Having just survived the calorie-fest of the Thanksgiving Holiday, show hosts Drs. Vivian Vega and Jackie Sherbuk reflect on the changing seasons--and that has them thinking NOT just about the weather, but about the climate. Here to discuss the impact of a changing climate on infectious diseases, they welcome Dr. Lauren Rybolt, Chief Infectious Diseases Fellow at the University of South Florida Morsani College of Medicine. Dr. Rybolt mentions that in the Tampa Bay Area, especially after the recent hurricane season, we are already seeing the effects of climate on infectious diseases occuring in real time. The direct effects of two rapidly intensifying hurricanes, Helene and Milton, that impacted Florida in late September and October have led to increased outbreaks of vibrio vulnificus and other waterborne infections, subjects which were highlighted in our very first episode.Dr. Rybolt explains that the effects of climate change are not singular, but are the results of many interconnected processes. For instance, how warming oceans affect precipitation patterns over the U.S., leading to flooding in some areas and drier conditions in others. Flooding can pollute streams and rivers and contaminate water supplies, while displacing people and increasing exposure to mosquito-borne disease; whereas drought can lead to water insecurity and the health consequences of extreme heat on vulnerable populations. Man made deforestation and land development can worsen the effects of flooding, and at the same time exacerbate climate change due to the loss of ecosystems and carbon-absorbing vegetation, for instance. Dr. Rybolt goes on to describe how the changing climate is producing an expanding zone of tropical infectious diseases in areas where they never occurred. More cases of Malaria and Dengue have been reported in Florida in recent years, for example. Tick-borne infections such as Lyme disease can expand to non-endemic regions due to climate change, but in Florida, we have been protected by a certain ubiquitous reptile. Certain fungal infections such as Candida auris have also spread beyond their typical "endemic areas" as well.Lastly, during their "ID News" segment, Vivian and Jackie discuss World AIDS Day, trends in vaccination and respiratory virus season, as well as the recent discovery of Bird Flu in raw milk.Thanks to Dr. Lauren Rybolt for appearing on this episode to share her knowledge about the infectious diseases impacts of climate change.Dr Vega would like to thank her friend Job Meiller, her YES Man, for the wonderful musical contributions and coming through on every idea she has. Dr. Richard Oehler, Professor of Medicine at the Division of Infectious Diseases at the University of South Florida Morsani College of Medicine, presents a review of bacteremia. Dr. Oehler begins by reviewing the epidemiology, personal and institutional costs of bacteremia. He then discusses blood culture collection and how false positive blood cultures occur. With the recent blood culture bottle shortage of 2024, he emphasizes the new concept of “blood culture stewardship”–when it is most appropriate to order a blood culture versus when it may be unnecessary. Next, Dr. Oehler reviews automated blood culture systems and other related identification technology, with a focus on new methods of blood culture isolate identification such as multiplex PCR and Maldi-TOF. The speaker then contrasts gram positive and gram negative bacteremia in general and by organism. The management of a positive blood culture with regard to treatment and source control is discussed. Lastly, ways to reduce the incidence of bacteremia are shared.

Gulfcoast Ultrasound Institute YouTube Channel On this episode of The Sonography Lounge hosts Lori and Trisha sit down with Dr. Charlotte Derr & Dr. Allyson Hansen to discuss the GUARD Protocol (Gastric Ultrasound for Aspiration Risk Determination). With the increased use of GLP-1 Agonists (Semaglutide, with brand names such as Ozempic, Wegovy, Rybelsus, Etc) for both diabetes and weight loss, there has been an increased number of aspirations in patients undergoing procedures. One of the effects of GLP-1 agonists is delayed gastric emptying which keeps patients feeling "full" thus reducing the desire to eat and overall caloric intake and blood sugar variations. Typically 6-8 hours not eating is enough time for a patient about to undergo an elective procedure to digest and empty their stomachs thus reducing the risk of aspiration while under anesthesia. With patients on GLP-1 agonists, we are seeing gastric contents remaining for several days after the last meal which can increase the risk of aspiration.  Dr. Derr and Dr. Hansen, discuss how they are using ultrasound and the GUARD protocol to determine the volume of food in a patient's stomach before elective and emergency procedures. Williams led the trauma team that treated police officers ambushed by a sniper in Dallas, Texas, on July 7, 2016, the largest loss of life for US law enforcement since 9/11. At a press conference days later, his heartfelt comments about the tragedy touched thousands, and Huffington Post named it one of the most memorable television moments of 2016. He has more than two decades of experience treating urban gun violence victims in Tampa, Boston, Atlanta, Dallas,and Chicago, and has served on the trauma faculty at UT Southwestern Medical Center in Dallas and at the University of Chicago. Williams graduated from the United States Air Force Academy with a degree in Aeronautical Engineering. After six years on active duty, he enrolled at the University of South Florida Morsani College of Medicine and then completed a general surgery residency at Harvard Medical School and a fellowship in trauma and surgical critical care at Emory University. After the Dallas shooting, Williams became an international spokesperson for racial justice. In 2017 Mayor Rawlings appointed him chair of the Dallas Citizens Police Review Board. His