Costumer Journey 2.0 - weg vom linearen Funnel, rein ins Chaos (Yvonne Romes-Schillack) | OMT-Podcast #281

Play Episode Listen Later Oct 20, 2025 54:24

This conversation with Dr. Carolyn Moyers revealed how personal experience with perimenopause transformed her from a general OBGYN into a Menopause specialist. Her journey began when her youngest son pointed out she was getting "mean as she got older," leading her to recognize her own perimenopause symptoms and start estrogen therapy.Dr. Moyers emphasized the critical importance of debunking perimenopause myths that continue to harm women. The most damaging myths include "it's all in your head," "your labs are normal so you're fine," "you're too young for perimenopause," and "hormone therapy just delays the inevitable." She stressed that perimenopause is a clinical diagnosis that can start in the early 30s and last 7-10 years before the final menstrual period. The discussion highlighted the complexity of managing perimenopause versus menopause, with Dr. Moyers noting that perimenopause is actually more difficult to treat because of the "pesky period running around." She advocates strongly for the Mirena IUD, which reduces bleeding by 90% in the first six months, making hormone management much more straightforward.A significant focus was placed on metabolic changes during the menopause transition, including the natural 0.6% annual decline in muscle mass. Dr. Moyers outlined a comprehensive approach including strength training, protein-rich diets, sleep prioritization, and stress management. She introduced the concept of "thought dumping" - an unedited journaling practice combined with identifying three daily wins and three goals for tomorrow.Dr. Moyers shared her unique integration of osteopathic manipulation into menopause care, offering quarterly OMT sessions as part of her membership model. This hands-on approach helps balance the autonomic nervous system and addresses the physical manifestations of hormonal changes, particularly beneficial for pelvic pain and pregnancy-related discomfort.Highlights:Debunking harmful myths: "you're too young" and "labs are normal".Why perimenopause is harder to manage than menopause.Osteopathic manipulation for hormonal balance and pelvic pain relief."Thought dumping" stress management technique with daily wins tracking.Comprehensive metabolic support including strength training and nutrition.Strategic use of GLP-1 medications as jumpstart tools for sustainable change. If this episode helped you understand perimenopause management and holistic care options, help other women find this conversation by subscribing to the channel and leaving a review on Apple Podcasts. Magewell's story from USB Capture to all-in-one production: how a user-first approach powers pro AV, broadcast, and streaming—plus where devices like Director One, Director Mini, and NDI converters fit in real-world workflows. Marc Aflalo sits down with Mike Nann of MVD (Mobile Video Devices)—Magewell's exclusive North American distributor—to trace Magewell's evolution and what makes their gear stick in both broadcast/media production and pro AV. Mike shares his 25+ years in video tech (from Digital Rapids' early Olympic streaming with Windows Media) and explains how Magewell moved from OEM engineering (2011) to its own lineup by finding and fixing the “gaps” between signals, software, streams, and screens. You'll hear how USB Capture popularized true plug-and-play ingest for HDMI/SDI; why the pandemic accelerated adoption; and how the company designs products around “technical invisibility” for pro AV—where the best gear disappears into the workflow—while still giving producers deep control. Osteopathic Manipulation Therapy In Sports Medicine

Dr. Dominic Maneen shares insights on OMT in sports medicine, covering his AT to DO transition and OMT applications for common conditions. Q: How did you transition from an Athletic Trainer (ATC) to a Doctor of Osteopathic Medicine (DO)? A: I transitioned to practicing sports medicine without surgery, as that field didn't align with my interests. I explored osteopathic medicine, identifying the core difference between DOs and MDs as a dedicated course in osteopathic principles. I maintained my athletic training certification to foster understanding and collaboration with ATCs, and a second course focusing on the musculoskeletal system further ignited my passion. During medical school, I also pursued an MBA, gaining insights into medical billing. Q: Can you describe your athletic training (AT) experience at HBU? A: I completed my undergraduate studies at UT, then worked as an ATC for baseball and softball at HBU. I entered the profession serendipitously, drawn by the phrase "sports medicine." It required rapid maturation, as I assumed an adult role despite being only slightly older than the athletes. I collaborated with Richy Valdez and several GAs, and student athletic trainers were indispensable since it was impossible to simultaneously oversee both baseball and softball. I recall an incident involving twin softball players: one sister not playing, the other on deck, with an accidental practice swing hitting the sister, necessitating a golf cart ride to the adjacent facility. Q: Why is low back pain a significant health concern, and how is it related to depression and lifestyle? A: Low back pain is the second most common reason people visit the doctor, with depression being the first. Patients typically present with symptoms that indirectly lead to a depression diagnosis, rather than overtly stating "depression." A sedentary lifestyle is often termed "the new smoking" in medicine, leading to tight, unused muscles. Hands-on manipulation therapy can be beneficial, and simple exercises like push-ups can improve posture by strengthening the neck muscles that support the head. Many individuals struggle with core muscle activation; focusing on proper technique and guiding them to engage their core will lead to increased strength over time. Q: How do you address flat feet in patients? A: Patients often present with concurrent back and knee pain. Structural analysis can reveal the cause, leading to recommendations for inserts or corrective devices for arch support, rather than immediate surgical intervention. Subsequent efforts focus on improving knee mechanics to alleviate symptoms. Q: What is your approach to concussion management? A: Myofascial release may sometimes require trigger point injections. However, most concussion cases can be effectively managed with muscle energy techniques. In older patients, some form of osteopathic manipulation may be necessary. Q: What are the key anatomy considerations for the neck? A: It is crucial to understand that the neck's complexity extends beyond superficial muscles like the scalenes and deltoids; smaller, deeper muscles also play a significant role in neck function. Q: What are your tips for the Athletic Training Room? A: Prioritize hands-on manipulation therapy and muscle energy techniques with athletes, rather than solely relying on stationary bikes or treadmills for warm-up.

