    Episode 67 - Innovating Community Fitness: Building Bridges from Research to Post-Stroke Rehab.New National Institute for Health and Care Excellence (NICE) guidelines in the UK recommend stroke patients receive at least three hours of rehabilitation a day, five days a week. This is a dramatic increase from previous guidance of 45 minutes a day. Currently there are big gaps in service when it comes to supporting people with stroke after they're discharged from care not only in the UK but worldwide. Often, survivors have nowhere to go to exercise and accessing a conventional gym for most is challenging and for some it's impossible.With our continued theme of exercise as the best medicine for stroke recovery, in this episode we are diving deeper into this topic to find solutions for expanding the reach of rehab into the community and highlighting champions innovating for more solutions to make these new rehab guidelines a reality.Meet Our Guest: Dr Rachel Young PhD MSc, BSc  Rachel is a senior research fellow at the Advanced Wellbeing Research Centre in Sheffield Hallam University. She is a chartered physiotherapist with expertise in neurological rehabilitation and exercise prescription. Rachel's research interests include the development and evaluation of rehabilitation technologies and accessible exercise solutions.  In our discussion Dr. Young reminded us that the NICE guidelines of three hours are not the first to recommend extensive rehabilitation. The National Clinical Guidance for Stroke launched in March 2023 actually advised six hours of activity a day, which may include activities of daily living, communication and exercise.We dove into why these guidelines sent a ripple through the stroke community, including how providers don't have the workforce to meet these new recommendations. We also discussed her research and how she believes technology, in the form of accessible exercise equipment, apps and wearables has to be part of the solution if we're to have any hope of meeting the guidelines. He moved to the Providence Bureau of the Associated Press and eventually to New York City. He was transferred to Lexington, Kentucky to work as a sports writer and moved to Charlotte, North Carolina after that. He worked at the Charlotte Observer and then returned to the Associated Press to work as the head of the Charlotte Bureau until 2006. Tim Whitmire was working for the AAP when he decided to look for work in Charlotte, NC.     Episode 64 - Achieving Balance (Part 1): Food as Medicine to Fuel Stroke Recovery A comprehensive stroke recovery plan of care often includes a combination of physical, occupational, and speech therapy, along with other medical interventions. These therapies are vital for regaining physical and cognitive abilities. Often overlooked, however, in the stroke recovery pathway is the quality of a stroke survivor's diet. Quality nutrition offers the potential to improve stroke outcomes by promoting healing, providing high performance fuels to support the physical demands of rehabilitation as well as offering key nutrients for boosting brain resilience to reduce the risk of future strokes.   We needed to look no further for an expert than our show co-host David Dansereau for today's episode on using food as medicine in stroke prevention and recovery. When David had a stroke at 39 from a congenital heart defect, he realized the challenges a stroke presented and took action by setting smart goals to optimize his diet in support of his long term rehabilitation plans. He ultimately achieved his goal of returning to running and completed the Boston Marathon. In this episode David shares his nutrition playbook and breaks down 3 key nutrition principles (his 3F's) that can provide a solid foundation for improving your nutrition IQ and complimenting your healing journey along with the help of your own health care team.   About David Dansereau David has a clinical background as an experienced physical therapist as well as a former registered dietitian and nutritionist in private practice. On this episode David will be sharing what inspired him to study nutrition and later write a best selling book (Body in Balance) and how these early experiences in healthcare, corporate wellness and behavioral health research have shaped his career today.     Pioneering Stroke Research: Dr. Broderick's TPA Breakthrough and StrokeNet Leadership

