I am joining the Endocrine Review team and hope to help my colleagues pass the endocrine boards! Todays question is: which of the diodinase enzymes is most active in illness?Diodinase 1, 2 , 3 or 4. And the Answer is Diodinase THREE ====== The learning objectives today:In order to better understand and interpret thyroid laboratory findings we first need to take a step back and discuss the thyroid hormone regulatory pathway. In particular the thyroid hormone axisStarting at the hypothalamus, thyrotropin-releasing hormone (TRH) induces the pituitary to secrete thyroid stimulating hormone (TSH). TRH interacts with the thyrotrophic cell receptor to influence TSH glycoclysation which informs TSH bioactivity. TSH production is made in a pulsatile circadian fashion with its peak between 2am-4am (referred to as the nocturnal TSH surge) and its trough between 4p and 8p. In a euthyroid human, TSH is produced at between 50-200 milliunits/day and can increase to up to > 4000 mU/day in primary hypothyroidism. The half-life of TSH is betwee n 50 and 80 minutes. TSH bioactivity changes based on TSH glycosylation variability; due to different glycosylation nocturnal TSH is less bioactive which is why TSH does not lead to increased thyroid hormone production at night. After TSH production, T SH binds to thyroid releasing hormone receptors on the thyroid follicular cells activating the thyroid synthesis cascade. The Thyroid Synthesis CascadeThere are five steps in the thyroid synthesis process. First the precursor protein thyroglobulin (TG) is made from thyrocytes in the thyroid follicular cells; this precursor protein does not yet contain iodine. Step #2 iodide is brought from the circulation to the thyrocytes and into the follicular cells. Step 3: the enzyme thyroid peroxidase (TPO) is activated and exacts three roles: oxidation, organification, and coupling reaction to ultimately form the four-iodine atom containing Thyroxine (T4). T4 is the major thyroid hormone product of the thyroid synthesis circulating in the bloodstream. T4 the inactive form of thyroid hormone is converted to the active form of thyroid hormone triiodothyronine (T3), comprised of 3 iodine atoms. The thyroid produces 90% inactive T4 thyroid hormone and 10% active thyroid hormone T3. Thyroid hormone is stored bound to thyroglobulin in the follicular stores. Upon TSH stimulation proteolytic enzymes cleave thyroglobulin and T4 and T3 are released into the bloodstream. Thyroid hormones are lipophilic and are transported in the blood via transport proteins. Both circulating thyroid hormones (T4 and T3) exert direct negative feedback on TRH synthesis in the hypothalamus. Peripheral deiodination of T4 to T3 in the liver and kidney accounts for approximately the other 80-90% of circulating T3. There are three types of deiodinase enzymes, type I (DIO1), II (DIO2), and III(DIO3).DIO1 primarily functions in the liver, kidney, and thyroid. DIO2 predominates in the brown adipose tissue, skeletal muscle, heart, and CNS. DIO3 is primarily located in the CNS, skin, and placenta. In the final episode of our Community-based Solutions for Substance Use Challenges season, Just Science sits down with Kristen Lee, Substance Use Program Coordinator at the Georgia Criminal Justice Coordinating Council, Lizann Roberts, Executive Director of the Coastal Georgia Indicators Coalition, and Tara Jennings, Strategic Planning Administrator for Chatham County Government, to discuss their COSSUP-funded Community Data Platform, which helps connect health and justice data for an individual, so that first responders can best meet their needs. For individuals who frequently cycle through jails, homeless shelters, and emergency departments, there is a need to increase access to community resources to break the cycle of justice involvement and reduce the burden on first responders. In Chatham County, Georgia, the Community Data Sharing Program provides real-time data across public health and safety services, helping first responders gain a holistic view of an individual's justice involvement and refer clients to the appropriate community resources. Listen along as Kristen, Lizann, and Tara discuss why they decided to use data sharing as a tool to better serve clients with a substance use disorder, how they addressed barriers such as data sharing and privacy concerns, and how the program has not only impacted participating clients, but also local first responders and policy makers. In 2005, Kristen Lee and her family were looking for a new home after their old one was destroyed in a flood. Kristen found the Bellaire house, which seemed perfect despite its dark history. So So I think it helped me create those retreats or those air pockets in my life that I need. Because I tend to just be an over worker, I love what I do. There's a lot of need in the world. So it helped me be more present with myself and not let my mind be a runaway train. Thoughts on Breathing Above all, I love the box breathing method. So It's a nice four-count holding and then releasing. So The reason I like it so much is that it's sustainable because sometimes if I make things too grand and too complex, I might stay with it for a while, but then fall off track. So I was that loud person that would stand up if I saw someone being hurt. Understanding the impact on one's life when these things happen, and how to help teach social-emotional skills and kindness are things that weren't necessarily present on the school bus in the 1980s. So Therefore, mindfulness would have helped me probably be a little bit more self-compassionate, and it would have helped me deal with that override of anxiety and fear that I was doing in those moments. So I love the exercise of thinking like, what would your past self say to yourself? Or what would you want your future self to say to the now. If I could look back I would say to myself, this is going to help you develop your empathy, this is going to help you be more mindful in how you show up and you contribute to the world. Don't internalize the messages of the meanness of the people, doing this kind of bullying that doesn't define you. Highlights from my conversation with Paula Roy first dropped as a very early episode (003) in July 2017. I believe it is well worth revisiting in this moment where book-banning seems to be edging out baseball as the national pastime here in the Home of the Free, and too many legislators and governors have convinced themselves that they know more about education than professional educators do. There is plenty to critique about dominant modes of schooling, from pre-kindergarten all the way through grad school, and lots that really needs rethinking and change. But as you enter that conversation, it's important to acknowledge the expertise of professionals like Paula Roy, who have spent years thinking about what and how and why kids learn, and who have experience negotiating the competing demands of real students' individual needs and capabilities as well as changing political contexts outside the school. Above all, it's important to take students seriously, and to keep them at the center of your work. In more than 25 years as an educator, I've never met anyone who does that better than she does. This classic episode inspired not only this beautiful drawing by artist and school administrator Al Morales (called “Curriculum,” after Paula's description) but also a five-minute video exploring the difference between sarcasm and humor, with animation by Roy Chambers (available on the original show page linked below).
