ASX 200 trades quietly up 11 points at 7805 (+0.1%) after a great day yesterday. Banks stalled close to record highs, NAB fell 0.5%, with ANZ up 1.2% on broker upgrades, MQG unchanged with the Big Bank Basket up to $210.71 (+0.4%). Financials mixed as PPT cratered 7.1% after a strategic review presentation. REITs better, although GMG fell 0.3% on an update, SGP up 1.1% with industrials slightly firmer, REA up 0.8% with TCL up 1.4% and WOR pushing ahead 1.2%. Tech seeing a late surge, the All-Tech Index up 0.5%. Resources mixed, Iron ore miners fell as prices eased back. FMG down 0.5%, RIO off 1.2% and MIN off 0.7%. PLS up 0.2% in the lithium space but PMT continuing the run from yesterday's drill results. Golds firm but idle as DEG went to shareholders and instos to raise $600m. Oil and gas stocks firmed, WDS up 0.1% and STO up 1.1%. On the corporate front, PPT announced some big changes, a sale of its name and Rob Adams to retire after doing a great job. Not. Olam upped its bid in NAM as a two-horse race continues. QIP saw a competing bid from IPH up 5.4%, and QAN up 0.8% on record demand for premium seats. FLT, ABB, DGH, and JIN have all reaffirmed guidance. LLL announced a deal to sell the entire Malian mine to Ganfeng for US$342.7bn (43cps). On the economic front, plenty of RBA analysis. Simon Shares Wild week for Naspers (JSE code: NPN), Prosus (JSE code: PRX) as Tencent (Hong Kong code: 700) got slapped down by the Chinese government. We're back at level 3, adjusted. Alcohol sales are allowed and Distell (JSE code: DGH) put out another great trading update. No doubt better if no alcohol bans, but the argument that your industry is under threat wears thin after such an update. President Ramaphosa also announced the return of the pandemic grant to the unemployed at R350 a month until March 2022. This pays to some 9.5million South African and means just over R3billion a month into the economy. How do we pay for it? Dividend tax on these super dividends. Apple $21.7billion in profits, almost doubling the previous year. Alphabet nearly tripled its profits to $18.5billion as advertising revenue grew 69%. Microsoft profits hit $19.1B, up 42% from the previous year. The market capitalization of Amazon, Apple, Facebook, Alphabet, and Microsoft is now over 30% of the entire S&P 500 index and over 40% for the Nasdaq100. Tigerbrands (JSE code: TBS) are recalling 20million tins. Two new ETFs coming to market. Global healthcare from Sygnia and a diverse and inclusive ETF from Satrix. Upcoming events; 19 August ~ JSE Power Hour: BEE shares and the JSE Forget super cycle, hello super dividends Anglo Platinum (JSE code: AMS) paid an R175.00 total dividend. Kumba Iron Ore (JSE code: KIO) paid a 7270c. In both cases, this amounted to a dividend yield (DY) of around 10% at the interim stage. No share buybacks as Anglo American (JSE code: AGL) own a significant majority of the shares and likely want the dividend back at home. The question is will other miners be as generous or will they do some share buybacks as well? If commodity prices stay where they are, we'll see the same again in six months and into 2022? If prices remain the same to the end of 2022 (big if?) we'll get a 40% return from just dividends? This is what super profits look like.