    In Episode 60 of the Know Stroke Podcast we sat down with the esteemed Dr. Joseph Broderick. Guest Bio: Joseph Broderick, MD, a distinguished stroke expert and neurologist, has been a crucial part of UC Physicians since 1987. In 1996, he became a professor of Neurology at the University of Cincinnati, and from 2000-2014, held the Albert Barnes Voorheis Chair for the Department of Neurology at the UC College of Medicine. From 2014 through the present, he has served as the Director of the UC Gardner Neuroscience Institute.   Contributing to medical care within the broader community as a founding member of the UC Stroke team, he has served as a courtesy staff member at 20 regional hospitals during the past 30 years and as a full staff member at all UC Health hospitals. Currently, he is the principal investigator for the National Coordinating Center for StrokeNET, an NIH-funded network of 25 regional stroke centers and 500 hospitals nationwide focusing on clinical trials and stroke care as well the principal investigator of two sizeable global stroke trials. Without a doubt, he has played a pivotal role in UCMC and UC being one of the leading Comprehensive Stroke Centers in the world.     Redefining Recovery: Unleash Your Potential with Sean Entin, aka the Stroke Hacker

    In Episode 59 of the show we sat down with stroke survivor Sean Entin aka the Stroke Hacker. Despite being paralyzed on his left side, Sean learned to walk, talk, drive, and speak in front of audiences, inspiring people to free their minds and bodies after a traumatic event. His mission is to help stroke survivors, caregivers, and families reduce anxiety, regain independence, and discover hope for a better future. Sean graduated from the University of Southern California with an emphasis in entrepreneurship. He went on to co-found two startups that went public; he even worked out with Navy SEALS to improve his fitness for fun. And with all this happening, he remained a dedicated husband and father of two young daughters. Then in 2011, on Thanksgiving weekend, it happened. Sean suffered a massive stroke at age 39. While induced into a 10-day coma, the neurosurgeon performed a craniectomy. That left Sean with a long road to recovery, but he never gave up. Despite Sean's grim prognosis, he defied the odds and birthed his mantra, “I can, I shall, I will.” Sean's journey has inspired him to launch Stroke Hacker, a program and community aimed at helping stroke survivors, individuals with traumatic brain injuries, and caregivers. Sean provides one-on-one coaching, encouraging survivors to face new challenges and helping them plan a recovery road map. Editor in Chief of Clinical Rehabilitation. Although Avril's main areas of interest are stroke rehabilitation and conducting randomized controlled trials, she is interested in all aspects of rehabilitation research. She has conducted trials and service evaluations of stroke units, occupational therapy pre-discharge home visits, falls reduction/management, interventions for people with MS, traumatic brain injury, low back pain and total hip replacements. She was one of fifteen woman identified by World Stroke Academy as an International Women in Stroke in 2021. Avril is currently conducting research into fatigue after stroke, which we discussed in this interview. She also works with falls, memory rehabilitation, tendon injuries, vocational rehabilitation, and upper limb recovery after stroke.     From a single weekend stroke caregiver and survivor retreat to a nationwide stroke support mission

    In Episode 56, we sat down with the United Stroke Alliance (USA) Co-Founder and Executive Director Marylee Nunley and CEO Larry Schaer to discuss how this dynamic duo has grown the alliance from a single person in a single community, to its expansion and now having offered 300 Retreat and Refresh Stroke Camps throughout the country. The Retreat & Refresh Stroke Camp program (now a division of the USA) was first developed in 2004 by Marylee & John Nunley (Founders) following John's stroke in 2001. We discuss the USA team's stroke support outreach efforts and how they have extended into communities throughout the country with their stroke awareness and prevention programs. The team estimates they have now educated millions of people on the signs of stroke as well as have supported thousands of stroke survivors, caregivers and their family members in their stroke recovery journeys.  About the United Stroke Alliance The United Stroke Alliance (USA) team is made up of passionate individuals who tirelessly strive to make a difference in not only stroke survivors and caregivers lives, but also for the members of every community through their stroke awareness and prevention programs. Next year will mark the 20th anniversary of the USA, which was founded in 2004 when one stroke caregiver, (Co-Founder Marylee Nunley) wanted to improve the quality of life for local stroke survivors and caregivers through a weekend retreat.     When a therapist really knows the patient perspective

