Episode 67 - Innovating Community Fitness: Building Bridges from Research to Post-Stroke Rehab.New National Institute for Health and Care Excellence (NICE) guidelines in the UK recommend stroke patients receive at least three hours of rehabilitation a day, five days a week. This is a dramatic increase from previous guidance of 45 minutes a day. Currently there are big gaps in service when it comes to supporting people with stroke after they're discharged from care not only in the UK but worldwide. Often, survivors have nowhere to go to exercise and accessing a conventional gym for most is challenging and for some it's impossible.With our continued theme of exercise as the best medicine for stroke recovery, in this episode we are diving deeper into this topic to find solutions for expanding the reach of rehab into the community and highlighting champions innovating for more solutions to make these new rehab guidelines a reality.Meet Our Guest: Dr Rachel Young PhD MSc, BSc  Rachel is a senior research fellow at the Advanced Wellbeing Research Centre in Sheffield Hallam University. She is a chartered physiotherapist with expertise in neurological rehabilitation and exercise prescription. Rachel's research interests include the development and evaluation of rehabilitation technologies and accessible exercise solutions.  In our discussion Dr. Young reminded us that the NICE guidelines of three hours are not the first to recommend extensive rehabilitation. The National Clinical Guidance for Stroke launched in March 2023 actually advised six hours of activity a day, which may include activities of daily living, communication and exercise.We dove into why these guidelines sent a ripple through the stroke community, including how providers don't have the workforce to meet these new recommendations. We also discussed her research and how she believes technology, in the form of accessible exercise equipment, apps and wearables has to be part of the solution if we're to have any hope of meeting the guidelines. Join host Polly Swingle as she talks with Dr. Kayla Diebold, PT, DPT and Chris Plague, MS, OTR about the latest evidence in stroke rehab and the development of a high intensity stroke program at The Recovery Project. Learn the practical applications of the APTA's Clinical Practice Guidelines for stroke rehabilitation, optimizing neuroplasticity, task specific training, and the benefits of a comprehensive individualized program. Kayla Diebold, PT, DPT, CSRS is a Lead Physical Therapist and the Site Coordinator of Clinical Education at The Recovery Project.  She has specialized in the treatment of neurological diseases and disorders since 2017 with special interest in Parkinson's Disease, Stroke, and Vestibular disorders.  She is a Certified Stroke Rehab Specialist and heads the medical programming group for stroke at The Recovery Project. Kayla also has certifications in PWR! and Rock Steady Boxing for the management of Parkinson's Disease. She is a guest lecturer in the Physical Therapist Assistant program at Macomb Community College and frequently presents on topics involving physical therapy management of neurological diseases and disorders across the continuum of care.Christopher Plague, MS, OTRL is an Occupational Therapist at The Recovery Project. He specializes in the treatment of neurological conditions and has a specific passion for stroke and spinal cord injuries. Chris has certification in Upper Extremity Motor Rehabilitation. He is also a Certified Inclusive Fitness Trainer through ACSM. Chris is the lead OT for the Concentrated Stroke Rehab program at The Recovery Project. He has spent time in volunteer roles for adaptive sports and recreation programs through Michigan State University, community colleges in Southern California, Special Olympics of Michigan, and others.

Tanya is a Registered Music Therapist based in Sydney, Australia. In 2019, Tanya was invited to speak about her PhD research at the inaugural TEDxNewtown event. More recently, Tanya was named the inaugural recipient of the 2020-2021 Australian Music Therapist of the Year award for her contribution to the field of music therapy. Over the years, Tanya has presented internationally and nationally on her research and clinical work. Her research can be found in journals such International Journal of Stroke, Nordic Journal of Music Therapy, Australian Journal of Music Therapy, amongst others. Over the years, Tanya has established multiple ongoing music therapy programs in Australia and India. Guest: Sarah Breshears, MA, CCC-SLP: Today we continue our special series in collaboration with the Dysphagia Outreach Project in honor of Dysphagia Awareness Month, this time to speak about strokes with Sarah Breshears. This amazing woman is an SLP in her sixth year of practice working in an acute hospital setting, is the creative powerhouse behind Short and Sweet Speech, and on top of it all, she is the Instagram Manager for Dysphagia Outreach Project. She leads a team of five women who work to create content that is engaging, informative, and reaches those who would benefit from DOP's mission. We kick things with the moment of divine intervention that set Sarah off on her track to becoming an SLP before chatting about how she got involved with DOP. From there, we wind our way into the heavy topic of strokes, beginning with what happens to the brain when somebody has one, the effects it can have on their life, and how to recognize early symptoms. We also get into how strokes can result in dysphagia, and then move onto best practices for intervention and recovery, where we highlight the necessity of PT and OT. Our conversation covers the importance of instrumental assessment for dysphagia and the dangers of recommending the chin tuck swallow wholesale as well.
