Die Gegensätze beginnen gleich im Nachtzug von Bukarest nach Chișinău. Im zügigen Tempo durcheilt der Zug Rumänien auf dem Weg zur moldauischen Grenze. Im Bordbistro spielt der Barkeeper mit dem Schaffner ein Brettspiel, die Musik kommt aus einer neuen Bluetooth-Box. Im Abteil steckt das neue Ladegerät von unserem Reporter Valentin Beige in einer recht alten Steckdose. Nicht nur der Nachtzug steckt voller Gegensätze, auch das Land Moldau selbst ist voll von ihnen. Cafés, die von den Preisen und der Machart her auch in Budapest oder Berlin liegen könnten, treffen auf außergewöhnliche Kellerbars. Oder auch die Stadt Chișinău, die mit ihren beiden Gesichtern aufwartet: dem sowjetisch geprägten Teil und der historischen Altstadt mit kleinen Häusern und geschotterten Straßen. Und geschotterte Straßen führen auch quer durch das Land. Unter anderem auf dem Weg in die Region Orheiul Vechi, nach Alt-Orhei. Die Region ist seit der Steinzeit besiedelt und besticht mit beeindruckenden Felsenklöstern, wunderschönen Ausblicken und ganz viel Charme. Und um eines kommt kein Moldau-Besucher herum: Wein. In Moldau dreht sich vieles um Wein. Bis vor wenigen Jahren lieferte das Land viel Wein nach Russland. Seit einem Embargo ändert sich das und Moldau setzt jetzt auf Qualitätsweine. Doch ohne Exporte könnte die moldauische Weinbranche nicht überleben. Valentin Beige ist in den Nachtzug und nach Moldau gereist. Selbst die Grenzkontrollen nachts um drei haben ihn nicht abgehalten. Aber ein Tipp für alle Nachtzugliebhaber: auf der Fahrt von Chișinău nach Bukarest ist die Grenzkontrolle am Abend und danach winkt eine Runde Schlaf in diesem ganz besonderen Nachtzug. So besonders wie das gesamte Land Moldau.

Today's guest is Aaron Uthoff. Aaron Uthoff, PhD, is a sport scientist and coach whose work sits right at the intersection of biomechanics, motor learning, and sprint performance. His research digs into acceleration, force application, and some less conventional forms of locomotion, including backward sprinting, with the goal of connecting solid science to what actually works on the field, track, or in rehab. Backward running shows up all the time in warm-ups and general prep. Most of the time, though, it's thrown in casually, without much thought about what it might actually be doing for speed, coordination, or tissue loading. In this episode, Aaron walks through his path into performance science, which is anything but linear. From skiing in Montana and playing desert sports, to football and track, to a stretch training horses in Australia, his journey eventually led him to research mentors in Arizona, Scotland, and New Zealand. That broad background shows up clearly in how he thinks about movement. One of the big takeaways from our conversation is Aaron's overview of research showing that structured backward running programs can improve forward acceleration and even jumping ability. We also get into how backward running can be used as a screening and coordination tool, and where it fits into rehabilitation, including what's happening at the joints, how muscles are working, and how to progress it without forcing things. For the past ten years, he has also led the CARIN Alliance, a bi-partisan, multi-sector alliance uniting industry leaders to advance the adoption of consumer-directed exchange across the U.S.In January 2023, Ryan joined Keith Figlioli on the podcast to discuss the myriad of new possibilities emerging in healthcare as a result of better access to data. In this episode, he recounts the progress and obstacles since that conversation, but more importantly, helps unpack the flurry of new activity.Topics Ryan and Keith covered include:ACCESS & TEMPO. These are the latest examples of two new government programs that Ryan believes will remove barriers to innovation. ACCESS is a CMS initiative that now makes it possible for technology companies to bill Medicare directly for digital health services – and get paid only when patients achieve specific, measurable clinical outcomes. Ryan explains how ACCESS is a breakthrough for transparency and has the potential to change contracting for digital health vendors as health system may now ask to share risk. TEMPO is a program from the FDA that complements ACCESS by allowing participating companies to bypass traditional device clearance processes through “enforcement discretion,” provided they share real time data with the FDA. Ryan explains how this oversight lowers cost and complexity for startups and accelerates the path to market for new digital health solutions.Removing administrative roadblocks. In early 2025, Ryan's team at Leavitt Partners published a paper titled, “Kill the Clipboard” that offered recommendations to cut administrative costs, lower the burden on consumers and providers, and modernize the health care data exchange ecosystem. Ryan discussed recommendations like the need for stronger enforcement of information blocking rules and suggestions for the government to change its certification program to focus on APIs, versus functionality of EHRs. He explained how these things would allow health systems to control their own data, build cloud-based workflows, and integrate with payers and innovative companies more easily.Linchpins for data liquidity. Ryan believes that achieving true data liquidity in healthcare requires three foundational elements: a cloud-based data store, an API endpoint, and robust digital identity credentials. With these in place, he says organizations can exchange data securely and efficiently, supporting everything from public health to quality measurement and pharmacy exchange.
