Video: Commentary   Niall Ferguson – Woke Totalitarianism) (18:14) 13 stillborns in one hospital in 24 hours (1:54) Nurse blows whistle: C19 Shots and Spike in Baby issues; Refuses to inject poison. (1:30)   Asthma Can Be Reduced By Increasing Vitamin D Levels And Consuming Ginger Tel Aviv University (Israel) & Columbia University, November 13, 2022 According to research published in the journal Allergy, Dr Ronit Confino-Cohen and colleagues at Tel Aviv University analysed data from more than four million Israeli's that are members of the nation's largest healthcare provider — finding that of the 21,000 with asthma, those with a vitamin D deficiency were 25% more likely than other asthmatics to have had at least one flare-up in the recent past.

The effect of the vitamin is strongest in people with asthma and other lung diseases who are predisposed to respiratory infections. People with the worst vitamin D deficiency were 36 percent more likely to suffer respiratory infections than those with sufficient levels, according to research in Archives of Internal Medicine. “Vitamin D has significant immunomodulatory effects and, as such, was believed to have an effect on asthma — an immunologically mediated disease,” said Confino-Cohen. “But most of the existing data regarding vitamin D and asthma came from the pediatric population and was inconsistent. Our present study is unique because the study population of young adults is very large and ‘uncontaminated' by other diseases.” Professor Catherine Hawrylowicz from King's College London explained that findings already suggest that supplementation with vitamin D may one day be used not only to treat people who don't respond to medications but also to reduce the doses of dangerous steroids in other asthma patients – thus reducing the risk of harmful side effects. Of the 21,000 asthmatics studied, the team reported that those with vitamin D deficiency were at a higher risk of an asthma attack. Meanwhile, a team of US-based researchers have suggested that ginger compounds could also be effective in reducing the symptoms of asthma. The study, led by Elizabeth Townsend from Columbia University, investigated whether purified extracts of ginger that contained specific components of the spicy root could help enhance the relaxing effects of bronchodilators in asthmatic people. The research team explained that they studied the effects of three separate components of ginger: 6-gingerol, 8-gingerol or 6-shogaol when exposed to airway smooth muscle (ASM) tissue samples that were caused to contract by exposing them to acetylcholine. “We demonstrated that purified components of ginger can work synergistically with Beta-agonists to relax ASM,” said Townsend – adding that tissues treated with the combination of purified ginger components and isoproterenol showed significantly greater relaxation than those treated only with isoprotereno. Indeed, one of the three ginger components, 6-shogaol appeared most effective in increasing the relaxing effects of the Beta-agonist. (NEXT) Natural matcha green tea extract found to effectively kill breast cancer cells University of Salford (UK), November 05, 2022 A recent study carried out by the University of Salford has found that one kind of green tea in particular, matcha, can kill breast cancer cells effectively. The scientists at the university's Biomedical Research Center used a process known as metabolism phenotyping on breast cancer stem cell lines. They discovered that matcha “shifted cancer cells towards a quiescent metabolic state” while stopping them from spreading. Best of all, this was achieved using a rather low concentration of just 0.2 mg/mL. In addition, they discovered evidence that matcha affects the signaling pathways that promote cancer stem cells in a way that may make it a viable alternative to chemical cancer drugs like rapamycin. The scientist explained that the tea essentially suppresses oxidative mitochondral metabolism, preventing these cells from refueling. This causes them to become inactive and eventually die. University of Salford Professor of Translational Medicine Michael Lisanti said: “Our results are consistent with the idea that Matcha may have significant therapeutic potential, mediating the metabolic reprogramming of cancer cells.” (NEXT) Inflammation in midlife linked to brain shrinkage later Johns Hopkins University School of Medicine, November 11, 2022 People who show signs of inflammation in middle age are more likely to suffer from brain shrinkage later in life, a possible precursor to dementia or Alzheimer's disease, researchers said Wednesday. The findings in the journal Neurology are the latest to uncover an association between dementia and inflammation, in which the body's immune cells rev up in response to harms like smoking, stress, illness or poor diet. However, the findings stopped short of proving any cause-and-effect relationship. “These results suggest that inflammation in mid-life may be an early contributor to the brain changes that are associated with Alzheimer's disease and other forms of dementia,” said study author Keenan Walker of the Johns Hopkins University School of Medicine. “Because