This extensive reissue includes a wealth of previously unheard alternate takes, outtakes, and demos. As their fifth and final studio album, Synchronicity features hit singles such as "Every Breath You Take," "Wrapped Around Your Finger," "King of Pain," and "Synchronicity II." The album has sold 15 million copies globally, with 8.5 million in the USA alone. This reissue project, which took three years to complete, was created with the band's involvement and endorsement. It includes a 60-page hardcover book with comprehensive new sleeve notes by music journalist Jason Draper, detailing the album's creation and history. The box set contains: CD1: The original album, including "Murder By Numbers," remastered from the original tapes. CD2: 18 tracks featuring all original 7" / 12" B-sides plus 11 exclusive non-album bonus tracks, available on CD for the first time. CD3 and CD4: Previously unreleased alternate takes of all the Synchronicity songs. CD4 also includes unreleased Police tracks, such as an early version of Andy Summers' "Goodbye Tomorrow" (later renamed "Someone To Talk To"), a demo of Stewart Copeland's "I'm Blind" (later "Brothers on Wheels" for the Rumble Fish soundtrack), a first take of "Truth Hits Everybody," and rare covers of Eddie Cochran's "Three Steps To Heaven" and Chuck Berry's "Rock and Roll Music." Treatments for chronic lymphocytic leukemia (CLL) and mantle cell lymphoma (MCL) have advanced in recent years, with more new developments on the horizon. James Gerson, MD, of the University of Pennsylvania, Philadelphia, reviewed some of these advances and future directions while describing how he would treat three patients. Host David H. Henry, MD, posed the following cases for consideration. Case 1 In a 75-year-old male with no comorbid illness, routine blood work revealed a WBC count of 15,000/mcL. The manual differential showed mature lymphocytes and smudge cells. The patient has no constitutional symptoms, but there is suspicion of CLL. What to do? An incidental finding of elevated WBC is the most common presentation for CLL, Dr. Gerson noted. Flow cytometry is how most diagnoses are made. If the patient’s blood sample is CD5+ and CD20+, in the vast majority of cases, the patient has CLL. The main alternative diagnosis is MCL, so Dr. Gerson recommends checking for cyclin-D1 overexpression and translocation (11;14), which would be present in MCL. Dr. Gerson also recommends fluorescence in situ hybridization (FISH), cytogenetics, and next-generation sequencing to assist with prognostication. When the exam is normal and the patient is asymptomatic, no imaging is required. For this patient, treatment should be deferred until disease progression. Case 2 A 75-year-old, fit male has a WBC of 25,000/mcL, noted after the patient reported not feeling well, having a distended abdomen, night sweats, and weight loss. Blood work shows a hemoglobin level of 10.5 g/dL and a platelet count of 130 x 103/mcL. What to do? Because this patient is symptomatic, treatment is indicated, Dr. Gerson said. However, because of the pandemic, Dr. Gerson would likely delay therapy, perhaps until after COVID-19 vaccination. To assess risk, Dr. Gerson would perform immunohistochemistry, FISH, and next-generation sequencing in this patient. Patients with 17p deletion have high-risk disease, those with TP53 missense or nonsense mutations have even higher-risk disease, and patients with both a deletion and a mutation are “at excessively high risk,” Dr. Gerson said. He favors giving a BTK inhibitor to patients with TP53 mutation/17p deletion because of results from the CLL14 trial (N Engl J Med 2019; 380:2225-36. https://bit.ly/38pVHbf). However, because of the “small signal” in the trial, Dr. Gerson said plenty of his colleagues use a BTK inhibitor interchangeably with a BCL2 inhibitor and anti-CD20 therapy (e.g., venetoclax and obinutuzumab). Dr. Gerson said ibrutinib and acalabrutinib have similar efficacy, according to unpublished results of the ELEVATE-RR trial (https://bit.ly/38onIjy). Both drugs inhibit platelets, and there appears to be a higher risk of atrial fibrillation with ibrutinib. Case 3 A 75-year-old, fit male has an elevated WBC, noted after complaints of bone pain, weight loss, night sweats, and enlarged lymph nodes. There is suspicion of MCL. What to do? MCL is a complicated disease and “incredibly” heterogeneous in clinical and pathological behavior, Dr. Gerson noted. The classic finding in MCL is cyclin-D1 overexpression caused by (11;14) translocation, but there are atypical translocations as well. The approach to first-line treatment of MCL varies and may include cytarabine-based induction, bendamustine plus rituximab, or rituximab alone. Brexucabtagene autoleucel, a chimeric antigen receptor T-cell therapy, is changing the landscape of treatment for relapsed/refractory MCL, Dr. Gerson said, as this treatment may offer a cure. Investigational therapies for MCL include the BTK inhibitor pirtobrutinib (LOXO-305) and hematopoietic stem cell transplant. Pirtobrutinib is under investigation in patients with MCL, CLL, and other indolent lymphomas that have progressed on or are intolerant to first-line therapy (Lancet. 