Abstract: Endometritis seriously affects women's normal life and work. It has been found that microRNA-123-3p (miR-124-3p) expression is abnormally high expression in the patients of chronic endometritis. However, the underlying mechanism for miR-124-3p regulation of endometritis development remains unclear. In our study, we treated human endometrial epithelial cells (HEECs) with LPS to simulate endometrial injury in vitro. Then, HEEC was treated with miR-124-3p mimics and miR-124-3p inhibitor. Next, exosomes were separated from bone marrow-derived mesenchymal stem cells (BMSCs). In addition, BMSCs were co-cultured with HEEC. Later on, dual-luciferase reporter assay was carried out to validate the regulation between miR-124-3p and DUSP6. Results indicated that LPS inhibited the viability of HEEC in time and dose dependent manner. MiR-124-3p inhibitor reversed apoptosis and viability inhibition of HEEC which were induced by LPS. In addition, we also found exosomes could transfer miR-124-3p from BMSCs to HEEC. Besides, BMSCs/anti-miR-124-3p Exo was observed to abolish LPS-induced viability and proliferation inhibition of HEEC by inducing the apoptosis of HEEC. Moreover, BMSCs/anti-miR-124-3p Exo alleviated inflammation of HEEC induced by LPS via upregulating DUSP6 and downregulating p-p65 and p-ERK. Furthermore, BMSCs/anti-miR-124-3p Exo protected against LPS-induced endometritis in vivo by upregulating DUSP6 and downregulating p-p65 and p-ERK. In conclusion, we found that BMSCs/anti-miR-124-3p Exo might be a promising new alternative to treat endometritis. Copy rights belong to original authors. Today we are talking about Endometritis, a silent offender. Endometritis is inflammation of the lining of the uterus, mostly caused by an infection. It can be due to chlamydia, gonorrhea, tuberculosis, or a mix of normal vaginal bacteria. The reason Endometritis has earned its title as the silent offender, is because if often flies under the radar. What you don't know is there, you can't address. Both acute and chronic endometritis are implicated in unexplained infertility and recurrent miscarriage. I see it most often in failed implantation and chemical pregnancy. Chronic endometritis is difficult to catch and can be easily overlooked, since its symptoms can be extremely subtle. Many patients I have had don't even realize they had it until they got tested.Chronic endometritis causes a persistent low-grade inflammatory response which is largely asymptomatic but generates an unfriendly environment for a pregnancy to implant and thrive in. If you listen to my other podcasts, you'll hear me talk about this low-grade inflammatory response and how it wreaks havoc in our bodies. While the research tells us that endometritis can be pesky for fertility and may contribute to failed implantation, what many people don't know is that it is actually more likely to occur after miscarriage or childbirth, it is also more common after a long labor or C-section.A study by Kimura et al (2019) reported chronic endometritis was present in 2.8-56.8% of infertile women, 14-67.5% of women with recurrent implantation failure, and 9.3-67.6% of women with recurrent pregnancy loss.  But how can you find out if you have it and get help? When the infection does show signs, they typically consist of pelvic discomfort, abnormal vaginal bleeding and/or an unusual vaginal discharge. In the vast majority of cases though, chronic endometritis is a silent condition. Tune into the episode to learn the most reliable diagnostic procedures, the natural medicine approach to take if you have it, and how you can help prevent it.

