Just how well served or poorly served are we by the engineering and the design of our roads? Residents of Waiuku are reeling after a crash yesterday that has left an adult and two children dead. Some locals say they've been calling for safety measures to be introduced on Masters Rd, known as ‘roller coaster road', for years. One poor man who was first on the scene yesterday lost his best mate on the same road two years ago and says he fears for his life every time he turns out of his driveway. He said if they could start focusing those speed cameras on actual known death trap sites, like Masters Rd, instead of on random corners or at the bottom of pretty safe downhills where they're just revenue gathering —please could they put those cameras where it would save people's lives instead of just collecting the tax?— “that would be a great start”. We need them on this road. He described the area where the crash occurred as being akin to an old school BMX jump. I received an email a couple of days ago from a woman whose friend had his house destroyed in May by a speeding driver in a stolen car. She said she'd spoken at a Whangarei Council meeting to plead for barriers to stop this happening again, but they've refused to fund it, despite the fact that 10 years ago the same house was totalled and had to be rebuilt because the same thing had happened. And pleas from that owner to put barriers were ignored then too. Franklin's Whitford-Maraetai Rd has seen crashes every month. Locals say it's a regular traumatic experience to drive the roads, despite efforts from Auckland Council to improve it. There's been road widening along parts of the stretch, resurfacing of some of the bends in asphalt, and it's reduced the number of accidents there, but locals say they still expect to hear that awful sound of metal crunching every time there's rain. What is a dangerous road? It's very subjective, but one indicator is a high number of fatal or serious crashes over an extended period of time. If you look at Old North Road in Waimauku, from 2014-2019 there were 13 deaths and serious injuries in five years. State Highway 2 from Katikati to Tauranga, one of the highest risk roads in the Bay of Plenty: between 2009-2018, 27 people were killed and 77 seriously injured. In response to that, speed limits were lowered in a number of places, and then flexible wire rope median barriers were put in on sections of State Highway 2 South of Katikati. But you'll remember we've had people phoning in about those because they say it's impossible to turn into side streets – it makes residents lives a misery having to drive for kilometres before they can turn around. So yes, it might stop cars from crashing into one another on a dangerous stretch of road, but it also means risky behaviour from those who are looking to avoid travelling many kilometres just to turn around and come back again. State Highway 1 from Kawakawa to Springs Flat, Northland: 14 deaths, 41 serious injuries from 2012-2016. There is the factor of people not wearing a seat belt, that causes deaths, where they've been drinking or drug impaired, of course that comes into play, but at the same time, there are stretches of road that New Zealanders drive that are completely and utterly unforgiving. You make one small mistake, and humans do, a moment of distraction, one small mistake, and the consequences are absolutely devastating because the roads are unforgiving. Many of them are still the goat tracks that they once were. Just had a bit of metal put on them and call them a highway. At what point do you get a road engineered? And again, it probably comes back to the resource management and the RMA and the problems we have with getting permission to reconfigure roads around the country. You would think though, in the case of the woman who emailed me, that putting a barrier up on the corner to stop a car leaving the road and barrelling into a house for the third time - surely a barrier fence wouldn't be a huge cost to ratepayers of Whangarei? The Army UH-60M Black Hawk helicopter that fatally collided with an American Airlines passenger plane Wednesday night over the Potomac River was not equipped with experimental autonomous flight capabilities, defense officials familiar with the ongoing federal investigation told DefenseScoop. There's said to be no survivors in the aftermath of the tragic crash, which happened around 9:00 p.m. local time on a notoriously highly-congested flight path in the National Capital Region. The Army is closely supporting the U.S. National Transportation Safety Board-led investigation into the incident, officials said. In statements, press briefings and one-on-one conversations Thursday, several defense officials shed new light on the mid-air collision. Their comments confirm that — despite the Army's unfolding experimentation with AI and autonomous software — the helicopter involved was not equipped with or deploying any such systems. More than 200 state and local governments are using a new, nonpartisan talent program called Civic Match that's connecting outgoing federal workers and campaign staff with new government jobs. The Civic Match program, which was launched in November by the workforce nonprofit Work for America, is a talent matching platform designed to help federal employees remain in public service, but at the state and local level. More than 1,200 outgoing federal workers and campaign staffers from both Democratic and Republican campaigns have signed up to use the platform, and 220 state and local government hiring managers are reviewing