A large, expanded-cohort pooled analysis of neoadjuvant immunotherapy for patients with resectable Stage III melanoma has reported very high rates of durable survival. The findings from the world’s biggest center of expertise in melanoma were announced at ESMO Congress 2024. The study included patients from clinical trials and real-world studies who had pure immune checkpoint inhibitor neoadjuvant therapy, or combinations including BRAF/MEK targeted therapy. After giving her talk in Barcelona, lead investigator Georgina Long, AO, PhD, MBBS, FRACP, Professor and Co-Medical Director at the Melanoma Institute Australia, University of Sydney, gave Oncology Times reporter Peter Goodwin the details.

Gulfcoast Ultrasound Institute YouTube Channel On this episode of The Sonography Lounge hosts Lori and Trisha sit down with Dr. Charlotte Derr & Dr. Allyson Hansen to discuss the GUARD Protocol (Gastric Ultrasound for Aspiration Risk Determination). With the increased use of GLP-1 Agonists (Semaglutide, with brand names such as Ozempic, Wegovy, Rybelsus, Etc) for both diabetes and weight loss, there has been an increased number of aspirations in patients undergoing procedures. One of the effects of GLP-1 agonists is delayed gastric emptying which keeps patients feeling "full" thus reducing the desire to eat and overall caloric intake and blood sugar variations. Typically 6-8 hours not eating is enough time for a patient about to undergo an elective procedure to digest and empty their stomachs thus reducing the risk of aspiration while under anesthesia. With patients on GLP-1 agonists, we are seeing gastric contents remaining for several days after the last meal which can increase the risk of aspiration.  Dr. Derr and Dr. Hansen, discuss how they are using ultrasound and the GUARD protocol to determine the volume of food in a patient's stomach before elective and emergency procedures. This quick and simple bedside technique can drastically alter a provider's pre-procedure course of action.  CHARLOTTE DERR, MD, RDMS, FPD-AEMUS, FACEP Dr. Derr is the Professor of Emergency Medicine and the Fellowship Director of the Advanced Emergency Medicine Ultrasound Fellowship Program at the University of South Florida Morsani College of Medicine in Tampa, FL and serves as the Co-Medical Director at Gulfcoast Ultrasound Institute. Sue Stanley-Green-BioSue Stanley-Green received her BS from The Ohio State University and her MS from Purdue University.  Prior to joining the Women's Tennis Association (WTA), she was the Athletic Training Program Director and Associate Professor at Florida Southern College in Lakeland, FL.   Her athletic training career included serving as Associate Head Athletic Trainer at the University of Kentucky, Head Athletic Trainer at Centre College, Physician Extender for Kentucky Sports Medicine Clinic, and Director of Medical Services for Intramural Recreational Services at East Carolina University.  Sue's service to the Athletic Training Profession includes currently serving as the chair of the NATA Honors and Awards Committee and as a member of the NATA Historical Commission.  She previously served as a member of the NATA Board of Directors, a two-time Director on the Board of Certification, President of the Southeast Athletic Trainers Association, Vice President of the Kentucky Athletic Trainers Society, and Co-Medical Director of the Bluegrass State Games.  Awards that Sue have earned include the NATA, SEATA, and ATAF Halls of Fame, NATA Most Distinguished Athletic Trainer Award, SEATA Award of Merit, SEATA Backbone Award, American Academy of Podiatric Sports Medicine Excellence in Athletic Training Award, and the Distinguished Athletic Trainer Award from the American Orthopedic Society for Sports Medicine.   In addition to working for the Women's Tennis Association, Sue is also an AT Spotter for the NFL.  Sue is married to Al Green, an athletic trainer also in the NATA and SEATA Halls of Fame.  They have a daughter, Logan Green England who is full time in the Kentucky National Guard.

