Want to talk about the principles of neuroplasticity as they apply to occupational therapy? On today's show we'll discuss the first 5 principles from the Evidence-Based Review of Stroke Rehabilitation (EBRSR) 2018 update and provide examples of how to apply them in OT practice. Briana Elson, MS, OTR/L, BCPR, CBIS is an OT clinician and mentor at a top rehabilitation hospital with a goal of helping all OTs provide consistent high-level care no matter where they work or who they work with. Principles discussed today: Use it or lose it with prism glasses Use it and improve it with bilateral arm training, CIMT, mirror therapy and mental practice Specificity with task specific training using FES, PSFS and COPM Repetition matters with EMG triggered NMES, amount of reps needed/time needed, and robotics Intensity matters with VR training, HRR (75-85%), aerobic exercise, strength training, and dual task training

Gözlem aletlerinin ve yeni yöntemlerin gelişiminde müslüman astronomların öncellerine kıyâsla kaydettikleri büyük ilerlemeler hakkında, İslâm astronomisine yönelik modern bilimsel araştırmaların gerçekten de erken sayılabilecek bir aşamasında, bilim adamı C. A. Nallino'nun edindiği genel izlenimden şunu duyuyoruz: “Araplar hem trigonometrik formüllerin kullanımında hem de aletlerin sayısı, kalitesi bakımından ve gözlem teknikleri sayesinde öncelleri Yunanları övgüye değer bir biçimde aşabildiler. Gözlemlerin hem sayısı hem de kesinliğinde İslâm ve Yunan astronomisi arasında çok dikkat çekici bir farklılık kendini göstermektedir.” Müslüman astronomların ele aldıkları diğer bir konular kompleksi ise, yeryüzünün dönmesine (rotasyon) ilişkin görüş ve hipotezler ile gezegen teorileridir. Coğrafyacı İbn Rüsteh (Hicri 3./Miladi 9. Yüzyılın son çeyreği) birçok teorinin yanı sıra, dünyanın merkezinde değil de evrende bulunduğuna, güneşin ve en uzak gök kürenin değil, dünyanın kendisinin döndüğüne ilişkin teoriyi aktarmaktadır. El-Bîrûnî, bu sorunun tatmin edici bir açıklamasına ulaşmak için ciddi olarak çaba sarf etmiştir. Hindistan'a dair eserinde (miladî 1030) şöyle demektedir: “Yeryüzünün dönmesi astronomi biliminin sonuçlarına hiçbir şekilde zarar vermez, bu konuya ait olan şeyler bu kabulde de aynı şekilde mantıksal olarak birbirleriyle bağlantılı kalır. Ayrıca, dikkat edilmesi gereken bir başka husus da, el-Bîrûnî'deki alıntılardan çıkarıldığı kadarıyla Ebû Ca'fer el-Hâzin'in Hicri 4./Miladi 10. Yüzyılın ilk yarısında gezegenlerin dönmelerinin (rotasyonlarının) görünüşte simetrik olmayışına yeni bir açıklama getirmesidir. Today I am talking with Dr. Marissa Barrera, the Assistant Dean of Health Sciences and Director of the Graduate Program in Speech Language Pathology at the Katz School of Science and Health at Yeshiva University in New York City. On today's episode, we talk about cognition, speech, and swallowing symptoms in multiple sclerosis and how to advocate for the care that will set you up for success down the road. Dr. Marissa A. Barrera is the Assistant Dean of Health Sciences and Director of the Graduate Program in Speech-Language Pathology at the Katz School of Science and Health, Yeshiva University. Dr. Barrera is an internationally recognized medical Speech-Language Pathologist, Multiple Sclerosis Certified Specialist, and a leading expert on the use of modalities for speech and swallowing rehabilitation, including Neuromuscular Electrical Stimulation (NMES), Surface Electromyography (sEMG) and Neuromuscular Taping. As a researcher and author, Dr. Barrera has lectured around the globe on a wide range of clinical care topics ranging from MS fatigue, dysphagia, NMES, motor speech disorders, cognition and neurogenerative diseases. Medical History: No significant medical issues Other Information: Recreational tennis player, recently started competing Physical Therapy Examination(s): Tenderness over the lateral epicondyle. Pain on resisted wrist extension Physical Therapy Plan of Care: None, this is the first visit 1. What is the MOST likely diagnosis for the patient's presentation? A) Golfer's elbow B) Tennis elbow C) Olecranon bursitis D) Cubital tunnel syndrome Answer: B) Tennis elbow Rationale: Tennis elbow, or lateral epicondylitis, is characterized by pain over the lateral epicondyle and is often exacerbated by resisted wrist extension. The patient's symptoms, along with her recent increase in tennis games, align with this condition. Abstract: Self-initiated behavior is accompanied by the experience of willing our actions. Here, we leverage the unique opportunity to examine the full intentional chain, from will (W) to action (A) to environmental effects (E), in a tetraplegic person fitted with a primary motor cortex (M1) brain machine interface (BMI) generating hand movements via neuromuscular electrical