the processes that lead to brain cell loss begin decades before people start showing any symptoms, it is vital that we figure out how these processes that happen in middle age affect people many years later.” The study was based on 1,633 people with an average age of 53. Researchers tested their blood for levels of five markers of inflammation—not in any specific part of the body but rather throughout it—including the white blood cell count. An average of 24 years later, participants took a memory test and underwent brain scans. Those who had higher levels of inflammation at midlife on three or more biomarkers had an average five percent lower brain volume in the hippocampus and other areas associated with Alzheimer's disease, said the report. The effect was similar to having one copy of a gene—called apolipoprotein E (APOE) e4—that predisposes people to Alzheimer's, researchers said. People with higher inflammation also performed slightly worse on a memory test, remembering on average five of 10 words they were asked to recall, compared to 5.5 in the non-inflammation group. Outside experts described the study as large and rigorously conducted, but stressed that it did not study whether patients went on to develop Alzheimer's disease, only that some showed signs of brain shrinkage and memory loss. “This research points to inflammation as a potential early indicator of later brain degeneration, but we cannot say whether inflammation could be causing brain shrinkage or if it is a response to other damaging processes that might already be underway,” said Carol Routledge, director of Research at Alzheimer's Research UK. (NEXT) Adult ADHD has become epidemic—experts explain why University of California at Berkeley, November 7, 2022 Attention-deficit/hyperactivity disorder is growing up. Stephen Hinshaw and Katherine Ellison authors of ADHD: What Everyone Needs to Know, confirm that adults—and particularly women—are reporting to clinics in record numbers, becoming the fastest-growing part of the population receiving diagnoses and prescriptions for stimulant medications. “ADHD was never just for kids, and today many adults are getting the help they've needed for years,” says Hinshaw, a professor at the University of California at Berkeley and a respected global expert on ADHD. Ellison, a Pultizer-prize winning journalist, notes, “At least half of all children diagnosed with ADHD will continue to be impaired by their symptoms as adults, suggesting that approximately 10 million U.S. adults qualify for the diagnosis. Adult ADHD can lead to suffering through commonly accompanying disorders such as anxiety, depression, substance abuse, antisocial behavior, and gambling or Internet addictions. Social ties may well be frayed, with high risk of difficulties in intimate relationships. And, people with ADHD are also more likely to have a bitter history of academic and professional failures.” Indeed, researchers have found that adults who have been diagnosed with ADHD are up to 14 percent less likely than their peers to have a job. On average they also earn 33 percent less compared with people in similar lines of work and are 15 percent more likely to be receiving some form of government aid. The bottom line is that adult ADHD is not only real but has potentially devastating consequences, the experts agree. Hinshaw and Ellison also reveal a recent rapid rise in adult prescriptions for ADHD medication. One of the biggest surprises is that women of child-bearing age have become the fastest-growing group of consumers of ADHD medications. The number of annual prescriptions of generic and brand-name forms of Adderall surged among women over 26 years old, from a total of roughly 800,000 to some 5.4 million. (NEXT) Study shows clear new evidence for mind-body connection University of Calgary Department of Oncology, November 3, 2022 Impact of meditation, support groups seen at cellular level in breast cancer survivors For the first time, researchers have shown that practising mindfulness meditation or being involved in a support group has a positive physical impact at the cellular level in breast cancer survivors. A group working out of Alberta Health Services' Tom Baker Cancer Centre and the University of Calgary Department of Oncology has demonstrated that telomeres – protein complexes at the end of chromosomes – maintain their length in breast cancer survivors who practise meditation or are involved in support groups, while they shorten in a comparison group without any intervention. “We already know that psychosocial interventions like mindfulness meditation will help you feel better mentally, but now for the first time we have evidence that they can also influence key aspects of your biology,” says Dr. Linda E. Carlson, PhD, principal investigator and director of research in the Psychosocial Resources Department at the Tom Baker Cancer Centre. “It was surprising that we could see any difference in telomere length at all over the three-month period studied,” says Dr. Carlson, who is also a U of C professor in the Faculty of Arts and the Cumming School of Medicine, and a member of the Southern Alberta Cancer