BUFFALO, NY- October 11, 2024 – A new #editorial was #published in Oncotarget's Volume 15 on October 1, 2024, entitled, “Transplant or no transplant for TP53 mutated AML.” As highlighted in this editorial, TP53 mutations (mut) occur in 10–15% of acute myeloid leukemia (AML) cases, commonly associated with therapy-related AML (t-AML) and complex cytogenetics (CG). TP53-mut AML is inherently resistant to conventional chemotherapies and continues to show a poor prognosis, even with venetoclax-based therapies. Allogeneic hematopoietic stem cell transplant (allo-HCT) remains a potential curative option, though only 10–15% of patients receive it. In a recent study, allo-HCT was the only variable significantly improving survival, despite only 16% of patients successfully bridging to it. In their editorial, researchers Talha Badar, Moazzam Shahzad, Ehab Atallah, Mark R. Litzow, and Mohamed A. Kharfan-Dabaja from the Division of Hematology-Oncology and Blood and Marrow Transplantation and Cellular Therapy Program at Mayo Clinic (Jacksonville, Florida) evaluated the outcomes of TP53-mutated AML patients based on data from the Consortium of Myeloid Malignancies and Neoplastic Diseases (COMMAND). The study found a “dismal” survival rate of 8.5 months, with no significant difference among treatment types, and allo-HCT was the only variable associated with improved survival. The authors also report on the “better long-term outcomes” when allo-HCT was performed during Complete Remission 1 (CR1) in previous observations. They acknowledge the limitations of their retrospective analysis, including selection bias, data heterogeneity from participating institutions, and the lack of complete molecular data prior to allo-HCT that might have influenced the results. The process of identifying which patients with acute myeloid leukemia (AML) can benefit from allogeneic stem cell transplantation in first complete remission (CR1) has taken a step forward thanks to analysis of the UK NCRI AML17 and AML19 studies, reported at the 65th ASH Annual Meeting and Exposition. Patients who achieved molecular residual disease (MRD) negativity in their peripheral blood were at low risk of relapse, and had no benefit from allogeneic transplant in CR1, including those with the FMS-like tyrosine kinase 3 (FLT3) internal tandem duplication mutation of the NPM1 (nucleophosmin 1) gene, that is generally considered to be a marker of poor risk. Peter Goodwin spoke with Jad Othman, MBBS, from King's College London and the Guy's and St Thomas' Hospital in London, and now based at the Royal North Shore Hospital in Sydney, Australia. Othman explained how testing for the FLT3 mutation of the NPM1 gene can be used along with assessment of molecular MRD to help choose patients who can benefit from transplant and spare those for whom the risk/benefit ratio is adverse.

As you know, the CR1 visa, or conditional resident visa, is for immigrant spouses who have been married for less than two years when they enter the United States with their visa. With this status, the spouse can move to the United States on a "conditional" basis. During this time, the couple is expected to stay married for at least two years. After this period, the couple can jointly file to remove conditions from the visa holder's permanent resident card. The visa holder will then receive the IR-1 visa (or the permanent green card), which lasts for 10 years.

And somebody that has their green card approved after their two-year wedding anniversary will receive the full 10-year green card. Whaley School is a separate day school with about 100 students with behavioral needs. Some students stay through graduation, but goal is to get them back to a neighborhood school. Sister school closed down and opened an elementary wing, so now a k12 school! 100% special education population - level 4, level 5 is more like a residential treatment center. Tier 3 in MTSS/RTI behavior. Students can access neighborhood schools. No after-school activities at Whaley. Sometimes a parent goes with them to activities at other schools. Why not inclusion at Whaley school? Some of the behavioral needs are so intense that staff members do not have the support to deal with those severe behaviors. Our motto is working through the problem. Physical aggression they are showing towards others. Strategies: Crisis Recovery Hallway - able to talk with intervention coaches to walk them through the conflict cycle, with whomever that may be! What are you going to do when you go back? Teaching kids to advocate for themselves. We really believe in no suspensions We give the same curriculum that they get at the other schools, All of their communication becomes behavior. An intervention coach in each classroom and there in the CR room. Intervention with Strategies and Support To prevent recidivism, we need to teach skills to help them. CR1 - something blows up and you lose your cool. Quickly getting back to class. CR2 - spend a little more time talking about what is going on. Choose: mindfulness, exercise, or work. CR3 - stay there the rest of the day. How to help the teachers welcome kids back into class. Every 5 minutes is a new beginning. It’s ok to start fresh at the beginning of the next class. Also have a few items that are specifically going to put them in the ISS room, Threatening, posturing, etc. Eliminating behaviors A lot of times teachers may see reactions, rather than instigating actions. Suggestions, be very loud and say, “I don’t like that, don’t do that.“ Check in and check out. 1st period talking about goals that they have. When they go to checkout, tell how they did. Doing so many checkpoints throughout the day. Behavior data every week. Data review meetings can be called at any time to try to get a student back into the regular school. Data review with the team, then we call a meeting with neighborhood school. Meet at the neighborhood school. Sometimes self-sabotage - Extra help for kids that are getting close to transitioning. Make sure kids feel that they have support when they leave. Why not do a different curriculum with these students who are very different? Students could have access to resource classes in all subject areas. How to be a transformative principal? Build close relationships with students, be out there and make sure that kids know you think they are important.
