Most of us will never have our insulin levels tested, yet this single hormone is the body's master regulator of energy. When it stops working properly, almost every system in the body is affected, from how we store fat, to how we age. Today's guest is Dr Ben Bikman, a scientist and world-leading expert on insulin and metabolism. He's a Professor in the Department of Physiology and Developmental Biology at Brigham Young University (BYU) and has spent years researching how changes inside our cells lead to common metabolic disorders, such as obesity, type 2 diabetes, and dementia. He frequently publishes his research in peer-reviewed journals, speaks at scientific conferences all over the world and is the author of 2 books, Why We Get Sick and How Not to Get Sick. In this conversation, we discuss: Why insulin is the body's “master hormone”, guiding how we store and use energy and how resistance to it can cause a wide range of chronic health problems. The fact that almost 9 in 10 adults show signs of poor metabolic health, and why this is a global issue that affects countries far beyond the United States. How ethnicity and genetics shape the way we store fat, explaining why two people of the same weight and size can face very different risks of conditions like type 2 diabetes or heart disease. Why focusing only on blood glucose misses the early warning signs of poor metabolic health, and why measuring insulin levels offers a clearer and earlier picture of risk. The visible clues your body may already be giving you – such as skin tags or darker, velvety patches of skin around the neck or armpits – that can indicate chronically elevated insulin. Practical strategies to bring insulin down, from reducing refined sugars and starches to spacing out meals and experimenting with fasting in ways that work for both men and women. So often, we're told that chronic illnesses are inevitable or a natural part of ageing. But as Ben explains, many of these conditions have a common origin – and by focusing on insulin resistance, we can take powerful steps towards prevention and even reversal. Their practice, Cherokee Children's Dentistry, has been serving the needs of children in the greater Atlanta area since 2007. Mazzawi is passionate about working with children and serving as an advocate for all child related issues. Mazzawi has been heavily involved with the American Academy of Pediatric Dentistry and organized dentistry throughout her career. She served as the president of The College of Diplomates of the American Board of Pediatric Dentistry in 2018 and has also been a part of several committees and councils for the American Academy of Pediatric Dentistry. Currently, she is the chairman of the Scientific Program Committee for the Annual Session. In this episode, we talk with two authors who have papers featured in the special issue of the Journal of Genetic Counseling on Research Methods in Genetic Counseling. In the first segment we explore implementation science and its utilization in bridging the gap between research and clinical practice. In our second segment, we talk to an author about retrospective chart reviews and the benefits and drawbacks of this methodology.   Segment 1: A guide to utilizing implementation science for genetic counseling   Alanna Kulchak Rahm is a certified genetic counselor and implementation scientist with a PhD in Health and Behavioral Science. She has spent her career specializing in the implementation of genomics and precision health in healthcare systems. For over 25 years, she has conducted research on the utilization of genetic information by individuals and healthcare systems, new paradigms for identifying individuals with genomic risk, and new service delivery models for genomic testing. She has been a driver for the integration of implementation science and patient engagement to understand and study the integration of genomics into the learning health system, and is a tireless advocate of implementation science in genetic counseling. She has participated in and led many workshops and trainings on implementation science in genetics, serving as a faculty mentor for the NIH Training in Dissemination and Implementation Research in Cancer (TIDIRC) and recently as a co-lead of the Training in Dissemination and Implementation Research in Genomics and Precision Public Health (TIDIR-GPPH). She is currently a Program Director in the Division of Genomic Medicine at the National Human Genome Institute (NHGRI) where she directs the Network of Genomics-Enabled Learning Health Systems and other programs and continues to advance the integration of implementation science and genomics.   