The question has been ever present since I went through school in 2006-2009 - In what direction does the profession need to move?Over the last 10 years, I've seen a lot and I've learned a lot. No matter how confident I am with my assessments and treatments, I can always do better. I can always learn more. And my outcomes can always improve. I think the profession as a whole needs to operate from this mindset. We can never stop learning, adapting, improving, and recycling the process.Aside from that, this podcast focuses the true giants in the conservative care space. I highlight chiropractors such as Craig Liebenson and Greg Rose, but also Kelly Starrett and Gray Cook (physical therapists). They have all collaborated to give us epic texts that guide us in terms of assessment and treatment. So now we circle back to the question - where does the chiropractic profession need to go?Well, we obviously need to see more than 10% of the population if we want to see our communities beat the chronic disease monster. But, a better question may be - how do we do that?I firmly believe that if the profession shows more solidarity in the following areas, we will see massive improvement over the next decade:Consistent physical capacity assessments (PCA) for acute care patients and patients starting care for chronic issuesPioneering research to broaden the PCA assessment to include more upper spine and extremity dataA flow towards Functional Movement Screens with sub-acute and progressing patients to reduce recurrence and maintain/develop capacitiesCap off treatment with a patient that can safely and confidently walk into CrossFit/Orange Theory/F45/HIIT/Functional TrainingIf we are the pioneers in not only improving patient outcomes, but fighting chronic disease, we will win in the court of public opinion and substantially increase the objective data that supports our profession as being the best place for pain patients to start.Here's to the next 10 years crew!!!Good Luck and #keepmovingDr.J

Transferrin saturation >45% is highly suggestive of iron overload as a contributing factor, including hemochromatosis, and would warrant referral to hematology. Other key tests to order include: CBC, ESR, CRP, Cr and lytes, LFTs and hepatitis serology (B and C), abdominal U/S, glucose, A1C, lipids, and HFE gene testing if you’re suspicious of hemochromatosis.3. Good rule of thumb: High ferritin is anything >300, but for those in the 300-1000 range, lifestyle modification targeting the non-iron overload causes is appropriate, with hematology referral for ferritin above 1000 as this plus high transferrin saturation is specific for hemochromatosis and phlebotomy may be necessary. Alcohol cessation is also recommended in hemochromatosis.

Ferritin below 15 equals iron deficiency. In the setting of chronic inflammation, ferritin below 30 and transferrin saturation below 20% can be used as cutoffs. Iron deficiency can cause anemia and non-anemic fatigue. Treatment is appropriate in both cases.2. Treat underlying causes when possible. Causes can be split into decreased iron intake and increased blood loss. Think about decreased iron intake with vegetarianism, autoimmune gastritis, H. pylori gastritis, celiac disease, gastric bypass, as well as blood loss – commonly GI (including malignancy) and heavy menstrual bleeding. 3. To replete iron deficiency, aim to restore 1-1.5 g of iron, whether that is more rapidly with iron infusions or over a few months with oral iron supplements. Pay attention to amount of elemental iron in what you’re prescribing. Oral iron should be taken at least 2 hours after meals with Vitamin C and without calcium or PPIs. Check for response to iron supplementation in 4-6 weeks.

Dr. Timothy P. Novelli, of North Canton, Ohio has been in practice since 1985. At the Parker Seminar in January in Las Vegas in front of an audience of more than 3000, Dr. Singer received the Lifetime Achievement Award. Dr. Singer graduated Chiropractic School in 1972 from what was then Columbia Institute of Chiropractic which went on to become New York Chiropractic College. In 1981, his practice took off like a rocket with nearly 50 new patients per week and a total of over a thousand visits a week in 9 months. He then entered the world of Chiropractic Consulting. For the last 33 years he has traveled over 6 million miles to help Chiropractors succeed in practice from Australia to Italy. His company has helped nearly one thousand clinics per year for 33 years to get more new patients, educate them better and earn more income. During this period of time he delivered over 10,000 seminars and teleconference. He and his company appeared twice on the INC. 500 list of the fastest growing privately owned companies in America.