leadership helped unite the Dallas Police Department, community activists, police associations, and City Council to revamp the role Dallas civilians play in police oversight. In addition to his role as a trauma surgeon, Williams served as a congressional health policy advisor in the US Senate and the Speaker of the US House of Representatives, is a guest commentator on CNN, keynote speaker, opinion writer featured in the Chicago Tribune, Dallas Morning News, and Newsweek, and host of the podcast Race, Violence & Medicine. He originally began his research path at Florida State University in the Exercise Science studying the influence of nutrition, exercise, supplementation, and environmental extremes on health-based outcomes in normal and clinical populations. From Florida State University, he transitioned into Biomedical Research where he received his doctorate in Biomedical Sciences with the Metabolic Medicine Lab at University of South Florida Morsani College of Medicine with a focus on studying metabolism and metabolic therapies for health, disease, and performance outcomes. Personally, Andrew Koutnik has also used exercise, nutrition, and lifestyle factors to manage Type-1 Diabetes for over 12 years. Melissa Wilson, PA-C, MPAS from UPMC Hillman Cancer Center, and Vernon Sondak, MD, Chair, Department of Cutaneous Oncology, Moffitt Cancer Center and Research Institute in Tampa, Florida, join host Raymond Liu, MD, for a discussion about the future of melanoma in 2023 and beyond. Treatments combining immunotherapies with targeted therapies, which disable the carcinogenic products of mutated cancer cells, have further increased treatment efficacy and durability. Toxicity and resistance, however, remain critical challenges to the field. During this podcast, our guests review past treatments and novel therapeutic interventions and discuss the term "de-escalation of care" and future directions as well as how to communicate with your healthcare team. About Our Guests Dr. Sondak is Chair of the Department of Cutaneous Oncology at the H. Lee Moffitt Cancer Center and Research Institute in Tampa, Florida. He holds the Richard M. Schulze Family Foundation Distinguished Endowed Chair in Cutaneous Oncology, and is also a Professor in the Departments of Oncologic Sciences and Surgery at the University of South Florida Morsani College of Medicine. Since 2004, when he came to Tampa, the Cutaneous Oncology Clinic at Moffitt has grown into one of the largest multidisciplinary treatment centers for pediatric melanoma and related conditions in the world. Dr. Sondak has been a leader in studies of surgical treatment of melanoma and other cutaneous malignancies, particularly in the application of sentinel lymph node biopsy and lymph node dissection to the staging and treatment of melanoma, and has been instrumental in training many surgical oncologists in these techniques over the years. Dr. Sondak also has a strong research background: he served as Principal Investigator of the Moffitt Skin SPORE, a major NCI-funded “team science” grant conducting translational research in melanoma and other cutaneous malignancies. His research interests include surgical treatment of melanoma in adults and children; adjuvant (postoperative) and neoadjuvant (preoperative) therapy of melanoma; and evaluation of new therapies for patients with localized or disseminated melanoma. Melissa Wilson, PA-C, MPAS, has a wealth of knowledge and experience helping melanoma patients and their families. Melissa is up-to-date about the latest melanoma treatments and research. A natural trailblazer, Melissa is proud of her collaborative work as an NCCPA-certified physician assistant senior since 2005 with the distinguished melanoma specialist Dr. John Kirkwood at the University of Pittsburgh in the Cancer Institute Division of Hematology/Oncology, Department of Medicine. I'm Geraldine Carroll, a reporter for ASCO Daily News. Today I'm delighted to welcome Dr. Jhanelle Gray. She is the department chair of thoracic oncology and co-leader of the Molecular Medicine Program at the Moffitt Cancer Center. She's also a professor at the University of South Florida Morsani College of Medicine and chair of the 2022 ASCO Annual Meeting Education Program. Dr. Gray will tell us about the hot topics and must-see educational sessions at this year's [ASCO] Annual Meeting. Dr. Gray, it's great to have you on the podcast today. Dr. Jhanelle Gray: Thank you for having me. I am excited to be here with you today and for the opportunity to chat with you about the upcoming 2022 ASCO Annual Meeting and the educational sessions. ASCO Daily News: Well, the theme of the Annual Meeting is advancing equitable cancer care through innovation. Can you tell us how equity and innovation are reflected in the Education Program? And what would you say are the must-see sessions in this year's program? Dr. Jhanelle Gray: I am excited about sharing and hearing the latest advances in our field so we can move toward impact innovation and equity across our global cancer care community. [In] many of the sessions that we have, the attendees will join us either online or in person, and really will help us come together with a common goal of reducing the cancer burden. The presidential theme from Dr. Everett Vokes has really helped us to formulate what these sessions are. A few of them that I think really align with where we want to go for this 2022 ASCO [Annual] Meeting are things such as looking at strategies to advance cancer equity in our cancer clinical trials. We also have sessions such as “Artificial Intelligence in Oncology: The Current Field and Where It Is Headed,” and this touches on our innovation piece. We also have some really great keynote speakers such as a session—our ASCO Town Hall, moderated by Dr. Monica Bertagnolli, a past ASCO president and she'll be talking to us about the future of the conduct of clinical trials after COVID-19. I hope this gives you a sense of the