Dr. Scott Harrington's interest in medicine and healthy living began early in life, deeply influenced by his mother's struggles with weight. Watching her navigate these challenges, he developed a fascination with diet books and a desire to help others achieve better health. However, it wasn't until 2012, after watching the documentary "Forks Over Knives," that his perspective shifted significantly. The film, which highlights the benefits of a whole-food, plant-based diet, inspired him to embrace veganism and adopt a more holistic approach to health.   Military Experience and Plant-Based Transformation   As an active-duty Army physician, Dr. Harrington faced personal health challenges, including maintaining his own weight. Despite efforts to manage his weight through portion control and intense exercise, he often felt hungry and deprived. A turning point came when a Special Forces soldier introduced him to the plant-based diet, emphasizing its performance benefits. This encounter, coupled with the influence of "Forks Over Knives," led Dr. Harrington to adopt a plant-based diet, transforming his approach to health and wellness.   Integrating Military Insights into Healthcare   Dr. Harrington's military service, including deployments in Afghanistan, profoundly shaped his understanding of trauma and recovery. He emphasized the importance of normalizing responses to traumatic experiences, such as PTSD, and helping patients view their reactions as typical under extreme conditions. This empathetic approach has been crucial in his practice, allowing him to build trust and provide effective care for veterans and others dealing with significant stress and trauma.   Transitioning from Military Service   After years of long deployments and rigorous schedules, Dr. Harrington sought a change from his demanding life in active duty. Desiring more time with his family and a less strenuous routine, he decided to transition to a career that allowed him to work from home. This shift led him to a telemedicine provider called American Well, similar to Teladoc or Doctors On Demand, where he served as a staff doctor.   The Telemedicine Experience   During his two years at American Well, Dr. Harrington was licensed in multiple states, allowing him to reach a wide range of patients. While the job provided the convenience of working from home, it lacked continuity of care. The nature of telemedicine meant he often saw new patients with minor ailments, leaving him unfulfilled professionally. He found himself yearning for deeper, more meaningful patient interactions and the opportunity to discuss lifestyle medicine more comprehensively. While progress in digital healthcare has been building for several years, in many ways we were not prepared for such a sudden and dramatic shift. Research into the efficacy of teletherapy is sparse, the tools at our disposal are not always reliable or intuitive, and licensure regulations changed overnight to allow providers to practice across state lines. For the last decade, telemedicine has been dominated by urgent care—and it’s delivered a great deal of value for patients in need of quick access to a doctor for something like the stomach flu. But for the technology to reach its full potential, the experience needs to expand beyond ‘blind date’ telehealth. What if we used telecare to enable established clinician-patient relationships? How might follow-up care improve if medical professionals could, for example, check in on cancer patients every day?Roy Schoenberg is the President and CEO of American Well, the United States’ leading telehealth platform. Since its rollout in 2008, the American Well ecosystem has onboarded more than 200 of the nation’s largest payers, health systems, employers and retailers. At the end of the day, health care should be about helping patients find their way to health while doctors, nurses, and other clinicians don’t burn out in the process. It’s becoming increasingly indisputable that the way to get to this North Star efficiently is through human-centered health care. Human-centered health care is a term coined by Dr. Sylvia Romm, and it’s a play on the term customer-centered design. How do we innovate? How do we use technology to intensify the human experience for both provider and patient? How do we rid ourselves of friction points and create a continuum of care that is sticky and makes getting healthy as enjoyable as Instagram? In this health care podcast, I speak with Sylvia Romm. She’s an MD and an MPH with a background as a researcher and a telemedicine entrepreneur prior to coming to Atlantic Health System as their chief innovation officer. We talk in this podcast about human-centered health care—what this means, what the success factors are, and how to make it happen. We also take into account the assorted challenges to overcome on the way there. This interview was recorded moments before COVID-19, and I say that as a good thing. Dr. Romm brings up telehealth as, let’s just say, a first step toward actuating human-centered design in health care. Clearly in the past, that was quite a hurdle. No longer. So, those health systems or you other stakeholders in the mix who have gotten the telehealth thing nailed, listen on for ways that you can leverage your success. And for those of you who haven’t, well, here’s a little extra motivation. You can learn more by connecting with Dr. Romm on Twitter at @sylvia_romm.  Sylvia Romm, MD, MPH, is driven by a passion for transforming health care delivery to patients and communities. She brings her background and expertise as a clinician and an entrepreneur to her role as chief innovation officer for Atlantic Health System. Firmly believing that a patient-centered focus is vital to health care innovation, Dr. Romm works with Atlantic Health System’s team members and physicians to find new ways to improve access to high-quality, affordable care. She also forges relationships with local and national innovation partners and works to expand the organization’s research profile. Dr. Romm is an avid author and speaker in the areas of health care, technology, and health information technology (IT) policy. She has written articles for various publications—including NEJM Catalyst, Forbes, KevinMD, and the Huffington Post—and was named one of Fierce Healthcare’s 8 Influential Women Reshaping Health IT and Becker’s Women in Health IT to Watch in 2020. A board-certified pediatrician, Dr. Romm has served in a variety of clinical leadership roles throughout her residency and as a hospitalist. Before joining Atlantic Health System, she was vice president of clinical transformation for American Well, the largest video-based telemedicine company in the United States. In addition, she was the founder of MilkOnTap, the nation’s first telehealth company focused on the needs of nursing mothers and lactation support. Dr. Romm earned her Master of Public Health in global health from Harvard TH Chan School of Public Health. In this episode of HOPA Now, President David DeRemer hosts a panel discussion regarding the increased need for telehealth during the COVID-19 pandemic. The panel consists of three well-informed doctors that are representing both the East and West coasts.  