Nederland kan jaarlijks ruim 350.000 ziekenhuisopnames en bijna 500.000 consulten voorkomen door patiënten thuis te volgen in plaats van in het ziekenhuis. De technologie is er, werkt en groeit hard, maar de brede opschaling komt moeizaam van de grond. De grootste obstakels zitten niet in de zorg zelf, maar in de manier waarop die wordt bekostigd en in de versnipperde uitwisseling van patiëntgegevens. In deze aflevering van BNR Beter bespreekt Nina van den Dungen de opkomst van thuismonitoring en de virtuele verpleegafdeling. Te gast zijn Michael van Herwerden, internist-intensivist bij het Erasmus MC en initiatiefnemer van de virtuele verpleegafdeling, en Jan Willem Spijkman, sectorbankier gezondheidszorg bij ING en medeauteur van een themastudie over thuismonitoring in ziekenhuizen. Van Herwerden legt uit hoe de virtuele verpleegafdeling werkt: de diagnose wordt in het ziekenhuis gesteld, maar het herstel verplaatst naar de huiskamer. Patiënten meten thuis een paar keer per dag zelf hun bloeddruk en zuurstofgehalte en vullen een vragenlijst in; een gespecialiseerde telenurse beoordeelt die waarden binnen het uur en schakelt bij afwijkingen de behandelend arts in. Bloedafname loopt via een regionale prikdienst, en zelfs een infuus met antibiotica kan thuis, met hulp van een technisch thuisteam. Het Erasmus MC blijft medisch verantwoordelijk alsof de patiënt is opgenomen. Zorgpaden voor onder meer hypofyse-operaties, bacteriële infecties en hartfalen laten zien wat al kan en waar de grenzen liggen, bijvoorbeeld bij medicijnen met een nauwe marge. Spijkman schetst de urgentie: het personeelstekort loopt op van zo'n 70.000 vacatures nu naar ruim 240.000 in 2034, terwijl de zorgvraag door vergrijzing en multimorbiditeit blijft groeien. Ongeveer de helft van de ziekenhuiszorg zou thuis kunnen plaatsvinden. Toch remt de bekostiging de opschaling af: een patiënt die naar het ziekenhuis komt, levert via de diagnose-behandelcombinaties meer op dan een belletje of een meting op afstand. Volgens Spijkman ligt de sleutel bij afspraken tussen ziekenhuizen en zorgverzekeraars, zoals een aanneemsom voor complexe zorg, zodat ziekenhuizen ruimte krijgen om zorg thuis op te schalen. Transitiegelden uit het Integraal Zorgakkoord en de komende doorbraakmiddelen van VWS moeten die verschuiving verder aanjagen. Ook de uitwisseling van patiëntgegevens komt aan bod. Nederland koos ooit tegen een landelijk EPD, wat volgens Van Herwerden een grote misser was. Waar België en Finland met systemen als Kanta vooroplopen, werken initiatieven als Digizorg, Zorgviewer en CumuluZ hier nog aan een landelijk dekkend netwerk, met de Europese EHDS-verordening in 2031 als stok achter de deur. Lose your job at 25 and someone will help you find another. Lose it at 55 and the talk quietly turns to how you might wind down towards retirement.Policymakers tend to assume job search training works for the young and not the old, so they rarely spend money trying. Bas van der Klaauw (Tinbergen Institute) thinks they got that wrong.In this week's VoxTalks Economics, he tells Tim Phillips about a Dutch experiment that put older unemployed workers through an intensive programme built on one idea: teach people over 50 to find work the way younger workers already do, by working their social network.Participants left unemployment faster, there was a 10% increase in job finding, and the savings in benefits more than covered the cost. The catch: it helped the better educated most and was tested in a recession. Her earlier work with van der Klaauw includes a study of how cutting the unemployment insurance entitlement period affects job finding.Research and concepts discussed in this episodeOlder workers and long-term unemployment. Older unemployed workers tend to have job finding rates around half those of younger workers, and during the recession the study covers, more than half of older job seekers risked becoming long-term unemployed. Van der Klaauw attributes the gap to a combination of factors rather than any single cause: more generous and longer benefit entitlements that weaken the incentive to take a lower-paid job quickly, and employers who favour younger hires expected to grow with the firm over a longer horizon.STEP (Successfully to Employment Program). A Dutch job search assistance programme developed during the post-2008 recession for unemployed workers aged 50 and above who had not found work within a few months of claiming unemployment insurance. It ran to 10 group sessions of around four hours each plus two individual meetings, covering interview practice, CV writing and social media, with a particular emphasis on activating the participant's social network. Participants were encouraged to have at least one conversation a week with a contact about possible work. The programme cost roughly 470 euros per participant.The experiment. The study covers everyone aged 50 to 63 who entered unemployment insurance in the Netherlands between November 2014 and July 2015 and remained unemployed for three months, about 50,000 people. Assignment to