In this week's episode, host Karli Burridge speaks with fellow obesity specialist PA, Ledyi Ballesteros, about how Ledyi's own struggles with her weight led her to the field of obesity medicine, and how she has developed the obesity program at Gonzaba Medical Group, a multispecialty clinic in San Antonio TX, which serves a primarily low-income, elderly, Hispanic population. Ledyi explains how she developed the obesity program, and how she is able to help her patients using culturally sensitive, economic, patient and family-centered approaches. Ledyenska “Ledyi” Baez Ballesteros, MPAS, PA-C Ledyi—a bilingual provider treating patients who speak Spanish and English—began working in internal medicine and urgent care at Gonzaba Medical Group (GMG) in San Antonio, Texas in 2011. She founded the obesity medicine program at GMG in 2015 after gaining around 100 pounds after complications with her first pregnancy in 2014. Ledyi, who now has two kids, subsequently embarked on her own weight loss journey the following year, and her patients began to ask her about it. The resulting pilot program started with an estimated 60 patients. Today, she runs the obesity medicine program at GMG with nearly 700 patients, 75% of whom are Hispanic. Ledyi completed AAPA's Obesity Management in Primary Care Training & Certificate Program in 2018 and earned the Certificate of Advanced Education in Obesity Management for NPs and PAs from the Obesity Medicine Association in 2020. For the past 2 years, she has served as the the Secretary of PAs in Obesity Medicine, and she has been instrumental in helping PAOM become an official caucus of the AAPA. Ledyi has presented on the topic of obesity to members of AAPA, the Association of PAs and NPs, Bexar County Society of PAs, the Society of Army PAs, and the Texas Association of PAs. The name for this drug is Zepbound™ and it is, the first and only obesity treatment of its kind that activates both GIP (glucose-dependent insulinotropic polypeptide) and GLP-1 (glucagon-like peptide-1) hormone receptors. Sarcopenic obesity represents a condition of excess adiposity (body fat) and low muscle mass or muscle function and strength. This condition is far more prevalent and problematic than many realized, and certain patient populations are at increased risk for sarcopenic obesity. New, highly-effective anti-obesity medications such as semaglutide 2.4mg (Wegovy), and anti-diabetes medications such as semaglutide 2.0mg (Ozempic) and Tirzepatide (Mounjaro) and could potentially increase the risk of sarcopenic obesity, unless clinicians and patients are properly educated on how to prevent patients from losing an excessive amount of muscle mass and function.Listen in as host Karli Burridge interviews obesity specialist NP Sandra (Sam) Christensen about sarcopenic obesity, how we can recognize it in our patients, and how we can prevent it!Sandra is a Family NP, who has been specializing in obesity medicine since 2005. How can we match our patients with the best treatment options that will work for them? This is the big question when treating a vastly heterogenous disease like obesity. Rather than using trial and error, what if we could phenotype our patients and choose a treatment that is most likely to be successful for that specific patient? It may sound too good to be true, but this is the type of science and precision medicine that Dr. Andres Acosta and his team at Mayo are making a reality. Dr. Acosta's goal is to treat and ultimately cure obesity, which he considers this century's greatest health epidemic. Dr. Acosta's work on individualized obesity therapy aims to identify the right therapy for the right patient, minimizing side effects.Andres Acosta, MD, PhD is a Physician scientist, Obesity and Diabetes Expert, and serial entrepreneur. He is faculty at the Mayo Clinic in Rochester Minnesota and focuses his research on gastrointestinal physiology to understand the complexity of food intake regulation and obesity. Dr. Acosta's laboratory utilizes a combination of genetics, physiology, pharmacology, proteomics, metabolomics, and gastrointestinal and brain imaging to understand food intake regulation and to modulate them for the treatment of obesity. Medications like Ozempic (aka semaglutide, a medication approved for type 2 diabetes) and Wegovy (also semaglutide, but at a slightly higher dose, and indicated for obesity), are all over the media, resulting in high demands and even medication shortages. In response to the high demand, and the limited insurance coverage for these medications, some compounding pharmacies are offering compounded peptides, like semaglutide, and some clinicians are offering these compounded medications to their patients. What are we to make of this? Are these medications safe? Effecive? Legal?