I'm already on a 2nd repeat course of antibiotics after 2nd biopsy still came back positive for CD 138 stromal cells.  My MD is suggesting to go straight into a transfer after second round of antibiotics without an additional biopsy. I'm afraid I may have an antibiotic resistant form and am wondering if you ever perform endometrial scarring or D&C to treat it?  Would you recommend to have a confirmation that the endometritis is gone before implanting? Also I am already on depot lupron 2nd month for endometriosis. Is it a concern that I will not get a period that would shed the bad cells somehow before a transfer? Ziel der vorliegenden Studie war die quantitative und qualitative Bestimmung der Gesamtfettsäuren in bovinen Serumproben sowie die Darstellung der Konzentrationsverläufe der einzelnen Fettsäuren während des peripartalen Zeitraums. Des Weiteren sollte festgestellt werden, ob zwischen Kühen, die postpartal an einer Gebärmutterentzündung erkranken, und gesunden Tieren Unterschiede in den Konzentrationen einzelner Fettsäuren bzw. Fettsäuregruppen vorliegen. Die in dieser Studie verwendeten Serumproben stammen von 50 Kühen eines Milcherzeugerbetriebes in Brandenburg. Von den ausgewählten Tieren zeigten 25 Tiere während des Untersuchungszeitraums Symptome einer klinischen Gebärmutterentzündung (puerperale Metritis, klinische Metritis, klinische Endometritis oder Pyometra) und wurden demnach als uteruskrank eingestuft. Die anderen 25 Tiere wiesen keine gesundheitlichen Störungen auf und wurden als uterusgesund eingestuft. Von jedem Tier wurden während des Zeitraums 17 Tage vor bis 28 Tage nach der Kalbung sechs bzw. sieben Serumproben gewonnen. Für die quantitative und qualitative Analyse der Gesamtfettsäuren in den Serumproben wurden die Fettsäuren zu Fettsäuremethylestern (FAME) derivatisiert. Die Probenanalyse wurde mit einem Gaschromatographie/Massenspektrometrie-System (GC/MS-System) durchgeführt. Außerdem wurden die Konzentrationen der freien Fettsäuren (NEFA) in den Serumproben mittels Enzym-Test bestimmt. Sowohl vor wie auch nach der Kalbung konnten bei verschiedenen Fettsäuren Konzentrationsunterschiede zwischen uterusgesunden und uteruskranken Tieren festgestellt werden. Die Konzentrationen der gesättigten Fettsäuren Octadecansäure (C18:0) und Tetradecansäure (C14:0) zeigten in beiden Tiergruppen nach der Kalbung einen leichten Anstieg, wobei die Werte der gesunden Kühe ab dem elften Tag p.p. signifikant höher lagen als bei erkrankten Kühen (P

Ziel der vorliegenden Untersuchungen war es, Zusammenhänge zwischen peripartalen Veränderungen von stoffwechselrelevanten Blutserum-, Milch- und Körperkonditionsparametern einerseits und Erkrankungen der Gebärmutter andererseits zu untersuchen. Weiterhin sollte geprüft werden, ob es möglich ist, für die Parameter, die Veränderungen aufweisen, Grenzwerte zu ermitteln, mit deren Hilfe ein erhöhtes Erkrankungsrisiko erkannt werden kann. 105 Tiere eines Milcherzeugerbetriebes wurden über einen Zeitraum von 2 Wochen vor bis 4 Wochen nach der Kalbung parameterabhängig in der Woche 2 a.p. und 1 a.p. jeweils einmal und an den Tagen 1, 4, 11, 21 und 28 p.p. untersucht. Dabei wurden Parameter der gynäkologischen Untersuchung (Uterusgröße und -tonisierung, Zervixgröße, Funktionsgebilde der Ovarien, Ausfluss), der Konditionsbeurteilung (Body Condition Score [BCS] und dessen Veränderung), klinisch-chemische Parameter (Bilirubin, β-Hydroxy-Buttersäure [β-HBA], Gesamteiweiß [GE], Albumin, Aspartat-Aminotransferase [AST], γ-Glutamyltransferase [γ-GT] und Glutamatdehydroxygenase [GLDH]), Parameter der Milch (Fett, Eiweiß, Laktose, FEQ, FLQ, Milchmenge) sowie das Auftreten von Gebärmutterentzündungen ermittelt. Für die Auswertung wurden die Tiere in 2 Gruppen eingeteilt: „uteruskrank“ (Tiere mit puerperaler Metritis, klinischer Metritis, klinischer Endometritis und Pyometra) und „uterusgesund“ (alle anderen Tiere). Für Blutparameter, für die signifikante Unterschiede zwischen den beiden Gruppen festgestellt wurden, wurden Grenzwerte (GW) ermittelt, um Kühe mit erhöhtem Risiko für die untersuchten Gebärmuttererkrankungen identifizieren zu können. Die GW der verschiedenen Parameter wurden zu den unterschiedlichen Untersuchungszeitpunkten mittels Sensitivität, Spezifität und Odds Ratio verglichen. Kühe mit Entzündungen der Gebärmutter wiesen höhere Bilirubin- (Tag 4 p.p.), β- HBA- (Tag 1), Albumin- (Tag 1) und AST-Werte (Tag 11 p.p.) sowie niedrigere GE- (Tag 11 p.p.), Albumin- (Tag 11 p.p.) und GLDH- Werte (Tag 4 p.p.), geringere Eiweißgehalte im Anfangsgemelk und im berechneten Durchschnittsgemelk (Tag 4 p.p.), geringere Laktosegehalte im Endgemelk sowie im berechneten Durchschnittsgemelk (Tag 4 p.p.) und eine geringere Milchmenge bis zum 28. Tag auf. Auch die Rast- und Güstzeiten waren bei Kühen mit Gebärmutterentzündung verlängert. Für die Parameter Bilirubin, β- HBA, Albumin und AST wurden folgende Grenzwerte berechnet: Bilirubin am Tag 4 p.p. ≥ 7,05 µmol/l (Sensitivität (Sens.) 53%, Spezifität (Spez.) 83%, Odds ratio 5,7), β- HBA an Tag 1 p.p.≥ 0,68 mmol/l (Sens. 53%, Spez. 79%, Odds ratio 4,4), Albumin an Tag 1 p.p.≥ 34,35 g/l (Sens. 50%, Spez. 72% Odds ratio 2,7) sowie an Tag 11 p.p. < 31,85 g/l (Sens. 81%, Spez. 79%, Odds ratio 16,8), AST an Tag 1 p.p. ≥ 80,8 U/l (Sens. 41%, Spez. 86%, Odds ratio 4,3) und an Tag 11 p.p. ≥ 117,8 U/l (Sens. 47%, Spez. 84%, Odds ratio 4,5). Abschließend ist festzustellen, dass einige der berücksichtigten Parameter (Bilirubin, β-HBA, Albumin und AST) für das frühzeitige Erkennen eines stoffwechselbedingt erhöhten Risikos für das Auftreten von Gebärmutterentzündungen genutzt werden können.

Ziel der vorliegenden Untersuchung ist es, Beziehungen zwischen verschiedenen Stoffwechselparametern und verschiedenen Störungen der Fertilität (verzögerter Zyklusbeginn, Brunsteinleitung aufgrund von nicht einsetzendem Zyklus, Ovarialzysten, verzögerte Uterusrückbildung, Erkrankungen des Uterus, positiver Zervixtupferbefund und verlängerte Therapiedauer) bei Milchkühen festzustellen sowie Grenzwerte für Erkrankungsvorhersagen und damit für die Erkennung eines erhöhten „Krankheitsrisikos“ der einzelnen Fertilitätsstörungen zu ermitteln. 