treatment or control was based on the last digit of the social security number, putting roughly 20% in the control group. Because participation was voluntary (an encouragement design), the authors report both the effect of being offered the programme and, using random assignment as an instrument, the effect of actually taking part. Around 54% of those in the treatment group took up STEP.What it did to job search behaviour. The job application register lets the authors watch how people searched. Participants made fewer applications to posted vacancies and did more networking, exactly the shift the programme was designed to produce. The change in method did not raise the number of job interviews, but it was accompanied by faster exits from unemployment.Cost effectiveness. Participation cut cumulative benefit payments by about 715 euros within 18 months, comfortably above the 470 euro cost, making STEP cost effective for the benefits administration. For participants, the lost benefits were almost fully offset by higher earnings from working sooner, so there was no large income gain to the individual, but no loss either.Who it helped. Effects were strongest for the better educated, those with higher pre-unemployment earnings and those not previously working through a temporary work agency. There was little or no effect on the lowest educated, who also had the lowest take-up. The authors find no significant difference by gender or by age band within the 50 to 63 range.Trainers and group composition. Effectiveness varied significantly across trainers, but no observed characteristic (gender, age, experience, contract type) explained which trainers did better. Group composition mattered too: participants did better when their group contained some lower-educated members, which argues for mixed rather than streamed training groups. One reading is that trainers may concentrate their attention on the more employable members of a group.Does it generalise? Two caveats. The programme was evaluated in a recession, when people were losing jobs through no fault of their own (frictional unemployment), and it may do less when work is easier to find. And it was designed for that kind of unemployment, not for the structural problem of workers whose skills no longer match available jobs, where van der Klaauw suggests training or retraining, rather than job search help, is the relevant tool.

Soviet soldiers forcibly packed her and her mother onto a train with thousands of their neighbors and deported them to remote Siberia, leaving them stranded to survive the brutal winter in subhuman conditions. Looking back, Ida shares their struggles: foraging for food, trying to reunite with her imprisoned father, spending weeks in a desolate hospital with typhoid fever, and adapting to shifts in the political climate to make the long journey home to Poland. Ida published this acclaimed memoir in her native Polish in 2011. Here, Ida's granddaughter, Isabella Skrypczak, translates her babcia's words and provides additional context—including describing the remarkable life Ida has gone on to live as a pioneering doctor. In the vein of Anne Frank's The Diary of a Young Girl, A Polish Girl in Siberia: Surviving and Transcending Exile (Disruption Books, 2026) chronicles Ida's experiences on a lesser-known front of the Second World War. In deze aflevering neem ik je mee in een week vol overdenkingen over tijd, zorg en wat we nu eigenlijk écht nodig hebben om te floreren. Tyrza zit in haar laatste weken van groep 8 en de mails over kamp, de musical en het afscheidsfeest vliegen me om de oren. Waar ik vorig jaar nog moeite had met het idee dat ze naar de middelbare school zou gaan, voel ik nu dat de tijd rijp is. Alles komt wanneer het zover is.Dat brengt me bij een prachtig Surinaams spreekwoord dat we tegenkwamen voor een schoolproject over cultuur: "Jullie hebben de klok, wij hebben de tijd." Dat raakte me enorm. In onze kapitalistische maatschappij rennen we onszelf vaak voorbij, terwijl de kunst van "No Spang" (maak je niet druk) juist zo waardevol is. Ik merk dat ik zelf ook vaak in die ratrace zit, ondanks dat ik een deel mijn eigen agenda probeer te bepalen.Izaï gaat naar groep 6. We hebben gesprekken op school over hoe we hem het beste kunnen helpen. Hij heeft zoveel potentie, maar in het huidige schoolsysteem loopt hij soms vast. Het zou zoveel helpen wanneer we kinderen écht kunnen bieden wat ze nodig hebben.Ik ben zelf op onderzoek uitgegaan en heb educatieve kaartjes gekocht om met Izaï te kijken wat hém helpt om zich gezien te voelen. Hij koos kaartjes als "inzet is goed" en "gaat steeds beter". Het deed me beseffen dat we in de basis allemaal hetzelfde nodig hebben om te kunnen groeien: veiligheid en gezien worden voor wie we daadwerkelijk zijn.Of het nu gaat om Izaï op school, ikzelf in mijn drukke agenda, of mijn rol als hulpouder (ook al weet ik niks van de regels bij tennis!): we doen het samen. Want zoals ze zeggen: it takes a village to raise a child. Ik blijf leren, elke dag weer, en dat is precies wat mij laat floreren.