exciting topics we have as we work to identify and address the challenges in this global cancer care field. ASCO Daily News: Thanks. Well, a couple of other sessions that are really trying to address these challenges are 2 joint sessions. So, I'd like to ask you about those. The first one involves ASCO and the American Association for Cancer Research, or AACR. And the second one features ASCO and the European Cancer Organization (ECO). Can you tell us about the topics of these sessions and why you think it's important for participants to see these particular sessions? Dr. Jhanelle Gray: Thank you. That's a great question. And thank you to AACR and ECO for their engagement and collaboration in planning and designing these sessions. We work to ensure that both organization's priorities and expertise are truly represented. The ASCO-AACR joint session is titled, “ASCO/American Association for Cancer Research (AACR) Joint Session: The Promise of DNA Damage Response and Repair in Cancer,” and the ASCO-ECO joint session is on HPV vaccination prevention and treatment. These sessions include hot topics in oncology and were planned intentionally with a common approach that is across DNA damage repair and HPV vaccines. I really want the audience to hear: What is the existent data from which we can learn? How do we work to expand upon these gains across various tumor types? What are those key opportunities to expand platforms, and they should include diagnostics and therapeutics across global populations? Overall, I think both of these sessions will help the audience to understand not only what present-day data is, but also learn where these fields are heading in the future. ASCO Daily News: Thank you. Well, the ASCO Voices session is a favorite of the ASCO Annual Meeting. The speakers this year from Nigeria, Ireland, Germany, and the United States will share personal stories focused on equity, global health, and innovation. I've had a chance to interview the speakers and their stories really capture the human spirit and convey a true desire to find innovative ways to improve the lives of patients and survivors. Is this session 1 of your favorites at the [ASCO] Annual Meeting? Dr. Jhanelle Gray: Absolutely. The ASCO Voices is truly a compelling session. It helps to highlight where we should focus in what can seem like a very busy meeting. It helps all of us, including health care professionals, industry partners, caregivers, to take that breath and recenter. Our focus is ultimately the patient, and these personal stories help to crosscut that oncology continuum. We have, of course, chosen those that helped to showcase and support the importance of the presidential theme. And you'll see that many of those have topics focused on issues that are most relevant to global health, innovation, and/or cancer equity. On today's show, D'Agostino discusses the Warburg effect, which explains the way in which cancer cells produce energy. Rather than through mitochondrial oxidative phosphorylation, cancer cells stay alive and proliferate using energy produced by the ancient fermentation pathway. This understanding points to a critical theory: cancer develops when there has been damage to the mitochondria in cells, limiting or altogether eliminating the production of energy through oxidative phosphorylation. There are many causes of cancer, including viruses, inflammation, irradiation, the aging process, and carcinogens, but what could be the underlying culprit among all causes is mitochondrial damage, which causes inflammation which further damages mitochondria. D'Agostino explains this process in depth, but also discusses a range of metabolic drugs being used to combat cancer, offering a less damaging form of cancer treatment than chemotherapy. In addition to discussing certain types of exercise and diets that can boost mitochondrial health, he talks about the combined use of radiation therapy and hyperbaric oxygen therapy. He explains that when hyperbaric oxygen and targeted radiation are used, only about five percent of the amount of radiation is necessary, which could provide huge benefits to patients and avoid the negative side effects of using radiation and/or chemotherapy alone. And so again, we have a really great conversation. The person that I'm really excited to talk to today and introduce to you all, Dr. Brian H. Williams. Dr. Brian H. Williams is a first-generation college graduate who earned a degree in Aeronautical Engineering from the United States Air Force Academy. After six years of active duty military service, he followed a different call to serve and enrolled into medical school at the University of South Florida Morsani College of Medicine. He did his general surgery residency at Harvard Medical School/Brigham and Women’s Hospital in Boston, Massachusetts and a fellowship in trauma surgery and surgical critical care at Emory University/Grady Memorial Hospital in Atlanta, Georgia. Upon completion of his training, Dr. Williams served on the faculty at UT Southwestern Medical Center in Dallas, Texas, where he taught and mentored students, residents, and fellows. Dr. Williams is well-known for his role in treating victims of the July 7, 2016, Dallas police shooting. He was the trauma surgeon working on the seven injured officers who were emergently transported to Parkland Memorial Hospital. At a press conference following the tragedy, his heartfelt comments about racism, gun violence and policing touched thousands. Unbeknownst to Dr. Williams, his impromptu speech became a viral media event, and his life of comfortable anonymity ended. In addition--Brian: That's a mouthful. [both laugh]Zach: It is, but it's real though. In addition to his work as a trauma surgeon, Dr. Williams travels the country as a thought-provoking speaker sharing his unique insight on resilience, gun violence, and racial justice. Dr. Williams, how are you doing?Brian: I'm doing fantastic, Zach. Thanks for having me on.Zach: