Haemorrhage, hypertensive disorders, and sepsis are responsible for more than half of maternal deaths worldwide. Further, it is estimated that for every death, there are 50 pregnant people with life-threatening morbidity from sepsis. Heartbreakingly, the incidence of puerperal sepsis has risen over the last decade, in some cases doubling, with increasing rates of severe sepsis contributing to mortality. Underlying this trend is increasing virulence of group A streptococcal (GAS) infection. This is suspected to be due to the predominance of emm1 and emm28 genotypes, which have higher associations with mortality, as well as increasing maternal risk factors for infection such as obesity and DM. PLUS, certain GAS antibiotic resistance is on the rise. Group A streptococcus infection remains an important contributor to pregnancy and puerperal morbidity and mortality. Early recognition, diagnosis and aggressive management are Important for favorable outcomes given the serious risk of sepsis and streptococcal toxic shock syndrome. In this episode, we will highlight this alarming rise of Group A strep in the peripartum period including the most recent 2021 International Society for Infectious Disease in Obstetrics and Gynecology (ISIDOG) guidelines regarding GAS in pregnancy. They initially appeared to be a perfect match with similar spiritual values, but things changed drastically after they got married. Ayana's husband started exhibiting controlling and verbally abusive behavior, and she couldn't understand where it was coming from. Friends and family were concerned about Ayana and tried to check on her, but they were unable to share their concerns before the wedding due to her husband's request for secrecy. 2: Unraveling the Truth Ayana gradually realized her husband had deep-seated issues and unresolved trauma from past relationships, which led to a hatred of women. He would use Scripture to condemn her instead of using it to support and guide her on her spiritual journey. Ayana's husband also struggled with financial control, withdrawing money during arguments and insisting on separate financial plans. 3: The Accusation After Miscarriage yana and her husband experienced a traumatic miscarriage after arguments and stress surrounding her previous husband's near-death experience. Heartbreakingly, Ayana's husband accused her of "murdering" their unborn children, causing immense pain and emotional distress. 4: A Mother's Struggle: The Desperate Cry for Safety Ayanna discusses hitting a breaking point in her marriage, facing abuse on every level, including physical, and her realization that she needed to seek help for the sake of her children. The traumatic incident with her ex-husband that led to a life-altering injury and its impact on Ayanna and her family. Ayanna's internal conflict and emotional turmoil when her current husband demanded her not to make physical contact with her ex-husband during a visit. Ayanna shares the intense emotional and physical abuse she endured during a heated argument with her current husband. The fear and helplessness she felt when he aggressively confronted her and the moment she realized she needed to escape for her own safety and the well-being of her children. Ayanna's internal struggle with the decision to leave and her faith-based approach to seeking guidance from God. 5: Finding Support and Strength Ayanna's discovery of Leslie Vernick's work and Conquer, which provided her with the language and understanding of emotional destructive marriages. The importance of recognizing her worth as a child of God and setting boundaries to prioritize her safety and well-being. 7: Embracing Healing and Wholeness Ayanna's gradual transition towards healing and wholeness after filing for divorce and separating from her abuser. Who are we, if not people who live on hope, thought to thought, day to day, year to year. Often knowing about possibilities, often just whistling in the wind. It could be a change of fortune, a lucky break, a chance encounter, a person we'd always loved. Everything, even what seems to be the minutest of an incident, has the potential to change lives, and more often than not, it does.   And until it does, hope binds us to invisible tethers.   Gurus talk about yearning, as they talk about the journey, and remind us not to lose the experience of what we go through.   To know that the journey of feelings is often more precious than what we finally get.   The untetheredness of anguish, the ecstasy of possibility, the world building, the smart turn of phrase, the laughter, the look, the sheer joy of something which could only be defined as tender. That is the road to finally getting something. Heartbreakingly , and retrospectively, when we finally get - what we wanted, who we wanted, how we wanted - it is often bereft of glory. Compared to the striving, what we finally get seems so much lesser - less glittering, less flawless, less satisfying.   