In this segment we discuss: Implementation science (IS) as a bridge between research and clinical practice in genetic counseling Misconceptions about IS, key frameworks like RE-AIM, and practical applications in daily work Using IS to identify and reduce inequities in genomic medicine Future integration of IS into training, research, and professional practice Link to the 2025 annual conference on dissemination and implementation    Segment 2: Leveraging hindsight: A retrospective chart review how-to for genetic counselors   Dr. Ramsey is the Section Chief of Individualized Therapeutics in the Division of Clinical Pharmacology, Toxicology and Therapeutic Innovation at Children's Mercy. She is leading the implementation of a pharmacogenomics program that is fully integrated with the electronic health record, developing model-informed decision support for several medications, and Co-director of their Pediatric Clinical Pharmacology Fellowship Program. Before joining Children's Mercy, Dr. Ramsey was an Associate Professor and co-director of the Genetic Pharmacology Service at Cincinnati Children's Hospital. She completed her postdoctoral fellowship in Pharmacogenetics at St. Jude Children's Research Hospital and received her PhD in Molecular, Cellular, Developmental Biology and Genetics from the University of Minnesota – Twin Cities. My guest today on the Online for Authors podcast is Twila Mason, author of the book Piece by Piece.  Twila is an award-winning romance author. She has been writing her own stories since she learned how to read. She is a writer of romance, women's fiction, poetry, and short stories. Her work explores raw emotions and the complexities of life while finding the hope within every obstacle.   After obtaining a PhD in Cell and Developmental Biology, she switched gears to follow her lifelong dream of putting her fictional stories out into the world. She lives the rural, small-town life in Missouri with her husband, two kids, and menagerie of animals. When she's not reading or writing, Twila can be found crafting, farming, doing home improvement projects, or going on adventures with her family.   In my book review, I stated Piece by Piece is a friends-to-lovers contemporary romance that takes a hard look at the drug culture - and what it does to families. Aria spends her childhood looking after her baby brother, Ben, because her mother simple cannot. As a drug addict, she is often high, always unreliable, and rarely offers motherly support. When her mother spends the rent money, Aria arrives home to find the family is now on the street. She turns to an old friend, Luke, who found a way out of the old neighborhood.   Although there is an immediate spark between them, Luke and Aria dance around the attraction because neither is ready to commit. We get front row seats as we watch them battle their demons and navigate a new world where peace, hope, and love have a chance to exist. As completely foreign concepts, neither one really knows what to do when life isn't a battlefield.   That's not to say everything is easy. Moving past trauma and finding the right path is not easy. Nor is forgiveness - of others or of self. And what to do when the ones you should love are the ones that keep hurting you?   This story is sweet as two young people fall in love. In today's OncClub episode, we had the pleasure of speaking with Branden Moriarity, PhD; and Beau Webber, PhD, about a study evaluating neoantigen-reactive CISH knockout tumor-infiltrating lymphocyte (TIL) therapy in patients with metastatic gastrointestinal cancers. Dr Moriarity is an associate professor in the Division of Pediatric Hematology/Oncology, co-director of the Center for Genome Engineering and the Genome Engineering Shared Resource (GESR), and a member of the Center for Genome Engineering at the University of Minnesota in Minneapolis, where he is also a faculty member in the Department of Pediatrics; the Microbiology, Immunology and Cancer Biology (MICaB) PhD Graduate Program; the PhD Program in Molecular, Cellular, Developmental Biology and Genetics; the Cellular and Molecular Biology (CMB) Graduate Program, and the Masters Program in Stem Cell Biology. He is also a member of the Stem Cell Institute at the Masonic Cancer Center. Dr Webber is an associate professor in the Division of Pediatric Hematology/Oncology, co-director of the GESR, and a member of the Stem Cell Institute at the University of Minnesota Medical School, where he is also a faculty member in the Department of Pediatrics, the Molecular Pharmacology & Therapeutics Graduate Program, and the Masters Program in Stem Cell Biology. He is also a member of the Masonic Cancer Center. Parent-specific epigenetic marks (imprints) leading to parent-specific gene expression are crucial for normal growth and