Questions? Thoughts? Send a Text to The Optometry Money Podcast! We'll answer your question on the show.In this episode, we break down tax-efficient investing for ODs, showing you how to pair the right investments with the right accounts to maximize your after-tax wealth over your career. What You'll LearnTaxes can be the difference between two optometrists with identical investment returns ending up with vastly different retirement wealth. This episode breaks down how to invest tax-efficiently by pairing the right investments with the right accounts. You'll learn which investments create tax-inefficient income (bonds, REITs) versus more favorable income (stock index funds), and the strategy for placing them across your taxable accounts, pre-tax retirement accounts, Roth accounts, and HSAs. The key is treating all your accounts toward the same goal as one coordinated household portfolio instead of managing each separately. Key TakeawayDon't let the tax tail wag the dog. Start with good, prudent, evidence-based investments - then optimize their placement to minimize taxes along the way. Practicing oncologist, adjunct professor at Duke University, and chief medical officer at AccessHope Yousuf Zafar discusses his article "Remote second opinions for equitable cancer care." Yousuf explains how a patient's zip code often dictates their survival odds despite biology showing that rural and urban patients have identical potential for recovery when given the same protocols. He details how the explosion of precision oncology has made cancer care vastly more complex, leaving community practices struggling to keep up with over 100 unique cancer subtypes. The conversation highlights how virtual reviews allow subspecialist expertise to flow from academic hubs to local clinics, changing treatment plans in over 52 percent of cases. Yousuf also emphasizes that this collaborative model supports rather than replaces local doctors by connecting them with just-in-time knowledge and clinical trial opportunities. Learn how technology is redrawing the map of medical expertise to ensure life-saving knowledge travels faster than the disease itself. Partner with me on the KevinMD platform. Can you really "have it all"? Dr. Sharon Malone OB-GYN, New York Times bestselling author of Grown Woman Talk, and Chief Medical Advisor at Alloy Health joins me for an honest conversation about what it really takes to balance medicine, motherhood, and everything in between. Dr. Malone and I talk about the things we don't usually say out loud. How do you balance being a great doctor, a present parent, and a supportive partner when society tells you to excel at all three simultaneously? Dr. Malone practiced medicine before and after the Women's Health Initiative, so she has a unique perspective on how hormone therapy went from being standard care to being feared and how that fear disproportionately affected women of color. We discuss why only 1% of Black women who are eligible for hormone therapy are actually on it, despite suffering the longest and most severe menopausal symptoms. We also talk about her work with Alloy Health, her new podcast The Second Opinion, and why she believes menopause is inevitable but suffering is not. Plus, we get into the uncomfortable truth about medical racism, implicit bias, and why your gut feeling matters more than your doctor's ego.HighlightsGive yourself grace in midlife perimenopause decreases your coping threshold for everything you're already juggling.Women of color carry the weight of representing their entire group, not just themselves, which adds invisible pressure.Diversity in healthcare leadership literally changes what research gets funded and what treatments get offered.Only 1% of Black women eligible for hormone therapy are actually on it, despite having more severe and longer-lasting symptoms. The "adipose tissue theory" that Black women don't need hormones because they make more estrogen on their own is completely false.Racialized medicine affects everything from endometriosis diagnosis (only thin white women?) to pain management assumptions. If your doctor gets mad that you asked for a second opinion, you need a different doctor.Guidelines are guardrails, not laws medicine requires both confidence and humility.Dr. Malone's book Grown Woman Talk is everything you need to know about navigating midlife with confidence and information. And remember: trust your gut. The conversation walks through the foundational ideas behind the book, beginning with retirement income styles and why there is no single “best” strategy for everyone. Wade explains the importance of aligning retirement income decisions with personal preferences, comfort with risk, and behavioral realities rather than forcing a one-size-fits-all approach. The discussion then expands into efficiency-focused retirement planning, highlighting practical ways retirees can improve outcomes through Social Security claiming decisions, tax planning, and organization for incapacity and estate planning. The episode concludes with a framework for understanding the three major risks retirees face: longevity risk, market risk, and spending shocks, as well as why planning becomes especially critical during the transition into retirement. Takeaways Retirement income planning does not have a single correct answer; multiple viable strategies exist, and the best choice depends on personal preferences and behavior. Understanding your retirement income style helps prioritize which strategies, tools, and chapters of the planning process deserve the most focus. Retirement efficiency means getting more after-tax spending power or legacy from the same set of assets, often by making better decisions rather than taking more risk. Social Security claiming decisions remain one of the most impactful and accessible efficiency opportunities for many retirees. Strategic tax planning, including Roth conversions, can create immediate and long-term benefits without requiring market forecasts. Organizing documents for incapacity and estate planning is a major but often overlooked source of efficiency with both financial and psychological benefits. Retirees face three primary categories of risk: longevity risk, market risk amplified by withdrawals, and unpredictable spending shocks. The years leading up to and immediately following retirement are a fragile transition period where early planning creates significantly more flexibility and better outcomes. Living with chronic back pain often means slowly adapting your life around it. Over time, everyday things – sitting comfortably, traveling, sleeping through the night, keeping up with family, start to require planning, compromise, or avoidance altogether. The hardest part isn't usually the pain itself, but the fear that this may simply be how life will always feel. After nearly 20 years of back pain and multiple procedures, Rachel, a teacher and mom of two, found herself in that exact place. She describes years of temporary fixes "surgeries that helped, until they didn't," trying everything from lumbar fusion to chiropractors to injections, discectomy and physical therapy, yet lasting relief never came. Eventually she was told disc replacement surgery wasn't an option for her. Instead, she was warned that her future would likely involve spinal fusion after spinal fusion, each one further limiting her mobility and quality of life. She felt trapped between continuing to manage her pain and facing an uncertain future of additional surgery. Rather than accepting that outcome, Rachel decided to keep asking questions. In this episode of Get Back to Your Life®, Rachel shares how seeking another opinion led her to two-level disc replacement surgery with Dr. Colin Haines at VSI®, and how that decision changed what daily life looks like for her today. Her story isn't defined by a single milestone; it instead highlights the return of everyday life… the moments that make life feel normal again… better sleep, greater independence, and the ability to fully participate in activities she had slowly stepped away from. If you're facing chronic back pain, been told your options are limited, or wondering whether disc replacement may still be worth exploring, Rachel's story serves as a hopeful reminder that being told "no" doesn't always mean the story is over, and that asking the right questions can open doors many people don't realize still exist.

This week we look at a Dan Savage column. The reader is bored with the sex, but also gets shamed from his wife for wearing a ‘gay' T-shirt. Dan's advice has some helpful ideas and affirmations (despite it being a short Q&A). How does the politics of the advisor affect the advice? What is the reader / advisor relationship? Then I offer my own extra advice. Are there other little microfascisms going on? How to talk about issues. How dis/inhibition is a problem and evokes Irving Yalom's ooze. What does thinking about affects, rather than causes and effects, do? It's a long one I'm afraid!