And thus go things in life, and thus do love stories find their beginnings, their middles and their ends. Too many affairs end at the consummation. And it would be a tragedy to have that as the only remembrance - and not the tease and the expectation and the imagination and the excessive giving and the extravagance leading towards it all.   And because of that, every story stands stunted, it's rich repository of the best of what we human beings are capable of lying discarded with a sheen of regret, as if it meant for nothing. If Boris Johnson had locked down earlier, Peter Owen would still be alive today, this is the firm belief of his daughter Becky. Instead, this beloved father and grandfather caught Covid 19 shopping in Sainsbury's on March 19, three days before Johnson belatedly put the country into lockdown. Becky vividly describes the void left behind by the death of her father, a proud Welshman from Aberystwyth who “always made time for people.”Equally as powerful is her description of the surreality of his death and feelings of distorted grief because all the rituals that usually go with death were denied. “I have nothing in my head to say that it's really happened,” Becky explains. “I've gone from seeing my dad before lockdown to seeing a pot of ash, there was nothing in between. That's a pretty abnormal thing“There was no proof that he died. I have no idea where he died, what the hospital ward looked like that he was on, or the bed, I don't know where he was cremated, I don't know any of the circumstances around his death.” Heartbreakingly, the nature of his death meant: “I've no idea what his last words to me were meant to be.”As with so many people in the Covid-19 bereaved community, Becky has only recently faced her loss and grief: “Coming out of lockdown was very difficult. Endometrial cancer is the fourth most common cancer in the US and is increasing in incidence each year. Heartbreakingly, despite postmenopausal bleeding presenting as the main symptom in all patients with endometrial cancer, there is a large racial disparity in endometrial cancer mortality in African-American women compared to White women. The 2 largest factors contributing to this are advanced stage of diagnosis and histological cancer type. In this episode, we will review the ABOG MOC article under “Health Equity and Patient Safety” for Q1 2023. Does transvaginal ultrasound perform the same in African-American women as it does in white women? Why or why not? Let's get to the data in this episode.

Sarah Beaumont had held high-level management roles and worked all over the world. But when a chronic condition left her in constant pain she was told she'd never work again. Now she's launched and runs her own business helping other people, and tells her story in today's episode.Diagnosed with Ehlers Danlos syndrome and Fibromyalgia – both long term chronic illnesses – Sarah became bed-bound, saying "there were times I would crawl on my hands and knees just to get to the bathroom.”Heartbreakingly, Sarah then went through a difficult divorce and says she "gained a huge amount of weight because I was so sedentary and I couldn't cook healthy, fresh meals so I ate a lot of junk. I lost my career and I lost my life before illness, I grieved for them for a long time.”But told she would never work again, Sarah could not accept that as her future: “I'm not a quitter and very slowly I have rebuilt my life, one piece at a time. With music that touched on themes of violence, bravado, and machismo -- particularly with respect for Punjabi men – Wala was an outspoken individual whose passing has left many with an empty feeling in their hearts. And now his fans are struggling to come to terms with the controversial circumstance revolving around his death. Wake Up to Lyme - Laura Harris (42) was recently diagnosed with Late Stage Chronic Lyme Disease (Lyme Neuroborreliosis - meaning it has hosted itself in the brain). Laura has three other tick-borne infections, named Bartonella, Babesiosis (cousin of Malaria), and Mycoplasma Pneumonia. Heartbreakingly, she has also passed these infections congenitally to her two children, Joshua (now 20 years old) and Elisha (now 14 years old). They have both been tested and were diagnosed in the last couple of years, following her own diagnosis and having learnt about the childhood symptoms, they had both been suffering with. Laura was first bitten by a tick about 30 years ago, on a Guide camp, in the New Forest, Hampshire, and then again about 18 years ago in the Alver Valley, Hampshire. Both are prevalent areas for Lyme Disease, in England. Both times Laura removed the ticks and doesn't remember having a bulls-eye rash, which is also the case for a third of people bitten. On the whole, she has lived a blessed and beautiful life, with her two children, travelling, dancing and doing a career, that she loved for over 20 years, supporting people with addictions and becoming a Counselling Psychotherapist, but she has had the challenge alongside, of getting through the terrible bouts of unexplained illness, the best she could and then carrying on again, when able. Until a car crash 10 years ago, that stopped her body being able to keep it at bay