Richard J. Schwab, MD, DABSM, Head of Sleep Medicine, University of Pennsylvania, and the Co-Medical Director, Penn Sleep Center, discusses the pathogenesis of obstructive sleep apnea, and obstructive sleep apnea treatment. Podcast Points: What is obstructive sleep apnea? What causes sleep apnea in some, when others seem to avoid it, and what can you do about it? How do soft tissues impact sleep? Dr. Schwab provides some background on his work and the various sleep problems that exist. He talks about sleep apnea, and why there is so much left to understand. Why does it happen when we sleep? He discusses soft tissue structures and lateral walls and other factors that can lead to airway paths collapsing. Dr. Schwab's extensive research seeks to target the pathogenesis of obstructive sleep apnea by using advanced upper airway imaging techniques. His studies help further explain the motion of various key structures of the upper airway and the role they play in airway closure. Dr. Schwab talks about the biomechanics of apneic events. He provides information on how they utilize magnetic resonance imaging and electronic beam computed tomography during sleep, as well as wakefulness, to study patients. Dr. Schwab talks about abnormal craniofacial structures as well as soft tissue, and how they can potentially impact sleep apnea occurrences. He discusses mouth breathing, studies they conducted on tongue fat, and how it all could impact breathing issues. As he states, if you naturally have a narrow airway, as movement occurs when you sleep, apnea could be initiated. He provides an in-depth discussion of how tissues move, and studies they have done on wakefulness. But he states there are more studies on sleep and breathing that they plan to do in the near future. Dr. Schwab, through his exhaustive research annually, collaborates regularly with members of the Departments of Radiology and Biomechanical and Computer Engineering. And together, the scientific researcher/developers have designed an extremely advanced, computer graphics-based analysis software that can assist with modeling, in three dimensions, of the biomechanical interrelationships that exist between soft tissue structures and the upper airway. A new melanoma vaccine which has almost halved the rate of recurrence is being hailed as cancer's “penicillin moment”. In an early stage clinical trial including 79 Australian patients, the personalised vaccine, made by Moderna, produced a 44 per cent reduction in the rate at which skin cancer returned. In addition to his clinical role, Dr. Kim conducts NIH-funded sports cardiology research at Emory. In this episode of the Female Athlete Podcast, I speak to sports cardiologist Dr Jonathan Kim. We start off exploring the new field of sports cardiology, and how it's evolved recently with rising awareness of heart conditions in athletes. Exercise results in adaptations to the heart that are normally beneficial, such as lower resting heart rates and increased cardiac muscle strength, otherwise known as “athlete's heart”, however, as Dr Kim explains, some changes in the heart can pose risks and indicate underlying problems. We discuss cardiac fatigue following strenuous events like a marathon, underlying the importance of taking enough rest to fully recover. It's not just older individuals at risk of heart problems, in fact, even the youngest and fittest athletes can be at risk. Dr Kim outlines causes of heart conditions independent of age and fitness related to genetic and inherited pathways: knowing your family history is the best prevention tool. Cardiac issues can also be acquired and athletes aren't immune! While exercise certainly is medicine, it is not a cure-all or vaccine against disease! This episode covers some of the warning signs of cardiac illness, many of which overlap with the symptoms of REDS: exercise intolerance, increased RPE, decreased performance and motivation, and increased fatigue. Chest pain is not always present! Working with a team of professionals -cardiologists, dietitians and psychologists- is important in understanding the cause of symptoms: REDS, a heart condition, and/or something else. Other more specific signs requiring medical attention include unexplained breathlessness, chest tightness when warming up (even if it goes away during exercise), and lightheadedness/ fainting. We touch on returning to exercise post Covid-19 for both asymptomatic and symptomatic cases, and Dr Kim provides reassurance and comfort debunking the extremely conservative advice that was initially circulating. By resting until asymptomatic for 3 days, before gradually progressing training load, the majority of people can return to their usual activity levels in just a few weeks (disclaimer: every case is different of course). In addition she has reviewed for prestigious journals, such as BMJ, JCO, Cancer, Cancer Prevention and Epidemiology, and served in several committees including her current position as the