Man, thank you for being here. So let's get into it. You were already known within your field, but you were thrust into the national spotlight after treating victims of the July 7th, 2016, Dallas police shooting. You were the trauma surgeon working, and so you were actively, right--like, I remember even in that video, you were--it was clear that you had just got done working. Like, you were--you were working. You know, I'm really curious. Considering your personal experience with police and the history of policing in black communities, what was going through your mind, like, just treating--like, in that situation? Can you talk about just what--of course there's no question as to your oath and your commitment to deliver care, but what I'm trying to understand--so, like, I want to be very upfront with that. What I'm trying to understand though is, considering your own experiences and your own identity, like, what was it like? Was it automatic? Was it just like, "Look, this is what I do?" Like, can you walk us through that experience?Brian: Sure. In that moment, when the officers were coming in, nothing else mattered. I just fell back on my training. So my experiences, my life experiences, that was not a factor in how I approached what I did, and, you know, it's a large team of nurses and doctors and students. So it wasn't just me, although I was the trauma surgeon that was on call that evening at the hospital. But in that moment, I'd give them the same sort of care I give any patient. Like, I do not differentiate based on occupations or race or ethnicity or--you know, all of those ways we try to categorize people as being different. That matters not to me. At the moment, I just saw a human being that was severely injured and critical, and I am trained to do things to try to save their lives. So that was what--like you said, it was automatic. It just was a crisis. My training kicked in, and I went to work.Zach: And so then talk to me a little bit about, like, so--you know, so after the care had been delivered and, you know, after you were done performing surgery and care, again, to the victims, you know, you had the conference. At what point did then, like, all of the emotions and thoughts and things come rushing back?Brian: Well, let me walk through the timeline of those few days. The shooting was on July 7th, 2016, but you may or may not recall that on July 5th, there was a shooting, Alton Sterling, in Baton Rouge, and on July 6th, that was Philando Castile in Minnesota. And then we had July 7th. So then on July 7th there were actually protests happening all around the country to bring awareness to this ongoing issue. People remember Dallas because of the tragic shooting that occurred there. This is happening all over the country. So I was aware, in those preceding couple of days, of those two deaths, and you could imagine that the public discourse was basically a screaming match about black lives matter and blue lives matter and all lives matter, and there's all this negative talk. So when I went to work on July 7th, I was aware of that but didn't expect this sort of tragedy to occur. A few days later, on July 11th, was when the press conference occurred that you referenced. So there was a couple days between the time of the shooting and the time of the press conference where I pretty much just cut myself off from society. I wasn't watching the news. I wasn't listening to the radio. I wasn't reading the paper. I just was in my own little bubble, because that night was--it's the worst night of my career. It's something I still think about to this day. It just really got to me for a number of reasons, but the big thing was that this was fueled by intolerance and hate and racism, and all of these elements that we don't discuss about in an honest, open manner fueled this event, and to lose any patient--but that happened on a night that was particularly volatile and unfortunately became historic for all of the wrong reasons. And going into the press conference, these were the things that I was thinking about - you know, what's going on in our country? What role am I playing to bring us together? Am I doing enough? What have I done with my life? There was just a mix of a lot of different emotions and thoughts which I didn't have the answers [to] or wasn't really able to process completely, which takes us to the press conference that you mentioned, and that all kind of spilled out in the moment without any plans or preparation. I just planned to sit there, just to be seen, because my wife felt that the country needed to see that there was a black surgeon there that night trying to do the right thing.Zach: You know, so let's talk a little bit about the conference, right? So, like, at the conference following the shooting, you said, quote, "I want the Dallas police officers to see me, a black man. I support you. I will defend you. I will care for you. That does not mean I do not fear you." Can we talk about, like, what you meant here? Like, what does that--and it's interesting, right, because it's almost--one could almost argue that those things are... like, there's a duality there, right? So, like, when you said this, what did you mean?Brian: Right, and that's exactly the word I was going to use. That's the duality that I think many black people in this country deal with. So to break it down into two parts, when I said "I support you," I'm a child of a military veteran. I have a lot of military veterans in my family. I went to the Air Force Academy. I was an Air Force officer. So I know what it means to wear a uniform, I know what it means to serve something greater than yourself. I know what it means to make sacrifices to serve a greater good. So although I'm not a police officer, that sort of ethos is not something that was foreign to me. Zach: And so, you know, because you took all of this--like, you took all of this, right? Like, so your fear, your frustration, your dedication as a public servant