Institute. “Further research is needed to better quantify these potential health benefits, but this is an exciting discovery that provides encouraging news.” The study was published online in the journal Cancer. A total of 88 breast cancer survivors who had completed their treatments for at least three months were involved for the duration of the study. The average age was 55 and most participants had ended treatment two years prior. To be eligible, they also had to be experiencing significant levels of emotional distress. In the Mindfulness-Based Cancer Recovery group, participants attended eight weekly, 90-minute group sessions that provided instruction on mindfulness meditation and gentle Hatha yoga, with the goal of cultivating non-judgmental awareness of the present moment. Participants were also asked to practise meditation and yoga at home for 45 minutes daily. In the Supportive Expressive Therapy group, participants met for 90 minutes weekly for 12 weeks and were encouraged to talk openly about their concerns and their feelings. The objectives were to build mutual support and to guide women in expressing a wide range of both difficult and positive emotions, rather than suppressing or repressing them. The participants randomly placed in the control group attended one, six-hour stress management seminar. All study participants had their blood analysed and telomere length measured before and after the interventions. Scientists have shown a short-term effect of these interventions on telomere length compared to a control group, but it's not known if the effects are lasting. Dr. Carlson says another avenue for further research is to see if the psychosocial interventions have a positive impact beyond the three months of the study period. (NEXT) Lion's Mane Mushroom for Your Brain and Neuronal Health Green Med Info, November 8th 2022 Lion's mane mushrooms (Hericium erinaceus), with their shaggy, mane-like spines, stand out among fungi not only for their appearance but for their mild, sweet, seafood-like flavor. Like other mushrooms, lion's mane are multi-faceted healers, with antimicrobial, antihypertensive, antidiabetic and wound healing properties among their many therapeutic properties. Of the 68 diseases and conditions that lion's mane mushroom may support, many of them relate to the nervous system, including cognitive function, memory, dementia, depression, anxiety and sleep disorders. Lion's mane mushroom contains more than 35 beneficial polysaccharides that may help prevent or treat cancer, gastric ulcers, diabetes, hyperlipidemia, liver injury and neurodegenerative diseases, according to a review published in the International Journal of Biological Macromolecules. With a long history of use in Traditional Chinese Medicine, including for brain and neurological health, it's now known that two terpenoid compounds– hericenones and erinacines — in these mushrooms and their mycelia may stimulate the synthesis of nerve growth factor (NGF). Active compounds in lion's mane mushrooms may also delay neuronal death in neurodegenerative diseases, including ischemic stroke, Parkinson's disease, Alzheimer's disease and depression, while promoting nerve regeneration in cases of neuropathic pain or age-related hearing loss (presbycusis). Some of their top therapeutic benefits include: Regenerate Damaged Nerves – Lion's mane has been shown to trigger neurite outgrowth in brain, spinal cord and retinal cells. It also stimulates the activity of nerve growth factor, which is important for the growth and differentiation of neurons. Boost Cognitive Function – In adults with mild cognitive impairment , those who took lion's mane powder three times a day for 16 weeks significantly increased their scores on a cognitive function scale compared with those who took a placebo. Hericenones in lion's mane are believed to be responsible for some of the mushroom's beneficial effects on brain neural networks and improvements to cognitive function. This brain-boosting mushroom has also been found to improve memory in mice.[xi] Fight Depression – Lion's mane may ameliorate depressive disorder through a variety of mechanisms, including neurogenic/neurotrophic and anti-inflammatory pathways. Animal studies suggest that lion's mane may reverse depressive behaviors caused by stress by modulating monoamine neurotransmitters and regulating BDNF pathways. Support for Neurodegenerative Disease – neurotrophic compounds are known to pass through the blood-brain barrier and have been used to treat cognitive impairments, Alzheimer's disease and Parkinson's disease. In a study of patients with mild Alzheimer's disease, those who took lion's mane mycelia capsules for 49 weeks had improvements in their ability to carry out daily activities, such as personal hygiene and preparing food. Improve Mood and Sleep Disorders – Among overweight or obese people with sleep disorders or mood disorders, lion's mane was effective in relieving symptoms. Eight weeks of lion's mane supplementation decreased depression, anxiety and sleep disorders in the study, while also improving mood disorders of a “depressive-anxious nature” and boosting the quality of sleep at night.

