Dr. James Andrews interview. Baseball is a development game. High School pitchers, when stress of the UCL is initiated 80 mph, danger zone 85 mph, UCL can tear at any moment 90 mph. Genetics play a role in the strength of the UCL (former MLB pitcher's son for example. Showcase baseball, ban radar guns from high school down to younger levels. Max effort throwing with heavy balls and baseballs, max effort long toss. Dr. Meister interview. Manipulation of the baseball to create new designer pitches, Dr. Meister can look at MRI and see what pitches are being manipulated based on the type of tears. Elbow positioning - this research has been around for over 30 years. Birmingham clinic DR. Andrews and Dr. Fleisig. At proper elbow position, at foot strike, 90 degrees shoulder abduction and 90 degrees elbow flexion. Allows for proper external/internal rotation. When shoulder works correctly there is less stress on the elbow. Jim Curnal shows this through pics of pitchers at foot strike. Prime movers are prime movers and stabilizers are stabilizers should be the focus. Hip Mobility - Baseball Flows Max Effort Throwing - The lost art of pitching. The built in safety valves (both mental and physical have been circumvented. Modern pitchers (throwers) are trained to bust through the barricade, instead of improving movement patterns and improving rhythm and timing to eliminate these barricades, ex. Hinge the front hip in order to get through the hip, create extension out front then rotate the trunk. All conversations that assess individual pitches to pitchers' success (Pitch Ninja video analysis) does not provide answers for what makes a successful pitcher. We compare pitches and relate it to success but we don't realize that a pitchers' combination of pitches is the package for success. Developing pitchers who can pitch is difficult. The technology shows us the result but who can teach the how to a variety individuals? Justin Orenduff - 94% of HOF pitchers went overhead with their hands. He calls it rhythmic cadence. Nolan Ryan often stated when he was struggling that he had to find his rhythm and timing. Koufax - levers in proper position. Gallo - transitions. Rooney - TripleSpin We have eliminate this to eliminate rhythm and timing problems (which has had the reverse result). We have taken the athleticism out of the pitcher. The difference between guys who can research, guys who can teach and guys who did it TC - chiropractic medicine story. My rehab from fractured neck and facial nerve damage. Dr. Ferguson Energy systems and recovery - you need all of the the energy system to be a complete pitcher. Aerobic conditioning aids in recovery. Workload / Intensity / Rest Why are pitchers out of breathe throwing a bullpen (recent experiences). Try harder mentality, max effort Anaerobic threshold training.

Why Are Youth Pitchers Getting Tommy John Surgery?: Spring Training Special (with Nikhil Verma, MD)

Baseball is back! Spring training cactus (and grapefruit!) leagues are in full swing, and the sports community is all abuzz with prognostications for the upcoming season. Pop quiz, dear listener! Shohei Ohtani. Bryce Harper. Kerry Wood. Jose Canseco. Chris Sale. What do they have in common? They're all Major League Baseball players, and they all (in addition to over 1,000 other MLB players) underwent Tommy John ulnar collateral ligament (UCL) surgery. Until the last 10 years or so, orthopedic surgeons performed Tommy John/UCL reconstruction surgery almost exclusively on professional baseball pitchers. However, with increased specialization of younger athletes, and increased demands on young athletes' throwing arms, the rate of UCL elbow injuries has skyrocketed. Young athletes aged 17-22 are the most common group receiving this surgery. Why?? Is blowing out your UCL "inevitable" in an overhead athlete? What factors come into play that increase risk of UCL tears? Can we prevent these injuries? How?? Thankfully we have brought our friend and colleague, and baseball injury specialist, Nikhil Verma, MD, to tease out everything "Tommy John"! Dr. Verma is a board-certified orthopedic surgeon, specializing in sports medicine, and has worked at Midwest Orthopaedics at RUSH since 2004. He is currently the head team physician for the Chicago White Sox (since 2017, taking over for one of our former guests, Dr. Chuck Bush Joseph), and is the director of the Sports Medicine division and is Program Director for the sports medicine surgical fellowship at RUSH.  He has robust research and academic presence, with tons of publications under his belt, and he is an associate editor for the Arthroscopy Journal, and on the editorial board for the Journal of Knee Surgery. He is a member of numerous academic societies in orthopedics, sports medicine, and Major League Baseball.  He also has been elected by his peers to “Best Doctors in America” every year since 2007, which is quite fitting as he is a wonderful human being and beloved colleague :) He also recorded this episode with Your Doctor Friends while driving in his car on the way to the airport to fly out to Spring Training in Arizona to help out his beloved White Sox, now that's dedication! But that doesn't begin to capture the myriad ways James is driving change. He explains how the art of diplomacy has helped him achieve his vision of change; creating a shared value in the diagram for the artists, the execs and the listeners. He talks about how he learned the skill of being able to flow "from Harvard to Harlem" or "from jersey to suit" to bring people along on the journey. This lecture was given by James Andrews, he explored the effect of orthodontics on the periodontium, an area under