and as a consummate professional, and you mobilized that into an effective partnership to actualize change, right? And so here's my challenge though, right? My challenge is I can't look at any point in American history where police have done right by black people. So, like, just the historicity of policing in America for black bodies, and, like, not to mention, like, the pathological narratives that majority media propagates, as well as the institutional systems and laws that make holding police accountable so incredibly challenging. And so I'm really curious, 'cause I--I know that I'm not the only person who has these challenges. I don't doubt that, Dr. Williams, that some species of this has been on your mind at some point of time, and so I'm curious to know, like, with those things in mind, what was your journey to become, like, the chair of the Citizens Police Review Board, and can you explain what it meant to manage through those relationship dynamics?Brian: Yes, I'm on-board with what you're saying as far as the challenges, and I guess we'll get back to that in a second, but as far as the journey to the Citizens Police Review Board, that was--the mayor's office reached out to me about potentially joining the board as a chair, and that was because a prior chair was turning out, so he needed someone new. Now, the Citizens Police Review Board is meant to be this body that will hear complaints from citizens about their interactions with police, and they can bring them to the board, we can deliberate and try to give them some resolution. So that's what the board existed for at that time. I didn't know that the board existed when I was asked. I didn't know what it did. I didn't know if it was worthwhile. I didn't know if I had the time. I had all of these questions about the board, but I asked around and learned about it, and I said--I thought, "Yes, this can be something good for the community. It can be a voice for citizens," and I felt that I could make a positive contribution to all of this. It was definitely challenging. I learned a lot about the Dallas Police Department. I learned a lot about community activists. I learned a lot about various board members in City Hall. So there are a lot of stakeholders working towards public safety, and to bring them all together to come to some sort of collaborative effort to ensure that the public has trust in their police department is challenging, and I think it's actually now an ongoing journey now that the board has been revamped and given more support as far as resources, personnel and a budget, which we did not have when I was a chair. Zach: And so I'm curious though, right, like, when you talk about--it's just interesting, because I don't think that we have a lot of examples in American history when it comes to, like, relationships where the underrepresented voices have, like, actual authority over a majority group and things don't become strained, like, either quickly or over time, and I think authority and accountability is a struggle for anybody, right? So I don't think that that's unique or exclusive to dominant and sub-dominant groups, but I'm really curious about, like, what did it look like, especially--like you said, at the time that you were the chair there was not a budget. Like, what did it look like to really be the chair on this review board and talk about right behaviors? Like, do you feel like you were able to have honest dialogue? Do you believe that there was, like--do you believe that you had the actual authority to kind of, like, drive substantive change? Like, what did that look like for you?Brian: So I think that everyone involved knew exactly who they were getting with me as chair. For one, they saw my statements at that press conference. So I [?] there. Two, I wrote an opinion piece that published in the Dallas Morning News where--actually, I wrote two regarding the police, one that talked about the history of slave patrols and how this distrust in black communities goes back for hundreds of years. It just doesn't happen overnight. And I talked about, you know, police departments have historically been there to maintain control over communities of color. It wasn't about public safety or protecting their rights, it was about keeping communities of color in line. So that is the history with which we need to reckon in order to move forward. So everybody knew exactly what they were getting with Dr. Brian Williams, [laughs] which, you know, had its pluses and minuses. I think the benefit was they couldn't accuse me of having any kind of agenda, right? I was criticized from both law enforcement and, you know, black civilians for the comments I made. And I, you know, received praise as well. So I feel I was pretty much solidly in the middle of all of that that I could equally appeal to and offend anybody that was involved in moving the Police Review Board forward.Zach: So your journey didn't stop there, right? Like, what did you learn about yourself? Like, what were some of the main things that you learned about yourself that then prompted your transition from Dallas into the South Side of Chicago?Brian: Well, and I'll just say, you know, that last comment I was obviously kidding when I said offending people. [both laugh] But I guess the point there was I was moving forward with this mission to ensure a voice for the citizens of Dallas with integrity, and I did not try to have any sort of self-gain from it. It was about serving the city of Dallas and the people of Dallas.Zach: Which is rare, right? 