Acute Care for Elders (ACE Units) have been around for over a quarter of a century. Randomized trials of ACE units date back to 1996 when Seth Landefeld and colleagues published a study in NEJM showing that they improve basic activities of daily living at discharge and can reduce the frequency of discharge to long-term care institutions. But if ACE units are so great, why do so few hospitals have them? On today's podcast we talk about ACE units with geriatricians Kellie Flood and Stephanie Rogers. They recently published a paper in JAGS looking at the current landscape of ACE units in the US. In the podcast we go over these issues and more: What are ACE units and what structural elements go into them (see the picture below for a nice summary)? Which patients are eligible to go to an ACE unit? What are the benefits of an ACE unit? If ACE units are so great, why are they not so common? Daniel Levin is an award-winning writer living in Los Angeles. He holds an MFA in poetry from the University of California, Irvine, where he taught creative writing and rhetoric. His writing has appeared in Provincetown Arts, The Sarah Lawrence Review, The Westchester Review, and The Bennington Review. During his time at Sarah Lawrence College Dan, as a 19-year-old sophomore got involved with Larry Ray the leader of what turned out to be a dangerous cult. Dan has written about these harrowing experiences in his new memoir SLONIM WOODS 9 A stunning firsthand account of the creation of a modern cult and the costs paid by its young victims. In the first half of their two-part conversation, Daniel shares with Rachel how he first came into contact with Larry Ray the dangerous cult leader, currently on federal trial for sex trafficking, extortion, conspiracy, and a string of other crimes. He discusses how Larry was able to evade detection on campus even while living in dormitories. Together Rachel and Daniel examine what he was feeling as he fell under the control of this manipulative and dangerous man, pointing out the red flags and explaining what prevented him from seeing them at the time. Anderson Center’s executive and artistic director Stephanie Rogers has got her tickets for Champagne Confetti at the Sheldon Theatre in Red Wing, Minn., and she highly recommends you join her. Champagne Confetti is the brainchild of Aby Wolf and Eric Mayson, and it brings together some of the finest musical talent in the Twin Cities to create music that defies categorization. You can hear classical, jazz and pop all at once in their tunes. Champagne Confetti performs at 7:30 p.m. on Saturday, Feb. 29. Poet and English professor Doug Green thinks you should check out “From Puerto Rico … With Love?” The show, which consists of two solo performances, is being remounted by Teatro del Pueblo Thursday through Sunday at the Avalon Theater in Minneapolis. Green says both performances will make you laugh, and will leave you smarter than before when it comes to this unincorporated territory of the United States. Music lover and occasional gospel singer Marie Denholm is looking forward to "The Sounds of Gospel" this Saturday at Bethel University. The concert, directed by Jevetta Steele, traces the evolution of gospel music and features some great vocal talent. Denholm says it’s a great way to mark Black History Month while enjoying some powerful, uplifting music.
You're the attending physician on a teaching service. Your resident says we shouldn't order a CT because CT's are over-used for this condition, and represent overuse, waste, and low-value care. In this case, however, you suspect that's not the resident's real reason. The real reason behind the resident's decision is that they are serial minimizers - residents who make little of potentially important findings. You feel they might be hiding their minimizing behind the sexy, trendy notion of providing "high value care." Does this sound familiar to you? It did to me. I've been in the awkward situation of being the consulting palliative care physician saying to the primary team, I know they have cancer and that's the most likely explanation for this abdominal pain. I also worry that this pain is out of proportion and different from other pain I've seen, and I think it should be evaluated with further testing. Kind of strange to say that as the palliative care doctor. Kind of strange as well to hear that perspective exposed by Chris Moriates and Vinny Arora, who spend most of their academic careers fighting against wasteful low-value tests and treatments (they run a non-profit called Costs of Care). To be sure, they note this problem is not as great as overuse of consultants, tests, and treatment. The challenge, as Stephanie Rogers our guest host (and guest fiddle player!) points out, is coming up with the right words to teach "right sizing" care to the patient in front of us. And what are the root causes of minimizing? Blame the house staff would be the easy way out, but Chris and Vinny don't take that road.