any longer she deteriorated rapidly. Laura knew in her core and intuition that she had it, especially after listening to Yolanda Hadid's audio book "Believe Me-My Battle with the Invisible Disability of Lyme Disease". It led her to research and fight the system to get private testing and now private treatment for her and her children also. It is a message of hope however, as she is determined to beat this! And also raise awareness with prevention and help those that already have Lyme and undiagnosed/misdiagnosed, or are aware but don't know where to turn next. Today I am joined by the wonderful Lotte Bowser. I first heard about Lotte and her fiancé Ben's love story in the summer of 2020 when Ben was undergoing treatment for stage 3 soft tissue sarcoma, an extremely rare type of cancer. Later that year, their journey led them to Mexico where Ben began to receive treatment at a specialist centre in Tijuana. Heartbreakingly just as things were beginning to look hopeful, Ben contracted coronavirus and passed away. And here began Lotte's unfathomable journey into widowhood, at just 30 years old. In this episode Lotte and I chat about her and Ben's unconditional and purest kind of love and how it evolved pre and post diagnosis. We also discuss how the use of psychedelics has been paramount in helping Lotte understand that there is so much more to be explored after we leave our physical bodies. "Six air changes per hour and HEPA grade filtration of indoor air for our children is an investment in their futures for long beyond when the COVID-19 pandemic fades from view. We know there is poor indoor air quality in multiple schools, from inadequate ventilation to air pollution to wildfire smoke. Kids learn better and have less absenteeism in healthy air. COVID-19 has just added fuel to that fire. Let this be a torch to light our way forward to healthy indoor air- starting right now. Most importantly, we need to make sure this opportunity is available to all children, particularly those who have often been left behind in this pandemic. Heartbreakingly, there are kids who never logged on last year or showed up to school, and may have families afraid to send them this year. There are federal funds available from the COVID-19 relief bills to ensure that we can provide this assistance even to already financially-distressed schools. Nothing will ever make our children completely safe, which is the persistent ache of being a parent. However, they deserve a school year in person if possible, and we as the adults in their lives, should do our best to make sure they get it. Don't buy your kids' teachers apples this year. Kristen Carbone had always wanted to work in an art museum, but after landing her dream museum job she soon faced a harrowing reality: her mother's breast cancer had returned with a serious metastasis. Packing up her life, Carbone moved to Baltimore to be closer to her ailing mother. Heartbreakingly, her mother passed before turning 50. After her mother's death, Carbone uprooted herself again and continued her museum work. But just three months after her 30th birthday, Carbone herself was diagnosed with breast cancer. She made the brave and terrifying decision to have a preventative mastectomy and reconstruction. “I felt really committed to the choice,” she says. As she learned to live in her new body, however, she experienced first-hand the challenges that many women face after treatment. Carbone reached out to hundreds of women experiencing the same issue before founding Brilliantly, a lifestyle brand whose programs and products are designed to support women living in post-mastectomy bodies. In 1963, Dian Fossey spent all her savings and took out a bank loan to fulfill her dream of going to Africa; it was the beginning of a journey that led to her drastically altering scientific and public perceptions of mountain gorillas. Unforgotten: The Wild Life of Dian Fossey and Her Relentless Quest to Save Mountain Gorillas by Anita Silvey, recounts Fossey's life and work so that younger readers can come to know this extraordinary woman's accomplishments, which still resonate today. Fossey had no experience or formal scientific training, but she was smart, passionate, and strong-willed — and she connected with paleoanthropologist Louis Leakey, who helped her reach her goal of studying animals in the wild. Fossey set up a research camp in Rwanda and threw herself into tracking and observing mountain gorillas. Over the next 18 years, Fossey got closer to gorillas than any human ever had. In 1973, Fossey recorded only 275 gorillas living in Volcanoes National Park, and there are more than twice that many today as a result of decades of ardent conversation efforts that she initiated. Written for middle graders, Unforgotten is filled with captivating photographs and fascinating facts about both Dian Fossey and her treasured mountain gorillas. Young readers will learn about a gorilla's typical day, including diet and sleeping habits, and how she came up with their names. Heartbreakingly, Fossey was murdered at her camp in 1985, and her death remains a mystery to this day. Heartbreakingly, she gained her wings less than a year after diagnosis but her memory still provides thousands of children smiles daily! Welcome to the second threesome of Season 3!  