development, yet their mechanisms of establishment and maintenance are not fully understood. In humans, approximately 200 imprinted genes have been discovered, and improper imprinting can manifest in growth restriction, obesity, intellectual disabilities, behavioral abnormalities, and an increased risk of certain cancers. While the use of pluripotent stem cells, especially those in the naïve state, have advanced aspects of modeling early development, a persistent issue hampering bona fide naïve hPSCs is the erosion of imprints. Our guests on today's episode will discuss genomic imprinting, its function, impact, and a new reporter system of imprinted gene expression in hPSCs that enables real-time visualization of loss-of-imprinting at single-cell resolution. This assay provides an important tool to help discover how to improve the imprint fidelity of naïve hPSCs and hence their application for studies of human development and regeneration. At the Artificiality Summit 2024, Michael Levin, distinguished professor of biology at Tufts University and associate at Harvard's Wyss Institute, gave a lecture about the emerging field of diverse intelligence and his frameworks for recognizing and communicating with the unconventional intelligence of cells, tissues, and biological robots. This work has led to new approaches to regenerative medicine, cancer, and bioengineering, but also to new ways to understand evolution and embodied minds. He sketched out a space of possibilities—freedom of embodiment—which facilitates imagining a hopeful future of "synthbiosis", in which AI is just one of a wide range of new bodies and minds. Bio: Michael Levin, Distinguished Professor in the Biology department and Vannevar Bush Chair, serves as director of the Tufts Center for Regenerative and Developmental Biology. Recent honors include the Scientist of Vision award and the Distinguished Scholar Award. His group's focus is on understanding the biophysical mechanisms that implement decision-making during complex pattern regulation, and harnessing endogenous bioelectric dynamics toward rational control of growth and form. Michael Levin is a Distinguished Professor in the Biology department at Tufts University and associate faculty at the Wyss Institute for Bioinspired Engineering at Harvard University. Prof Levin holds the Vannevar Bush endowed Chair and serves as director of the Allen Discovery Center at Tufts and the Tufts Center for Regenerative and Developmental Biology. Prior to college, Michael Levin worked as a software engineer and independent contractor in the field of scientific computing. He attended Tufts University, interested in artificial intelligence and unconventional computation. To explore the algorithms by which the biological world implemented complex adaptive behavior, he got dual B.S. degrees, in CS and in Biology and then received a PhD from Harvard University. He did post-doctoral training at Harvard Medical School, where he began to uncover a new bioelectric language by which cells coordinate their activity during embryogenesis. His independent laboratory develops new molecular-genetic and conceptual tools to probe large-scale information processing in regeneration, embryogenesis, and cancer suppression. Dr. Barbara Katzenback is an Assistant Professor in the Department of Biology at the University of Waterloo. Barb studies how frogs defend themselves from diseases to stay alive, and she also investigates how the environments that frogs live in impact their ability to defend against diseases. Frogs and other amphibians are critical components of the food web, they are important for pest control, they are indicators for the health of their environment, and they produce interesting molecules on the surface of their skin that may have relevance for human health. Barb's hobbies outside of science include engaging in a variety of physical activities like running, yoga, and hiking with her husband and two dogs. She's also experimented with soccer, spin classes, cross country skiing, rock climbing, and inner tube water polo. Some of her other interests include pottery, art, crafting, baking, gardening, sewing, and leisure reading. Barb earned her BSc with Honors in Immunology and Infection from the University of Alberta. She received her PhD in Physiology, Cell, and Developmental Biology from the University of Alberta as well. Barb was next awarded a Natural Sciences and Engineering Research Council (NSERC) Postdoctoral Fellowship to conduct research at Carleton University, followed by an NSERC Banting Postdoctoral Fellowship to conduct research at the University of Waterloo before joining the faculty there. In our interview, Barb speaks more about her experiences in life and science.