Co-Chair for the SWOG Prevention and Epidemiology Committee.Dr. Robson is the Chief of the Breast Medicine Service in the Department of Medicine at Memorial Hospital, New York, Attending Physician on Breast Medicine and Clinical Genetic Services, and a Member of the Memorial Sloan Kettering Cancer Center. He is an associate editor for the Journal of the National Cancer Institute and a Fellow of the American Society of Clinical Oncology (ASCO), as well as a past chair of the ASCO Ethics Committee. His clinical research is on the optimal application of germline information to the management of cancer patients. He has been a lead investigator for trials of PARP inhibitors in patients with BRCA mutation–associated breast cancer and is currently developing new models for the acquisition of germline information, including "mainstreaming" through test ordering by primary oncology providers and broad genomic screening in the context of somatic mutational profiling. In this episode you will hear from Professor Alan Cooper; a Clinical Dermatologist himself, who was diagnosed with stage III melanoma which progressed to stage IV.  In this episode, Alan is joined by his treating clinician - Professor Georgina Long.   Professor Alan Cooper Professor Alan Cooper was diagnosed with stage III melanoma in 2013, and despite multiple surgeries and two trials of experimental treatment it progressed to stage IV. The melanoma eventually responded to a combination of radiation therapy and immunotherapy which commenced in early 2017. At the time of diagnosis, Alan was Clinical Professor of Dermatology at the Northern Clinical School of the University of Sydney, and Head of the Department of Dermatology at Royal North Shore Hospital. Over the years he has served in a number of capacities in The Australasian College of Dermatologists and has been involved in the training and assessment of medical students and dermatology advanced trainees. He supervised a research unit and was involved in fund raising for dermatology research through state, national and international foundations. Alan is a Director of MSCAN.   Professor Georgina Long Professor Georgina Long is the Co-Medical Director of Melanoma Institute Australia (MIA), and Chair of Melanoma Medical Oncology and Translational Research at MIA and Royal North Shore Hospital, The University of Sydney. She leads an extensive clinical trials team and laboratory at MIA, with a focus on targeted therapies and immuno-oncology in melanoma. Professor Long is the author of over 370 peer-reviewed publications in clinical and translational research in melanoma.

Tompkins County has seen a surge in cases of COVID-19. There are 90 active cases in Tompkins County today, compared to just 4 two weeks ago. Data from the Tompkins County Health Department shows a rise in breakthrough cases too. All of this is largely being attributed to the more transmissible Delta Variant., but it's prevalence has not yet been determined. According to the American Academy of Pediatrics, 1 in 4 children in the United States are part of an immigrant family unit. By definition, immigrant children are those born outside the United States or who have at least one foreign-born parent. In the last decade, global conflicts have propelled the migration of these families, creating a more diverse migrant population and given rise to divisive sociopolitical policy in our country. Like all children, the experiences of this patient population are diverse and have direct impacts on their biopsychosocial development. Additionally, this patient population may face unique challenges of inequity that can threaten their health and wellbeing. In this episode, we will talk through considerations that pediatric medical providers can make that are uniquely supportive to the refugee and immigrant patient population. For this discussion we are pleased to be joined by a colleague from the broader pediatrics community in Denver, Janine Young, MD. Dr. Young is the Medical Director of the Denver Health Refugee Clinic; Co-Medical Director of the Denver Health Human Rights Clinic and Medical Advisor for the Colorado Refugee Services Program. About Kyle Getter Kyle Getter enters his fourth season at the University of Virginia and first as an assistant coach. Getter spent three seasons as UVA’s director of recruiting/player development. Getter joined the Cavaliers after serving three seasons as an assistant coach at Liberty under former UVA associate head coach Ritchie McKay. About Dr. John MacKnight Dr. John MacKnight is the Co-Medical Director for Sports Medicine at UVA and is the Primary Care Team Physician. He also serves as an associate professor at UVA for Clinical Internal Medicine and Orthopaedic Surgery. MacKnight is responsible for the comprehensive primary care needs of UVA student-athletes as well as the oversight of pre-participation medical screening and policy production to ensure the health and welfare of the student-athlete population.