Eve Segal, PharmD, BCOP is the Lead Clinical Oncology Pharmacist at the University of Washington Medical Center/Seattle Cancer Care Alliance in Seattle, WA.  Maurice Alexander, Pharm.D., BCOP, CPP is a Clinical Manager Hematology/Oncology Pharmacy Services at UNC Chapel Hill, and John Valgus, Pharm.D. MHA, BCOP, CPP is the Assistant Director of Pharmacy at UNC Chapel Hill.  They discuss the need that COVID-19 has created to rethink traditional patient care, address various implementation strategies for telehealth, and offer a look at the potential future of telehealth.   In this episode you will learn: What did telehealth look like before COVID-19? ● Telemedicine has always been part of a traditional practice, but concerns about patient relationships and access to technology limited full adaptation of telehealth. ● Prior to COVID-19, UNC only conducted 25 telehealth visits per month. ● Telehealth visits were limited by a narrow set of eligibility requirements.   Implementation strategies for telehealth ● The adaptation of telehealth is now essential in order to deliver care as safely as possible. ● Telehealth care at UWA is currently limited to those who have been credentialed, and they are working to expand telehealth credentials to all oncologists. ● At UNC, AmWell has helped meet the increasing need to expand telehealth services. ● Telemedicine platforms have improved exponentially because physicians, pharmacists, and hospital administrators are now all on board.   Rethinking traditional patient care ● Telehealth tends to improve care as patients feel more comfortable asking questions in a personal environment. ● More dedicated patient education sessions allow for increased preparation for treatment. ● Telemedicine offers increased convenience for patients but also requires improved coordination of care and lab testing. ● Staffing models have remained largely unchanged through the transition to telehealth. ● COVID-19 has forced us to break down the barriers that have limited the use of telehealth in the past.   Platforms that support telehealth ● Health and human services technology rules have changed to allow doctors to use any platform including Skype for Business, FaceTime, Google Hangouts, or Zoom. ● Zoom offers a HIPPA-based platform that includes a virtual waiting room. ● American Well offers a telehealth solution that can be embedded into your patients’ health records. ● Doximity allows doctors to connect with patients with minimal steps. ● Security and user-friendliness need to be the two biggest priorities for selecting successful platforms.   Looking to the future of healthcare ● The slow reopening of states will allow hospitals to offer elective outpatient procedures.  ● Pharmacies have the opportunity to embrace telemedicine and to offer best practices for their patients. ● The increased convenience and safety measures offered by telehealth benefit patients in a way that can and should be sustained beyond COVID-19.   Mentioned in This Episode: HOPA AmWell Zoom Healthcare Plan Doximity   Quotes: “In the past month, the culture around telemedicine has completely changed. It went from a nice option to something that is absolutely necessary.” — John Valgus   “Our leadership has recognized that we need to adapt in order to deliver care as safely as possible.” — Eve Segal   “Telemedicine gives us the opportunity to safely manage patients with an even higher level of quality than in the past.” — John Valgus   “Telehealth is an excellent, convenient service that should sustain itself beyond the time of COVID-19” — Maurice Alexander

Dr. Lindsay Henderson is a clinical psychologist and currently the assistant director of Psychological Services at American Well. Before her position as assistant director, Lindsay worked as Staff Psychologist in the Child and Adolescent Division at McLean Hospital and also has experience in Harvard Medical School as Clinical Instructor. Her work revolves around adolescents and helping them cope with distress and emotional difficulty. Today, Lindsay talks about how today’s kids and young adolescents suffer stress primarily due to lack of sleep brought on by excessive use of smart phones and tablets. She also gives tips to parents regarding how to properly handle their kids’ relationship with technology.   “Our technology progresses much more than parenting can keep up with.” Dr. Lindsay Henderson In this episode, you’ll learn: There is a dysfunction in parenting in today’s society particularly on sleep issues with children. Advances and changes in society make it difficult for kids and adolescents to learn how to get to sleep and stay asleep for a good amount of time. Integrating technology in children's lives safely and healthily is not as fast as advancements in technology. We should not have phones and tablets before bedtime. Constant access to technology provides a constant stimulation which results to a child never truly learning to tolerate their own thoughts or emotions by themselves, especially because social media is addictive. Kids constantly have tablets or phones that turn their attention away from the task at hand. They never learn how to focus on one thing. Families should implement tech-free time in the household. Tech-free time in the morning before school is important as it sets the tone of the day in our brains to go forth engaging mindfully with one task at a time. Emotional skillset may not develop and cultivate when kids turn their attention towards technology use. Sleep is a challenging task emotionally. Sleep is the ultimate separation from reality as we are vulnerable when we close our eyes for hours at a time. When people are unable to develop skills to tolerate separation it can be difficult to get yourself ready to sleep. Poor quantity of sleep correlates (not causes) trying drugs and alcohol at an earlier age.   Key Takeaways: Sleep is something that needs to be worked on constantly. It's a habit that needs to be practiced and developed.   Mentioned in this episode: American Well Limiting Kids Screen Time article on the Amwell blog   Rate, Review, Connect, Inspire   Stay updated on new episodes, guest interviews, and health, wellness, and fitness information and resources by subscribing to the HealthCastNow Podcast Show on iTunes.