51 Tiere eines Milcherzeugerbetriebes wurden über einen Untersuchungszeitraum von zwei Wochen a.p. bis 14 Wochen p.p. parameterabhängig im Abstand von einer bis zwei Wochen untersucht. Dabei wurden Parameter der gynäkologischen Untersuchung (Uterusdurchmesser, Zervixdurchmesser und deren Veränderungen), des Energiestoffwechsels (Energiebilanz, Energiebedarf und Energieaufnahme), der individuellen Futteraufnahme (Trockenmasseaufnahme), der Konditionsbeurteilung (Body-Condition-Score [BCS], Rückenfettdicke [RFD], Muskeldicke [MD] und deren Veränderung), klinisch-chemische Parameter (β-Carotin, Bilirubin, Aspartat-Amino-Transferase [AST], Creatinkinase [CK], Glutamat-Dehydrogenase [GLDH], γ-Glutamyl-Transferase [GGT], Alkalische Phosphatase [AP], β-Hydroxy-Buttersäure [BHB], Freie Fettsäuren [FFS] und Insulin-like-growth-factor-I [IGF-I]), Parameter der Milch (Milchleistung und Milchinhaltsstoffe Laktose, Harnstoff, Fett und Eiweiß) sowie die Häufigkeit von Erkrankungen ermittelt. Für die Auswertung der Daten wurden die Versuchtiere in folgende Gruppen eingeteilt: Zyklusbeginn (≤2 Wochen / >2 Wochen), Brunsteinleitung aufgrund von nicht einsetzendem Zyklus (ja / nein), Ovarialzysten (ja / nein), Uterusrückbildung (≤3 Wochen / >3 Wochen), Uteruserkrankung (o.b.B+GKI+GKII / GKIII+GKIV), BU-Zervixtupferbefund (negativ / positiv) und Dauer der Endometritistherapie (1 Woche / 2 Wochen / ≥ 3 Wochen). Für die Prognoseverfahren der verschiedenen Fertilitätsstörungen wurden für die einzelnen Parameter zu den Zeitpunkten mit signifikanten Unterschieden Grenzwerte gebildet. Mittels Sensitivität und Spezifität wurden die Grenzwerte der jeweiligen Prognosen miteinander verglichen. Kühe mit einem Zyklusbeginn >2 Wochen wiesen höhere Bilirubin- (Wochen -2 bis 6), höhere GLDH- (Wochen -2 bis 8), höhere Milchharnstoff-Konzentrationen (Wochen -2 bis 5), niedrigere AP-Konzentrationen (bei Tieren älter als drei Jahren; Wochen -2 bis 12), geringere Milchleistung (alle Wochen), geringeren Energiebedarf sowie geringere Energie- und Trockenmasseaufnahme (alle Wochen) als Kühe mit einem Zyklusbeginn ≤2 Wochen auf. Kühe mit Brunsteinleitung wiesen höhere AP-Konzentrationen (bei Tieren älter als drei Jahren; alle Wochen), niedrigere AST-Konzentrationen sowie kleinere Zervixdurchmesser (Wochen 0 und 1) als Kühe ohne Brunsteinleitung auf. Kühe mit Ovarialzysten wiesen höhere AST-Konzentrationen (Wochen -2 bis 7), höhere FLQ- (Wochen 0 bis 5) und niedrigere FEQ-Werte (Wochen 1 bis 8), größere Durchmesser im rechten Uterushorn (Wochen 2 bis 6) und in der Zervix (Wochen 0 bis 5) sowie niedrigere Milchlaktosegehalte (alle Wochen) als Kühe ohne Ovarialzysten auf. Eine Retentio secundinarum war bei Kühen mit Ovarialzysten 2,6-mal häufiger festgestellt worden als bei deren Gruppenpartnern. Kühe mit Uterusrückbildung >3 Wochen wiesen höhere BHB-Konzentrationen (Wochen 2 bis 14), niedrigere Milchlaktosegehalte (alle Wochen), höhere FFS-Konzentrationen (alle Wochen) sowie weniger ausgeprägte negative Energiebilanzen (Wochen 2 bis 12) als Kühe mit Uterusrückbildung ≤3 Wochen auf. Kühe mit purulenter Endometritis wiesen höhere Bilirubin-Konzentrationen (Wochen -1 bis 6), höhere FLQ-Werte (Wochen 0 bis 6), größere Veränderungen des BCS (3. Messung) und der RFD (3. Messung) sowie niedrigere Milchlaktosegehalte (alle Wochen) als Kühe ohne purulente Endometritis auf. Kühe mit positivem BU-Zervixtupferbefund wiesen höhere Bilirubin-Konzentrationen (Wochen -2 bis 1), höhere FLQ-Werte, größere Durchmesser