Judo onderwijs is veelal nog traditioneel (ouderwets). Gelukkig is er veel onderzoek en ontwikkeling met betrekking tot motorisch leren en onderwijs. Ik ga een aantal podcast afleveringen maken over Judo onderwijs 2.0. Oftewel wat is achterhaald, wat kunnen we veranderen, aanpassen in het judo onderwijs om het aantrekkelijker, beter te maken voor onze judoka's. Hoe maken we betere judoka's, atleten. Allemaal met de insteek om judo leraren aan het denken te zetten en ze te dwingen verder te kijken dan het traditionele lesgeven op de tatami.  De eerste podcast zal gaan over ASM. Durant son enfance en Corrèze, Thomas suit les traces de son grand frère Fabien. Il commence ainsi le rugby à 7 ans du côté d'Egletons avant de partir en sport études à Ussel. Recruté par Aurillac, il passe une saison dans le Cantal avant de s'engager à 17 ans dans le club phare de la région : L'ASM Clermont Auvergne. Il y peaufinera sa formation et y entamera ce qui sera le début d'une carrière exceptionnelle ! Champion du Monde -21 en 2006, il fait ses débuts en équipe première de l'ASM la même année et s'y imposera au fil du temps. En 2009, il honore ses premières sélections avec le XV de France et en devient rapidement un cadre, étant notamment un des grands artisans du Grand Chelem 2010. La même année, il remporte le premier Brennus de l'histoire de l'ASM, près d'un siècle après la création du club. Par la suite ralenti par des blessures, il se forge un mental d'acier et parvient à revenir au plus haut niveau ! Thomas poursuit son aventure Auvergnate jusqu'en 2017, puis s'engage à la Section Paloise. Après 2 saisons et le corps usé par ses combats passés, il décide de raccrocher les crampons. Il saisit alors l'opportunité d'apporter son expertise de la mêlée au pack Béarnais et en devient l'un des entraineurs. Grand passionné de bons gueuletons, de moto et de tatouage, Thomas sait se projeter au delà du rugby et il est plein de projets avec sa petite famille. C'est vraiment un personnage aussi humble qu'attachant et j'ai passé un super moment en sa compagnie ! According to the World Bank, building enough renewable energy infrastructure to keep global warming below 2C will require more than 3 billion tons of minerals. Reducing emissions quickly is crucial to minimizing risk for the world's most climate-vulnerable communities, many of whom are on the front lines of a crisis they did not create. But unless we are careful, ramping up mining in order to decarbonize could actually worsen inequity and injustice. “How do we do this quickly, safely, and sustainably, in ways that benefit all?” asks Lauren Risi, Director of the Wilson Center's Environmental Change and Security Program in this week's New Security Broadcast.  Risi explores this question with Roger-Mark De Souza, a Global Fellow with the Wilson Center and Vice President of Sustainable Markets at Pact, an international development organization with decades of experience improving health, governance, sustainable markets, and local stakeholder engagement in mining activities. What Pact is most known for, says De Souza, is how it engages communities: “[It's] very much a co-creation process in partnership with and led [by] communities.”  Pact's broad portfolio includes work on gold in Ghana, mica in Madagascar, cobalt in the Democratic Republic of Congo, and gemstones in Tanzania and Kenya. In the Great Lakes region of Africa, the organization has spent over a decade improving mining activities for the 3T minerals (tin, tantalum, and tungsten) through a program called ITSCI. De Souza explains that the project, which is implemented in partnership with the International Tin Association, “[looks] at the supply chain [of] the three T's with a focus on social protections, traceability, and due diligence.” According to De Souza, ITSCI is the only program that fully adheres to the OECD's guidelines for due diligence.  Across the world, Pact also works closely with artisanal and small-scale mining (ASM) operators—which De Souza calls “the hidden labor force of the mining sector.” Distinct from the more formalized, and more mechanized industrial mining sector, ASM accounts for a vast majority—as much as 90 percent—of the mining workforce worldwide. Artisanal and small-scale mining can bring significant economic benefits both for local populations and for global markets. “[ASM] is a tremendous source of livelihoods and income for communities,” says De Souza, “and [it] is critical to supply chains.”  