Horizontal is still Slow Radio. It’s still consensual eavesdropping. It’s still us reclining, sharing secrets, in your ears. But while the first two seasons had me lying down as though post-coital and stargazing with only one person at a time, in Season 3 I’ll often get horizontal with two people at a time: people who are in some kind of relationship with one another. In this case, they are co-hosts, business partners, and friends. In this episode, 76, and the following, episode 77, I lie down with Dominick Q and Bryan Stacy of The Discerning Dick, a monthly series focused on generating worthy inquiry and meaningful introspection into the forces that shape a man's sexuality, behavior and intimate relationships.  I attended the first three Discerning Dick events at The Assemblage in Manhattan, and it gave me an echo of the feeling of the day I got to be a fly on the wall during a men’s group meeting in Ashland, Oregon in 2008. Undoubtedly, what I saw and heard was changed somewhat by the fact of my witnessing. But what I heard was still a powerful window into minds and bodies that I wanted desperately to understand. Dominick Q is a men’s group leader, an executive mentor, a speaker, and a writer. Bryan is a sexual health advocate, a testicular cancer survivor, and the co-founder of the app biem, which is designed to allow people to easily get tested for STI’s and share their test results with partners.  Bryan and I have been planning to produce an event together — look out for TESTFEST 2019! Dom & Bryan also co-host the “Man Amongst Men” podcast, which they call a “High Achieving Man's Journey to Personal Development.” They are both men who have spent the majority of their lives focused on standard notions of external success. And now they are focused on what that energy might look like, and what it might eventually do out in the world, if they first focus on their inner world.  They are cisgendered straight white males trying to heighten their self-awareness in order to do better, to be constantly in the process of becoming better men without claiming to have arrived at it, and sincerely making the effort to bring other cis straight white men along with them.  I can think of many men that I would like them to talk to. Men also suffer from the indoctrination of a culture that shames us for our natural human sexuality yet objectifies women to sell things to nearly everyone. These men aim to be a bridge between a man who doesn’t know that he’s swimming in water… and the sex educators and personal development leaders of the world. They aim to speak directly to men who feel stuck in a lackluster sex life or mired in sexual habits that are destructive or secretive, or both. Just as it is a white person’s responsibility to educate themselves and other white people about intersectionality, injustice, and the perniciousness of racism, so too is it a man’s responsibility to educate himself and other men about intersectionality, injustice, and the perniciousness of sexism. I deeply, Heartbreakingly believe that a lack of self-inquiry combined with sexual repression is a direct cause of the lion's share of violence on this planet. They are aiming for the root. And I want to do everything in my power to stand for that.  Dominick and Bryan don't claim to be experts; they just aim to be willing: willing to be educated, to be called-in, to adjust their worldview, and to tell other men what they've discovered, publicly. Heartbreakingly, we've been here before. On one hand I'm not sure it's my place as a white man who mostly writes Rick Ross fat jokes to speak about the justice system's absolute disregard for black life. I'd much rather use whatever meager influence I have to direct people towards those who are writing more powerfully than I ever could. At the same time, I also believe that silence is consent, and it just doesn't feel right to remain silent about an issue that's been the only thing I've been talking about off the internet for the past two days. So as much as there's a part of me that wants to hide and just talk about Lil Wayne potentially leaving Cash Money, I'm devoting the bulk of this week's podcast to talking about Eric Garner, Ferguson and what can only be described as America's systematic war on black lives. I can't pretend it's even remotely enough to matter, but I just can't stay silent. That also means that I considered pulling this week's interview with Danez Smith, an amazing poet and performer I recently came across. I spoke to him two weeks ago, before either the Mike Brown or Eric Garner of the non-indictments, and I was hesitant to place our conversation in this context. But listening back to our conversation, I realized that since so much of his work is about remaining human despite overwhelming odds, this might actually be a compelling week to bring him into the podcast.