'Cause I think, especially, like, in the political climate that we're in today, right, like, you see these voices, like, on the far right who--like, they're black voices. Like, they're tokens, like, coming and, like, sharing specific talking points and narratives without any, like, real intellectual substance behind them, and I think what really intrigued me about you--'cause I've spent a majority of my life in Dallas, and my mother is still in Dallas. I have family in Dallas, and so I was very familiar with--like, with your work and your statements, and they run very true to me and sensitive for me considering that I'm from that area, right? So what I'm curious about is did you ever feel any pressure to kind of, like, lean one way or the other or take on certain agendas or certain talking points that you yourself didn't agree with from a principle perspective?Brian: The short answer to that is yes, and I should say, you know, I wasn't immediately embraced by citizens that were working on police reform. [They didn't?] know who I was. You know, I was a new quantity, and people have been working on this reform for decades, right? These are Dallas natives that were born and bred here in Dallas, and I was--Zach: The activist culture is deep, right? There's a lot of community servants and activists who have been on the ground. So yeah, I'm right there with you. And it's hard. It's hard to break in, right? Like, when you're new and, like, the main thing you have when it comes to community activism--from what I understand, because I would not consider myself a community activist because I know that I want to respect that work, but what I understand is, like, really it's your relationships--your social capital is, like, gold, right? That's, like, the only thing you have, and if you're unknown, then it's hard to, like, you know, break the ice.Brian: Exactly. And I will say your podcast is a form of activism.Zach: I appreciate that. Thank you. Thank you, Dr. Williams, man. You got me blushing, man. [both laugh] Brian: You're doing it, man. You're doing it.Zach: Man, I really appreciate it. So let's talk about South Side, Chicago. Like, you transitioned, you went there. Like, what was the call or the impetus to transition from Dallas to Chicago?Brian: Well, my journey in health care--I mean, I've always been very mission-driven about what I can do to eradicate racial health care disparities, and that is a nation-wide mission, right? That can occur anywhere. Now, as a trauma surgeon, I'm particularly focused on gun violence as well. So South Side, Chicago, you know, there's a lot of gun violence here. It's frequently talked about in the media in ways that aren't--I think that dehumanizes the population that's there living within these violent communities. There was a new trauma center that opened up in the area, and several of my mentors were here at the trauma center, so there was this perfect storm of the mission that I want to serve with a community that was very active in getting this trauma center here built with people I know that had flocked here, and I said, "You know what? I would like to be a part of having impact that will cross generations," right? And I think it's happening right now, and that's why I wanted to join this group here. As far as Dallas, you know, that was not an easy decision. I had been in Dallas--I was in Dallas for 9 years. That's the longest I've been in one spot my whole life. It's now my de facto home. [laughs] If Texas will accept me, it's pretty much my de facto home. I've been moving my entire life as a military kid, as a military officer, you know? I feel home in Dallas. I still follow what's going on in Dallas. I'm interested in what's gonna happen to my home city. Zach: And so, you know, I'm interested, right, in addition to this you're a respected health care leader. Can we talk a little bit about how your work and the legacy of racism impacts health care inequity, right? Like, so you're coming in, and you're in Chicago, and yes, like, you're supporting--there's a gun violence issue in South Side, Chicago, and I--you know, honestly, I really do wish that some folks never found out about Chicago, because I feel as if it's, like, the default when anybody ever wants to pathologize black folks. It's very annoying. It's just like, "Golly, I wish that--anything Chicago, I just wish y'all wouldn't have known about it." But, you know, in your work, can you talk a little bit about, like, how health care inequity shows up, right? Like, that's been an ever-growing talking point or just point of awareness, right, like, in headlines and mainstream media, growing awareness around health care inequity for black and brown folks juxtaposed to majority members, white counterparts. Can we talk a little bit about, like, what you've seen from a perspective of inequity and kind of, like, how you've combated that as a black surgeon?Brian: I would like to see us get to a point where we just get real about what health care disparities are and health care inequities. This is the legacy of racism in this country. It's about health, poverty, housing, education, employment. Like, there are so many things to unpack and address. Health care is one part, and that's where I happen to be, you know, an expert in that particular field, but I recognize that what I do in the hospital is not gonna be enough to uplift these communities in need. And like you said, I don't like to pathologize Chicago either. I'm coming here to help, but I don't know how to talk about it without being offensive to people that live here, right? Like, who am I to talk about their community? So I'm trying to be sincere about my desire to contribute, to uplift the community without being offensive to the folks that live here and are actually gonna be doing the work for a long time. So I completely agree with you that even I feel like an outsider sometimes in doing this work.Zach: So then, you know, I think--and I'm really excited, and I'm thankful that you've been able to come on the podcast, because I think what really intrigued me about having you on, beyond you just sharing your story and the work that you've done and that you do, is around, like, the concept of effective relationships and building relationships with individuals that you may not feel immediately safe with or comfortable with, and I know that that involves a certain level of