Explore a complex case of Thoracic Outlet Syndrome, double crush, & triple crush injuries. Learn about diagnosis, treatment, & patient support. Q: Overview of Neck, Nerve, or Both? A: My wife experienced numbness in her upper extremities. As an athletic trainer (AT), I initially suspected Thoracic Outlet Syndrome (TOS). She eventually sought medical attention and was diagnosed with TOS. Conservative treatment, including medication and a home exercise program, was recommended. However, therapy seemed to worsen her symptoms. An MRI of the vascular system came back normal, except for a benign thyroid mass. A cervical MRI revealed stenosis in C5, C6, and C7, which, when combined with her other symptoms, provided clarity. I consulted a spine surgeon, who advised exhausting conservative options before considering surgery for insurance coverage. My wife, who has a fear of anesthesia, opted for pain management and received injections. An EMG to rule out carpal tunnel syndrome indicated 70% carpal tunnel involvement. An upper extremity specialist diagnosed a double crush injury, meaning nerve compression at two sites. She underwent left-side carpal and cubital tunnel release, followed by a two-level cervical fusion. Post-surgery, she regained sensation in her pinkies. The right side was subsequently treated. She experienced relief for about a year before numbness recurred. Another round of CT, MRI, and EMG scans returned normal. Pain management suggested a shoulder issue, leading to an MRI of the shoulder. While the report mentioned a slight rotator cuff tear, a physician who reviewed the scans deemed her shoulder pristine. This doctor’s examination of her neck, however, exacerbated her symptoms, leading to a diagnosis of lower brachial plexus TOS. Despite my initial concern about the previous treatments, the doctor affirmed that the cervical fusion was necessary. A C8 nerve block did not provide relief but offered further diagnostic information. We were then referred to a specialist in Dallas, where a diagnostic nerve block in the scalenes provided immediate relief. Months later, Botox injections were administered for extended relief. The doctor’s words, “How does it feel to not be crazy?” significantly validated her experience. She then underwent a first rib removal on the left side, which resulted in an 11-day hospital stay with complications including two chest tubes, a needle aspiration, and 100cc of fluid in the pleural cavity. Upon returning home, she began physical therapy but developed shortness of breath with deep breaths due to a costochondral fracture, likely from the chest tube or aspiration. She is currently awaiting a consultation for ultrasound injections to address this. The journey continues. Q: What inspired you to share this story? A: The primary motivation was to share the complex journey and its takeaways. This case involved a “triple crush” — compression at the first rib, cubital tunnel, and carpal tunnel. A key takeaway is the importance of acknowledging when you “don’t know, but know the next step.” In healthcare, we often focus on obvious issues, but a broader perspective is often needed before returning to specific concerns. It’s also crucial to acknowledge the psychological toll on patients without answers, highlighting the profound impact of chronic pain. Q: You mentioned how emotionally draining the journey was. What advice do you have for clinicians to support patients? A: Remind patients that recovery is an “ultramarathon, not a sprint.” Reinforce this message, as other healthcare professionals will likely echo it. Sometimes, when my wife is in pain, she doesn’t want to discuss it, and that’s acceptable as long as I am there to support her. The doctor’s validation, “How’s it feel to not be crazy?” significantly improved her emotional well-being. Supporting patients in seeking further opinions is also vital. Q: Overlapping issue on a personal level, how did you navigate the multiple diagnoses? A: My ability to navigate this well stems from my access to top medical professionals through my profession. My connections as an athletic trainer allowed me to consult doctors and seek referrals. While they may not know specific TOS treatments, they can guide us to the appropriate specialists. Q: Any difficulties separating the Athletic Trainer (AT) role from the husband role? A: There were no difficulties in separating the AT from the husband role. My wife knew me as both from the beginning of our relationship, so there was no separation or conflict. Reinelda Estupinian,known lovingly as Reina, joins us again to share rare family photos and her firsthand memories of life inside the Roddenberry household. Hired in 1974 as a nanny for six-month-old Rod Roddenberry, Reina became a trusted part of the family for over three decades, eventually working closely with Majel Barrett Roddenberry at Lincoln Enterprises. Reina shares candid and touching stories of those years: traveling with the family, managing Majel's whirlwind convention life, and offering emotional support through Gene's declining health and Majel's final days. In each dystopian future, life is awful but everyone agrees the best way to get through it is to watch people get hunted down in the titular game show! Arnie's a cop who took a while to realize he was a baddie, while Glen Powell is an ordinary guy who wants to do the right thing but forgot to get his anger issues fixed. On the way we'll meet side characters of varying wackiness, hostages of varying acting abilities, and satire of varying levels of focus! This week on The Trek Files, we take a mid-season dive into the pre-internet letter-writing era of fandom with a look at Interstat #109, a Star Trek "LOC-zine" published just after the announcement of The Next Generation in late 1986. With Mission Log host and producer John Champion joining Larry, we revisit that first wave of passionate, skeptical, and sometimes prophetic reactions from fans adjusting to the idea of Star Trek without Kirk, Spock, or the original cast. From fears about recasting to early excitement about a new crew, these letters reveal a fandom both resistant to change and deeply hopeful for Trek's future. And while the tone may be more thoughtful than today's drive-by social media culture, the underlying emotions haven't changed. Together, John and Larry reflect on how fandom evolves, why backlash often masks deep investment, and how publications like Interstat helped shape the Trek we know today—slowly, one stamp at a time.

