In nature, enzymes are the catalysts that make much of biology work. They jumpstart chemical reactions that either wouldn't happen, or would happen super slowly. They break down food, build other molecules, extract energy, and more. What if we could harness evolution to engineer designer enzymes that do other specific jobs that benefit us?  Putting that idea into practice changed the game for chemistry, and earned Frances Arnold the Nobel Prize prize in 2018. She called it “directed evolution.” Today, thousands of labs use her methods to coax enzymes into doing things no one ever thought of. She joins Host Flora Lichtman to talk about where she sees this approach going in the future, and the personal evolution that brought her into science. I have 315 total /240 LDL/70HDL/50 Trig. I am 5.4 height, 120lb and gluten free and dairy free for 13yrs and eat very low carbs and sugar. Never taken statins because they make my joints hurt but had high cholesterol for years but 300 was my highest. What can I take that can help and what should i eat. Thank you Pati      Anonymous: Hi Dr. Cabral. Just wanted to confirm. Podcast 3576 reviewed a PubMed study that gave a minimum daily dosage of 10,800 FU to obtain desired results. The podcast gave me the impression that your supplement exceeded the 10,800 FU/day since only the 20,000 FU number was given. However, 3 plus does per day would be needed? Please can you clarify? Proteolytic Enzyme supplement. Natto Serving dosage on label is - (20,000 FU/g) 150mg My understanding is Natto amount is 3,000 FU 20,000 FU/g x .150g =3,000 Is this correct?      Ashley: Hi Dr. Cabral, I have a detox meal timing question for you. When I did my last set of the big 5 labs, my health coach told me that it would be OK for me to eat at 8 and 3 instead of 11:30 and 6:30 as I feel much better when I fuel my body in the morning. When I don't, I feel ravenous in the evening. However, the admin in the FB group disagreed and said I shouldn't do that. Can you please clarify and let me know how big of a difference it makes to eat later instead of earlier? I feel like if eating earlier in the day helps me actually stick to the foods I'm supposed to eat and I'm still fasting for the same lengths of time that  it would be OK. If it's a bigger deal than I realize, will you please explain the reasoning? Thanks so much!      Tommy: Hi Dr. C, I'm an IHP who's come a long way with healing, but I feel stuck for the remainder. I've attempted a detox only once in my life. After the 4th day I was so wiped out that I couldn't really even work. I've also got high heavy metals but the protocol causes me such histamine reactions (even at a lower dose) that I just can't see it through. I feel stuck, I play for a soccer team but even training is too hard on my nervous system and leaves me wired. Maybe I should just try gentler detox like sauna and castor oil packs but I'm not sure it will be enough, and the sauna may just deplete me further. Jahresstart. Viele Menschen wollen etwas verändern: weniger Hunger, mehr Kontrolle über das eigene Essverhalten, Gewichtsreduktion ohne ständigen Verzicht. In diesem Zusammenhang taucht eine Empfehlung besonders häufig auf - im Fitnessstudio oder auf Social Media: mehr Protein essen. High Protein gilt als einfach, effektiv und wissenschaftlich gut begründet. Doch was steckt wirklich dahinter? In der zweiten Folge unserer HEALTH NERDS Spezialserie nehmen wir die eiweißreiche Ernährung systematisch unter die Lupe. Wir erklären, ab wann eine Ernährung aus wissenschaftlicher Sicht überhaupt als proteinreich gilt, welche Mengen in Studien verwendet werden und warum High Protein oft weniger als eigenständiges Ernährungskonzept wahrgenommen wird, sondern eher als Ergänzung zu Low Carb, Keto oder klassischer Fitnessernährung. MCT Öl gilt als schneller Energielieferant. Ein Esslöffel im Kaffee soll genügen, um den Stoffwechsel effizienter arbeiten zu lassen, die Konzentration zu steigern und Fett direkt in Energie umzuwandeln. Kaum ein anderes Fett wird derzeit mit so klaren Leistungsversprechen beworben. Doch hinter diesem Trend steckt weniger Magie als ein besonderer biologischer Mechanismus. MCT steht für Medium Chain Triglycerides, also mittelkettige Fettsäuren. Entscheidend ist ihre Kettenlänge. Im Fokus stehen vor allem C8 und C10 Fettsäuren, Kaprylsäure und Kaprinsäure. Sie kommen natürlicherweise unter anderem in Kokosfett und in Muttermilch vor, dort jedoch eingebettet in ein komplexes Nahrungsgefüge. Als isoliertes Öl werden sie heute gezielt eingesetzt, um einen bestimmten Stoffwechselweg zu nutzen. In diesem HEALTH NERDS Deep Dive ordnen Podcast Host Felix Moese und Gesundheitswissenschaftler Matthias Baum die biochemischen Besonderheiten von MCT Öl ein. Der zentrale Unterschied liegt im Verdauungsweg. Während langkettige Fettsäuren Galle, Enzyme und den Transport über das Lymphsystem benötigen, werden mittelkettige Fettsäuren deutlich schneller aufgenommen. Sie gelangen direkt über die Pfortader zur Leber und werden dort rasch oxidiert. Dieser Weg erklärt, warum MCT Öl schneller Energie bereitstellen und die