emotional labor for you, even today, right? I'm curious though, like, if you could give younger professionals any advice about building relationships--and when I say relationships, I'm thinking more like coalitions for your personal and professional development and journey. Like, if you could, like, boil it down to, like, three things, what would they be?Brian: I would say, first and foremost to young professionals, nothing is worth sacrificing your dignity for acceptance. What I mean is that the papers and the promotions and the titles, like, if you have to leave part of who you are at home, if you have to compromise your integrity and your ethics and your purpose to achieve those goals, those goals aren't worth it, so do not hand over your dignity for acceptance. That's one. Two, you need to set your boundaries. If you don't set your boundaries someone else will set them for you, and you may not like them. And actually I believe that if you set your boundaries, that will lead to greater connectedness with people, not less, because you are respecting who you are and what you stand for. You're not letting anyone else compromise that for you. And the third thing is just always keep your end goal in mind. As you're going through life, your profession, like, think about what it is you want to accomplish, where you want to be. If you never lose sight of that, then all that noise and chaos that you encounter on the way, you'll be able to filter through that and not lose sight of the end objective. So people call it your North Star, your purpose, but I think your end goal, whatever that is, never lose sight of that.Zach: Man, Dr. Williams, this has been a great conversation. Returning guest, Dominic D'Agostino, is a researcher and professor in the department of molecular pharmacology and physiology at the University of South Florida Morsani College of Medicine. On today's episode, he offers fascinating insight into a recent mission he completed in collaboration with NASA, which involved a number of experiments carried out under sea with the purpose of emulating activities that are essential to space missions. He explains how different space analogs under the sea can mimic neutral buoyancy and microgravity in space, the use of mini DNA analyzers to analyze the microbiome of the habitat and surface of astronauts' skin, the physiological effects of hyperbaric pressure which presents three times the amount of oxygen and nitrogen than the average person is accustomed to, a number of extravehicular activities (EVAs) to include the testing of astronaut rescue devices and drills for sampling rock or coral, the health impact and ways of dealing with extremely high concentrations of carbon dioxide, and decompression sickness. He also discusses what he considers the biggest challenge in space, which is living in confined habitats for long periods of time. Navigating these issues means addressing team cognition and psychology, as well as the practical concerns associated with performing a variety of scientific experiments and tasks in such small and enclosed spaces. Dr. Dominic D’Agostino returns to STEM-Talk to give Ken and Dawn an update on his research into ketogenic nutrition. Dom was the guest on episode 14 back in 2016 when ketogenic diets didn’t even show up on a list of the top-10 diets that people Googled. Since then, the search term “ketogenic diet” has risen to the top of the list. In today’s episode, Dom talks about his past 10 years of research into ketogenic diets and what he is learning about the physiological benefits of nutritional ketosis. Dom is tenured associate professor in the Department of Molecular Pharmacology and Physiology at the University of South Florida Morsani College of Medicine.He also is a research scientist here at IHMC.Throughout his career, Dom has been a researcher with a diverse background in neuroscience, molecular pharmacology, nutrition and physiology. Show notes: [00:02:55] Dawn begins the interview mentioning that when she and Ken started hosting STEM-Talk, the ketogenic diet wasn’t on the list of the top-10 most Googled diets of 2015. Today, however, Dawn points that ketogenic diet is number one on the list. She asks Dom if he foresaw sudden mass interest in a ketogenic diet coming. [00:04:12] Ken asks Dom for his thoughts on how the ketogenic diet has went from being very obscure to becoming a household term. [00:06:04] Ken comments on the evolutionary component of the ketogenic diet and how our ancestors must have gone in and out of ketosis based on the availability of food. He also comments on the unique aspect of the ketogenic diet, being that it has an objective measurement, and asks Dom to talk about that. [00:06:59] Dawn comments on the cynicism regarding the ketogenic diet, particularly from nutritionists. She asks Dom to address the criticism and pushback that the ketogenic diet receives from so many nutritionists. [00:10:02] Ken mentions that some fields are resistant to change and new science due to the emotion behind established theories. Dom agrees and then talks about how people, even doctors, are resistant to new data and new science. [00:11:13] Dom talks about the most common misconceptions and overrepresentations of the ketogenic diet. [00:12:54] Ken discusses his dissatisfaction with the term “ketogenic diet” since the word diet implies the mandated consumption of certain food items. He goes on to say that if one is in ketosis, then, by definition, they are doing a ketogenic diet, even though they may be in ketosis because they have been fasting and haven’t eaten anything.  