2021 Mar 6;397[10277]:892-901. https://bit.ly/3rvRv1h). In this episode, Bo Bennett speaks with James Woods, an openly-atheist congressional candidate for Arizona's 5th district. Though he lost in the November 4 general election, his trailblazing campaign helped pave the way for other openly secular candidates in the future. Read about James Woods (in his own words): I've lived in Arizona my whole life. I was born in the Arizona East Valley and raised by good-hearted working class parents. My dad is one of my best friends and supporters. We lost my mom to cancer in 2002, but I think she would be proud of how close and strong our family has remained. I graduated from Dobson High in ‘97 and went to Mesa Community College, working toward a career in the tech industry. I wanted the kind of job where I could support myself and a family, but also contribute to the quality of life in Arizona by developing technology-based solutions to the problems our state faces. A month before my 27th birthday, I was hospitalized for a rare illness that nearly killed me. I didn't have health coverage. I survived organ failure and amputations. While my medical team fought to keep me alive, my vision started fading. On a Monday I told the doctor that things were starting to look dim, and by Friday I never saw again. I ended up on dialysis and even spent time in hospice. I would not be alive today without all the people who invested in me. You invested in me through your contributions to government programs like Medicaid, Social Security Disability and Nutrition Assistance. My dad invested in me and became my caregiver. And someone I never met gave me a new chance at a healthy life through organ donation. After all of that generosity and support, it's time for me to start giving back. Last February I received a new kidney, and from my hospital bed in Phoenix I signed the paperwork to run for the US Congress in Arizona's CD5. After my long struggle to get the health care I needed and the losses I experienced when I couldn't get it, I knew I had an opportunity to start advocating for change. There is a damaging disparity between the decision-makers in Washington and the people who have to live with the decisions they make. We deserve representatives who know what it's like to face hard times and what's needed to overcome them. I can provide that representation. I have already overcome some tough odds, and I believe it's time to overcome the conservative ideology that hurts the most vulnerable among us.

Schizophrenia has often been observed to be associated with alterations of the cellular immune system. In this study we monitored numerous immune parameters in the course of antipsychotic treatment. 40 patients diagnosed with an acute exacerbation of schizophrenia were tested before and during treatment with antipsychotics. The percentual distribution of lymphocyte subgroups (CD3, CD4, CD8, CD19, CD5, CD16/56, CD25, HLA-DR, gd, CD11a, CD11b and CD49d) was measured by FACS analysis. 20 healthy volunteers served as controls. In the acute state of psychosis a significant elevation of the B-lymphocyte fraction was observed, while the percentage of T-lymphocytes was decreased. These values levelled to those of the control group in the course of treatment. Natural killer cells, integrine expressing cells and CD5+ B-cells showed alterations in numbers after several months of treatment. The latter may be a side effect of antipsychotic treatment. The findings in the acute state of schizophrenia could be seen as a nonspecific stress reaction or an effect of the altered metabolism of neurotransmitters. On the other hand the observed changes in the immune system could be due to infectious diseases or autoimmune processes provoking the symptoms of schizophrenia.

Bei schizophrenen Patienten sind immunologische Veränderungen nachweisbar, die auf eine Aktivierung des Immunsystems hinweisen und die seit langem diskutierte Hypothese stützen, dass das Immunsystem in der Pathogenese der Schizophrenie involviert ist. Es gibt Anhaltspunkte, die die Annahme eines inflammatorischen - sei es infektiös oder autoimmunologisch getriggerten - Prozesses im Gehirn schizophrener Patienten stützen, der zu einer erhöhten Permeabilität der Blut-Hirnschranke führt und für die psychopathologische Symptomatik verantwortlich gemacht wird. In dieser Arbeit wurden mittels Dreifarben-Durchflusszytometrie (FACS-Analyse) Lymphozyten-Subpopulationen von 31 schizophrenen Patienten bestimmt und sowohl mit Befunden gesunder Kontrollpersonen, als auch desselben Patienten vor und nach Neuroleptikabehandlung verglichen. Gemessen wurde die Expression einer Auswahl von Oberflächenantigenen, die den Aktivierungszustand des Immunsystems anzeigen