In this episode, we welcome Dr. Edward Jauch, the Chief of System Research, at the Mission Research Institute/Mission Health System.  Dr. Jauch served as the Chair of Stroke Council for the American Heart Association/American Stroke Association and was the primary author for the 2013 Acute Ischemic Stroke guidelines. He is a co-author of the flagship American Heart Association / American Stroke Association (AHA/ASA) guidelines for telemedicine use in stroke, primary prevention of stroke, prehospital care of stroke, and stroke systems of care, as well as over 20 other AHA guidelines and scientific statements.  Collectively Dr. Jauch’s efforts have focused on improving access to care, increasing the quality of acute stroke care, and educating both the public and healthcare professionals on developing integrated stroke systems for care for all patients.Most recently, Ed was the primary author of the 2021 Recommendations for Regional Stroke Destination Plans in Rural, Suburban, and Urban Communities from the Prehospital Stroke System of Care Consensus Conference, which is the focus of today’s episode. This consensus paper provides local and regional EMS agencies and stroke advisory committees with guiding principles and recommendations for how to integrate the elements of a stroke system of care in three key regional settings: urban, suburban and rural settings. It also takes into account the differences in resources, hospital certifications, geography, and population density. After several months of data, we now have a slight understanding of the virus. We know the general trajectory of what to do when someone tests positive and begins to exhibit symptoms. But back in Summer 2020, we had no clue. Doctors, nurses, and other hospital staff were on their toes everyday trying to figure it out. In New York Presbyterian-Weill Cornell, one particular doctor, Dr. Leroy Lindsay, served as Co-Medical Director of the groundbreaking COVID Recovery Unit.Dr. Lindsay refers to himself as “air traffic control” in the hospital. Although it is already a tough job, when the pandemic hit, it only became more complicated. He discusses the many repercussions that people can face after surviving the virus, including lingering disruptions to the cardiopulmonary system and mental health. Tune in to this episode to hear straight from someone on the front lines about what this experience was like and about the COVID Recovery Unit at NYP.Dr. Leroy Lindsay obtained his medical degree from the University of Rochester. Richard J. Schwab, MD, DABSM, Head of Sleep Medicine, University of Pennsylvania, and the Co-Medical Director, Penn Sleep Center, discusses the pathogenesis of obstructive sleep apnea, and obstructive sleep apnea treatment. Podcast Points: What is obstructive sleep apnea? What causes sleep apnea in some, when others seem to avoid it, and what can you do about it? How do soft tissues impact sleep? Dr. Schwab provides some background on his work and the various sleep problems that exist. He talks about sleep apnea, and why there is so much left to understand. Why does it happen when we sleep? He discusses soft tissue structures and lateral walls and other factors that can lead to airway paths collapsing.  Dr. Schwab's extensive research seeks to target the pathogenesis of obstructive sleep apnea by using advanced upper airway imaging techniques. His studies help further explain the motion of various key structures of the upper airway and the role they play in airway closure. Dr. Schwab talks about the biomechanics of apneic events. He provides information on how they utilize magnetic resonance imaging and electronic beam computed tomography during sleep, as well as wakefulness, to study patients. Dr. Schwab talks about abnormal craniofacial structures as well as soft tissue, and how they can potentially impact sleep apnea occurrences. He discusses mouth breathing, studies they conducted on tongue fat, and how it all could impact breathing issues. As he states, if you naturally have a narrow airway, as movement occurs when you sleep, apnea could be initiated. He provides an in-depth discussion of how tissues move, and studies they have done on wakefulness. But he states there are more studies on sleep and breathing that they plan to do in the near future.  Dr. Schwab, through his exhaustive research annually, collaborates regularly with members of the Departments of Radiology and Biomechanical and Computer Engineering. And together, the scientific researcher/developers have designed an extremely advanced, computer graphics-based analysis software that can assist with modeling, in three dimensions, of the biomechanical interrelationships that exist between soft tissue structures and the upper airway.