But ASM, and mining more broadly, can also be accompanied by serious human rights risks. “There's a tension [when] mining is the foundation of communities' livelihoods,” observes Risi, because mining often simultaneously introduces child labor, hazardous working conditions, and environmental degradation—all of which undermine local livelihoods, health, and sustainability.   Pact's programming seeks to respond to some of these challenges. Under its ‘alternative livelihoods' program, for instance, Pact helps children exit mining and then supports them in developing sustainable livelihood strategies post-graduation. The program has had major success in certain places: “In some mining sites, we're able to get more than 90 percent of the children out of these mines,” De Souza shares.    Despite the challenges of ASM, its importance to local livelihoods and global supply chains means it merits attention in policy solutions. To that end, the World Bank, Pact, and other partners have developed a data hub called DELVE, which seeks to collate robust information about ASM and ultimately inform better decision-making. As a multipurpose tool, it serves a wide audience including communities, the mining sector, policymakers, and NGOs.  As the demand for critical minerals continues to rise, De Souza says improving transparency across ASM and industrial mining should be a priority. “[It's important to] have in place systems for better tracking, traceability, due diligence, tracking on conflict minerals.” For companies, looking at the risks in their supply chain is not just a moral imperative, he says. “It's also good business sense.” Encouragingly, corporate boards of directors and shareholders are increasingly asking for this information.  Beyond transparent supply chains, de Souza emphasized the need to formalize the ASM sector and strengthen gender equity in mining, where discrimination—like taboos associated with menstruation—can limit women's opportunities. Underlying these ways forward is the more fundamental philosophy that community voices and needs must be centered. Stone is not seeking re-election, so McKinnor has vowed to bring the bill back to life. Qui se cache derrière ce nom, Chinese Man, figure mystique à la fois omnipotente et invisible ? Pourquoi et comment, depuis plus de 15 ans, reconnaît-on toujours sa signature alors même que sa musique est multiple ? Pour le savoir, je me suis invitée à Marseille, dans les locaux de Chinese Man Records, juste avant le concert du groupe Chinese Man à la Fiesta des Suds. Chinese Man, "l'homme chinois" en français, a trois têtes : Sly, Mateo et High Ku. Trois têtes bien faites qui se sont connues à Aix-en-Provence à l'heure où certains vont à l'université. Trois passionnés de cinéma, de musiques et de danse qui, au début des années 2000, à Marseille, avec d'autres de leurs amis, ont monté le collectif Chinese Man Records. Aujourd'hui, Chinese Man, ce sont deux albums (Racing with the Sun en 2011 et Shikantaza, en 2017), des projets en solo (BOLD, Matteo, Rumble) etde nombreux projets collectifs comme les compilations The Groove Sessions (avec des artistes maison tels que Youthstar, Taiwan MC ou ASM) qui puisent leurs influences dans le hip hop, le trip-hop, mais aussi le dub, l'électro ou encore le reggae. Pour célébrer les 15 ans du collectif devenu label, producteur et tourneur, Chinese Man a invité deux autres groupes de la famille Chinese Man Records, Scratch Bandit Crew et Baja Frequencia, a composé le 5e volume de ses The Groove Sessions. L'occasion d'un nouveau live festif et audacieux (c'est leur marque de fabrique), que les Chinese Man ont pris un malin plaisir à jour à domicile pour fêter les 30 ans d'une autre aventure marseillaise et musicale qui les a vu grandir et les a souvent programmés : la Fiesta des Suds. Parce que je ne pouvais pas rater les anniversaires de ces institutions marseillaises, je me suis invitée, avant leur concert à la Fiesta des Suds, chez les Chinese Man. In this episode:Mike and Ed discuss the recent exploits of the Human Torch. In less than a week he has captured four super criminals, and he has done it with style. Meanwhile, Dr. Strange has been asked by the media to come