bzw. mit autoimmunologischen Prozessen in Verbindung gebracht werden. Dadurch war es möglich allgemeine immunologische Veränderungen des peripheren Immunsystems schizophrener Patienten zu erfassen und diese auch im Verlauf der Therapie mit Neuroleptika, sowie in Korrelation mit psychopathologischem Befund und Prognose zu betrachten. Die in dieser Arbeit gezeigten Ergebnisse stützen die Hypothese, dass bei schizophrenen Patienten eine Aktivierung des Immunsystems vorliegt. So fand sich bei Schizophrenen nach Behandlung im Vergleich zu Kontrollen eine Erhöhung der CD4-positiven T-Zellen – ein Befund, der auch bei Autoimmunerkrankungen gesehen wird und dort für die chronische Stimulation von B-Lymphozyten verantwortlich gemacht wird. Zusätzlich wurden Patientengruppen anhand des psychopathologischen Verlaufes und des Therapieerfolgs gebildet. Ein Zusammenhang bestand zwischen dem Ansprechen auf Therapie und CD8-positiven T-Lymphozyten: Die Patientengruppe mit niedrigerem Anteil von CD8+- Zellen, also weniger Suppressorzellen, zeigte ein schlechteres Ansprechen. Ein weiteres Aktivierungszeichen des zellulären Immunsystems ist die Expression des CD45RO-Moleküls anstelle von CD45RA auf CD4-positiven Lymphozyten. Auch hier konnte bei schizophrenen Patienten ein erhöhter Anteil von CD4+/CD45RO+- (Gedächtnis-) Zellen, insbesondere nach Behandlung, beobachtet werden, wobei wiederum Patienten mit schlechterem Ansprechen auf Therapie stärkere Veränderungen zeigen. Unterstrichen wird dieser Befund durch die parallele Zunahme der Expression des Adhäsionsmoleküls CD58 und CD60 bei Schizophrenen unter Behandlung, sowie zusätzlich bei schlechter Therapieantwort. Eine Expression dieser Moleküle weist auf die Fähigkeit zur transepithelialen Migration der Zellen, auf eine weitere Immunaktivierung und Stimulation von B-Zellen hin. Ebenfalls ähnlich wie bei Autoimmunerkrankungen wurde in der hier untersuchten Patientengruppe eine erhöhte Expression von HLA-DR gefunden. Dies, wie auch eine geringe Zahl von CD4+/45RA+-Lymphozyten, die als Suppressor-Inducer-Zellen bekannt sind, kann als fehlende Suppression von Autoimmunreaktionen interpretiert werden. Darüberhinaus scheint ein schlechtes Outcome der Patienten mit insgesamt erhöhter immunologischer Aktivität assoziiert zu sein. Die Ergebnisse im Bezug auf CD5+/19+- Zellen und NK-Zellen runden das Bild einer Modulation immunologischer Mechanismen im Verlauf einer schizophrenen Störung und durch neuroleptische Therapie ab. In der Literatur beschriebene Störungen der Blut-Hirnschranke und Reaktionen des hirneigenen Immunsystems weisen auf eine Interaktion von peripherem und zentralem Immunsystem im Rahmen der Erkrankung hin. In der akuten Phase der Schizophrenie scheint dabei die Aktivierung des Th2-Systems im Vordergrund zu stehen. Im Verlauf der neuroleptischen Behandlung findet dann ein Wechsel zum Th1-System statt. Veränderungen im Zytokinsystem sind von besonderem Interesse, da diesen Botenstoffen eine Vermittlerrolle zwischen Immunsystem und Nervensystem zukommt und so ein Einfluss auf neurometabolische Vorgänge erklärbar ist. Die in dieser Arbeit beobachteten Veränderungen unter Neuroleptikatherapie sprechen für einen immunmodulatorischen Wirkmechanismus dieser Substanzen. Für die Substanz Clozapin sind beispielsweise derartige Effekte bekannt. Bei der Schizophrenie handelt es sich um eine heterogene Krankheitsentität, für die es zahlreiche Erklärungsmodelle gibt. Die hier erhobenen Befunde stehen in Einklang mit Ergebnissen anderer Studien und können diese zum Teil bestätigen und ergänzen. Allerdings finden sich in der Literatur diskrepante und umstrittene Befunde bezüglich der zellulären und humoralen Veränderungen des Immunsystems, die die Komplexität des Krankheitsbildes deutlich machen und vielleicht als Ausdruck ätiologischer Unterschiede angesehen werden können.
We identified a novel T cell Ag in the South African clawed toad (Xenopus laevis) by a mAb designated 2B1. This Ag is present in relatively high levels on most thymocytes, approximately 65% of splenocytes, 55% of PBL, and 65% of intestinal lymphocytes, but is rarely seen on IgM+ B cells in any of these tissues. Lymphocytes bearing the 2B1 Ag proliferate in response to stimulation with Con A or PHA, whereas the 2B1- lymphocytes are reactive to LPS. Biochemical analysis indicates that this Ag is a differentially phosphorylated glycoprotein of 71 to 82 kDa. The protein core of 64 kDa bears both N- and O-linked carbohydrate side chains. The amino-terminal protein sequence of the 2B1 Ag shares significant homology with both the macrophage scavenger receptor type 1 motif and the mammalian CD5/CD6 family. The biochemical characteristics and cellular distribution of the 2B1 Ag suggest that it represents the CD5 homologue in X. laevis. While T cells constitutively express this highly conserved molecule, Xenopus B cells acquire the CD5 homologue only when they are stimulated in the presence of T cells
