Wat maakt de ziekte van Parkinson zoveel complexer dan alleen een trillende hand?Bas Bloem benadrukt in ons gesprek dat we Parkinson vaak veel te smal bekijken. Het is een sluipende afbraak van dopamine die invloed heeft op bijna elk facet van het mens-zijn: van je nachtrust en je darmflora tot je stemming en motivatie. Omdat dopamine de smeerolie is voor zowel je motoriek als je mentale welzijn, tast de ziekte de kern van je dagelijks functioneren aan.Bas legt uit dat reguliere pillen dit deels kunnen opvangen, maar dat sporten noodzakelijk is om het brein te dwingen zichzelf van binnenuit te onderhouden en weerbaarder te worden tegen deze brede symptomen.2. Wat houdt het concept van sporten als medicijn in de praktijk precies in?Voor Bas Bloem is sporten als medicijn geen leuke metafoor, maar een harde biologische noodzaak. Het gaat erom dat gerichte beweging in staat is om de progressie van de ziekte te beïnvloeden en symptomen effectief te onderdrukken.Waar medicijnen vaak als een bot instrument van buitenaf komen, zorgt sporten ervoor dat het lichaam zelf de juiste stoffen op de juiste plek aanmaakt. Dit proces stimuleert neuroplasticiteit, waarbij het brein leert om nieuwe omwegen aan te leggen rondom beschadigde gebieden. Het is een actieve vorm van zelfzorg die de regie teruggeeft aan de patiënt.3. Wat gebeurt er biologisch in het lichaam wanneer werkende spieren het brein helpen?Het geheim van de interactie tussen lichaam en geest zit volgens Bas in de myokines, zoals de eiwitten irisine en clusterine. Wanneer je intensief traint en je spieren flink aan het werk zet, scheiden ze deze stoffen uit in de bloedbaan. Eenmaal in de hersenen fungeren ze als een soort groeihormoon en ontstekingsremmer. Ze beschermen hersencellen tegen verdere afbraak en bevorderen de aanmaak van nieuwe verbindingen.Dit mechanisme verklaart waarom fysieke inspanning zo'n direct en meetbaar effect heeft op je neurologische gezondheid. Het brein profiteert letterlijk van het zweet op je rug.4. Wat vertelt de Park in Shape studie ons over de intensiteit die nodig is voor resultaat?De resultaten van de Park in Shape studie laten zien dat niet elk vrijblijvend rondje wandelen telt als medicijn. Bas legt uit dat de deelnemers in dit onderzoek drie keer per week trainden op een hometrainer, waarbij ze tachtig procent van hun maximale hartslag opzochten. Alleen bij die relatief hoge intensiteit zagen de onderzoekers op MRI-scans dat er daadwerkelijk nieuwe functionele verbindingen in de hersenen ontstonden.Voor elke sporter, met of zonder diagnose, is de les helder: je moet het systeem echt even onder druk zetten en je ademhaling flink laten stijgen om de herstelprocessen bovenin te activeren.5. Wat is de waarde van sporten als preventieve maatregel voor de gezonde duursporter?Ook voor de sporter zonder klachten is de boodschap van Bas Bloem relevant: je bouwt aan een functionele reserve. Door nu regelmatig en intensief te sporten, maak je je hersenen weerbaarder tegen de natuurlijke veroudering en mogelijke toekomstige schade.Bas waarschuwt hierbij wel voor de schaduwkant, zoals hersenschade door koppen bij voetbal of vallen zonder helm. Het doel is om je brein te prikkelen zonder het te beschadigen. My philosophy is simple: It's a meal, not a mortgage. Real food is the most important factor in good cooking. After surviving a heart attack in 2020 and the loss of my brick & mortar cooking school business, I've found new purpose cooking for my empty nest. Post heart attack, I'm reforming my butter-loving, heavy cream worshiping ways by making changes to how I cook, taking things in a healthier direction, without doing a complete 180 and still keeping things flavorful and delightful. The key here is healthier. I'm not afraid of butter, but I'm using a touch instead of stick. A tablespoon or two of cream or half-and-half, not a cup, and so on and so forth. And now that the kids are grown and living lives on their own, I'm re-learning how to cook for two. This week my guest is chef Christine Van Bloem. For 15 years Christine ran the successful Kitchen Studio Cooking School in Frederick, Maryland, a recreational cooking school. When the Covid pandemic hit, she had to stop doing in-person cooking classes. She pivoted to online cooking classes, hoping to go back to in-person. Then, her husband had a stroke, she had a heart attack, and the landlord of her business evicted her.That's enough for most people to throw in the towel. Christine took some time away from the kitchen, but now she's back. On the show, we discuss her new venture The Empty Nest Kitchen. She'll talk about what her cooking class model looks like today. In this episode, Dr. Bastiaan Bloem talks with Davis Phinney Foundation Executive Director, Polly Dawkins, about a recent article he co-authored, "The Elephant in the Room: Critical Reflections on Mortality Rates Among Individuals with Parkinson's Disease." In the article, Dr. Bloem et al. write, "Although the survival of people with Parkinson's has improved over time, people with Parkinson's still live fewer years than their age- and sex-matched population comparators, albeit at older ages this difference can be small." Dr. Bloem and his co-authors also acknowledge that doctors and researchers "often emphasize the tremendous impact of [Parkinson's] on an individual's life. However, in public awareness campaigns, we largely avoid discussions on the survival of people with Parkinson's..." It's understandable: the topic of mortality and Parkinson's is often painful. However, in this episode of The Parkinson's Podcast, Dr. Bloem argues out that openly acknowledging that Parkinson's shortens lifespans is a good thing for people with Parkinson's for a multiple reasons. In this episode, we explore Parkinson's disease (PD), which is the second most common neurodegenerative disorder of aging after Alzheimer's disease, and the most common movement disorder. It affects approximately 1 million people in the United States, and it is estimated that 60,000 new cases are diagnosed each year. Worldwide more than 10 million people suffer from PD. We described the disease in a patient story, discussed its epidemiology, causal pathways including genetics, signs and symptoms, medical and invasive treatments, prevention and future trends in research.
