This episode discusses four MMWR reports. In our first report, Idiopathic Pulmonary Fibrosis Mortality by Industry and Occupation - United States, 2020-2022, workers in some industries and occupations have higher-than-expected death rates from idiopathic pulmonary fibrosis, a progressive lung disease. In our second report, Enhanced Identification of Tobacco Use Among Adult Medicaid Members - King County, Washington, 2016-2023, identifying tobacco product use during Medicaid enrollment provides opportunities for states to help people quit smoking, improve their health, and lower health care spending. In our third report, Tobacco Product Use Among Adults - United States, 2017-2023, exclusive cigarette smoking has decreased, while exclusive e-cigarette use has increased, leading to no overall change in tobacco product use among adults from 2017 to 2023. In our last report, Detection of Vaccine-Derived Poliovirus Type 2 in Wastewater - Five European Countries, September-December 2024, variant poliovirus was detected in wastewater in five European countries last Fall.

Since 2005, global tobacco use has dropped by one-third, with 118 million fewer smokers, according to WHO research. A key driver of this progress is the WHO Framework Convention on Tobacco Control (WHO FCTC), one of the most widely embraced UN treaties. The FCTC, in effect for 20 years, regulates both tobacco supply and demand. With 183 parties covering 90% of the global population, it has led to major policy changes. Today, 138 countries require graphic health warnings on cigarette packs, dozens enforce plain packaging, 66 ban tobacco advertising, and over 25% of the world's population benefits from smoke-free laws. Higher taxes also make tobacco less affordable while funding health initiatives. In 2018, a legal protocol was introduced to combat illicit tobacco trade, which weakens control measures and fuels crime. Despite this progress, tobacco remains the leading cause of preventable death, linked to heart disease, cancer, stroke, and more. The industry, facing declining cigarette sales, now pushes e-cigarettes—misleadingly marketed as safer—while continuing to target young people. Only 56 countries are on track to meet the goal of a 30% reduction in tobacco use by 2025. Still, cutting global smoking rates by one-third is a major public health victory.

Tobacco use is a known modifiable risk factor for postoperative complications and revision surgery after anterior cruciate ligament reconstruction (ACLR). Previous studies focus on tobacco as a broad categorization of traditional smoking, smokeless tobacco, and other forms of nicotine use. It is unclear if differences in the type of nicotine used lead to similar adverse outcomes after ACLR.   In conclusion, smokeless tobacco use was associated with an increased risk of medical complications, ED utilization, and subsequent procedures compared with nonuser controls and traditional smokers. These findings highlight the importance of considering specific forms of tobacco use in preoperative screening for patients undergoing ACLR.     Click here to read the article.

The number of Americans using both cannabis and tobacco is increasing, with a particular surge among older adults, a new study finds. Researchers at Duke University School of Medicine who analyzed two decades of national data warn that the combination poses greater health risks than the dangers of using either substance alone. In 2021, 6.38% of U.S. adults reported using both cannabis and tobacco within the past month. While this might seem like a relatively small percentage, it's a big jump from previous years. Researchers attribute the trend to more states legalizing marijuana, and people becoming more accepting of and comfortable with cannabis. When looking at the 2021 data, younger individuals, people with lower educational attainment and residing in a state with a medical marijuana law, were most likely to report co-use,” said lead study author Dana Rubenstein, a fourth-year medical student at the Duke University School of Medicine and translational science researcher. Trends in cannabis and tobacco co-use prevalence have not been examined in many years while regulatory policy has been changing quite a bit. The campaign aims to prevent youth from using tobacco through powerful messaging and real-life stories, but how has The Real Cost remained relevant as the media landscape, young people, and the market of tobacco products all evolved rapidly? In this episode of Empathy Affect, we explore these questions and dive into the methods and public health impact of The Real Cost.April Brubach is the acting director of FDA's Center for Tobacco Products' (CTP) Office of Health Communications and Education (OHCE) and has worked at CTP since 2010. She joined CTP in 2010, working across a portfolio of tobacco prevention campaigns, education initiatives, and communication science and policy. She has previously built and managed the Health and Human Services' COVID-19 Public Education Campaign, and she has served as deputy for FDA's COVID-19 Joint Information Cell and communication program manager at the National Cancer Institute.Merrybelle Guo is a social scientist at FDA CTP's OCHE. Ankle fractures are common orthopaedic injuries that may be indicated for open reduction internal fixation (ORIF). Although the negative impact of tobacco use on perioperative outcomes of ankle fracture ORIF has been described, the potential impact of cannabis use on related outcomes is not as well established.   In conclusion, patients with tobacco use (alone or with cannabis) were at greater odds of 90-day adverse events following ankle fracture ORIF, but cannabis only users were not.   Click here to read the article.