    In Episode 55 we're joined by Michelle Ballasiotes, a pediatric stroke survivor and occupational therapist. Michelle suffered what is called a perinatal stroke which usually occurs between the middle of pregnancy and delivery. Her childhood consisted of weekly physical and occupational therapy for more than 10 years in order to gain mobility due to right hemiplegia, or paralysis on the right side of the body. She also has hydrocephalus, or an excess of cerebral spinal fluid, in her brain, resulting in having a shunt placed in her head at three days old.     Restoring Arm Mobility with Spinal Cord Stimulation

    In Episode 51, we were joined by Marc Powell to speak about the ground-breaking neuro-technology he and his team at Reach Neuro are working on to restore arm and hand movement in patients who survived severe strokes. Globally, every fourth adult over the age of 25 will suffer a stroke in their lifetime, and 75% of those people will have lasting deficits in motor control of their arm and hand, severely limiting their physical autonomy. In April 2023, the FDA granted breakthrough device designation for Reach Neuro's Avantis, a platform that delivers small electrical impulses to the spinal cord to restore shoulder, arm, and hand movement to post chronic stoke patients. The Avantis system is built to restore a patient's ability to control movement directly, giving them not only immediate relief but also allowing it to be possible for them to return to therapy. Early evidence has shown the effectiveness of the system in patients with chronic stroke up to 9 years poststroke, outside of the 6 month window where existing therapies are typically used.     Building Global Stroke Support Networks: Insights from Sarah Belson of the World Stroke Organization

    About this Episode: For episode 49 of the podcast we sat down with Sarah Belson, International Development Manager at the World Stroke Organization; leading, developing and implementing a program of capacity building activities for new and emerging patient stroke support organizations (SSOs) around the world. Some of the highlights from our interview: Global mapping of SSOs: One of the key challenges that stroke patients and their families face is a lack of awareness and support for their condition. To address this, the World Stroke Organization has been working to map SSOs around the world and develop a comprehensive database of resources and contacts for patients and their families. Framework for long-term sustainability: Another challenge that SSOs face is maintaining their operations over the long term. To address this, the World Stroke Organization has developed a framework for building capacity and ensuring the sustainability of SSOs. This includes training and support for organizational development, fundraising, and advocacy. Stroke Connector: Stroke Connector is an online platform developed by the World Stroke Organization to connect stroke survivors, caregivers, and advocates around the world. The platform includes a directory of SSOs, resources for patients and their families, and a forum for sharing experiences and best practices. Collaboration with SAFE: The World Stroke Organization has also been working closely with the Stroke Alliance for Europe (SAFE) to support Life After Stroke programs across Europe. This includes training and capacity building for SSOs, advocacy for better stroke care and support, and raising awareness about the impact of stroke on patients and their families.     In this episode 48 we sat down with stroke survivor and accomplished entrepreneur Tim Whitmire. Tim co founded the largest free men's fitness movement in the US. He speaks to us about how he grew a community built around fitness, fellowship, and faith into the largest mens' fitness movement in the US, and how those pillars transferred into his rehab journey. About our guest: Whitmire started as a freelance journalist, then got a job at the Associated Press and covered Harvard Men's Basketball. He moved to the Providence Bureau of the Associated Press and eventually to New York City. He was transferred to Lexington, Kentucky to work as a sports writer and moved to Charlotte, North Carolina after that. He worked at the Charlotte Observer and then returned to the Associated Press to work as the head of the Charlotte Bureau until 2006. Tim Whitmire was working for the AAP when he decided to look for work in Charlotte, NC. He ended up getting hired in a marketing and business development role at a sell side and investment bank. He then co-founded a firm called Black Arch Partners, where he worked for a few years. After leaving the second bank, he found his way into his current work as an executive strategy consultant. Will then asked him about his nonprofit, F3 Nation.     Stroke Has No Age Limit