Fargo, ND based Resurgent Health is bringing a new philosophy on healthcare Contact Resurgent Health: at their website, Facebook Page or by phone (701) 599-3950 An episode summary: Introduction and Direct Primary Care Explanation (0:00 - 6:38) The Ag Week podcast host, Al Winmill, introduces Aly Dahl and Kristine Martens from Resurgent Health, a direct primary care clinic in Fargo. They begin by explaining the concept of direct primary care, emphasizing that it's a membership-based model rather than insurance-driven. Patients pay a monthly fee for unlimited access to their providers, including in-person visits, phone calls, text messages, and virtual appointments. This model aims to foster stronger patient-provider relationships and provide more personalized care. They contrast this with the traditional insurance-based system, where providers often have limited time per patient due to reimbursement rates and are incentivized to see more patients to generate revenue. They highlight that their model allows for longer, more focused visits, and they are motivated to keep patients healthy rather than just treating them when they're sick. The transparency of their pricing, with no surprise bills, is also a key benefit. They state that the system is more convenient for patients, especially those with busy lifestyles, as much of the care can be delivered remotely. Direct Primary Care and Agriculture (6:38 - 10:26) The conversation shifts to the suitability of direct primary care for those in agriculture. Dahl and Martens explain that their model is particularly beneficial for farmers and ranchers due to its convenience and accessibility. They acknowledge that farmers often work long hours, especially during planting and harvest seasons, and may not have time for traditional doctor's appointments. Direct access to providers via phone, text, and virtual visits allows farmers to receive care without leaving their work. They also emphasize that their model is more proactive, focusing on preventative care rather than a reactionary approach. They discuss the economic benefits of direct primary care for agricultural businesses, noting that it can be a more affordable alternative to traditional health insurance, especially for small businesses struggling to offer comprehensive benefits. They are now offering their services as a benefit for small businesses. They believe that offering direct primary care can help attract and retain employees by providing them with access to quality healthcare. Health Challenges Faced by Farmers (10:26 - 13:09) The guests then discuss the unique health challenges faced by farmers and ranchers. They highlight musculoskeletal injuries as a common issue due to the physically demanding nature of their work. They explain that they focus on healing the body from the inside out and that Dr. Martens offers osteopathic manipulation techniques to help with pain management. They also address the high levels of stress experienced by farmers due to market fluctuations, weather conditions, and other factors. They acknowledge that many farmers self-medicate with alcohol or ignore their health issues altogether. They emphasize their commitment to being there for their patients during challenging times and helping them maintain their health. Addressing Stress and Osteopathic Manipulation (13:09 - 14:58) The discussion returns to the stress experienced by farmers, and the doctors reiterate that seeking medical care shouldn't add to that stress. They emphasize the flexibility of their scheduling, allowing for virtual visits or rescheduling without long waits. Dr. Martens elaborates on osteopathic manipulative treatment (OMT), a hands-on technique she offers. She explains that it's a gentle approach, combining elements of massage and chiropractic, aimed at aligning the body and promoting natural healing. OMT can be beneficial for various pain syndromes, migraines, and even GI issues. She notes that Resurgent Health is currently the only clinic in the area offering this service. Nutrition and Individualized Care (15:16 - 18:38) The conversation shifts to nutrition, with the doctors acknowledging that farmers often have healthier eating habits than city dwellers. They stress the importance of individualized dietary advice, recognizing the unique challenges faced by farmers who work long hours and have varying schedules. They explain that they don't believe in rigid dietary restrictions or "check-box" approaches, instead focusing on making small, sustainable changes based on each patient's lifestyle and needs. They give an example of working with a farmer who drinks a lot of Mountain Dew, suggesting a gradual reduction rather than complete elimination. They emphasize that generic nutritional advice is difficult because individual circumstances vary greatly. Practical Health Tips and Prioritizing Care (18:39 - 21:22) The doctors offer practical health tips for farmers and ranchers, particularly emphasizing the importance of preventative care and addressing the reluctance of men to seek medical attention. They cite a study predicting a significant increase in men's cancer rates by 2050 and urge men to prioritize their health. They highlight the convenience and personalized care offered by Resurgent Health, encouraging listeners to consider their model as a more comfortable and accessible alternative to traditional healthcare. They reiterate that the current healthcare system, not the providers, is often the source of frustration and that their model allows them to provide the kind of patient-centered care they believe in. Learning More and Scheduling Consultations (21:22 - 22:41) Winmill about how listeners can learn more about Resurgent Health. The doctors direct them to their website, resurgenthealth.net, and highlight their offer of free "meet and greets." They emphasize that these consultations are no-obligation and provide an opportunity to meet the doctors and learn more about their services. They also mention that they offer after-hours consultations on Monday evenings and Friday mornings, in addition to regular weekday appointments, to accommodate busy schedules. Concluding Remarks and Gratitude (22:43 - 22:58) The podcast concludes with Winmill expressing gratitude to Aly Dahl and Kristine Martens for sharing their insights and the story of Resurgent Health. Winmill commends the clinic for its innovative approach to healthcare and find their work inspiring. Winmill expresses hope for future conversations with the doctors. Dahl and Martens reciprocate the thanks, expressing their appreciation for the opportunity to share their message.