Deze aflevering begin ik met een stukje zelfreflectie. Ik heb feedback gekregen dat ik vaak "eh" zeg en ik probeer daar in deze aflevering heel bewust op te letten door stiltes te laten vallen en rustiger te ademen. Het laat zien dat ook ik nog elke dag leer en groei, precies zoals ik dat ook in het ouderschap doe.Voor mij betekent bewust ouderschap niet dat alles perfect gaat—ook ik schreeuw wel eens of maak fouten—maar het gaat om de bereidheid om in de spiegel te kijken. Ik focus me op het 'lezen' van mijn kinderen: wat proberen ze met hun gedrag te communiceren? Ik vind het belangrijk om een veilige omgeving te creëren waarin fouten maken mag. Of het nu gaat om leren lezen of blijven zitten op school, de prestatiedruk moet eraf zodat een kind gewoon kind kan zijn.Een groot deel van deze aflevering gaat over onze zoon Izaï en waarom we een "eetfeestje" vieren. Izaï heeft een geschiedenis met sondevoeding en een moeizaam groeiproces. We hebben de afspraak dat we bij elke bereikte mijlpaal in kilo's een feestje geven. Nu hij de 22 kilo heeft aangetikt, gaan we met vriendjes naar de cafetaria om dit succes te vieren. Het gaat hierbij niet om de cadeautjes, maar om het vieren van zijn groei op zijn eigen tempo.Ook blik even terug op de zware tijden: de ziekenhuisopnames en operaties. Hoewel het heftig was, heeft het ons gevormd tot de ouders die we nu zijn. Ik ben dankbaar voor de 'opening van mijn hart' die dit proces heeft teweeggebracht. Het heeft me geleerd om onvoorwaardelijke liefde te tonen en mijn kinderen te steunen in wie ze zijn, ongeacht hun prestaties.Ik sluit af met de boodschap dat geluk en hoe je je voelt je eigen verantwoordelijkheid is. Je begint bij de "man in the mirror". Ik hoop dat ik mijn kinderen meegeef dat zij zelf bepalen hoe ze zich voelen en dat een ander daar geen macht over heeft.Kortom: wees lief voor jezelf, vier de kleine en grote successen, en durf kwetsbaar en eerlijk te zijn in je groei als ouder en als mens.

Kroz priču o balansiranju između velikih životnih prekretnica, kao što su vjenčanje i uređenje naše prve nekretnine, te vođenja biznisa, otkrivam kako su se privatni život i poslovni život ispreplitali na načine koji su ponekad bili emocionalno iscrpljujući. Ipak, moja vodilja kroz sve te procese bila je riječ "igra", koja me podsjećala da si moram dozvoliti uživanje i neproduktivnost, čak i kada su rokovi i stres oko nepredvidivih situacija prijetili da preuzmu kontrolu nad mojom svakodnevicom.Također, dijelim s vama konkretne lekcije koje sam naučila donoseći teške odluke, poput odgode radionice ili smanjenja broja upisa u mentorski program unatoč velikim financijskim izdacima, jer sam znala da moram zaštititi svoje mentalne kapacitete. Ovi izazovi potvrdili su mi da dugoročni uspjeh i kredibilitet dolaze iz usklađenosti sa samom sobom, a ne iz forsiranja. El presidente de Ecuador Daniel Noboa decidió eliminar el subsidio al diésel que pasó de 1,80 dólares a 2,80. La medida sorpresa, sin convocar a mesas de diálogo ni concertar con los sectores más dependientes del combustible, ha generado la protesta social y gremial. RFI consultó la opinión de Leónidas Iza Salazar, líder indígena y excandidato presidencial. El presidente Noboa decretó el estado de excepción en seis provincias y llamó al diálogo en un país donde el combustible no solo es un bien económico sino un símbolo político. Dos gobiernos ecuatorianos han estado a punto de arder por intentar eliminar el subsidio al diésel. En 2019, la revuelta social obligó al presidente Lenín Moreno a derogar las impopulares reformas económicas que incluían un alza importante en el precio de los combustibles. Tres años después, durante el mandato del ultraconservador Guillermo Lasso, el movimiento indígena también logró bloquear el incremento del precio del combustible y forzar al gobierno a comprometerse a mantener congelados los precios del diésel y del gas licuado de petróleo. Los enfrentamientos dejaron muertos, heridos y una gran fractura social. Leónidas Iza Salazar presidía entonces la Confederación de Nacionalidades Indígenas del Ecuador (CONAIE), emblema nacional por la defensa del subsidio al diésel. "El tema de los subsidios a los combustibles está anclado en más de 30 años de política pública. Según una ley de 2005, los siguientes gobiernos deben construir una política de compensación social y productiva para que vayamos dejando el diésel poco a poco y que no sea una decisión agresiva, letal, como ha intentado cada gobierno de turno", dijo a Radio Francia Internacional