In 2019, middle distance runner Catriona become the first Australian woman in a decade to break the two-minute barrier in the 800m. Later that year, she set a new Australian record of 1:58.78, surpassing the 43-year-old record set by Charlene Rendina. One of the most dominant middle-distance runners in the world that year, Catriona describes her return to the sport as a ‘happy accident', after nearly a decade away from the sport while battling an eating disorder. This series is presented by Nike, championing athletes like Catriona, listening to athlete's needs and supporting them throughout all their journeys. One of my favourite parts of this episode was Catriona's discussion around her postgraduate degree in architecture, and in particular her thesis on running tracks, a beautiful connection that she feels between movement and making spaces a home. The trend saw female influencers posting videos of themselves wearing leggings, with the subtext promoting the idea that you need slim legs to do so. It quickly saw backlash over concerns that it could lead girls towards unhealthy eating or exercise behaviours, even leading to intervention from TikTok itself, now when you search for #legginglegs you'll be directed to Butterfly Foundation resources).  So why do we keep seeing these diet culture trends repackaged? On this week's episode I am excited to be introducing you to our next special guest Clinical Psychotherapist and Founder of Wings and Quill Jaqueline Byrnes. Wings and Quill provides a range of services for the treatment of eating disorders including but not limited to psychotherapy, peer support groups, workshops, group programs and more. Jacqueline is trained with a Masters in Clinical Psychotherapy with further post graduate studies in narrative approaches, research and eating disorders. Jacqueline also holds additional qualifications in English literature, education, creative and professional writing and nutrition. As a writer Jacqueline has several articles, journal contributions and creative writing pieces published and is hoping to have her first book published soon. Having produced a body of research in the qualitative narrative analysis of eating disordered experiences. Jaqueline has over 7 years of experience in her field where Jaqueline is now the Clinical Manager of Support Services for the leading national eating disorder organisation, The Butterfly Foundation. Jacqueline's work in psychodynamic approaches has been widely acclaimed with the popularity of her programs in Sydney for the Butterfly Foundation leading to a national service being implemented as state-based programs being offered across Australia. Most notably Jacqueline developed, implemented and managed the Butterfly Foundations e-health initiative ED HOPE a national Tele Web program, the first service of it's kind in Australia. Regarded as an expert in her field Jacqueline is a valued member of the Expert Advisory Board for the Butterfly Foundation and contributes to research projects on the recovery for those with eating disorders. Jaqueline is also an active member on a variety of boards and organisations including the Qualitative Research in psychology Initiative, NEDC, ANZEAD and CAPA. Jaqueline is a facilitator of workshops and programs as well as is a supervisor for other clinicians in the eating disorder field. On this week's episode I am excited to be speaking about the topic of diet culture and our relationship with our bodies. As the relationship that we have with our bodies largely influences our body image. On this episode Jacqueline will so kindly explore the relationship we have with self as the primary focus for wellbeing. While also contributing to the understanding we may have around bodies, eating disorders and body image. In this episode, we sit down with occupational therapist Dr Elysa Roberts to discover why OT can be effective in treating eating disorders.  First, what is an occupation? It's generally defined as “any activity in which one engages”, which means it's a lot more than just a job. An occupation can include eating, exercising, working, shopping, socialising, parenting, education, time spent on spiritual practice... the list just goes on. Occupations are all the ordinary and everyday activities of life. An eating disorder can be considered an occupation, but one that isn't good for your health or well-being. Recovery could also be defined as an occupation, and that's why Occupational Therapists can play a key role in a person's treatment team. Dr Roberts, an OT since the early ‘90s, says that she realised the importance of OT as a treatment modality through her own lived experience. “I'd go through waves of treatment, be discharged, recover, and then go through it again all while establishing my OT career. Unfortunately, what wasn't happening in those days was strong relapse prevention programmes, or relapse prevention being included as part of recovery.” Now things are changing. Dr Roberts describes how OT can help – first by bringing a unique and holistic understanding of an individual's mental, physical and sensory profile, but also by co-designing practical interventions that can make all the difference to ongoing recovery. Kai Schweizer, who identifies as gender diverse, first joined the Butterfly podcast in 2020 for the episode Gender, Sexuality and Eating Disorders. Our host Sam spent some time getting to know how Kai's emerging gender identity at an all-girls secondary school led to disordered