Ken and Dom discuss how knowledge about ketogenic nutrition has changed over time and that it is certainly possible to eat an unhealthy ketogenic diet. [00:15:35] Dom and Ken talk about the results of a recent Megan Roberts paper, “A Ketogenic Diet Extends Longevity and Healthspan in Adult Mice,”that showed a 13% increase in the lifespan of the mice along with remarkably improved healthspan. [00:20:26] Dom shares his thoughts on the potential of exogenous ketones in the context of blood glucose regulation. [00:27:07] Ken asks if Dom has been tracking Virta Health, which was founded by Dr. Jeff Volek who was interviewed in STEM-Talk episode 43. Virta Health has been publishing impressive results of its trials that show  people reversing type-2 diabetes via a well-formulated ketogenic diet. [00:29:13] Ken adds that the reported numbers from Virta show 60% to 70% of their patients going off their insulin medication or greatly reducing their insulin levels. Andrew Koutnik, biomedical researcher, discusses cancer cachexia, wasting in general, the impact of inflammation within the body, and various other issues. Koutnik is a seasoned researcher. His notable and extensive work studying nutrition and metabolism and their combined impact on health, disease, and even performance has generated much interest in the scientific medical community. He is actively involved in biomedical research with the Metabolic Medicine Lab at University of South Florida Morsani College of Medicine. Koutnik provides an overview of cancer cachexia, which he describes as a multi-factorial, dynamic, systemic wasting syndrome. And surprisingly, Koutnik states that in spite of its long history, the condition currently has no standard of care. Cancer cachexia is often characterized by a loss of skeletal muscle mass, sometimes with fat loss, that cannot be completely reversed by conventional and standard nutritional support and can result in progressive and significant functional impairment. Koutnik explains anabolic signals and synthetic responses as well as inflammatory responses. As he states, inflammatory signals may be inhibiting the synthesis response in the muscle. Koutnik explains, by citing specific examples, the ways in which atrophy is initiated, and why it occurs. And with up to 20% of cancer patients dying of this disease, it's clear that advanced research is needed ongoing. The biomedical research expert discusses the process of fasting and how it affects the body. And he provides information on past research and recorded studies on ketone bodies. Koutnik explains why adipose tissue is not always used first as an energy source during fasting. He states that in a normal, healthy response to a fasting scenario or carbohydrate restriction, adipose tissue, over time, becomes a preferential fuel. He explains that while fat is a very important fuel that we also need to use intermediary metabolites. Simply defined, metabolites are the intermediate products of specific metabolic reactions that are catalyzed by select enzymes that naturally occur within the cells. Koutnik goes into detail on inflammation issues that relate to atrophy, as it is an underlying current in many of these cases though it may not always be rapid. He talks about his theories on how the body sees and understands subtle information it is receiving regarding atrophy issues. And ultimately, he states that from a physiological perspective the body will always do whatever it can to survive. Koutnik continues his discussion on ketones. He cites various studies that provided new and significant information on ketones and specifically, exogenous esters. He explains how antioxidant responses are related to ketone presence. He states that much of the work in this area has been implemented in rodent-based studies, and now the desire is to try to translate these studies to the human scenario, and more research is needed to understand the compatibility with previous studies.

Dr Dominic D'Agostino is a professor in molecular pharmacology and physiology at the University of South Florida Morsani College of Medicine as well as a Visiting Research Scientist at the Florida Institute for Human and Machine Cognition. Dr D'Agostino tells us how he found his way into the filed of ketosis. He combined interests in nutritional and neuro sciences.  From there, Dom developed a deep expertise in underwater medicine and oxygen toxicity seizures where he made the connection with ketones and the brain.  What is Nutrtional Ketosis  Ketones - more specifically ketone bodies- are chemical produced by our livers. Ketone bodies are produced by avoiding most carbohydrates in the diet (eg sugars, breads, fruits, and certain vegetables). A much higher percentage of healthy fats are eaten during a ketogenic diet.   When someone follows this diet the body begins to make and utilize ketones more so than on it's typical  energy source - glucose. Nutritional ketosis is the sustained elevation of ketones in the blood. To be clear, as long as someone isn't completely lacking insulin (eg Type 1 diabetic), this low-level of ketosis is NOT the dangerous state of ketoacidosis which develops from having a lack of insulin.  What are the benefits of Nutritional Ketosis Lower states of brain inflammation. It may turn out that this dietary approach is the most powerful intervention we could entertain, in terms of providing the most salubrious environment for the body and the brain! I’m going to tell you right up front, today’s interview is a bit scientifically intense, but nonetheless absolutely worthwhile. I had the great honor to speak with Dr. Dominic D’Agostino. He is is an Associate Professor in the Department of Molecular Pharmacology and Physiology at the University of South Florida Morsani College of Medicine. He is also a Research Scientist at the Institute for Human and Machine Cognition (IHMC). His laboratory develops and tests nutritional strategies and metabolic-based supplements for neurological disorders, cancer and enhancement of physical performance and resilience in extreme environments.  His research is supported by the Office of Naval Research (ONR), Department of Defense (DoD), private organizations and foundations. Our time together explored the mechanisms involved in terms of how ketosis is so powerfully beneficial for brain cells and brain health.