In 1999, as an assistant professor, he started his first independent research laboratory at the Lerner Research Institute of the Cleveland Clinic. There, his research on polycystic kidney disease began with the support of a local family affected by PKD and whose family foundation still supports PKD research in Dr. Weimbs' lab today. In 2005, Dr. Weimbs moved his laboratory back to California to join the Department of Molecular, Cellular, and Developmental Biology at UCSB. The laboratory studies molecular mechanisms underlying PKD with an emphasis towards developing new therapies. Recent research has led to the discovery that dietary interventions and natural metabolites strongly slow PKD progression in animal models. This grant allowed him to establish an independent research group called 'neuronal nutrition physiology' at the Philipps University of Marburg, where he explored the impact of nutrient sensing in the brain on the regulation of body weight and glucose homeostasis as one of the youngest independent group leaders in Germany. From 2012-2014 in addition to his group leader role he was Head of Department for Animal Physiology at this institution before he was appointed at the University of Otago in 2014 in Physiology.   Throughout his career, Alex Tups has contributed significantly to his field, publishing 48 peer reviewed research papers, the vast majority as lead author. His research has contributed to our understanding of how the brain controls body weight and blood sugar levels. His work has been internationally recognized with notable awards, such as the Michael Harbuz Price in neuroendocrine research in the UK in 2009, the Ernst and Berta Sharrer Award in Germany in 2014, the Commercialisation Researcher Award at Otago University in 2023 and the Research Excellence Price by the Physiological Society of New Zealand in 2023. Dr. Ardelle Piper is a Royal College Certified OB/GYN, and focuses her gynecology practice on midlife women's health. She is known as a leader and pioneer in telemedicine, and provides menopause assessments over videoconference to women across Ontario since 2020. As a subject matter expert, Dr. Piper is frequently called upon as keynote speaker to provide menopause education to medical colleagues and the community, and collaborates with other social media educators. She advocates for improved appreciation of menopause in organizations and the general public, and health education in workplace wellness programs.Dr. Piper completed a BSc(H) in Cell and Developmental Biology, then achieved her medical degree (MD) at the University of British Columbia in Vancouver in 2006. Appreciating her passion for women's health and education, she pursued an Obstetrics & Gynecology residency and a MEd in Saskatchewan, with a thesis on School Based Sexual Health Education. Through this she enjoyed leadership opportunities as a provincial representative with the Canadian Association of Interns and Residents (CAIR) and was Chair of the Junior Member Committee of the Society of Obstetricians and Gynecologists of Canada (SOGC). During her obstetrical training and career, Dr. Piper welcomed into the world over 1500 babies, and co-chaired the Management of Postpartum Hemorrhage Simulation Workshops at the Federation of International Gynecologist and Obstetricians (FIGO) World Congress in Vancouver (2015) and Brazil (2018).She realized her interest in education, advocacy, and women's health aligned perfectly with the field of Menopause, and she achieved a final year of sub-specialty training in Midlife Women's Health at The Shirley E. Prof Levin holds the Vannevar Bush endowed Chair and serves as director of the Allen Discovery Center at Tufts and the Tufts Center for Regenerative and Developmental Biology. Prior to college, Michael Levin worked as a software engineer and independent contractor in the field of scientific computing. He attended Tufts University, interested in artificial intelligence and unconventional computation. To explore the algorithms by which the biological world implemented complex adaptive behavior, he got dual B.S. degrees, in CS and in Biology and then received a PhD from Harvard University. He also oversees the Center for International Education, the Office of the Registrar, and the Office for Institutional Effectiveness. Born in Bowling Green, Kentucky and raised in northern New Jersey, Del Mastro earned his B.A. at Wake Forest University, his M.A. at Middlebury College, and his Ph.D. at the University of Virginia. After defending his doctoral dissertation in August 1992, he relocated to Charleston, South Carolina to begin a career at The Citadel, where for 18 years he taught a variety of courses to include Spanish language and literature, and Hispanic culture and business. In July 2010 Del Mastro joined the College of Charleston where he served as Chair of the Department of Hispanic Studies until August 2019 when he transitioned to his current role as Associate Provost for Academic and International Programs in the Office of the Provost and Academic Affairs. Chris Korey, professor of biology and associate provost for student success, leads the office for the academic experience, which provides students with educationally purposeful activities that lead to deep learning, integrated social and intellectual development, and engagement with local and global communities. In this role, Korey leads eight units and multiple programs. Korey joined the College of Charleston's Department of Biology in 2003. Most recently, Korey served as Associate Dean for the School of Sciences and Mathematics, a role he has served since 2020. Korey earned his B.S. in Biochemistry from the University of Notre Dame and a Ph.D. in Cell and Developmental Biology from Harvard University. An active scholar, his past research focused on the development and plasticity of the invertebrate nervous system and model systems for studying human genes associated with neurodegenerative disease. Dr. Irving Weissman is the Virginia and Daniel Ludwig Professor of Clinical Cancer Research and Professor of Pathology and Developmental Biology at Stanford University. Dr. Weissman was the first scientist to identify and isolate mammalian blood-forming stem cells in mice, and his work has contributed to the understanding of how a single hematopoietic stem cell can give rise to specialized blood cells. He talks about developing new treatments for metastatic breast cancer and severe combined immunodeficiency. He also discusses myeloid bias in aging and strategies for commercializing new therapies.