    In 2020, William Kyler Lewis, 15, was riding in the car with his mom in College Station, Texas, when he suddenly turned to her confused. His head hurt, he was sick to his stomach and his vision was blurred. Soon, he couldn't remember anything for more than a few seconds. After his mom rushed him to the pediatrician, he got a shot and was told to go home. Instead, his mom drove 1½ hours to a children's hospital in Houston. Nurses there thought Kyler had taken drugs, but his mom insisted no, he was not that type of kid, and something was very wrong. A CT scan revealed Kyler had an acute spontaneous intracranial hemorrhage from a brain arteriovenous malformation (AVM) that he unknowingly had when he was born. A brain AVM is a condition that is not inherited. AVMs are a random occurrence due to a gene malfunction in utero where a mass of arteries connect directly to veins. Kyler received lifesaving external ventricular drain (EVD) surgery to control the fluid build-up in his brain. After the stroke, memory and neuro fatigue became a big problem. Kyler has undergone radiation and chemotherapy and takes medication for epilepsy, headaches, dizziness, weakness and tremors. Yet, Kyler is inspiring many. He uses local news, community functions and social media to educate the public that a child can have a stroke, too. He also organizes walks to highlight the importance of recognizing the symptoms of a pediatric stroke. Kyler's Facebook group, “A Cure for Kyler,” his CaringBridge page and YouTube channel educate and inspire pediatric stroke survivors by documenting his recovery.     Dr Eva Mistry; winner of the Robert G. Siekert New Investigator Award in Stroke

    Episode 46: In this weeks episode we sit down with a Dr Eva Mistry, assistant professor of clinical neurology and rehabilitation medicine at the University of Cincinnati in Ohio. Dr Mistry was announced the 2023 winner of the Robert G. Siekert New Investigator Award in Stroke. The award encourages new investigators to undertake or continue stroke-related research. Mistry's award-winning presentation (Abstract LB18), "Blood Pressure After Endovascular Stroke Treatment (BEST)-II: A Randomized Clinical," was presented at the International Stroke Conference in February 2023. Additionally Dr Mistry is the principal investigator of a new study with $9.6 million funding support from the Patient-Centered Outcomes Research Institute (PCORI) that will examine the effectiveness of a stroke treatment for patients with a pre-stroke disability, one of the first studies to focus on this population.     Episode 44: In this episode we sit down with Kim Malkin, Angels Initiative Lead for Australia to discuss the work she and her team are doing across the region to improve stroke care delivery. The Angels Initiative was launched in 2016 by Boehringer Ingelheim in partnership with the European Stroke Organisation (ESO), the World Stroke Organisation (WSO), the Stroke Alliance for Europe (SAFE), and many other national stroke societies, companies and health institutions. Their mission is aimed at reducing the burden of stroke for countless patients by working with hospitals, stroke teams, societies and individuals to build an innovative network of stroke-ready hospitals worldwide to reduce treatment delays and provide patients with the optimal standard of acute stroke care. About our guest: A move to Australia in 2004 provided not only more sunshine for Kim but also the opportunity to begin a long and diverse career with Boehringer Ingelheim – over a decade of which is in improving outcomes for stroke patients through the delivery of innovative preventative and acute stroke treatments.     Delivering stroke care on the roads and in the sky across Australia