those candidates for placement in government, according to program organizers. The Wayne County Prosecutor's Office announced on Tuesday, Dec. 10, that the mother, whose son was 16 years old at the time of the crash, will not be charged in connection to the November 2023 crash. Kiernan Tague, who was 16 at the time of the crash, was charged in March with second-degree murder for the Nov. 13, 2023, crash that claimed the life of this 18-year-old friend, Flynn Mackrell. Police say Tague was driving over 100 miles per hour on a residential street in Grosse Pointe Farms where the speed limit was 25 mph when he lost control and struck a utility pole and tree. Mackrell's mother, Anne Vanker, requested the teen driver's mother be charged, citing the case of convicted Oxford High School shooter Ethan Crumbley and his parents, who were the first parents in United States history to be held responsible for a school shooting committed by their child. But Wayne County Prosecutor Kym Worthy said on Dec. 10 that a panel of assistant prosecutors determined the mother “was not directly or indirectly involved in any way in the offenses for which we have charged her son.” “Parenting is hard in the best of circumstances,” Worthy said in a statement. “Most parents do everything they can to mentor their children and steer them in the right direction. The juvenile respondent's mother was no different. She took consistent, active steps to try to make sure that he stayed on the right path.” "There will be those that say she should have done more, but we have looked at all of the facts and those facts dictate no criminal charges here," Worthy said. "We will not be more specific about her actions because her son will be facing trial in February. (The defendant's mother) was not directly or indirectly involved in any way in the offenses for which we have charged her son. Despite his mother's attempts, it is our firm position that the respondent and the respondent only is directly responsible for the crimes that we have adult designated and charged him with." MacKrell's mother Anne Vanker told The News she thought there was enough evidence to prove the driver's mother knew he was reckless because a Grosse Pointe Farms police report showed the woman had monitored her son's driving habits via the Life 360 app that allows parents to view their kids' driving speed, hard braking and other activity. Mackrell's mother previously told the paper that she believed the teen driver's parents could have done something to prevent the crash that killed her son. MacKrell's parents said he was picked up by a 16-year-old friend who was driving his mother's white 2021 BMW X3. They were going to surprise another friend in Grosse Pointe Farms. Afterward, the driver sped over 100 mph while heading southwest on Ridge Road, officials said. That stretch of the road has a 25 mph speed limit. He lost control of the SUV, struck a utility pole, went airborne, and then hit a tree, witnesses said. They reported that the SUV was moving “at a very high rate of speed.” “The vehicle was traveling at such a high rate of speed that the engine, transmission, front axle, and passenger compartment all separated from each other and scattered all over the roadway,” the police report says. Vanker called the announcement "a weak decision." "The wounds never close, and I'm disappointed," Vanker said. "When your child is skipping school or smoking pot, a parent maybe can't control that. But she knew her son was driving more than 100 mph because of the Life 360 app; that was right in the police report. She didn't try to control his behavior. And she provided him with the murder weapon, which was the car, knowing he was an aggressive driver. "I know Kym Worthy does the best she can, and her office gets a lot of cases," Vanker said. "But this was a missed opportunity. So many kids are killed in car accidents every year, and parents keep giving them the keys to these cars, knowing how crazy they drive. This was a chance to hold parents accountable. Now, my son is dead, and my family is destroyed." Tague is not being charged as an adult but is adult-designated, meaning if convicted, a judge can sentence him as a juvenile or as an adult or blended sentence with the option of imposing an adult sentence if Tague is not rehabilitated. Fight instructor-related accidents and check ride failures at all levels of certificates and ratings are on the rise. Why? Insights are hard to come by. The report on a February 24, 2024 fatal training flight in South Africa that killed the student pilot and the instructor does nothing to help that issue. The Flight Safety Detectives' analysis of the document finds a lack of any insightful analysis of the factors that led to the fatal crash. There are no useful insights or lessons that could benefit aviation safety. Any accident investigation has to provide some kind of aviation safety benefit, and this investigation did not meet that standard.The flight involved a pre-solo student. The training manual may have included things not normally taught to a pre-solo student. Based on eyewitness testimony, the aircraft had a low and fast approach, followed by a wingover maneuver. The report does not answer the basic question of whether it was the student or instructor at the controls.Greg Feith points out key factual evidence not analyzed or explained in the report. Todd Curtis calls for more details about the flight instructor and the flight school. John Goglia surmises that the report was written by someone without an aviation operations background.