stimulation (NMES). This combined BMI-NMES approach allowed us to selectively manipulate each element of the intentional chain (W, A, and E) while performing extra-cellular recordings and probing subjective experience. Our results reveal single-cell, multi-unit, and population-level dynamics in human M1 that encode W and may predict its subjective onset. Further, we show that the proficiency of a neural decoder in M1 reflects the degree of W-A binding, tracking the participant subjective experience of intention in (near) real time. These results point to M1 as a critical node in forming the subjective experience of intention and demonstrate the relevance of intention-related signals for translational neuroprosthetics. Quad Inhibition and Muscle Atrophy can really slow down the recovery process. However, Jackie Kleihege and Jena-Claire Auten know it is essential for healing. Finding the balance between helpful and hurtful is kind of an art form. Looking at the timeline is essential so we can see the progress of where we have come. What is the research showing us? We do not have all the data yet from the quad tendon ACL We have 10 years of data on it What are some reasons we have inhibition? In the acute phase, it looks about the same. If we can interrupt the inhibition early then we have better outcomes It is really hard to study the brain when we are studying the knee How can we find this loophole or workaround to muscle atrophy? Decrease swelling Decrease local inflammatory response ICE??? Is there a time frame where are going too fast in turning off the inflammation? We still want the natural healing process to continue through that phase. And then for that to quietly fade out. NMES JK: I have probably underutilized it most of my career.       I needed to give something more and then added NMES and made a difference. What are your ideal volume and exercises to prevent muscle atrophy? PRE-OP therapy is critical There is not a magic wand protocol Pre-Op nutrition is super important as well. When we try to activate the quad and then the patient is absent for a week what is the problem? Latency is key - I want those first 7 days like they are falling downhill, then I dial it back. Often day 1 postop patients do not look bad, but then days 2-3 are worse. JA: Genetics plays a role in the inflammation process. Consistency is key, similar to Instagram and Facebook, they keep putting the ads in front of you and then you end up buying it… Continue to use the same methods so the patients know what is coming and can buy in Looking at the 12-16 week patient OPEN KINETIC CHAIN - the type of graft makes a difference as well I need to be able to read the signs and customize the options for them. The goal is quad communication Determining when to progress: Biofeedback can help give visual and auditory feedback. BFR: It is really as good as they say. The uses keep growing. Subluxation and Shoulder Pain “And my problem with taping generally is, it's kind of band-aidy because as soon as you take it off the arm could fall right back out. However, if you could get that to reduce pain and then have them do stuff, which then activates the SITS muscles and the deltoid and it brings that humerus back into where it needs to go, in the meantime because they are doing stuff...they're driving cortical change, which reduces spasticity, which may indicate more movement and now we get this upward spiral of recovery that could be quite helpful. Guest: Brooke Beilman, MS, CCC-SLP - Today's episode is dedicated to the SLP's role in head and neck cancer and we invite Brooke Beilman, MS, CCC-SLP, to speak about this important subject. Brooke is an SLP with experience in acute care, inpatient rehab, outpatient rehab, and skilled nursing settings. With certifications in manual interventions like myofascial release, neuromuscular electrical stimulation, NMES, and the Mcneill Dysphasia Therapy Program, Brooke is as brilliant as she is kind. We kick things off by hearing about how Brooke got the idea to become a speech pathologist and then bring this proactive intervention to patients with head and neck cancer, thereafter helping to develop a rehabilitative program for these patients during her CF. From there, we discuss the rise of the role of HPV in the development of head and neck cancer in younger patients and the implications of this. We get into the signs and symptoms of head and neck cancer, common treatment forms used by oncologists, and how this differs from those used by an SLP. Next up, we take a deep dive into the role of lymphatic impairment in head and neck cancer. Brooke helps us understand the lymphatic system and the role it plays in the body, and also weighs in on the condition of lymphedema and the role of myofascial release in treating it.