Ketonkörperproduktion erhöhen kann. Damit unterscheidet sich MCT Öl klar von klassischen Nahrungsfetten. Die schnelle Verfügbarkeit bedeutet jedoch nicht automatisch Fettabbau. Der Einfluss auf Körpergewicht und Fettmasse ist gering und immer an die Gesamtenergiebilanz gekoppelt. Auch MCT liefert neun Kilokalorien pro Gramm. Die beworbene Fettverbrennung ist daher kein eigenständiger Effekt, sondern eher eine Folge veränderter Energieverwertung im Rahmen bestimmter Ernährungsformen. Ein weiterer Aspekt ist die Verträglichkeit. Gerade weil MCT Fettsäuren den klassischen Verdauungsweg umgehen, reagieren viele Menschen empfindlich auf höhere Mengen. Übelkeit, Bauchkrämpfe oder Durchfall sind keine Seltenheit, wenn der Einstieg zu schnell erfolgt. Evolutionär betrachtet war der menschliche Stoffwechsel zwar immer mit mittelkettigen Fettsäuren konfrontiert, jedoch nie in isolierter, hochkonzentrierter Form. Genau diese fehlende Einbettung erklärt, warum Geschwindigkeit für den Stoffwechsel sowohl Vorteil als auch Belastung sein kann. Wann kann MCT Öl sinnvoll sein? Etwa bei eingeschränkter Fettverdauung, bei bestimmten ketogenen Ansätzen oder als gezielte Energiequelle für das zentrale Nervensystem. In this part, Dr. Longo will discuss the current treatment landscape and limitations in lysosomal disorders.Nicola Longo MD, PhDWhat I want to do today, is just place gene replacement therapy within the current landscape of lysosomal storage disorder treatment therapy. Gene therapy obviously has the potential of treating lysosomal disorder to correct the root cause of lysosomal storage disorder. The gene is defective, and what happen is that you can potentially either fix the gene or bypass the lack of the genetic product. But there are already therapies that are existing and are functioning. Obviously, in many cases, the lysosomal disorder is caused by defective production of an enzyme, which is defective.We can either replace the enzyme with enzyme replacement therapy, or provide chaperone for specific mutations that retain the synthesis of the enzyme, that however is not very functional. Another avenue that it is being reported is the utilization of substrate reduction therapy. A substrate accumulates, you prevent the synthesis of the substrate to reduce the accumulation of toxic material. What we know now is that this is not enough to produce many lysosomal disorders. In many cases, the lysosomal disorder result sometime in impairment of intracellular trafficking, and sometime in the function of other organelles.At the end, it results in the activation of the macrophagic system and inflammation. Already we have some therapy acting at this level. The end result of lysosomal storage disorder, there will be cell suffering and cell death, leading to a progression of the disease, and morbidity and mortality. Now, what therapy do we have available already? Obviously, hematopoietic stem cell transplantation has been around for quite some time.It has been the same thing that we do with gene therapy, except that instead of reintroducing the gene of the subject, we place gene of a subject who is not affected of the disease. This therapy has been proven effective in cases of MPS-1 and alpha-mannosidosis. But in many cases this has to be given way before symptoms start to be affected.Enzyme replacement therapy has been around for quite some time, starting with Gaucher disease, and now that it is available for a list of diseases that are there, so it's like Fabry, Gaucher, Pompe, different types of mucopolysaccharidosis, alpha-mannosidosis, acid lipase deficiency, 1 neuronal ceroid lipofuscinosis, and Niemann-Pick type A and B.Obviously the advantage of this therapy, they give back the enzyme that it is defective. But the disadvantage that many time they cannot enter specialized areas such as the brain. There is already the second generation of enzyme replacement therapy that it is available. With this second generation, some of the newer drugs are more effective in terms of cellular uptake, or in terms of having a prolonged half-life and prolonged activity.Then there are pharmacological chaperone therapy, and the one which is FDA approved is migalastat for Fabry disease, under study is ambroxol for Gaucher disease. The disadvantage of this therapy that only a selected number of mutations respond to this therapy.Substrate reduction therapy has been introduced for Gaucher disease many years ago with miglustat, and it was followed by eliglustat. Both of them are effective, and some of them more effective than other, simply because of the fewer side effects of eliglustat as compared to miglustat. But at the same time, eliglustat does not pass the blood brain barrier.Finally, the newer agents that are already administered, N-acetyl-L-leucine and arimoclomol, both approved for Niemann-Pick type C, they act more on the downstream effect of the lysosomal storage disorder, either by stabilizing neuronal cell activity or by reducing the inflammation that is present in the brain.In the next part, Dr. Longo will discuss gene replacement therapy in lysosomal disorders.

