Episode 55 of The Art of Aging Mindfully Podcast is all about moving beyond dogmatic approaches to health.On This show I share my personal evolution in my approach to yoga practice and nutrition. I was challenged and inspired by Master Iyengar who said “5 breaths is not enough for me to explore the asana and learn about it's full expression.” "Dogma" refers to a set of principles or beliefs that are accepted without question, often as part of a particular ideology, religion, or system of thought. In a broader sense, dogma represents a rigid and uncompromising adherence to established beliefs, even in the face of contradictory evidence or alternative viewpoints. It can limit critical thinking and stifle open-mindedness.Ways I have adjusted my yoga practice:I still train daily, 6/7 days are really structured, where one day is long, and the next is short. This alternates between “hard and soft days.”I have incorporated Iyengar Yoga Method, Strength Training and Running into my training, to complement Ashtanga Yoga Method. On my short vinyasa days, I use props and do some static stretching and postures outside of vinyasaI have added muscle to my body and am making tradeoffs with asanas I can afford to part with, in exchange for the extra size. I focus on yogic behavior code and ethics more than on the physical aspects of yoga. I I ask myself "Am I becoming a better person?"How I've become less dogmatic about eating:1. I Focus on Whole Foods and Nutrient Density 2. I Eat Moderate Portions 3. I Listen to my body: I Eat when I'm hungry and stop when I'm satisfied. This intuitive approach to eating can help you build a healthier relationship with food.4. Evidence-Based Choices: Make dietary choices based on scientific evidence and individual needs. Hey Everyone welcome to The Art of Aging Mindfully Podcast. I'm your host Jai Sugrim.On episode 50, I allow you the listener to peer right into my training logs for weeks 7,8, and 9 (out of 13 weeks) as I prepare for the Philadelphia Marathon, on November 19th, 2023.This three week block was a massive success, because I made my mileage goal of 50 miles per week for every week. Sachin Patel and Dr. Drew Taylor discuss Drew's early foray into medicine when he shadowed an orthopedic surgeon doing total knee arthroplasty, Drew's interest in following his father into baseball and medicine, and how Drew earned his Ph.D. in Biomedical Engineering. He picked total-knee arthroplasty for his seventh-grade science fair project. He was permitted to scrub in and shadow a surgeon to observe a total knee replacement surgery. The next day, he accompanied the surgeon on rounds and watched the patient stand up out of bed and embrace the surgeon. [4:54] Drew talked to the surgeon about witnessing this emotional experience for the patient. The surgeon told him that the metal and plastic parts will eventually wear down and the patient will be back for revision surgery. The surgeon predicted that in the future this operation will be done with the patient's cells to replace cartilage, bone, and tissues. [6:23] Years later, Drew worked at the same hospital on a biomedical skeletal tissues engineering team with the Division of Orthopedics, trying to engineer joints for patients on demand. It was his science fair experience that led him there. [8:42] Drew attended Baseball Spring Training after college but he was injured before the regular season, so he never played in the Major Leagues. He played for both the Blue Jays and the Phillies on their Minor League squads. Drew became a Ph.D. student in biomedical engineering at the University of Toronto while he played baseball. [11:22] Drew explains stem cells. The embryonic stem cell can form any cell. They are pluripotent. Other stem cells have started to develop but can still become one of several cells. They are multipotent. Technology takes time to develop to a level to benefit people. [16:36] Drew's company Acorn Biolabs focuses on giving clients access to their cells. Taking donor cells always carries a risk of rejection or tumors. The patients receiving their cells is always the best choice. [18:19] Acorn Biolabs collects cells from clients while they are younger. Younger cells are better than older cells to leverage for the rest of your life. Sachin points out that nothing can fix your body better than it can fix itself. [22:04] Drew addresses how and when it is best to harvest cells. Acorn harvests hair follicles, which have thousands of stem cells. The quality of the hair follicle begins to decline as early as in the twenties. About 50 hair follicles are plucked per client and placed in a solution. [24:59] Drew explains the process for protecting the harvested cells for transport and freezing them to -190 degrees Celsius for when you need them in the future. [27:50] Acorn Biolabs has some amazing methodologies they use to develop different cells you may need or the frozen cells can be sent to the laboratory of your choice. Drew lists some of the products the cells can produce. [31:22] Drew shares an example of a use case in research and development: creating your pancreas islet beta cells to produce insulin for type 1 diabetics. This project will take years before human application. Other use cases are closer, such as producing collagen to deliver to the skin. Hair regrowth and wound closures are other potential use cases and sports injuries. [34:05] Drew's brother is a type 1 diabetic. Drew made sure his follicles were banked as quickly as possible so he would have his youngest cells cryogenically preserved and available. Drew believes in his brother's lifetime we