forward and explain what magic is, and has refused. The hosts contrast the different choices that these two superheroes have made for handling public relations, and argue that Dr. Strange may be making a choice that leads to being ostracized like Spider-Man. Also: A lack of evidence does not mean something has been proven to not exist.Behind the comic:The three issues we discuss in this episode were all released on the same day (at least officially). Marvel's two most popular characters at the time were Spider-Man and the Human Torch, and ASM #21 is a great example of Stan differentiating the two teenagers: in the eyes of the public, the Torch can do no wrong, and Spider-Man can do nothing right.In these issues:Strange Tales #129:Story oneThree of the supervillains (who were given powers by Dr. Doom previously) have escaped from prison. Johnny “the Human Torch” Storm hears about it while on a date with his girlfriend on the golf course, and quickly races to investigate. He is overpowered by the trio after running out of energy and captured, but is able to fire a flash to alert his teammate Ben “the Thing” Grimm. Ben races to the scene but he is likewise overpowered. The two heroes are tied to train tracks, although the Thing is able to kick the train away, and thereafter capture the three supervillains.Story twoThe media want Dr. Strange to discuss magic on a TV show; he answers the requests with silence. TV presenters who decry Dr. Strange and, from their perspectives, “alleged” magical abilities have an old statue rumoured to have mystic powers that they wanted to ask Dr. Strange about. The four TV presenters mysteriously disappear, leaving only the old statue on the studio floor. Dr. Strange investigates and determines, with the help of his mentor, that the statue does have mystic powers connected with an ancient and evil magician named Tiboro. Dr. Strange confronts Tiboro and is able to best him after a fantastic battle, freeing the four TV presenters. The rescued quartet are grateful and want to put on a show about how magic is real, although Dr. Strange casts a spell so that they all forget what happened.Amazing Spider-Man #21The supervillain Abner “the Beetle” Jenkins is released from prison and immediately sets about planning his revenge against the Human Torch. Before the inevitable battle, we see how the public responds to the Human Torch - with love and admiration - and Spider-Man - with fear and trepidation. The Beetle first battles Spider-Man, and their battle leads to them crashing into the house of the Torch's girlfriend Dorrie. The Beetle kidnaps Dorrie, and when the Torch arrives at her place, he mistakenly thinks that Spider-Man was involved. The two battle and, once the Torch figures out that Spider-Man was not involved in kidnapping Dorrie, they team up to fight the Beetle, eventually overcoming the supervillain.Tales of Suspense #62:The episode opens with Iron Man captured and at the mercy of the Mandarin, who goes about monologuing his supervillain origin story (which involves capturing rings from an alien spacecraft). Iron Man breaks free from his confinement and is able to break up the Mandarin's efforts at starting a nuclear war. The two battle, with the Mandarin fleeing, vowing to destroy Iron Man another day.Assumed before the next episode:People are wondering if Dr. Strange is legit or a fraud, and they are still unsure about whether Spider-Man can be trusted. With increasing digitization and the use of cloud-hosted assets, managing attack surfaces continues to be a major challenge. A recent survey report on the state of attack surface management (ASM) finds security teams drowning in a flood of legacy and ineffective tools with limited discovery capabilities. The need for ASM platforms with advanced digital asset detection capabilities is revealed in the survey findings. https://www.linkedin.com/in/davidmonnier/ (David Monnier, Team Cymru Fellow), sheds light on the latest ASM platform capabilities and discusses the implementation challenges and success factors. What led to this very important study that y'all conducted? 07:41 -- David, would you like to add anything to that from a methodology and participant demographic standpoint? 