Ep 190 - Adult Congenital Heart Disease in the ED: Part 1

This is the first in a two part podcast series discussing Adult Congenital Heart Disease (ACHD) and how these patients may present to the Emergency Department (ED). Dr Sam Fitzsimmons, our guest on the podcast, is a Consultant in Adult Congenital Heart Disease at University Hospital Southampton. There is more information in this blog post. Look out for Part 2, which will be released next week, where we discuss Eisenmenger Syndrome, Transposition of the Great Arteries and Coarctation of the Aorta.  Background With advances in paediatric cardiac surgery, more and more patients with complex congenital heart disease are surviving to adulthood: in the 1950s you might expect a survival rate of about 10%, whereas now this is more like 85%. In fact, there are more patients in the adult congenital heart disease population than there are in the paediatric one (with 2.3 million adults vs 1.9 million children in Europe). Many patients with Adult Congenital Heart Disease are young and able to live a relatively normal life. This means that they can travel and take part in just the same sort of activities as those without ACHD. They may well turn up in your Emergency Department one day, regardless of whether you are a tertiary centre or a district general hospital (DGH). They are experts, and know their disease well, but this does not abstain you from a responsibility to know about them too! When these patients become unwell, they can go downhill very fast and you may not have the chance to discuss with them their exact lesion and its management. The anatomy and physiology of these patients is abnormal, so they may present in atypical ways, and may not respond to usual medical interventions: in fact, some of our usual treatments may even be harmful. However, starting with our usual 'ABC' approach is by far the best way to go, whilst gathering more information and contacting their specialist centre. Many patients will have their last clinic letter and ECG with them (which will also have the direct dial number of their specialist). And if they, or their relative, say there is something wrong you must believe them and do all you can to make sure they are fully investigated.     The presence of scars may give you some clues as to the patient's underlying condition and previous surgical repairs. (BMJ 2016; 354: i3905)   A General Approach Do your usual ABC assessment. Pay particular attention to the respiratory rate - this should be normal. Give oxygen if they look unwell. They should have a 'normal' blood pressure - any hypotension should be taken as abnormal and investigated. The Fontan Circulation This is not a condition in itself, but in fact the resulting circulation after a series of operations that could've been performed due to a number of different underlying conditions:   Tricuspid Atresia Double Inlet Left Ventricle Atrio-ventricular Septal Defect – unbalanced Pulmonary Atresia Hypoplastic Left Heart Syndrome In essence these patients are born with a single functioning ventricle, that has to be utilised to supply the systemic side of the circulation, whilst the Fontan acts as a passive means of returning blood to the pulmonary circulation.   It was first devised in the early 1970s by Dr Francis Fontan, so the majority of patients with this are in their mid thirties and younger. Potential reasons for admission to the ED - Fontan circulation 1, Arrythmia As the patient is entirely dependent on their systemic ventricle to work optimally, any disturbance of the delivery into it is very poorly tolerated. Thus, any arrhythmia is life threatening, even a mild atrial tachycardia. These patients need to be returned to sinus rhythm as quickly as possible and the recommended method for this is DC cardioversion in expert hands. Fontan patients have an incredibly fragile circulation and any change in their respiratory physiology can be life threatening, especially if it increases their pulmonary pressures (and thus prevents the passive flow within the Fontan circulation). These patients are not candidates for sedation in the ED and should have an experienced anaesthetist to manage them during the procedure. Beware if the patient comes in and tells you they are fasted! This means they have been in this situation before and needed DC cardioversion. 2, Haemoptysis Over time the patient develops venous hypertension within the Fontan connection. This causes the formation of collateral vessels, that may link into the bronchial arterial tree. If the patient presents in shock treat them as you would any other patient with emergency blood transfusion. Any haemoptysis, however small, may herald the beginning of a massive bleed. These patients need further investigation, probably a CT chest with contrast. These vessels may then be coiled by interventional radiology. 3, Cyanosis If the patient has a non fenestrated Fontan they should have normal oxygen saturations. However, if there is a fenestration there will be shunting and therefore a reduction in oxygenation. For patients this is trade of between being pink or blue, each of which have complications. Dr Sam Fitzsimmons Dr Sam Fitzsimmons is a Consultant Cardiologist in Adult Congenital Heart Disease (ACHD) at the University Hospital Southampton, UK. Sam also subspecialises in pulmonary hypertension and maternal cardiology. Working within a tertiary surgical ACHD centre, Sam delivers an ACHD on call service for emergency admissions, inpatient care, routine outpatient follow-up, intra-operative imaging and post-surgical care, as well as specialist clinics in Pulmonary Hypertension and Maternal Cardiology. Sam holds a Honorary Senior Clinical Lecturer post with the University of Southampton as she is passionate about teaching and in particular, she is enthusiastic about helping demystify congenital heart disease for many non-specialist to improve patient care. Sam is well published in peer review journals, cardiology textbooks and specialist guidelines.