will see these things made possible. [34:41] Drew addresses correcting problems with auto-immune diseases. Auto-immune problems are multi-layered and will take time to solve. Gene editing technologies like CRISPR can help eliminate some problems. Three-D printing of organs is on the horizon. [37:13] The biggest ethical considerations have been mitigated. The new technologies do not use embryonic stem cells. We can reprogram your cells. The importance is in intercepting your cells at their best. [39:11] It's difficult to predict for any individual at what age cells are not viable. Acorn Biolabs does not put a cut-off age for individuals to harvest their cells. The younger, the better. [41:51] Drew says there's a law of diminishing returns in working to maximize your health through lifestyle changes before harvesting your follicles. We are always getting older. Don't wait too long. Regenerative cell therapy may one day help us all grow our own replacement parts. People with heart failure could get a transplant for an entirely new heart, made out of their own cells. Burn victims, or people who just want wrinkle-free skin, could grow new sheets of the stuff — from their own tissues. Younger cells may provide better raw material for these therapies than older versions, and so a commercial cell banking industry has sprung up to provide the required cryogenic storage for about the price of a Netflix subscription. But is it worth it? INSIGHTS Revolution is right around the corner Regenerative cell therapy and commercial cell banking services are growing more popular, but they’re based on technology that’s been around for decades. So why now? According to Taylor, more than 1,000 clinical trials are happening in the field of cell and gene-based therapies. The Acorn Biolabs founder likes to compare it to the Wright brothers. They successfully flew the first powered aircraft in 1903. An aeronautics industry and commercial flight took decades longer to develop. Stem cell science is undergoing a similar curve. [Time code: 7:10] PRP and 3D printing of human replacement parts Currently, a goal of regenerative cell technology is to concentrate the growth factors and nutrients in platelet-rich plasma (PRP) and put it at the site of an injury. The idea is that this will enhance the inflammatory response in the surrounding cells and encourage healing. It’s used in soft-tissue sports injuries as well as skin-rejuvenation in dermatology. Another potential use for commercial cell banking is the 3D printing of replacement parts for the human body. For example, at Tel Aviv University, they took a biopsy of skin cells, multiplied them and were able to 3D-print a miniature human heart. “It was a major milestone,” Taylor says. [Time code: 9:30] Taylor’s argument that young cells are better for cell therapies “We need to be thinking about strategies that we could deploy to allow people to actually secure a better cell population to use as a starting material,” Taylor says of his work at Acorn Biolabs. In the past, what he’s found is that adults who are in pain will start looking at regenerative cell therapy. The problem? At that point, the cells and tissues in that area already are in a diseased state. What Taylor wants to help people do is get ahead of these problems by collecting younger, healthy cell samples and storing them for potential further use. “We really need to be thinking about this in advance,” Taylor says. “To be thinking about strategies we [can] do to allow people to secure a better cell population to use as a starting material.” [Time code: 13:55] Counterpoint Aaron Levine is an associate professor at Georgia Tech and a member of the International Society for Cell & Gene Therapy, where he was a Vice Chair on the Presidential Task Force on the Use of Unproven and/or Unethical Cell and Gene Therapies. The taskforce concluded that “these companies — that will take your cells and freeze them today for potential use sometime in the future — are too far ahead of the science.” Basically, in Levine’s view, the science is not yet there to justify cell storage. “These companies may be over promising to their customers,” Levine says. “The reality is that the vast majority of these cells will remain frozen indefinitely and never used clinically.” [Time code: 23:15] Critical thinking Taylor disagrees with Levine's point that cell banking is speculative — he argues that the science is there and will continue to advance. Not all cell-banking services are created equal, Taylor says. Levine’s investigation also was completed before Acorn Biolabs was founded. “There is a massive difference between companies that offer cell-banking services,” Taylor says, adding that it’s likely that certain organizations probably are over-promising. “Personally, I would be mortified to be grouped in with some of them.” [Time code: 26:47] Practical applications Although you can never make guarantees in science, regenerative cell therapy is already showing real-world results, Taylor says. This type of therapy could help heal athletic injuries, or be used as an aesthetic skincare treatment. But there are bigger potential uses, too. For example, he says, in Canada 24 patients have had keratinocyte and fibroblast cells harvested and leveraged to create sheets of skin, which were then used to treat severe burns. There are also about a dozen people in North Carolina who have had 3D bladders created using their own cells and implanted in their bodies. “We’re already seeing cells being leveraged in some of these therapeutics,” Taylor says. “So I’d push back a little bit” on Levine’s points.
