In celebration, I have a really special episode of the podcast for you. I am so excited to share with you my conversation with my dear friend and fellow IBCLC, Adrienne Guirguis. I cannot begin to count up how much I have learned from Adrienne over the years. She is everything you want your IBCLC to be: brilliant, compassionate, humble, confident, open-minded and accepting. She is a whiz at lactation triage on the hospital inpatient floor, patient and gentle with everyone who is in the room for a lactation consultation, and absolutely ravenous for new knowledge and information. I would never hesitate to refer a client to her for any lactation issue, and I believe she represents the ideal in an IBCLC. Let's get to know Adrienne!Adrienne Guirguis, a board certified lactation consultant, has over 25 years experience in helping breastfeeding infants and their families. She began her journey into the world of lactation after struggling to breastfeed her oldest son. She became an accredited La Leche League leader and was able to help babies breastfeed. Her experience with La Leche League led Adrienne to become a International Board Certified Lactation Consultant (IBCLC). She has been board certified for 20 years and has experience with a wide range of problems that may be experienced by new families. Adrienne has worked in hospital for over 10 years and then worked in community health for many years. She is a clinical practitioner, working hands on with families to improve feeding outcomes.Adrienne continues her lactation education, constantly attending conferences and courses to stay up-to-date on the information and skills needed to benefit those who most need breastfeeding help. She is a certified Specialist in Orofacial Myology. In 2022 Adrienne completed the Holistic Integrative/Functional Lactation course, a year long program followed by a residency. This course has increased her skills to provide the best care for the families with whom she works.Adrienne also is an educator, working within the community to teach others the skills necessary to become lactation counselors and eventually board certified lactation consultants. Over the span of more than 25 years, she has seen breastfeeding unfold over the hours, days, months, and years along the entire spectrum of infant and child development. From teaching prenatal breastfeeding classes to private prenatal consultations, from seeing newborns nursing in the hospital to the community clinic to the home visit, and babies and toddlers of all ages in support groups, Adrienne has seen a lot in her lactation career. One of her strengths is connecting with mothers and fathers, building relationships with them so that they can trust her with their lactation needs. Having a newborn brings the new parent into a vulnerable space, and Adrienne has a gift for engaging them in a way that brings them comfort and support so that they can enjoy their baby.In fact, her connections with her clients are so strong and so important to the parents she serves that her private practice's strongest referral source is word of mouth. People trust her with the lactation care of the people they love, and there is no greater testimonial than that. In September of 2021, Kerry left her job of over 10 years to start her own adventure in private practice. Her practice, Elevate Speech Therapy, focuses on helping those of all ages through challenges related to orofacial myofunctional disorders. She has done extensive continuing education in the areas of breathing, oral motor development, tethered oral tissues, and sleep. I got a message from a subscriber of my channel named Susi Quintana. She graciously corrected one of the tongue exercises on one of my sleep apnea and snoring videos and opened my eyes to the world of myofunctional therapy. Since then, I have sent patients her way and bounce information off of her on a regular basis. What I do with my throat opening and tongue and mouth exercising videos can help a lot of people but it is not tailored to a specific person with a specific diagnosis related to breathing, snoring, being tongue tied, etc. So, there is Susi who evaluates online and takes someone through a journey through this important part of your physiology.  Breathing, sleeping, even having a 60 year old be able to roll her tongue for the first time are some of the benefits I have seen myofunctional therapy accomplish. Myofunctional therapy helps people with tongue thrust, snoring, sleep apnea, swallowing problems, dental issues and even asthma, allergies, acid reflux, bruxism (clenching your teeth) and TMJ issues.  It concentrates on objective measurement and resulting oral facial muscle and tongue therapy. We don't think too much about fine tuning our tongues but it can make a difference that gives benefits for the rest of our lives.  When I use myofunctional therapists, I have them work with me as I adjust the cranial bones using endonasal cranial adjusting, apply lasers on the soft palate for snoring, do posture correction, TMJ therapy and other therapies that may, also, involve a dentist that specializes in maxilla expansion and other corrective therapies for the mouth. Susi Quintana is a Registered Dental Hygienist of over 20 years.  