The most well known are the Naspers~N (JSE code: NPN) A shares that give control to a few parties. Shoprite* (JSE code: SHP) has the same for Christo Wiese which he tried to sell back to the company. But they have zero economic rights and only are votable by Wiese More recently it surprised many that while Remgro (JSE code: REM) owns a little over 31% of Distell (JSE code: DGH) they ave a pile of B shares that gives them 56% voting control. In the older days, companies used pyramid structures, but those have long since largely left the JSE and no new ones can be listed. The challenge is how to know about them? The annual report will detail them. Generally, they don't matter massively, until they do. And then they matter. Wiese failed to receive the required number of votes to get back onto the Shoprite board in 2018, but then he used his special shares to basically override shareholders. Celles et ceux mobilisés réclament l'arrêt immédiat des suspensions de plusieurs postes prévus par les autorités du secteur éducatif, et l'augmentation des heures qui serait une chose impossible à gérer par les enseignants. Pour se faire entendre, quelques enseignants de l'EREA de Pessac et le représentant des élèves, ont accepté de parler à la clé des ondes. Podcast complet à retrouver ci-dessus La Clé Des Ondes : pourquoi cette manifestation devant la cour de l'EREA (Etablissement régional d'enseignement adapté) de Pessac? Thomas Albarran, professeur spécialisé à l'EREA de Pessac : On s'est réuni sur une double revendication qui est contextuelle à notre établissement. Une grosse baisse des moyens pour reprendre un peu le jargon de l'éducation nationale, on nous a annoncé une baisse de la DGH, donc la baisse de la dotation globale horaire, qui signifie une baisse de moyens, de dotations horaires des professeurs, accompagné de l'augmentation d'heures supplémentaires qui sont imposées maintenant au collègues. Donc, on se battait sur ces revendications locales qui viennent également s'ajouter à un gros lot de revendications nationales sur l'éducation en ce moment, pour ne citer que ça. Mais il y a beaucoup d'attaques sur le service public en ce moment, comme la précarisation des statuts des AED (Assistants d'éducation, NDLR) et des AESH (Accompagnants des élèves en situation de handicap, NDLR), les inégalités dans les primes qui ont été annoncées autour du COVID notamment, vis à vis des collègues professeurs documentalistes CPE (Conseiller principal d'éducation, NDLR), le statut d'infirmier scolaire qui est fragilisé, beaucoup d'attaques du service public qui vient s'ajouter à notre situation contextuelle qui a déclenché ce mouvement à l'EREA (Etablissement régional d'enseignement adapté, NDLR) de Pessac. Est-ce que vos revendications ont été entendus par le gouvernement ? Marlène Mercier, professeur documentaliste à l'EREA de Pessac : Par le gouvernement, je ne pense pas encore. Par contre, par le rectorat, peut-être qu'ils vont commencer à l'entendre, donc on espère, on maintient la tension ! nom de la photo Comment les élèves vivent cette période ? Christelle Pichou, infirmière à l'EREA de Pessac : Ils sont assez fragilisés ! On a beaucoup d'élèves qui sont en situation de handicap ! Beaucoup d'élèves à prendre dans leur globalité ! Donc, c'est aussi pourquoi je soutiens mes collègues, Je suis inquiète aussi pour mon statut, parce que les infirmiers vont être sûrement sortis de l'éducation nationale et nous seront décentralisés dans le territorial, donc nous sommes assez inquiets. Nous avons peur que les infirmiers ne soient plus dans les établissements ! Et à l'EREA, s'il n'y a plus d'infirmiers, je pense que ça sera très problématique car il y a quand même une grosse grosse fragilité de la part de nos élèves. Zana Koné, tu es le représentant des élèves, qu'est ce que vous réclamez aux côtés du personnel ? Nous réclamons de l'aide, parce que lorsque je suis arrivé ça n'allait pas du tout ! Je ne maîtrisais pas très bien le français mais grâce à l'encouragement des professeurs, je me suis accroché, mais s'ils disent qu'ils vont diminuer les profs, je trouve que ça serait très compliqué parce que les professeurs n'auront plus beaucoup de temps pour nous apprendre des choses utiles. Nous demandons vraiment de l'aide ! Si les revendications ne sont pas entendues, quel est le risque pour tous ces élèves et apprenants qui sont dans les EREA ? Thomas Albarran : Le risque plus largement qu'on dénonce depuis pas mal d'années, c'est la mort programmée de l'enseignement spécialisé en France. Au début, quand on tenait ce genre de propos, beaucoup de gens pouvaient penser qu'on exagérait mais ça fait plusieurs années qu'on observe notamment, au niveau collège, avec les SEGPA (Section d'enseignement général et professionnel adapté, NDLR) qui sont un peu plus connus du grand public. La disparition des SEGPA a commencé à être programmée avec la disparition des sixièmes SEGPA et de fermetures de beaucoup de divisions. Egalement