eating behaviours. He's now a respected specialist in LGBTIQA+ issues, including this community's experience of eating disorders."We know that treating your discomfort with your body, your dysphoria, or your incongruence through disordered eating doesn't solve the problem you're experiencing," he says. "It doesn't reduce the distress that you're feeling."While Kai's teenage years were a struggle, he found a path to recovery. "It was amazing how quickly, upon starting to feel more congruent in the body I was living in, that it became more comfortable to nourish and care about it," he says. "It is really easy to not care for the body you live in when it doesn't feel permanent. As my body became more of a home for me, it became more important to take better care of myself."Today, Kai's a sexologist and researcher focused on trans and gender-diverse health. Two in three trans young people have engaged in disordered eating behaviours, and Australia needs safer treatment options for the LGBTIQA+ community. Negative body image can impact almost every aspect of your life, especially your sex life. And while fulfilling intimate relationships is a key component of general wellbeing, the shame hammered into us by societal messages about what bodies should look like is too often carried into the bedroom. It can seriously mess with our sexual experience.“Self-judgement and self-criticism make us feel low, and when we're feeling low, we don't feel particularly sexy,” says sex and relationships therapist, Adriene Cobcroft.Sex educator and broadcaster Nat Tencic agrees. “You're literally naked,” she says. “If you aren't comfortable in your skin, you're not going to be able to be in the moment and you're not going to be able to experience pleasure.”The good news is there are ways to improve our body image and by doing so enhance our sexual enjoyment. When Nik Mitchell learned to embrace his vulnerability and be more body kind, he opened himself up to a fulfilling sexual relationship. Bella Davis learned how to get out of her head and into her body, fully allowing herself to enjoy the moment. Dating app Bumble's Lucille McCart's insists that authenticity is key. Katie is in recovery from an eating disorder, and says her struggles with body image and disordered eating were triggered in high school, when she began to feel on “on the outer” from her straight peers.One million people in Australia will struggle with an eating disorder during their lifetime, and a significant number belong to the LGBTIQA+ community. Yet only 25% of those living with an eating disorder will ever seek help because they don't fit the stereotype.Kai, who identifies as trans, developed an eating disorder that suppressed his puberty, because he thought only women got eating disorders. Mitch came out as gay in the middle of a relapse of anorexia, but was told that his identity was just a symptom of his being unwell.The history of discrimination against LGBTIQA+ people, along with not enough trained professionals to serve this community, is why many do not get the treatment they need. The good news is there are people working to change that. The stereotypical eating disorder sufferer is a young, wealthy, white woman, but there's a lot of evidence telling us that the stereotype is inaccurate.Based on current numbers, one in three people living with an eating disorder are men, and there are a lot of men suffering in silence. In this episode of the Butterfly podcast: Let's Talk, one of the country's top eating disorder researchers, Dr Scott Griffiths from the University of Melbourne, says there could be a significant number of dudes with eating disorders we don't know about.“Men are really reluctant to go and see a doctor in the first place,” he tells us.That rings true for the three men we profile in Episode Two of Butterfly: Let's Talk, including Carlton and Melbourne Football star Brock McLean.The truth is, anyone, from primary school kids to middle-aged men—including sports stars like McLean—can get an eating disorder. Over 1 million Australians (about 1 in 20) currently have a clinically diagnosed eating disorder, and approximately 10% of Australians (15% for females) will have an eating disorder at some point in their life. I was shocked to find out these statistics from Juliette Thomson, the resident psychologist at The Butterfly Foundation. The conversation today dives deeper into the unspoken reality of eating disorders - from my own personal experience and the perspective of a clinical professional. I facilitated this conversation, wanting to open a window of understanding on the development of eating disorders and how we can best identify and support loved ones who may be grappling with this mental illness. As it happened, this podcast taught me far more about the complexities of identifying and understanding this highly prevalent mental illness. Juliette and the Butterfly Foundation, passionately advocate that everyone’s experiences with eating disorders are different. There are also some shocking statistics regarding eating disorders that are worth being aware of​ For example, on average, Anorexia affects an individual for around 7 years! However, for some the experience can extend for decades whilst for others this could be only two years. I hope this conversation can assimilate with the Butterfly Foundation’s mission of being a catalyst for change and increase the awareness of eating disorders in our society. Certainly,​ there are high-risk groups, but anyone and everyone can be affected by an eating disorder. Lucy Lauer, is a Licensed Mental Health Counselor with 26 years’ experience as a Registered Dietitian. She specializes in helping people navigate the rough waters of disordered eating to achieve a healthier life and relationship with food. She has experience with Anorexia Nervosa, Bulimia Nervosa, Binge Eating Disorder, Nocturnal Eating Disorder, and the range of dysfunctional eating patterns that cause distress but may not have a diagnostic label.   