Dr. White is re-writing history with the countless lives he is saving and cutting edge changes to the medical community.   While on vacation in Italy, White received a phone call in the midst of dinner. It was emergency medical services calling from Rochester Minn., about a patient in cardiac arrest. Without hesitation, White leapt into action and began directing the paramedics on what to do next. (Something he does anytime his Bat phone rings) In this show, we get an exclusive look into the life of a great American hero, Dr. Roger White.   More about Dr. Roger White Roger D. White, M.D., FACC, is Professor of Anesthesiology, Mayo Clinic College of Medicine, and a consultant in the Department of Anesthesiology as well as Department of Cardiovascular Diseases, Mayo Clinic, Rochester MN.  He is the Medical Director of the City of Rochester and Olmsted County Early Defibrillation Program. He is also Co-medical Director of Gold Cross/Mayo Clinic Medical Transport and Principal Investigator with the Mayo Clinic Therapeutic Hypothermia Program. He was chair of the Board of Directors of the National Registry of Emergency Medical Technicians in 1983 and again in 1994.  Dr. White is a member of the National Association of EMS Physicians, American College of Cardiology, American Society of Anesthesiologists, and the Minnesota Society of Anesthesiologists.  He is a reviewer for Resuscitation, Annals of Internal Medicine, Critical Care Medicine, Mayo Clinic Proceedings, and Circulation.   He is a fellow of the American College of Cardiology.  In 2004 Dr. White received the Hans Dahll Award at the Emergency Cardiovascular Care Update Conference.  On-going clinical research includes assessment of patient outcomes following cardiac arrest, evaluation of performance of biphasic waveforms in clinical settings, and therapeutic hypothermia after cardiac arrest. Dr. Roger White's bibliography includes 327 publications, the majority pertaining to emergency cardiac care.

Listen NowAs a follow up to my April 1st conversation with Jim Gera concerning bundled payments, during this podcast Dr. Richard Morel discusses Medicare's other major payment reform program, Accountable Care Organizations (ACOs), or WESTMED Medical Group's three year experience as a Track 1 ACO.   The Medicare ACO program is a creation of the 2010 Accountable Care Act and participation in the program began in 2012.   Currently, there are 434 ACOs (over 90 percent participating in the "no risk" Track 1) caring for approximately 7.5 million Medicare beneficiaries.  The program to date has been a mixed success.  After two performance years (2013 and 2014) only 25 percent of participants have been successful, i.e., have earned shared savings.  (Performance year three or 2015 performance will be made known this September.)  CMS is currently in the process of revising how the agency calculates an ACO's reset financial benchmark.  It is anticipated these changes will improve program performance, or improve both provider interest in participating (or continuing to participate) in the program and participant success in earning shared savings.   During this 21-minute conversation Dr. Morel provides an overview of WESTMED, explains the organization's interest in becoming a Medicare Shared Savings Program or ACO participant in 2013, WESTMED's experience under their first three year agreement, what explained their success, challenges they've found with the program, how the program could be improved and their expectations now as a second agreement period Track 1 ACO.Dr. Richard Morel is the Co-Medical Director of WESTMED Medical Group in Yonkers, New York.  Prior to joining WESTMED in 2008 Dr. Morel was in private practice affiliated with Columbia-Presbyterian Riverdale Hospital for 12 years.  Dr. Morel is board certified in internal medicine.  He received his medical degree from Columbia University College of Physicians and Surgeons, did his postgraduate training at Columbia-Presbyterian Medical Center and  received his masters of medical management from Carnegie Mellon.  He is a fellow of the American College of Physicians and a member of the American College of Physician Executives.  For information regarding WESTMED go to: http://www.westmedgroup.com/. Of all gynecologic cancers, ovarian cancer is the number one cause of death. Women with an inherited BRCA1 or BRCA2 mutation can have a 15% to 40% risk of having this deadly disease, compared to a risk of less than 0.2% in women in the general population. Dr. Karen Lu, Professor of Gynecologic Oncology and Co-Medical Director of the Clinical Cancer Genetics Program at The University of Texas MD Anderson Cancer Center, says knowing the BRCA status helps better define a woman’s risk of ovarian cancer.
