Dr. Andrew Higgins (Crtitical Care Cardiology and Advanced HF / Transplant Cardiology at Cleveland Clinic) provides the ECPR for this episode. They discuss the following case involving a rare cause of non-ischemic cardiomyopathy. A young African American male was admitted for cardiogenic shock following an admission a month earlier for treatment resistant psychosis. He was diagnosed with medication-induced non-ischemic cardiomyopathy, which resolved with a remarkable recovery of his systolic function after discontinuation of the culprit medication, Clozapine. Episode notes were drafted by Dr. Essa Hariri. Audio editing by CardioNerds Academy Intern, student doctor Shivani Reddy. Enjoy this case report co-published in US Cardiology Review: Clozapine-induced Cardiomyopathy: A Case Report CardioNerds is collaborating with Radcliffe Cardiology and US Cardiology Review journal (USC) for a ‘call for cases', with the intention to co-publish high impact cardiovascular case reports, subject to double-blind peer review. Case Reports that are accepted in USC journal and published as the version of record (VOR), will also be indexed in Scopus and the Directory of Open Access Journals (DOAJ). Pearls - An Unusual Case of Non-ischemic Cardiomyopathy The diagnosis of drug-induced non-ischemic cardiomyopathy is usually one of exclusion. High clinical suspicion is needed to diagnose drug-induced cardiomyopathy. Missing the culprit medication causing drug-induced cardiomyopathy could be detrimental as there is a high probability of reversing a systolic dysfunction after stopping the offending medication. Clozapine is an effective medication for the treatment-resistant schizophrenia and is associated with reduced suicide risk. Clozapine is reported to cause drug-induced cardiomyopathy and is more common with rapid drug titration. Clozapine is more commonly associated with myocarditis. Close monitoring and vigilance are critical to preventing cardiac complications associated with initiating clozapine. The management of clozapine-associated cardiomyopathy includes clozapine cessation and heart failure guideline-directed medical therapy. Show Notes - An Unusual Case of Non-ischemic Cardiomyopathy We treated a case of clozapine-associated cardiomyopathy presenting in cardiogenic shock. Drug-induced cardiomyopathy is a common yet under-recognized etiology of non-ischemic cardiomyopathy. Clozapine is an FDA-approved atypical antipsychotic medication frequently prescribed for treatment-resistant schizophrenia and the only antipsychotic agent that has been proven to significantly reduce suicide among this patient population. However, Clozapine is reported to be associated with several forms of cardiotoxicity, including myocarditis (most common), subclinical clozapine associated cardiotoxicity, and least commonly, drug-induced cardiomyopathy. Clozapine-associated cardiomyopathy should be considered as a differential diagnosis in schizophrenic patients presenting with signs of acute heart failure.  Rapid titration of clozapine is a risk factor for clozapine-associated cardiomyopathy and clozapine-associated myocarditis. To date, there is no evidence or consensus supporting preemptive screening. According to the American Psychiatric Association, whenever clozapine-induced myocarditis or cardiomyopathy is suspected, a cardiology consult is warranted. Experts recommend, when initiating clozapine,

Dr. Dodd earned her Doctor of Pharmacy degree from Florida Agricultural and Mechanical University, a Master of Science in Pharmacy Administration from The Ohio State University, and a Master of Science with a focus in Applied Pharmacoeconomics from the University of Florida.  She completed her pharmacy practice and administration residency at The Ohio State University Medical Center.  Dr. Dodd has been a pharmacist for 22 years.  She transitioned into full-time entrepreneurship after a 17-year career in the pharmaceutical industry, where she had multiple roles within Medical Affairs. She has also managed a hospital pharmacy, and has experience in retail pharmacy. She educates women pharmacist entrepreneurs how to pivot in their careers and build a business that will help them live a life they love.  She also advocates for them to become their own advocates. Jerrica and I met for the first time at the first Medipreneurs Conference in Asheville, NC in April 2018.   Jerrica’s Dad told her that, “Every challenge or obstacle is there to prepare you for the next one.”   