Since the early 2000s, Tobacco use has declined steadily and in some cases very sharply nearly everywhere in the world except China. According to the world health organization, Tobacco use for people 15 years or older declined globally from 34% in the year 2000 to 23% last year. But in China, tobacco use has remained relatively stable -- falling just 1%, from 27 to 26 percent in the last two decades.  A new piece of investigative journalism offers one key explanation of why China has been such an outlier to this global trend, namely the political influence of China's national tobacco monopoly.  My guest today, Jason McClure, a correspondent with The Examination, a new non profit investigative news agency focused on global health. He is one of the authors of the report detailing the ways in which the state-run China National Tobacco Corporation successfully undermined Tobacco use reduction efforts in China. In May 2023, the MoHFW brought in the regulation of tobacco depiction on OCCPs or OTT content platforms, requiring them to put a health spot of a minimum of 30 seconds along with a prominent health disclaimer on online curated content. A recent report by researchers at the Koan Advisory Group, in collaboration with survey partner Youth Ki Awaaz, examines the trade-offs of mandatory health warnings during smoking scenes in OTT content, with data from a primary survey of about 1,900 OTT viewers. Carl Jaison speaks to Tamanna Sharma, Associate at Koan Advisory Group, on a major gap in existing research that prompted them to conduct this study: the need for more young consumer feedback. They discuss the key findings & results from the research and the key recommendations on alternate strategies for an industry-led response by OTT platforms. According to the World Health Organization (WHO), more than 8 million people are killed by tobacco each year.Unfortunately, most tobacco-related deaths occur in low- and middle-income countries, areas that are targets of intensive tobacco industry marketing. But the good news is that the scale of this human tragedy is preventable.The Bloomberg Initiative to Reduce Tobacco Use at Bloomberg Philanthropies works with national and local governments in more than 110 countries to help implement measures to protect people from harm, such as creating smoke-free public places, banning tobacco advertising, and increasing tax on tobacco products. This initiative builds on Mike Bloomberg's successful efforts in reducing smoking rates during his time as mayor of New York City. Since the launch of the Bloomberg Initiative to Reduce Tobacco Use in 2007, global smoking rates have fallen from 22.7% to 17.5%. While we're making strides to save lives around the world, there's still work to be done in the face of new challenges, such as the rise in flavored tobacco products and e-cigarette use among teens in the U.S.On this episode, Katherine Oliver sits down with Betsy Fuller, who is part of Bloomberg Philanthropies' team working to reduce tobacco use, to discuss our tobacco control work in the Philippines, how the country is working to combat youth vaping, and to share an update on global smoking rates around the world.

Prior to earning her doctorate, she worked as an elementary and middle school counselor in Southern California, was the district-level Tobacco Use, Prevention, and Education (TUPE) Coordinator, and was a site supervisor for practicum and internship students.  Her interests include assessment & data-driven decision making, incorporating creativity in counseling and supervision, and online counselor education. She is award winning and has served in multiple leadership roles with various state and national counseling associations, including California Association of School Counselors, Florida School Counselor Association, and Chi Sigma Iota International. Highlights From Today's Episode “For every person who dies from smoking, at least 30 people will live with a serious smoking-related illness. Smoking causes cancer; heart disease; strokes; lung diseases, including chronic obstructive pulmonary disease, which include emphysema and chronic bronchitis; and diabetes. 87% of all lung cancers are directly linked to smoking. . . . One out of every three cancer deaths are directly related to smoking.” Timestamp (TS) 11:50 “When and if a patient continues to smoke with a cancer diagnosis, it's because there's a high nicotine dependence. They're smoking to manage their withdrawal symptoms. The biggest withdrawal symptoms are anxiety and depression. And one might say that just being diagnosed with a cancer diagnosis is very anxiety-provoking. . . . They get very, very anxious, and the nicotine receptors in the brain will actually tell them to have a cigarette.” TS 15:58 “One of the byproducts of tobacco is carbon monoxide, and that is retained in the lungs. . . . And in eight hours, we can start to reverse that. In 24 hours, the risk of a heart attack decreases if you stop smoking. In about two weeks to three months after stopping smoking, your circulation starts to improve and your lung function increases.” TS 27:43 “As an oncology nurse, I think we need to start really focusing on some of the positive reasons why patients need to stop smoking in any prognosis that they have across the board. I think people respond to positive feedback better than negative feedback. That's why the benefits of smoking cessation for patients with cancer are so important to talk about.” TS 29:35 “It's the oncology nurse who might be the only cheerleader this person has to keep them motivated moving forward, and that's what we need to do.