el líder indígena y excandidato presidencial. El presidente ecuatoriano Daniel Noboa decidió el viernes pasado eliminar de forma sorpresiva el subsidio al diésel. Su medida silenciosa generó, de inmediato, las protestas de transportistas, agricultores y pequeños productores, temerosos de una cadena de aumentos que golpee directamente el costo de la vida. "Al ser el combustible un factor de producción, el incremento de su precio está ligado a toda la economía ecuatoriana", firma Iza Salazar. Y explica que la subida del diésel genera dos efectos inmediatos: "El incremento en el precio de los productos de primera necesidad, sobre todo los industrializados, no los productos de los campesinos. Un segundo efecto inmediato es la especulación, porque las unidades de producción y los vehículos del transporte recuperan el costo de inversión por el incremento en el precio del combustible. Pero, además, en el mercado, por estar en cadena con todo el sistema de producción, lógicamente se desata la especulación", precisa. La protesta se ha extendido a otros sectores sociales y gremiales. El presidente Noboa decretó el estado de emergencia aduciendo grave conmoción interna y, de esta forma, frenó el paro nacional convocado para este lunes. El mandatario también autorizó a fuerzas policiales y militares a desarticular reuniones en espacios públicos que amenacen la seguridad, dijo. "El cambio de sede del gobierno es una provocación" El presidente Noboa también trasladó el poder Ejecutivo a la ciudad de Latacunga, en la provincia de Cotopaxi, donde se concentran las comunidades indígenas más organizadas y combativas. Para el líder indígena, que encabezó en el pasado la batalla contra la eliminación del subsidio al diésel, el traslado del gobierno de Noboa a esta provincia es una provocación. "Venir a una de las provincias que se ha manifestado cada vez que ha habido una decisión de este nivel es un mensaje de provocación. Y a partir de ahí, capaz que intenta controlar la provincia y la sierra centro donde estamos ubicados", sostiene. En opinión de Iza, el gobierno "sale a provocar, pero se esconde con mil policías armados, mil miembros de las fuerzas armadas, del ejército, con tanquetas de guerra, con drones, se encierra en una ciudad. Entonces, creo que hay una planificación militar política alrededor de la decisión de venir a Latacunga". Finalmente, el líder indígena Leónidas Iza denuncia que la medida de supresión del subsidio al diésel adoptada por Noboa se origina en los compromisos de su gobierno con el Fondo Monetario Internacional (FMI). "Lamentablemente quiero decir que no es una decisión del presidente de la República, Daniel Noboa. El 15 de mayo de 2024, el gobierno de Noboa firmó 43 acuerdos con el Fondo Monetario Internacional y en este momento está agachando la cabeza y acatando la imposición del Fondo Monetario Internacional". El gobierno ecuatoriano intenta cumplir con el FMI y, al mismo tiempo, frenar las protestas con bonos compensatorios selectivos y temporales. Pero en Ecuador el combustible no solo es un bien económico, sino un símbolo político. Tocarlo sin consenso es una medida explosiva.

In this episode we're talking with Charlotte Knee -- a brand spanking new mom, hailing all the way from Ireland, who welcomed her little bundle of joy in a home birth. In this episode we get to hear some perspective on home birth in Ireland! Charlotte was kind enough to chat with us while her 6-week old baby daughter Iza hung out in the background. We understand the life of fresh parenthood, and we know time is one of the most precious things at this state. So we consider it an honor that she spent some of it with us to share her awesome story. Charlotte's story is an example of how we continue to stay fascinated with birth in general, especially home birth. Because it's so different everywhere you go. Living in the states, you just tend to hear about the variations in laws and practices from state to state. But there's a whole big world out there! Charlotte has never been a big fan of hospitals. And get this - she was born at home! So continuing the tradition, she hired a private midwife and she and her husband Max planned for their home birth. Another cool part of Charlotte's life - she and her husband live in a shared house. So community is very important to her, and has played a big part in her journey of pregnancy and motherhood. We learned all kinds of new stuff from her - like using a tens unit and laughing gas during labor. We're totally inspired by Charlotte's style.