The Australian Stroke Alliance is comprised of more than 30 organisations working together to transform stroke care across Australia and improve the lives of those affected by stroke. They are developing novel mobile imaging, digital health and emergency transport technologies to radically transform stroke outcomes and deliver commercial benefits for all Australians. Rather than transporting patients to hospital, they drive or fly Australian-designed life-saving brain scanning equipment to a patient during the Golden Hour for Stroke, transforming their chances of survival and recovery.     About Our Guest Dr. Mike Studer has been a PT since 1991, and an NCS (Neurological Clinical Specialist) since 1995. He is co-founder and co-owner of Spark Rehabilitation and Wellness in Bend, where he is an adjunct professor at the OSU DPT program leading the coursework on motor control. Mike was recognized as the 2011 Clinician of the Year in the Neurologic and (in 2014) the Geriatric Academies of the APTA. He received the highest honor available in PT in 2020, being distinguished as a Catherine Worthingham Fellow of the APTA in 2020. Dr. Studer has authored over 35 articles, 6 book chapters, and is a consultant to Major League Baseball on the motor control of pitching and hitting.     Episode 41 In our final episode of 2022 we had the pleasure to be joined by Pamela Duncan. Pam is a leading expert in stroke, neurologic recovery, aging and patient reported outcome measures and widely recognized for her efforts to reengineer post-acute care services and recovery following stroke. She develops consumer-oriented assessments to improve patient outcomes by guiding real-time clinical decision-making and recommendations for self-management, medical and social services. The Compass CP platform was created based on insight gained from the COMPASS study, led by Wake Forest University School of Medicine and The University of North Carolina and conducted in 40 hospitals and with community partners across North Carolina. The study was funded through a $14 million, five-year award from the Patient-Centered Outcomes Research Institute (PCORI), an independent, nonprofit research funding organization authorized by Congress in 2010.  COMPASS compared the health status of stroke patients who receive conventional post-hospitalization treatment to that of patients who receive comprehensive care based on a model developed by a team of physicians, nurses, therapists, pharmacists, health system and human services leaders, and patient and caregiver stakeholders.  Because of her outstanding contributions to the investigation, management, mentorship and community service in the stroke field, Duncan is a 2020 recipient of the American Stroke Association's David G. She serves as a global aging expert advising startups, large corporations and investors, with nearly 20 years dedicated to transforming the aging experience. Sarah is CEO of multinational staff-hosting company, MezTal in Guadalajara & CEO of the consulting firm Delight by Design, creating age-inclusive products, brands, spaces and experiences that delight the consumer at every age. Sarah has held key leadership positions across the US and Asia, developing and implementing strategic corporate innovation initiatives, including change management. She serves as Executive-in-Residence at Aging2.0,Principal Fellow in the Nexus Insights firm for aging transformation, and co- founder of AgeTech News. She formerly led all commercial activity for Seismic Holdings - a Silicon Valley-based company shaping human potential through integration of apparel and robotics to augment human strength and performance. Sarah's commercial expertise helped position the organization for esteemed recognition by the World Economic Forum, Fast Company, The Economist and CES. Prior to Seismic, Sarah led all global innovation for the largest post-acute care provider in the US, Genesis, serving more than 55,000 individuals a day. She combines her Occupational Therapy expertise with her operational, clinical and entrepreneurial experience to inspire global improvements in design and systems innovation worldwide. She designed an innovation center in China as a part of a vitality initiative to improve the health and well-being of global communities. About Our Guest Arun Jayaraman, PT, PhD, is an internationally recognized expert on sensors, exoskeletons, robotics and other emerging rehabilitation technologies. As Executive Director, Technology & Innovation Hub (tiHUB), he collaborates with commercial and academic partners for research collaboration, technology development and clinical outcome evaluation. He also leverages valuable industry and research expertise to accelerate Shirley Ryan AbilityLab's clinical adoption of next-generation rehabilitation technologies. Since joining Shirley Ryan AbilityLab in 2008, Dr. Jayaraman also has served as Director of the Max Näder Lab for Rehabilitation Technologies and Outcomes Research, which conducts outcomes-based research on rehabilitation technologies such as prostheses, orthoses, rehabilitation robotics and other adaptive technologies. In addition to his work at Shirley Ryan AbilityLab, he is Professor of Physical Medicine, Rehabilitation, Medical Social Sciences, and Physical Therapy and Human Movement Sciences at the Northwestern University Feinberg School of Medicine. Dr. Jayaraman's work has been published in more than 100 peer-reviewed journal articles, and has been funded by myriad leading organizations, including the U.S. Department of Defense, the National Institutes of Health, the National Science Foundation and the Michael J. Fox Foundation. He also holds various external leadership positions, serving as Director of Global Outreach for the Wearable Robotics Association and editorial Board Member for the International Journal of Stroke. Dr. Jayaraman also is involved with the American Orthotic Prosthetic Association, the Society for Neuroscience and the American Physical Therapy Association. Dr. Jayaraman holds a BS in Physical Therapy from SRM Medical College Hospital and Research Centre in Chennai, India; an MS in Physical Therapy from Georgia State University; and a PhD in Rehabilitation Medicine from the University of Florida in Gainesville.