She is also a Certified Orofacial Myologist (or COM) through the International Association of Orofacial Myology (or IAOM).  She is the first certified myofunctional therapist with a dental background in Houston, and the 5th in the entire state of Texas. She has her own myofunctional therapy business in the Houston, TX area – but provides virtual therapy as well – which means she can provide therapy to anyone anywhere. Some people find it adorable when a baby sucks his or her thumb. Although that is a normal occurrence, the continuation of the habit into the toddler and elementary school years of a child could negatively affect their oral and facial development.Thumb sucking can cause some children to create what is commonly known as a dental openbite (a vertical space between the upper and lower teeth where the teeth do not touch). This open bite is a dental issue most difficult to correct.Other children will suck their thumb to the point that it appears that their top teeth are “sticking out”. This would require orthodontic treatment to correct. But the biggest issue that thumb sucking causes is changes to the bottom jaw that can affect the normal growth of the jaw which negatively affects the child's face. This is caused by the thumb or finger preventing the lower jaw from coming forward and growing properly during thumb sucking.The negative effects to the teeth and jaw depends on the frequency, intensity and duration of the child's sucking habit.  Frequency is the number of times the child is sucking during the day.  Intensity is the amount of force that is applied to the teeth and jaw during sucking.  Duration is the total amount of time spent sucking.  The more frequent, the more intense and the longer the duration of the sucking habit, the more likely it is to affect the child's developing oral structure and jaw.Dr. Liliana Calkins is a Specialist in Orthodontics and Craniofacial Orthopedics at Sunrise Orthodontics Facial & Airway Development Center. Have you ever asked yourself the question ‘what exactly is Orofacial Myology' or ‘what does an Orofacial Myologist do? Orofacial Myologists are registered allied health practitioners who already hold extensive knowledge and familiarity with the head and neck and who have undertaken further (and ongoing) study to combine their pre-existing knowledge and experience to offer functional assistance for clients. In this episode, I explain how orofacial myofunctional therapy can be an important adjunct to traditional health services and the approach that Townsvile Orofacial Myology takes to help clients obtain and maintain improved health outcomes. Clients often seek ‘OM' to prepare for oral or airway surgery,  for rehabilitation afterwards or to help address incorrect habits or poor function. The issue of tongue ties affecting infant feeding is not new, but it has become more topical in the past few years as parents have begun to value the role that breastfeeding plays in early nutrition and oral development. The WHO recommends that babies exclusively breastfeed for 6 months and encourage that it continues up to two years of age. However, for those who choose this for their baby, what are the barriers to a successful breastfeeding (or even bottle-feeding) journey for a mother and baby? In what way does a tongue tie affect this?Dr Cara Riek, is an Arizona based health practitioner and International Board Certified Lactation Consultant (IBCLC ) who assists families to achieve positive breastfeeding outcomes through thorough evaluation of both the mother and baby and, if required, provides a release of the tethered oral tissues via laser procedure at her specialist practice, the Arizona Breastfeeding Medicine and Wellness Centre.In this episode, Dr Cara explains the physics of successful feeding and why the baby's muscle movement is so important. Let’s face it; as an oral health coach, I spend a lot of time in groups answering questions about cavities and children. The stories I hear and the pictures I see break my heart daily. It also keeps me going when I feel like giving up. Many dental professionals will tell you that flossing, fluoride and dental visits help banish cavities, while many natural healing people will state that diet and nutrition are the keys to healing and halting them.  And while both dental visits and nutrition can be beneficial to controlling cavities, I also feel that there are bodily habits and functions that often are overlooked by both sides of the oral health world which contribute greatly to cavities. Stacey Fletcher, RDH, BS, OMT joins me on today’s podcast and we talk about how mouth breathing, tongue ties, breastfeeding and bacteria all play a major role in the cavities in kid’s mouths, and why (don’t worry we won’t be telling you that we think breastmilk causes them). Not only is Stacey is a dental hygienist, she also is a myofunctional therapist and is finishing her clinical hours for lactation consulting.  She’s also enrolled in a holistic lactation course as well.  Stacey shares with us that her son’s mouth ties were what started her down the path of learning about these issues, and how that has helped shape her future thoughts on dental hygiene and cavity control/prevention in kids.