09:52 -- The first finding that I'd like to talk about is "the biggest reason organizations implemented ASM is to increase the visibility of Shadow IT in the enterprise." I think this is a very significant finding. If you would expand on this and also describe what Shadow IT means? 14:50 -- Over the years have ASM platform capabilities been enhanced to better monitor cloud-hosted assets? What's been the trend? 19:17 -- Moving along to another interesting finding, which states that "23% of the respondents said that identification of rogue or unclassified assets is the most valuable capability that ASM has provided their organization." I guess my question here is, shouldn't this be obvious? Shouldn't that be what an ASM is supposed to be doing? 20:56 -- What steps should an organization take, should the security analysts, the security professionals take, to ensure that their ASM platform is performing at a satisfactory level? 24:30 -- To what extent is AI being used to enhance the functionality, the capabilities, of these ASM platforms? 27:48 -- So talking about the human component, and in this discussion, we have been talking about attack surfaces, more from a physical standpoint, devices, and so on, so forth. How about humans as attack surfaces, as very vulnerable attack surfaces? Are we doing better in terms of securing this very vulnerable attack surface? Can tools help us secure that attack surface? What are your thoughts? 31:23 -- Is there anything that you'd like to address that you found interesting, or something that surprised you all? 34:22 -- Is there a way of notifying multiple personnel just to make sure that the alerts don't go unheard? 36:43 -- So if you had to make recommendations to potential buyers, or investors of this ASM platform, what does it take to prepare the organization, so they can effectively use such a platform? 40:33 -- I'd like you to share some final thoughts. Hospital and insurer in negotiations that could leave public employees paying out of network costs

July 20, 2022 — A recent announcement by Adventist Health about its negotiations with Anthem Blue Cross, the county's main insurance provider, has caused widespread panic. The contract between Anthem and Adventist, a faith-based nonprofit hospital system that manages all three of the county's hospitals, was originally due to expire on July 18, but has been extended to August first. Adventist posted FAQs on its website this week, directing patients to call the number on their insurance card for answers to most questions. But patients are advised that if they are in the hospital after midnight on August first, Anthem could choose to transfer them to another hospital. Some patients may be able to continue receiving care for some time as a “continuity of care service.” In a letter to patients last month, Adventist claimed that Anthem has enjoyed record profits for the past two years, but continues to pay Adventist “substantially less than other hospital systems. Anthem is one of our lowest-paying health plans, and we can't continue to provide quality care for patients at such significantly reduced rates.” Adventist offered to be interviewed on this subject, but we declined because President Judson Howe refuses to speak to us about the faith-based hospital's policies on abortion, which remains legal in the state of California. Recent studies show that much of the high cost of doing medicine in the state of California is due to a lack of competition, both in healthcare and insurance markets. According to a study by the California Healthcare Foundation, titled, “Markets or Monopolies,” “the preponderance of evidence suggests that hospital consolidation leads to higher prices… Furthermore, workers bear the burden of these increased premiums as employers depress wages to pay more for health insurance coverage.” The MediCare Payment Advisory Commission told Congress in 2018 that “hospitals with large market shares have the leverage to negotiate relatively high prices from commercial insurers.” This affects small local governments offering employer insurance, as well as private citizens who pay for their own coverage. The county's health plan is currently close to $5 million in the hole, with the county paying about 81% of the cost of skyrocketing claims, according to a presentation by Deputy CEO Cherie Johnson during last month's budget hearings. “So we would really need to change our whole plan,” she told the Board of