At 1-year follow-up, 74.1% of patients in the T-TEER group improved, compared to 40.6% in the OMT-alone group (P < .001). T-TEER significantly reduced TR severity, improved NYHA class, patient global assessment (PGA), and Kansas City Cardiomyopathy Questionnaire (KCCQ) scores, and had a high procedural success rate (97.3%) with a low 30-day major adverse event rate (0.7%). The findings support T-TEER as an effective intervention for symptomatic severe TR, warranting long-term follow-up to assess mortality and hospitalization impact.

More Than a Kissing Spot: Your New Baby's Face is a simple and deep dive into all the things people need to know about the mouth and tongue before they are exhausted parents. www.primalair.com/kissing-spot Biologists say that human babies are born about seven months early as compared to other mammals. Human babies are born earlier because of their head size; they'd never make it out at all if they lived in the womb for 16 months. Because of this early exit babies are extremely defenseless. All they have is their cuteness and crying to do everything necessary to get their needs met. It's up to the “hard wiring” of maternal instinct to take over for the rest, including making sure the baby is breathing through their nose. Shirley's book, More than a Kissing Spot: Your New Baby's Face is the most important child development book written to date. Written for soon to be parents, it takes the mystery out of one of the most unusual organs in body, the tongue. The tongue is the most important driver of facial development, and rarely mentioned in discussions of the face. Orofacial Myofunctional Therapy is a superhero therapy that combines simple exercises and techniques to help kids with everything from their speech to their eating habits to their behavior.  But how do you know if your child could benefit from this therapy? What exactly is OMT? What does a session look like? In this episode of the Soaring Child podcast, learn more about Orofacial Myofunctional Therapy, or OMT, from special guest Dr. Marielly Michell.  Dr. Mitchell's innovative approach to OT centered on orofacial myofunctional therapy (OMT) combines principles of cranial nerve integration, sensory processing, and motor function, allowing her to address neurological and airway dysfunction in infants and children that impact behavior.  She is the founder of Theraplay LA, the first pediatric therapy center to offer comprehensive services that include sleep monitoring in conjunction with virtual and in-clinic occupational, physical, and speech therapy services specializing in sensory processing, motor skills, feeding and swallowing disorders, OMT, nutritional support, and sleep training. And ideally, everyone should be placing their tongue suctioned to the roof of their mouth behind their 2 front teeth, with their lips sealed and breathing through their nose without tension in their jaw.” ”If your child has a low oral rest posture…basically mouth breathing, then that automatically puts them into a state of fight or flight.” ”When a child is mouth breathing for a long period of time, it will deactivate that area of the brain, and so myofunctional therapy really focuses on facilitating a closed mouth posture and activating the nasal passages and making sure that the person reaches a more ‘rest and digest' or parasympathetic state.” ”Breathing and sleeping really take a huge toll, and it's really an abstract silent epidemic that we're in with this.” ”A really good tip is to always do these exercises in front of a mirror with them so they can get some visual feedback.” ”We're in a silent epidemic of this.” How to Connect with Dr. Marielly Mitchell Website: http://www.doctormarielly.com  Instagram - http://www.instagram.com/doctormarielly  Dana Kay Resources:

N Engl J Med 2012;367:991-1001Background: Percutaneous coronary intervention (PCI) did not improve the outcome of death or nonfatal myocardial infarction in stable coronary artery disease, as seen in the COURAGE trial. Nonetheless, some skepticism remained within the cardiology community regarding the results of the COURAGE trial, with the belief that revascularizing ischemia-causing lesions could improve clinical outcomes (ischemic testing was not required in all patients in the COURAGE trial). While there was data supporting this idea, large randomized trials providing conclusive evidence were still lacking.Cardiology Trial's Substack is a reader-supported publication. To receive new posts and support my work, consider becoming a free or paid subscriber.Fractional flow reserve (FFR) is a pressure-wire-based measurement performed during coronary angiography to evaluate whether a coronary stenosis is likely to be causing myocardial ischemia. The investigators of FAME 2 trial sought to test the hypothesis that FFR-guided PCI with drug-eluting stents plus optimal medical therapy (OMT) is superior to OMT alone in patients with stable coronary artery disease.Patients: Eligible patients had stable angina pectoris and stenosis of 50% or more in at least one major coronary artery that has a diameter of at least 2.5 mm and supplying a viable myocardium. Patients with atypical or no chest pain were included if they had documented ischemia on noninvasive testing. To evaluate the hemodynamic severity of each identified stenosis, FFR was measured using a coronary guidewire during adenosine-induced hyperemia. Patients with at least one stenosis in a major coronary artery with an FFR of 0.80 or less were randomized.Major exclusion criteria were: Bypass surgery felt to be the preferred treatment strategy, left main disease, myocardial infarction within a week, prior coronary artery bypass surgery, left ventricular ejection fraction

N Engl J Med 2007;356:1503-1516Background: By the turn of the 21st century more than 1 million coronary stent procedures were performed each year in the United States and approximately 85% were undertaken electively in patients with stable coronary artery disease. This pattern evolved without a single clinical trial demonstrating a concrete improvement in hard endpoints with percutaneous coronary intervention (PCI) compared to optimal medical therapy (OMT) alone. We have already reviewed several of these trials including ACME, RITA-2 and the Atorvastatin vs Angioplasty trial. Each trial was relatively small and none showed a significant benefit for revascularization compared to medical therapy on death or MI.Previous trials involving PCI compared to standard care or OMT included less than 3,000 patients altogether, did not broadly use intracoronary stents (instead using balloon angioplasty only) and they did not employ what would be considered a contemporary standard of medical management. Thus many questions involving the efficacy and safety of PCI versus OMT alone for managing stable CAD remained unanswered. The Clinical Outcomes Utilizing Revascularization and Aggressive Drug Evaluation (COURAGE) trial was designed to test the hypothesis that up front PCI plus OMT would significantly reduce the risk of death and nonfatal MI compared to OMT alone in patients with stable CAD.Cardiology Trial's Substack is a reader-supported publication. To receive new posts and support our work, consider becoming a free or paid subscriber.Patients: Eligible patients had stable CAD defined as either 1) a coronary stenosis of >/= 70% in one or more proximal epicardial coronary arteries and objective evidence of myocardial ischemia (based on the resting ECG or with exercise or pharmacologic vasodilator stress testing) or 2) a coronary stenosis of >/= 80% and classic angina without provocative testing. Patients were excluded if they had persistent Canadian Cardiovascular Society (CCS) class IV angina, a markedly positive stress test defined by substantial ST-segment depression or hypotensive response during stage 1 of the Bruce protocol), refractory heart failure or cardiogenic shock, an EF /=50% (4%).Patients in the trial were adherent with medical interventions and motivated to improve diet and lifestyle. For all patients, BP and cholesterol were improved and activity levels increased over the course of the study. For example, in the OMT group, baseline SBP and LDL were 130 mm Hg and 102 mg/dl at baseline and declined to 122 mm Hg and 72 mg/dl at 5 years, respectively. Similar reductions were seen in the PCI group. Fifty five percent of patients performed moderate physical activity at baseline and this increased to 77% at 5 years. Dietary adherence improved across the board and smoking decreased. Patients were followed for a median time of 4.6 years and 9% were lost to follow up with no significant difference between group. Overall, PCI + OMT did not reduce the primary endpoint of death or nonfatal MI compared to OMT alone (19.0% with PCI vs 18.5% with medical therapy; HR 1.05; 95% CI 0.87-1.27). Periprocedural MI was numerically more frequent in the PCI group whereas spontaneous MI was nearly identical between groups (statistics not provided). There were no statistically significant differences noted in any of the endpoints reported with the exception of revascularization during the follow up period, which occurred less frequently in the PCI group (20% vs 30%; p
