In this month's episode of the Theatre, head and neck surgeon Rafal Niziol, dental surgeon Caroline McCarthy and oncologist Chris Nutting come together to talk about oral cancer. Together, they give an in-depth overview of the current state of the field, from challenges in diagnosis to opportunities created by new technologies and the importance of improving multidisciplinary collaboration.Oral cancer, also known as mouth cancer or mouth lining cancer, is one of the forms of cancer with the highest mortality rates – above 50% within 5 years of diagnosis for those diagnosed at an advanced stage. Research shows early diagnosis can have a critical impact on that rate, as early-stage cancers have a survival rate of around 90%. Yet, for a variety of reasons, oral cancer is still grossly underdiagnosed – with median delays between initial contact with a GP and specialist diagnosis as high as 106 days.At RCS and the Faculty of Dental Surgeons, we want our mission to advance dental surgical care for all patients to go hand-in-hand with practical action, and it is with this in mind that we set to tackle one of the biggest problems facing the profession. You'll often see in textbooks that alcohol is a major risk factor for oral cancer. They'll tell you that people who drink three to four drinks per day have about two to three times the risk, and with heavier drinking, that risk can go up to five times. They'll also emphasize that when alcohol is combined with tobacco, the risk can increase up to 15 to 30 times. But here's the thing—most of the time, they stop there. They tell you that it increases risk… but not really why. Our special guest is Dr. Alessandro Villa, DDS, PhD, MPH, Chief of oral medicine, oral oncology and dentistry at Miami Cancer Institute, where he also serves as program director of the Oral Oncology Fellowship. He is a Professor at the Herbert Wertheim College of Medicine at Florida International University. Dr. Villa specializes in oral mucosal and salivary gland diseases, oral precancers and oral complications related to cancer treatment. Before joining Miami Cancer Institute in 2022, he was Chief of Oral Medicine and Oral Oncology at the University of California, San Francisco (UCSF); he also directed the oral medicine residency programs at UCSF and Harvard University. His research focuses on oral leukoplakia, cancer therapy-related complications, and the prevention of HPV-associated oropharyngeal cancers. Dr. Villa is actively involved in several investigator-initiated and sponsored clinical trials and has authored more than 180 peer-reviewed publications. Dr. Villa provides an in-depth overview of the ADA's Living Guideline Program, explaining its purpose, how it works, and the key ways it differs from traditional guidelines. Living Guidelines are developed with the same level of rigor as traditional guidelines, but they use a distinct infrastructure design for continuous updates to emerging evidence, Dr. Villa shares. The development of these Living Guidelines is driven by a multidisciplinary panel that typically includes general dentists, dental hygienists, oral medicine and oral pathology specialists, oral surgeons, and other relevant experts. These panels also incorporate patients and additional stakeholders. Dr. Villa shares that they use the great evidence-to-decision framework to help dentists efficiently move from data to recommendations in a timely manner. All of this is built inside a digital platform. When evidence is conflicting or limited, Dr. Villa explains, the transparent process for Living Guidelines ensures clarity and integrity. The first set of recommendations from the ADA's Living Guideline program is on the evaluation of potentially malignant disorders in the oral cavity. Dr. Villa gives us more details about these recommendations, and the findings to date. BUFFALO, NY - May 9, 2025 – A new #research paper was #published in Oncotarget, Volume 16, on May 8, 2025, titled “METTL3 promotes oral squamous cell carcinoma by regulating miR-146a-5p/SMAD4 axis." In this study, researchers Jayasree Peroth Jayaprakash, Pragati Karemore, and Piyush Khandelia from the Birla Institute of Technology and Science, India, discovered that a molecule called METTL3 contributes to the development and spread of oral squamous cell carcinoma (OSCC). The study shows that METTL3 increases the levels of a small RNA molecule called miR-146a-5p, which blocks SMAD4, a key tumor-suppressing gene. These findings help explain why oral cancers are difficult to treat and may offer a new target for more effective therapies. Oral squamous cell carcinoma is a common and aggressive cancer affecting the mouth and throat. It has a high death rate, mainly due to late detection, treatment resistance, and the cancer's ability to invade nearby tissues. In this study, the researchers focused on METTL3, an enzyme that adds chemical tags known as m6A marks to RNA, which change how genetic information is used by cells. They found that METTL3 is unusually active in OSCC cells, causing an increase in miR-146a-5p. This molecule, in turn, blocks the function of SMAD4, which helps control how cells grow and die in our bodies. “METTL3, the primary m6A RNA methyltransferase, is significantly upregulated in OSCC cells leading to increased global m6A levels.” When METTL3 was reduced or chemically blocked, miR-146a-5p levels dropped and SMAD4 levels increased. This shift slowed the growth of cancer cells, increased their death, and made them less likely to spread. When researchers reintroduced miR-146a-5p or lowered SMAD4 levels again, the cancer-promoting behavior returned. These results show that the METTL3–miR-146a-5p–SMAD4 pathway plays a key role in OSCC. The findings open up new possibilities for treatment. Drugs that block METTL3 or miR-146a-5p or that restore SMAD4 could slow or stop tumor growth. One such drug, STM2457, which targets METTL3, has already shown promise in lab studies. As research progresses, targeting this molecular pathway may offer a new strategy in treating OSCC. This discovery improves our understanding of how OSCC develops and avoids the body's defenses. By interfering with this newly discovered pathway, future treatments may become more successful, improving survival rates and quality of life for people with this disease. In this episode, we got real about something most of us don't think about nearly enough: oral cancer—and why your mouth might be trying to tell you something important. April is Oral Cancer Awareness Month, and we wanted to shine a light (literally and figuratively) on how vital it is to be aware of what's happening in our mouths. We opened up about how oral cancer claims one life every hour in the U.S., and yet many people don't even know how to check for it—or that they should be checking in the first place. I shared my own story. Even though I didn't have oral cancer, I was diagnosed with breast cancer—and the first warning signs actually started in my mouth. As a dental hygienist, I knew my gums were healthy. So when they started bleeding without explanation, despite perfect brushing and flossing habits, I knew something was up.
