As you all know, the vaccines were already approved during the Holiday season last December 2020 and the distribution has already started. However, based on my experience, there are some lapses about it and I wanted to talk to you about that.     This is an entirely different topic for my podcast, but I do believe that there is a relation between family personalities and dealing with health care issues within the family.  Some of the interesting points Sandra brought are: How she helps all family members understand their personality types Once you understand who they are (your kids) on a core level better so that you can honor them for who they really are.     #12 MTM ~ Medication Therapy Management

    This is a lengthy and very informative episode, with a very close to my heart colleague. Swapna Chakrabarti, RPh.  We are talking about Medication Therapy Management and how patients, caregivers, and family members can benefit from it.  Since it is a service included on Medicare part D plan, if you have more than 5 medications or more than 2 chronic diseases, you should demand it Some of the most relevant points we made are: who is this episode for? This is for patients that are Medicare Part D, and have all the benefits of their insurance because this type of service is included. Not free, but included in what you already paid. So it's no extra payment. If you are a caregiver for somebody that has Medicare Part B and has medication therapy management as a service as well. And for everybody else that has a family member, a loved one, a parent, a grandparent, an in-law that might be having or that could have issues with their medications, and you know that they need help, but you don't know what to do "I went to pharmacy school in India. And we came here for graduate school, not as a practice not oriented towards practicing pharmacy but more towards the research. So I came to Pittsburgh for my masters and did my masters in medicinal chemistry and I hated it. I don't know why. But then what I know why because when I was in pharmacy school, the aspect of the medicinal chemistry, which is basically not just the synthesis of drugs, but how the drug actually works in your body, at the chemical level was so fascinating". It's hard, It is too it's hard. It's complicated. It's not the pharmacist that you see dispensing, filling up the vials with your medication. That is probably the last step in a long series of steps that we have taken to understand your medication. And that's not the only thing we do In the healthcare professions, Pharmacists actually top the lists of the most reliable healthcare professionals and that is definitely a historical reason for it. But we definitely want you to know, that we are not in there just to make money. So then we just fill your prescription bottles and get you out of there as soon as possible. That is not what We want to do so don't look at us through that lens. We are people, we are compassionate, we want to help you. So if you have to wait a little bit longer to get your prescription at the pharmacy, that's probably a reason for it. I think it's one of the noblest professions because we are there to help people use our expertise. But yeah, we are people too, we get our frustrating this we have. So when you are dealing with treating them, as we would expect a pharmacist to treat you kindly treat them kindly to as human beings, it's we are not just in there, just to save face just to make money off of the prescriptions that we are filling for you. ******************************************************************** If you liked this episode write us a review and recommend and share it with those that might need or want this information. Well, WRONG! There is A LOT of fears that we keep in our mind, from as far as childhood, and unfortunately, they could affect the rest of our lives. That is why I invited my Executive Coach to discuss it. We did not only have fun but also we covered a lot of ground on this subject.  Dr. Carla Michelle is an executive coach and a family therapist. Among other stuff we talked about: Motivation is a Motive with an Action Confront your fears. You are missing out! Communication is key, not just about a plan, but communicating your feelings and your emotions and relieving yourself of that stress, or those stressors that come with life. Have a discovery call with you, and how we can how you see this event, being of service to you how I can be of service, and another therapist could be a service to you. We have coaching sessions.     #8 Pre-Diabetes ~ Dr Theresa Morrow

    We all have heard of Diabetes, but do you know what Pre Diabetes is? Well, we have Dr. Theresa Morrow, PharmD, who is an expert in this area and is going to educate us, with her sweet personality and her very relatable story. She is also developing a workshop where she will guide, support, and instruct you on what to do after you get diagnosed as Pre-Diabetic. Some of the most relevant points she made are: I'm just a Coal Miner's Daughter. I grew up in South West Virginia, that is rural, Appalachian Virginia. It's at the corner in about 30 minutes from the state of West Virginia. But when I graduated, there was great shortage of pharmacists We mix that IV medicine so we have a sterile room just like it's an operating room. We mix the medicine in that sterile room. And then we package it and deliver it to the patient's home where they can actually have the medicine at home so they can get better over time And I'm trying to do some stuff on the side with what I really care about what matters to me, which is the pre-diabetes I am working on creating a workshop called Pre-Diabetes Plan. And in that plan, I want to help people with pre-diabetes. And I have a passion for that because I have pre-diabetes. I've had it for 10 years, and I have such a struggle. When you're of a certain age like I am, and you have insulin resistance, and so many things that contribute to the weight gain, it's just very difficult. And it's discouraging, and you feel like you feel hopeless. If you're overweight, you probably should be checking it much younger and there are screening recommendations on when all that should be done. So if you don't know what your A1C is, I would say talk to your doctor and see if you qualify for that. She had three toes amputated before she was ever diagnosed as a diabetic. Which, you know, we kept saying she's got to be a diabetic and they can't say No, she's not. No, she's not. And you know, so now I hope today that would never happen. But still, that is an example that shows you can't just look at one point in time about what your blood sugars are doing because it does fluctuate and when it fluctuates, it can damage the nerves in your body and affect your kidneys and so many things that are affected...