The lack of time to tell stories is painfully evident in "The Examples". Things Discussed: Bryce Underwood: Progress isn't linear. Jordan Marshall and the offensive line: The redshirt freshmen will save us! They're already playing at close to a Big Ten level. Defense: Purdue was very college crappe. I asked them to be vanilla, they were, and I'll take my lumps. Mad at special teams for not putting them back enough because the way Purdue plays they need to be able to take a TFL or a sack, or need to have a 4th down strategy. We're rotating an extreme amount, also there are extreme levels of injury: Any team that loses Rolder, Sullivan, and Barham isn't going to be great at LB. Protected the kids by blitzing them. Vance disappointed in the DTs: they were in the wrong gap sometimes on those LB blitzes. I saw Pierce do that once in the 1st half, haven't gotten to the 2nd half yet. DTs to me are Benny & Pierce, then a big gap to a lot of innings eaters, where Etta is more of a pass-rush option and Payne, Williams and Ike. You're playing a tempo team and they're only in there for three snaps. Running the DTs on and off the field is wearing them out as much as playing. Three snaps is three deadlifts of a 300-pound guy, then you're sprinting a 300+ pound body off the field. Too much rotation: They got on a 19-play drive with a quarter break and at no point in that drive did Michigan have their best available players on the field at over 50% of the spots. Brandt is not on Moore's or Guy's level. Ratios are way off. Brandt plays as much as Derrick, more than Guy. Are guys going to leave if they don't get PT? That's true but 1) That doesn't explain having starters playing 40% of snaps, 2) That's not the culture of this program. The culture of this program wasn't "You get yours" it was "Egos don't matter as much as the Team." That 2023 team had multiple "starters" because Harrell and Stewart could both play at a high level, sophomore D-Mo and McGregor could both play at a high level. If it's a program goal to get guys snaps against Purdue, go and beat Purdue first and then when you're up three touchdowns get your guys in. Offense and Bryce: He's young and doing young guy things like fleeing the pocket and forcing throws he's used to throwing. He had a terrible half and a decent half. Can't really blame Chip because he's giving him the reads, e.g. a wheel to Klein that takes advantage of Purdue taking away screens, and he throws it at Klein's feet. Also: he doesn't have receivers who make those plays for him when they're there. Klein should have caught that ball. Semaj had another one whistle through his hands. McCauley didn't adjust to a ball in the endzone. Go watch any freshman QB and you see the same things: Watch JJ in 2021. Watch Gardner when he was young. Go under center? Might help the run game but Crimes Against Man Panda. Craig talks about the 1939 Bears. Men's Hoops: Oakland was bad. Open corners were just there, they were missing two players, run a complicated zone with a new team. Trey McKinney is a knockdown shooter, and he has a little Darrius Morris (RIP) in his game. He's not Mike Smith, won't have short-arm turnovers. Good sign he can use his long arms productively. Cadeau: Wonder if he's going to have more TOs against Wake Forest. If he can keep turnovers under control this team will be very good. Defense has a very high floor. Michigan has three bigs instead of two this year, means their second-best lineup won't be a massive drop from their best. Didn't have Cason, might be that sixth guy of the year (or your starting two?) who can be Plan B for point guard and competently relieve Roddy when they full-court press him. Solution for press: Mara? Just have the freaky tall guy pass it without putting the ball on the floor. Women's Hoops: Very interesting team. They're five-out, swarming. Lots and lots of turnovers.