Jerrica serves a number of roles:   Coach women pharmacist entrepreneurs Functional medicine care provider Leader and public speaker Executive Editor (PharmaSis Magazine) A typical week involves a mix of her many roles:  from working with a patient to coaching a single client or a group of clients to public speaking and networking.  Jerrica shared a success story about a patient she coached.  Using Jerrica’s input and ideas, the patient lowered her Hemoglobin A1C from 10.1 to 6.3, without changing her medications!  Good food-shopping and preparation choices, dietary and nutrition changes, movement/exercise improvements, and more led to the lowered Hemoglobin A1C.  The patient’s trust in Jerrica and the patient’s meaningful changes led to the success.   How do patients and coaching clients find Dr. Jerrica Dodd?  Website, word-of-mouth, and social media.  Check out the links (above) in the show notes! If you are interested in working with Dr. Jerrica Dodd, visit her website or social media links (see above).  An initial consult is available at no charge to explore whether a working relationship and the timing of the coaching is right for both her and her clients.  Patients need to be ready to take charge of their health, and pharmacist entrepreneurs need to be ready to dream.

Dr. Ron Gharbo is an expert in the body-brain connection and in today’s conversation, he endeavors to help Laura understand the intricacies of Heart Rate Variability (HRV) and how it is intimately connected with and an indicator of one’s emotional intelligence (EQ), which is highly correlated with success in leadership. Recent advances in wearable technologies give Dr. Gharbo predictive insight into an individual’s energy reserves, which allows him to assess whether they are physically equipped to deal with physical and emotional stresses. “Energy is finite,” he explains. If an individual actually understood his or her personal “energy bank,” he or she could make better decisions about what to engage in or take action on. “If you knew your reserves were low, would you still take this action?” is a question he poses as he walks Laura through how it works. HRV is incredibly sensitive and requires skilled interpretation into the physical and emotional variables that impact it. Practitioners can gather enough data over time to understand an individual’s HRV and make healthy modifications through exercise, breathing, and mindful awareness. Dr. Gharbo takes this level of insight and information to a new level when he expresses interest in doing a large-scale experiment within a corporation allowing participants to self-modulate behavior and HRV through a phone app. It’s a fascinating conversation and an intriguing idea for sure; if you’re in HR and want to take him up on this experiment, connect with him below. Ron’s Bio Raouf (Ron) Gharbo D.O. is board certified in Physical Medicine and Rehabilitation (PMR) and Neuromuscular Electrodiagnostic Medicine and is faculty at the Virginia Commonwealth University Department of PMR, the hub for the National Veterans Administration Chronic Effects of Neurotrauma Consortium.  In 2020, Dr. Gharbo submitted a novel approach to whole health: Autonomic Rehabilitation: Adapting to Change. Dr. Gharbo was the clinical HRV expert for the largest randomized control trial for inducing cardiorespiratory synchronization to reduce chronic pain. He gave the keynote address at the 50th Anniversary for the Academy of Applied Psychophysiology in 2019, the Richard Baer Lectureship at The Ohio State University Medical Center 2018. His submission, on reproducible placebo analgesia at the International Conference on Opioids in 2017 was selected as a top presentation.  He is Riverside Health System faculty for the emerging leaders program. He speaks to all new health system providers and through ANS Health, LLC, he coaches athletes, coaches, and corporate leaders on the Physiology of Leadership and sustainable high performance. To hear Dr. Jerrica’s take on leadership, listen to her interview as she discusses the importance of giving, leading with grace, and the necessity of making difficult decisions.   Dr. Jerrica Dodd Bio Dr. Jerrica Dodd is a pharmacist, entrepreneur, coach and leader.   