Thank you for joining us for our 2nd Cabral HouseCall of the weekend!   I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks…   Lynette: I have Celiac Disease and worry about cross-contamination when eating at restaurants or friends' homes. People have suggested taking a gluten digestive enzyme, but I don't want to digest gluten. Is there anything I can take prior to eating that would rid my body of gluten, should I unknowingly come into contact with it? I'm a "silent celiac" so don't experience immediate GI upset, but instead suffer joint pain, headaches, and skin issues afterward. Living gluten-free out in the world is tough, so any suggestions to make things easier and safer are appreciated. Thanks!                                          Anonymous: I received a complimentary bottle of CBD Gummies -- thanks! Are there any contraindications with taking them? I'm thinking about them for my older parents, who are on the typical American old-age pharmaceuticals, including high blood pressure medication, statins, and Gabapentin. For reference, I am a 49 year-old perimenopausal female. I started using about 8 mg of DHEA orally daily. I gave it a few weeks to resolve and then began 25 mg of 7 keto DHEA daily and have had no acne breakouts at all. On my call with my IHP through Equilife to follow up from the labs, I was told that 7 keto DHEA is the same as regular DHEA and is likely just a marketing tool. Can you tell me why I reacted significantly different to the 7 keto DHEA? AABP executive Director Dr. Fred Gingrich reviews a paper published in The Bovine Practitioner with two of the authors. Joining today are Dr. Greg Habing, professor in the Department of Preventive Medicine at The Ohio State University College of Veterinary Medicine, and Dr. Emily Nogay, clinical assistant professor at The Ohio State University Large Animal Ambulatory Services.  Salmonella Dublin is a host-adapted and zoonotic pathogen affecting many dairy farms. It can be associated with high morbidity and mortality and is often multi-drug resistant which elevates its importance to dairy farmers and veterinarians. Surveillance and diagnostics are an important aspect of managing diseases in our herds. Nogay reviews available diagnostic tests and the advantages and disadvantages of each one. The ELISA test has a sensitivity of 65%, but is used to screen herds for the disease. The objective of this study was to determine if the four U.S. commercially licensed Salmonella vaccines would result in a positive Salmonella Dublin ELISA test result.  We walk through the results of the paper and discuss some of the findings, including the impact of the vaccines on the serum ELISA results and some potential reasons for the ELISA results after vaccination. Habing discusses future opportunities for research on this disease, including the need for better diagnostic tests, including a PCR test to screen herds.  AABP's peer-reviewed journal, The Bovine Practitioner, publishes original research studies, reviews, case series, and case reports intended to provide clinically relevant research to private practicing veterinarians. Gene Mack, CEO and President of Gain Therapeutics, is combining AI-powered drug discovery with the development of allosteric modulators, drugs that bind to unique sites on proteins. The company's AI platform, Magellan, is crucial for accelerating drug discovery by reducing the time for computational screening of potential drug compounds.  Their lead compound is showing promising results as a potential disease-modifying therapy for Parkinson's disease, aiming to halt the progression of the disease rather than just treating symptoms. Gene explains, "So allosteric modulators of protein, it's a bit of a word salad, but what we're trying to achieve here is finding unique binding sites on proteins that are sort of away from the active site of that protein." "So, a lot of physics calculations go into these binding site calculations. The idea is to complete these quickly during the screening of hundreds or thousands of compounds. This process takes 10 to 15 minutes to run a set of computations and determine if a particular molecule is a fit for a specific protein. If that takes 10 or 15 minutes per compound, it's not a very big deal to go to that library if you need to get through billions, trillions of those compounds, and you need that computational speed to really fire up." "We are able to speed up those calculations from, let's say, 10 minutes to milliseconds. You can screen through much larger numbers of compounds and potentially even construct new molecules that are not known to the public domain, which would be a real key innovation." "What we think we have in our lead program, which is GT-02287, another molecule that was discovered through our application of Magellan. What we hope we have in GT-02287 is a disease-modifying approach to Parkinson's. CREBBP mutations create a zombie enzyme that competes with its wild-type counterparts.