Thomas Weimbs is a professor at the University of California in Santa Barbara (UCSB) where he directs a research laboratory focused on polycystic kidney disease (PKD). He received his doctoral degree from the University of Cologne, Germany, and then conducted postdoctoral research at the University of California in San Francisco. In 1999, he started his first independent research laboratory at the Lerner Research Institute of the Cleveland Clinic as an assistant professor. There, he started to research PKD after he was approached by a local family affected by PKD who financially supported PKD research in his newly-founded laboratory. Their family foundation still supports PKD research in Dr. Weimbs' lab today. In 2005, Dr. Weimbs moved his laboratory back to California to join the Department of Molecular, Cellular, and Developmental Biology at UCSB. His laboratory studies molecular mechanisms underlying PKD with an emphasis towards developing new therapies. Research from Dr. Weimbs' lab has led to a better understanding of PKD and led to novel methods to deliver therapeutics specifically to polycystic kidneys. Some of the most recent research has led to the discovery that dietary interventions and natural metabolites strongly slow PKD progression in animal models. This led to the launch of Santa Barbara Nutrients with the goal to make these approaches available for the nutritional management of PKD and to support kidney health. Research in Dr. Weimbs' laboratory has been funded by grants from federal agencies, including the National Institutes of Health and the Department of Defense, by private foundations, including the Lillian Goldman Charitable Trust, the Amy P. Goldman Foundation and the Jarrett Family Fund, as well as several biotech companies. Liza Konnikova, MD, PhD, FAAP, is a neonatologist at UPMC Children's Hospital of Pittsburgh and an assistant professor in the Departments of Pediatrics, Developmental Biology and Immunology in the University of Pittsburgh School of Medicine. Her lab focuses on the develop of neonatal immunity at mucosal surfaces and its role in the pathogenesis of diverse diseases such as sepsis, preterm labor, necrotizing enterocolitis, and very-early onset IBD (VEO-IBD). She has pioneered the application of mass cytometry to frozen mucosal biopsies, helping to improve our understanding of mucosal immunity. This innovation has helped to identify novel populations of immune cells in VEO-IBD and to perform serial monitoring of IBD activity during drug trials.

Dr. Shankar Srinivas is a Professor of Developmental Biology in the Department of Physiology Anatomy and Genetics based in the Institute for Developmental and Regenerative Medicine at the University of Oxford. He is also a Zeitlyn Fellow and Tutor in Medicine at Jesus College. Using mouse and human embryos as model systems, his group looks at the control of patterning and morphogenesis during the establishment of the anterior-posterior axis, gastrulation, and early cardiogenesis.  He discusses how tissues respond to forces during early development, characterizing cardiac progenitors, and his experience training internationally.

Dr. Thomas Weimbs received his doctoral degree from the Department of Biochemistry of the University of Cologne, Germany, in 1993. He conducted postdoctoral research at the Department of Anatomy, University of California at San Francisco until 1999. In the same year, he joined the Department of Cell Biology in the Lerner Research Institute of the Cleveland Clinic as an Assistant Professor where he established his research laboratory focusing on investigating membrane trafficking and epithelial cell polarity as well as molecular mechanisms underlying polycystic kidney disease. In 2005, Dr. Weimbs was recruited to the Department of Molecular, Cellular, and Developmental Biology and moved his laboratory to UCSB. Dr. Weimbs is currently an Associate Professor in MCDB and in the Neuroscience Research Institute. In this episode, Drs. Tro and Thomas talk about how Dr. Thomas became interested in researching PKD (Polycystic Kidney Disease), what PKD is, research that Dr. Thomas has done on the impact of ketosis on PKD, a recently published paper on the negative impact of the ketogenic diet on kidney health, how Dr. Thomas' colleagues have reacted to his research, and lifestyle intervention programs using ketogenic metabolic therapy.