increasing interest within aesthetics to achieve the ideal ‘pink aesthetics' with the increasing adult population receiving orthodontic treatment. His lecture was based on, is orthodontics good or bad for the gingiva? What is the starting point ? Increase in adult orthodontics from 1970 by 800% 50% of adults have some element of periodontal disease Untreated adult population 51% dehiscence 37% areas of fenestration Evangelista 2010 Facial type and bone morphology Tunis 2021 Dolichocephalic = narrow alveolus and elongated to compensate for vertical growth Brachycephalic = larger alveolus Dolichocephalic - Red flag patients Tooth movement:  What happens when teeth move buccally? facial tooth movement Wennström 1996 Reduced bucco lingual width Therefore, reduced free gingiva Increased risk only if tooth is moved out of the alveolar housing What type of movement Tipping (uncontrolled) increase likelihood of recession Condo 2017 Proclination causes recession, but inconclusive  Thickness more relevant than final inclination Yared 2006 How to decide what to do? U.S. Medal of Honor awardee Sgt. Leroy Petry joins Tim to tell his Medal of Honor story, from a life and death battle in Afghanistan to the very definition of the word, “honor.” Sgt. Petry is a retired U.S. Army Ranger who is one of the few to receive the military's highest honor, and one of the very few medal recipients who have survived to tell their own story. This episode was first released October 20, 2020. https://traffic.libsyn.com/secure/shapingopinion/Encore_-_Sgt._Leroy_Petry.mp3   In April of 1862, a group of Union Soldiers in the middle of the Civil War had an assignment. They were supposed to make it across Confederate lines to steal a Confederate train car and ride it to Union lines. Along the way, they were supposed to destroy track and depots, cutting off the Confederate supply lines and transportation. That group of Union solders was called “Andrews Raiders.”  Twenty-five men volunteered for the mission that ended in a dramatic train chase and capture by Confederate forces. Eight of the original 25 volunteers escaped. Three were declared missing. Another eight were hanged. Among those who were executed was leader James Andrews. Another six found their way back to the Union Army as part of a prisoner exchange a year later. That following March, the survivors met with President Lincoln who thanked them for their service and their efforts in the daring mission, and he told them they'd be the first to receive a new honor. The Medal of Honor. And with that, he had a prototype of the medal and gave it to the youngest member of the group, Private Jacob Parott. Jacob Parott was the first in the Army to receive what is now regarded as the highest honor any member of America's military can receive. The Medal of Honor is the award for valor in combat for all members of the armed forces. Since 1862, more than 3,400 such honors have been bestowed, many if not most of them, posthumously. Not many who earn such an award, live to talk about it. Today, the Medal of Honor is awarded sparingly to service members who as the Army says are, “the bravest of the brave; and that courage must be well documented.” Since the medal is awarded sparingly, and so many of those who receive it die in combat, there are few recipients alive today to tell their story. Retired Sgt. Leroy Petry of the U.S. Army Rangers is one of those few warriors. The U.S. Army Ranger Creed Recognizing that I volunteered as a Ranger, fully knowing the hazards of my chosen profession, I will always endeavor to uphold the prestige, honor, and high esprit de corps of the Rangers. Acknowledging the fact that a Ranger is a more elite Soldier who arrives at the cutting edge of battle by land, sea, or air, I accept the fact that as a Ranger my country expects me to move further, faster and fight harder than any other Soldier. Never shall I fail my comrades. I will always keep myself mentally alert, physically strong and morally straight and I will shoulder more than my share of the task whatever it may be, one-hundred-percent and then some. Gallantly will I show the world that I am a specially selected and well-trained Soldier. My courtesy to superior officers, neatness of dress and care of equipment shall set the example for others to follow. Energetically will I meet the enemies of my country. I shall defeat them on the field of battle for I am better trained and will fight with all my might. Surrender is not a Ranger word. I will never leave a fallen comrade to fall into the hands of the enemy and under no circumstances will I ever embarrass my country. Readily will I display the intestinal fortitude required to fight on to the Ranger objective and complete the mission though I be the lone survivor. Rangers lead the way! Dr. James Andrews is known as the Godfather of Sports Medicine and was one of the first orthopedic surgeons to utilize arthroscopy. He was also one of the first Doctors to market himself to big-time athletes and celebrities while performing world-class surgeries. Dr. Andrews is the team doctor for the Tampa Bay Rays, Auburn Tigers, and Washington Commanders. Andrews has performed surgery on many high-profile athletes. He first became known among athletes when Roger Clemens' agent advised the pitcher to visit Andrews in 1985. Andrews provided a second opinion to team doctors, performed successful surgery on a torn labrum, and Clemens made a full recovery. Andrews would gain a reputation as an "athlete-centric" doctor, operating on athletes including Drew Brees, Bo Jackson, Michael Jordan, Jack Nicklaus, John Smoltz, Triple H, Brett Favre, Chris Godwin, and Adrian Peterson.