In 35 years of practice, Lucy has treated disordered eating from both a nutritional and psychological perspective. She has presented workshops at the local, state and national level on many aspects of disordered eating for psychologists and counselors, physicians, dietitians, parents and teachers. She established and facilitated the Space Coast Eating Disorders Professional Consortium from 2009-2011, and supervises Florida Tech Psychology students in a special practicum focused on Eating Disorder treatment.    She participates regularly in continuing education programs and has had over 60 hours of specialized training in ED treatment in the last 3 years alone. Specific topics have included ACT and other evidence-based interventions, medical complications, trauma impact, co-occurring disorders (substance abuse, self-injury, etc), assessment for appropriate level of care, body image, art and other expressive therapies, and using a team approach to treatment. She has also been invited to site visits at residential treatment centers in South Florida and Colorado.        What are the various types of eating disorders? Anorexia Nervosa, Bulimia Nervosa, Binge Eating Disorder   Since eating disorders have a direct impact on physical health as well, tell us a bit about the team approach to treatment when collaborating with Registered Nutritionists, Physicians, etc... The gold standard of ED treatment involves a team of clinicians working together. The team consists of a mental health counselor or psychologist, Registered Dietitian (RD), primary care physician, and if needed, a psychiatrist. In an out-patient setting, it is usually the mental health clinician who coordinates care by communicating with other team members re. treatment goals, progress toward goals, and psychoeducation, as needed. The RD monitors weight, food intake and specific food-related issues and may also help set exercise goals. Medical parameters such as labs, EKGs and bone-density are ordered by the PCP and shared with other team members as markers for progress. When all team members are not under the same roof (which is usually the case), communication is done through phone calls, written assessments and updates and occasionally, face-to-face team meetings. If the client is a minor, the parents are also part of the team and are included in treatment planning, progress updates and psychoeducation so they can more effectively support their child’s recovery.    Many people with an eating disorder have a fear of recovery.  Talk a bit about this and how a clinician can work with someone resistant about recovering from their eating disorder. An eating disorder is experienced as a life-raft in the rough sea of uncertainty that is a normal part of life. Despite the fact that eating disorders are deadly and create enormous physical and psychological pain, they also serve important functions such as protection from intrusion, a sense of control when life feels chaotic, a way to make complex issues seem more manageable and many others. In order to help a person let go of this safety net, the clinician has to help them:  1) recognize the costs to other valued parts of their lives such as relationships, energy & concentration for other persuits; 2) Identify the functions of the ED and Develop other ways to address those. This often requires practicing acceptance of the inherent uncertainty of life, imperfection of themselves and others, and the messiness of living in a human body with all of its flaws and limitations. Sometimes, cognitive growth and recovery cannot take place until weight and nutritional balance is restored and this takes education, support, encouragement and a leap of faith on the part of the client.    I know you facilitate a wonderful group for people with an eating disorder.  Tell us a bit about that group and how it's different from individual therapy? Our 8-week eating disorder therapy group employs a combination of mindfulness practice, self-compassion and some elements of ACT (acceptance & Commitment Therapy), which helps people identify personal values that the ED has robbed them of. There is a lot of discussion about perfectionism, fear of failure, body hatred, habitual responses to difficult emotions and practice slowing down and experimenting with alternative perspectives and behaviors. Much of the benefit from the group comes from the common experiences of the members who usually feel alien and outside of “normal” human connections. ED is a very isolating illness and it is progressive, so over the years, people feel more and more alone and ashamed of the disorder in addition to all the other things about themselves they already hate.     Where can clinicians go for more information, trainings, certifications, etc... My favorite resources are programs presented by The Renfrew Center & the Eating Recover Center, both of which have residential treatment facilities in several US cities and do a lot of research, outreach and education.