She holds a Doctor of Pharmacy degree from Florida Agricultural and Mechanical University, a Master of Science in Pharmacy Administration from The Ohio State University, and a Master of Science with a focus in Applied Pharmacoeconomics from the University of Florida. She has also managed a hospital pharmacy, and has experience in retail pharmacy. Her recent educational pursuits has been the completion of a Nutrition Health Coaching Certificate from the Institute of Integrative Nutrition. Currently, she is enrolled in the Institute of Functional Medicine and aiming to introduce a functional medicine perspective to the pharmaceutical management of member patients of Your Pharmacy Advocate, LLC where she is the founder and CEO. Her experience also includes speaking to diverse audiences including health care providers. With her background in pharmacy, leadership training and her passion for people, Dr. Dodd enjoys encouraging people to be their best and to get the most out of every opportunity that life presents. Her favorite scripture is Luke 12:48…”to whom much is given, much is required” and Dr. Dodd endeavors to live life giving back from the abundant ways in which she has been blessed. Nicole Walker’s Interview Commentary I really enjoyed this interview with Dr. Jerrica and I hope you did as well. I agree with Dr. Jerrica’s notion that leaders must be able to make decisions, sometimes without the insight and support desired at the time. If we aim to move forward as leaders, we must understand that decisions are required! As Dr. Jerrica suggested, we must learn to rely on and be confident in our experience, intuition, and knowledge to guide us. When we become comfortable with the possibility of failing, we understand that failure is just an opportunity to course correct if we keep pushing. A quote by Winston Churchill reads, “The price of greatness is responsibility.” I admire Dr. Jerrica’s decision to put herself first by adding self-care into her morning routine so she can be a better leader. Taking time to nurture ourselves, is not only smart it is required if we want to show up at our best. Dr. Jerrica shared three things she does for herself daily, which is meditate, pray, and work-out. These are great ways to take care of ourselves so we are healthy and on-point. Dr. Jerrica suggested we schedule all of our priorities, with self-care being at the top of the list, to ensure we accomplish our goals and stay focused.  A quote by Eleanor Brown reads, “Self-care is so important. When you take time to replenish your spirit, it allows you to serve others from the overflow. You cannot serve from an empty vessel.” I can relate Dr. Jerrica’s analytical yet optimistic mindset, as I have the same. Dreaming can be difficult for a person whose mind works off of facts and figures, yet it is essential if we want to reach our highest potential. Positivity can’t be quantified in numbers, but it can be quantified in observation. When we expand our horizons we open our minds to the possibilities we have. When our dreams energize and excite us, we should follow them. As Dr. Jerrica said, when we start thinking about what is possible, we can build anything we want. A quote by Walt Disney reads, “If you can dream it, you can do it.” Nicole Walker’s Takeaway of the Week Dr. Jerrica mentioned the importance of having shared values with those we do business with, to ensure we have the same priorities and goals. When our expectations differ from those around us, we view things differently and are driven by different measures which can leave room for disappoint in the long run. If we want to be aligned with those we work with, we have to ensure our values are similar and we are operating on the same “frequency”.    I will make it my business to dig deeper in my conversations with others to see if we have the same values before moving forward. I understand that my idea of complete and superb is not a universal view; so, I will do my due diligence to mitigate the risk of misunderstanding be seeking those in alignment with my values. A quote by an unknown author reads, “Sometimes…The issue is simply that their ceiling is your floor.” Nicole Walker’s Leadership Challenge My leadership challenge for you would be to think about the one thing you can take away from this episode and adopt into your life.  I know it’s hard to absorb too much information at one time and it’s even harder to try and implement too many changes at once.  When I attend a training or listen to podcasts I aim to walk away with a least one thing that stuck out to me and one way that I can change as a result.  I challenge you to do the same!  Don’t forget to subscribe to She Leads Podcast for first access to future episodes. And also, like and share this episode of She Leads Podcast entitled “Learn How To Course Correct with Dr. Jerrica Dodd”. Thanks and until next time “Be Empowered and Empower On”!