on voit aussi la baisse des moyens sur les établissements qui accueillent exclusivement des élèves en situation de handicap comme nos collègues des IME (Institut médico-éducatif) et des ITEP (Les Instituts Thérapeutiques, Educatifs et Pédagogiques), on voit que la réduction des moyens entraîne une réaction en chaîne. Nous, sur cet établissement, il y a 5 à 6 ans de ça, on avait une minorité d'élèves en situation de handicap. A force de fermer des places dans les structures avec encore plus de moyens que nous sur des handicaps plus graves, ça fait un petit peu les vases communicants. Moins il y a de places dans les IME et les ITEP, par exemple, plus on va avoir des élèves en situation de handicap, et aujourd'hui on se retrouve avec deux tiers d'élèves en situation de handicap sur l'EREA de Pessac, alors qu'on est seulement censé gérer la grande difficulté scolaire qui n'est pas le même type d'enseignement, et d'encadrement, or on voit bien l'enjeu derrière sur le service public. Une structure spécialisée comme un IME par exemple, c'est des enseignants, des éducateurs, des psychomotriciens, des psychiatres, donc le prix d'une journée dans un établissement spécialisé pour un élève coûte très cher à l'Etat, et c'est aussi ce qu'on dénonce, c'est le fait de faire disparaître ces places pour faire des économies, mais des économies à quel prix ? nom de la photo C'est à dire que ces enfants là, s'ils n'ont pas cette prise en charge là, que vont ils devenir ? C'est bien là qu'on pose cette lutte comme une lutte collective et au nom aussi des élèves et non pas seulement une lutte corporatiste pour nous. Si ces établissements ont des moyens qui diminuent ou disparaissent, c'est bien la prise en charge de ces élèves qui va devenir inexistante, qui va poser un gros problème parce qu'on pourrait pour la partie des titulaires dire égoïstement : "nous on est fonctionnaires... que demain il y ait des moyens ou pas de moyens on aura toujours notre travail..." Mais c'est bien pour ça aussi qu'on lutte, c'est que, pour nous, il faudra que ces élèves soient pris en charge. Que ce soit la grande difficulté sociale ou les situations de handicap, il faut bien aussi se dire qu'il y a pleins de gens qui pensent qu'ils sont éloignés de ça mais ça peut toucher tout le monde. N'importe qui de n'importe quelle classe sociale peut avoir un enfant demain et se retrouver par la malchance de la vie avec un enfant en situation de handicap, et c'est des sujets qui concernent vraiment tout le monde. C'est ce pourquoi on se bat, c'est que pour n'importe quel enfant quelque soit sa difficulté en situation de handicap puisse avoir une scolarité dans le service public en France. Comment qualifiez-vous ce manque de réactivité des décideurs ? Marlène MERCIER : Manque de réactivité ? Je crois que les décideurs, ils le programment ! C'est pas un manque de réactivité ! Mais ils sont des comptables à très court terme. Ça serait bien de rallonger le terme parce que comptable à court terme, on y perd sur toute la ligne. nom de la photo On va créer des situations de tensions, des situations de faiblesses, des situations de dangers, des situations qui sont invivables pour ces jeunes. Sans structures, y' a qu'à regarder certains pays ou y'a pas de structures qui accueillent ces jeunes qui se retrouvent à la rue sans rien. C'est pas possible, c'est pas vivable ! En France, 5ème ou 6ème pays le plus riche au monde au 21ème siècle, en 2021, non ! Moi je dis non ! C'est pas possible, je ne peux pas laisser faire ça ! Thomas Albarran : Et puis de toute façon, cette question des coûts, c'est une fausse question dans le sens ou, des personnes en grande difficulté d' un moment à un autre, il y a de multiples études qui l'ont déjà montré, elles vont coûter à la société d'une manière différente. Est ce qu'on met des moyens dans la santé, dans le médico-social et l'éducation et l'insertion professionnelle pour que ces jeunes aient un avenir ? Ou l'argent qu'on ne mettra pas là, on le mettra ailleurs, autrement dit dans la police, dans le carcéral, dans l'hôpital psychiatrique si nous. Si on ne réussit pas notre travail, qu'on a pas les moyens de les réussir, on va créer d'autres problèmes ailleurs, et on va créer d'autres dépenses ailleurs. Or personnellement, en tant que contribuable, je préfère largement financer une éducation de qualité, que de financer toutes les parties qui vont devoir rattraper les choses que l'éducation n'a pas pu mener à bien avant. Discussion of the Scream trilogy can be found here.Show Synopsis:Scream is a 1996 smart satire of the horror film genre, directed by Wes Craven and written by Kevin Williamson. The film revitalized the slasher film genre in the mid 1990’s, similar to what Halloween (1978) did for the 1970s, by using a standard concept (teens being brutally killed off being one example) with a tongue-in-cheek approach that successfully combined straightforward scares with dialogue that satirized slasher film conventions.