Supervisors. “We need to be looking at the deductibles, the co-insurances…Fully insured is where we're looking at, so we know, with a fully insured plan, what your payments are every single month. Right now, we don't know. We project what we believe our monthlies will be, but it's unpredictable. We could get a $60,000 claim week, and then the next week, we could get $116,000.” Julie Beardsley, President of SEIU Local 1021, which represents most county workers, said it's time to cut loose. The union is in a battle with the county, which is offering a 0% cost of living allowance. “The county has been stalling about looking into new plans,” she said at a union rally during the budget hearings. “Obviously Adventist has kind of monopoly here in the county and they can charge whatever they want, but we need to look at other plans.” Adventist told its patients that in the last five years, it's given away more than $276 million in charity to those in need. We have not had the opportunity to review detailed financial information for Adventist Health. But according to a report by Stat, a healthcare-focused news website produced by Boston Globe Media, nonprofit hospitals in the U.S. received an estimated $25 billion in tax exemptions in 2015. Its authors, Ge Bai and David A. Hyman, write that “many nonprofit hospitals do not provide enough charity care to justify their exemptions…More than one-third of nonprofit hospitals (36%) provided less than $1 of charity care for every $100 in total expenses.” Nonprofit hospitals are exempt from federal, state, and local property taxes, and donations to them are tax-deductible. Ge and Hyman argue that, “If nonprofit hospitals are unwilling to provide sufficient charity care to justify the amount of their current tax exemption, there is no reason we should deprive local communities of the property tax revenues that allow them to fund local schools, parks, and other public services.” Assembly member JimWood's office responded to our inquiries with a statement reading in part that the Assembly member understands that, “In this case, Adventist is the one initiating the action to renegotiate, requesting higher reimbursements in a number of areas…On the other hand, Anthem is seeking an agreement that would not reflect significant cost increases to the employers, employees or others with Anthem coverage. It's important to note that the entities being affected, such as the school districts, city of Ukiah and (the) county, are self-insured and, as such, are tax-payer funded and can only absorb certain cost increases before they have to look for other ways to make it up, like cutting costs. This is not a situation that Asm. Wood wants to see happen and commends these self-insured entities for being responsible to the taxpayers and by pushing back against any attempt to raise the cost of coverage beyond what they can afford.” Some of the largest employers in the county, including the county government, the city of Ukiah, and the Ukiah Unified School District, rely on Anthem for their insurance. The county has 961 employees with 810 dependents, while the school district has about 750 employees with approximately 350 dependents. The city of Ukiah has 194 workers and 234 dependents, and is consulting with a regional insurance provider about its options. And the only answer is, we need to contribute more.”

René Wormhoudt is the High-Performance Coach (Strength & Stamina) of the Dutch Men's National Soccer team and CEO / Co-founder of the Athletic Skills Model Company (ASM). ASM is built around the philosophy that all athletes benefit from multidisciplinary training, providing educational programs for coaches and instructors, developing training materials, and designing formats for fields and courts that allow for multidisciplinary practices and training. Inspired by the work of Professor Geert Savelsberg two decades ago, René started to challenge the belief of coaches that early specialization was beneficial to youth players, outlining its detrimental effect on the overall development of motoric skills of athletes. With the support of the legendary soccer player/coach Johan Cruyff, he reformed the renowned soccer academy of FC Ajax Amsterdam, and partnered with Professor Savelsbergh to develop new initiatives that transformed Dutch thinking on sports education and coaching.