im linken Uterushorn (Wochen 2 bis 7) und in der Zervix (Wochen 1 bis 8), niedrigere Milchlaktosewerte (alle Wochen) sowie niedrigere IGF-I-Konzentrationen (alle Wochen) als Kühe mit negativem BU-Zervixtupferbefund auf. Die Kühe mit Endometritistherapie über 3 oder mehr Wochen wiesen niedrigere BHB-Konzentrationen (alle Wochen) sowie geringere Energie- und Trockenmasseaufnahmen (alle Wochen) als Tiere mit Endometritistherapie 2 Wochen“ wurden folgende Ergebnisse festgestellt: Der Grenzwert für den Energiebedarf betrug in Woche 1

Die Bakterielle Vaginose stellt in der gynäkologischen Infektiologie nach wie vor eine der häufigsten Erkrankungen der Vagina dar. Die auftretenden Symptome wie gräulich homogener Fluor vaginalis oft verbunden mit fischartigem Geruch sind für die Patientinnen sehr belastend und werden nicht selten aufgrund fehlender Diagnosestellung nicht gezielt behandelt. Bis jetzt konnten die der Bakteriellen Vaginose zugrunde liegenden Ursachen noch nicht hinreichend geklärt werden. Um die immunmodulatorischen Effekte zu untersuchen, die mit den Veränderungen der vaginalen Mikroflora im Sinne der Bakteriellen Vaginose assoziiert sind, und in Hinblick auf mögliche neue sowohl therapeutische als auch diagnostische Ansätze, bestimmten wir bei insgesamt 81 nicht schwangeren Frauen die pro-inflammatorischen Zytokine Interleukin-6 und IL- 12 und das anti-inflammatorische Zytokin IL-10 im Vaginalsekret mittels Elisa. Wir untersuchten 45 Patientinnen im Durchschnittsalter von 30,0 Jahren mit der klinischen Diagnose Bakterielle Vaginose und 36 asymptomatische Frauen im Durchschnittsalter von 33,7 Jahren. Interleukin-6 und IL-12 werden über die Immunantwort vom TH1-Typ vermittelt und zählen zu den pro-inflammatorischen Zytokinen, während Interleukin-10 hingegen ein TH2-vermittelter anti-inflammatorischer Immunmediator ist. Ziel war die Beurteilung der Rolle der zellvermittelten Immunität beim Krankheitsbild der BV. Die Auswertungen unserer Messergebnisse zeigten weder für die proinflammatorischen Zytokine IL-6 (p = 0,782807) und IL-12 (p = 0,671506) noch für das anti-inflammatorische Zytokin IL-10 (p = 0,389744) einen signifikanten Unterschied im Vergleich zur Kontrollgruppe. Die für die BV-typischen diagnostischen Marker, wie beispielsweise der pH-Wert, waren statistisch signifikant (p < 0,0001) verändert. Für die Aminprobe ließen sich in der Patientengruppe 42 / 45 positive Ergebnisse finden, während in der Kontrollgruppe 0 / 36 positiv waren. Anhand unserer Ergebnisse scheint die lokale Immunität zumindest in Bezug auf die von uns bestimmten Zytokine bei der Bakteriellen Vaginose eine untergeordnete Rolle zu spielen. Demzufolge lassen sich kaum Anhaltspunkte erkennen, aufgrund derer neue therapeutische Ansätze im Sinne einer Immuntherapie als effektiv zu sehen wären. So gilt eine Behandlung mit Metronidazol oder Clindamycin weiterhin als Mittel der Wahl, eine begleitende Therapie mit Laktobazillen zur Stabilisierung der Mikroflora wäre zu überlegen. Bei Frauen, die an BV erkrankt sind, kommt es vermehrt zu Komplikationen sowohl während der Schwangerschaft, wie ein Spontanabort oder eine Frühgeburt, als auch bei nicht schwangeren Frauen, wie eine Adnexitis oder Endometritis.