    This episode is quite informative and a bit deep into the pharmacy world for those that are not part of it. I believe the information that Sharzad had shared with you is going to be useful and an eye-opener for your own health and the health of those you love. If you get the chance to work with her, DO NOT DOUBT IT! She is sweet, has so much compassion, and SO KNOWLEDGABLE!!! Some of the most relevant points she made are: There are times that women think that oh, well, this is how it's supposed to be or my mom went through it the same way or No; my neighbor didn't have any symptoms. So maybe I'm just kind of a hypochondriac and thinking that there's something, you know, wrong, where it's not. I meet with the patient talk to them, I go over their past medical history, their family history, risk factors for developing a different disease, their symptoms, their blood work. I talked to them about hormonal and non-hormonal ways to help improve their symptoms that may include lifestyle modifications, it may include supplements, I do, go over their supplements, make sure that they're taking the right things that are needed. For example, a postmenopausal woman may be at higher risk for developing bone loss and break your bone osteoporosis, right. So we want to make sure that they're taking the calcium, magnesium, vitamin D, etc. I appreciate the opportunity that my patients have given me over the past 20 years, I've probably done close to 20,001 on one consultation with patients Women who experienced vaginal dryness because their hormone levels have dropped, oftentimes end up having bladder problems, such as a leak, Urinary tract infections. Yeah. So things that are related to the bladder, they constantly have to go to the bathroom.     You are going to LOVE this episode!!!! Compounding Pharmacies are essential for a lot of us. We need to know what they can do for you, how it works, and what alternatives do they offer, so we (and our loved ones) get what we need! Some of the most relevant points she made are: We can in a compounding lab, add special flavors that make it more palatable where you can actually not taste the bitter awful taste of a drug like that. We can make it great marshmallow flavored, or tutti frutti flavored or chocolate mint flavored, etc. So we can improve palatability. If you're gluten intolerant, we make sure to make the product with not containing any gluten sources at all. lactose intolerant, if you can't swallow pills and we'll make it as a liquid or a suppository. If you needed applied topically, we can apply it topically. topical pain creams are a big mover in our compounding pharmacy. And that can combine more than one ingredient that helps to relieve chronic pain into a cream that you rub on externally to relieve that pain.insurance companies today rarely cover compounds. They just figured out, I'm not going to have to pay for this. I don't want to pay for this. So I'm going to say not covered. So often they do not cover But if it's not going to be covered by the insurance and the custom preparation is necessary for that patient, we work with the patient to make it the most affordable we can if the doctor prescribed a very expensive, active pure chemical ingredient, and there's a less expensive one for example, that would work equally as well. We Contact the doctor and ask if we can substitute it to make it more affordable for you the patients.Most of our compounding prices for a one month supply range in the 40 to $125 range for a one month supplyJust like in the food industry. We want you want a pure food product. In a restaurant inspectors inspect restaurants to make sure the kitchens are clean, and they're being cooked in a in a safe and sanitary measure. Because when you are going through the time that we females go through, that we have lower level of progesterone, estrogen and testosterone, we need a replacement, but not everybody has the same needs. So if you start taking stuff that are commercially available, you might not have exactly what you need Similar lab inspections can occur in compounding labs, by state boards of pharmacy or the Food and Drug Administration. Hormone replacement therapy is one of our very big movers; males and females a lot we compound products like estrogen or progesterone or testosterone into a custom made preparation. There are commercially available alternatives for those some are oral some are topical, but they're only made in a very small range of strengths. So for example progesterone capsules for women are only made in a 100 and a 200 milligrams and that's it. But perhaps you the patient can't tolerate the hundred-milligram strength it makes you too sleepy When you are going through the time that we females go through, that we have lower level of progesterone, estrogen and testosterone, we need a replacement, but not everybody has the same needs. Also, I give you a general view of what we pharmacists do at a hospital, how we collaborate with other health care specialists and how every-single-patient in a hospital NEEDS a pharmacist! Some of the most relevant points of these episodes are: Independent retail pharmacies are small local businesses  They are focused on community health, simply because the owners are part of that community. Independent pharmacies have more space and time for the patient to interact with the pharmacy staff These pharmacies normally have space for consultations with their patients and for other activities like vaccinations and Medication Therapy Management  The time these pharmacies dedicate to their patients in 3-4 times longer than what big chain does.
