Welcome to "Ditch the Lab Coat," the podcast where we break down complex medical topics with leading experts shaping the future of healthcare. In today's episode, we're diving into the nuanced and critical conversation surrounding Fetal Alcohol Spectrum Disorder (FASD) with our distinguished guest, Dr. Susan Rich. With a career spanning decades, Dr. Rich is a trailblazer in child and adolescent psychiatry and an ardent advocate for public health. She's here to shine a light on prenatal alcohol exposure's often overlooked impacts on brain development. Did you know that not only alcohol consumed during pregnancy but even prior to conception can lead to multigenerational health issues? Dr. Rich unpacks how prevalent this is, affecting 1 in 20 children, yet remains largely unrecognized. Throughout our discussion, we'll explore systemic challenges, the myths perpetuated by the alcoholic beverage industry, and most importantly, the critical steps we need to take for prevention and support. Whether you're a healthcare professional, a parent, or just intrigued by one of public health's hidden crises, this episode promises to be both enlightening and empowering. Get ready for a thought-provoking journey as we seek a healthier future for the generations to come.Key Takeaways :The Hidden Impact of Alcohol on Fetal Development Dr. Susan Rich explains that fetal alcohol spectrum disorder affects approximately 1 in 20 children (5% of the population), with far-reaching consequences beyond the commonly known physical characteristics.Early Pregnancy Vulnerability Critical developmental impacts occur as early as the third to fourth week post-conception, often before women know they're pregnant. About 13.5% of childbearing age women in America binge drink, and approximately 50% of pregnancies are unplanned.Beyond Traditional Understanding While medical education traditionally focused on severe cases with facial deformities and intellectual disabilities, Dr. Rich reveals that only 10-15% of affected children show these classic features. The majority have neurodevelopmental issues without obvious physical signs.Male Factor Contribution Alcohol consumption by men up to three months before conception can affect sperm quality through epigenetic modifications, potentially making the embryo more susceptible to alcohol exposure during pregnancy.Economic and Social Impact The alcohol industry generates approximately $220 billion annually, while the cost of caring for just those with full fetal alcohol syndrome (not including the entire spectrum) was estimated at $5.4 billion as of 2003 data.Prevention and Treatment Approaches Dr. Rich advocates for better public health messaging, comprehensive support systems for affected children, and the need for healthcare providers to better understand and recognize the full spectrum of alcohol-related developmental disorders.03:14 - Alcohol's multigenerational impact explained.08:27 - Alcohol's epigenetic effects on offspring.13:17 - Unplanned pregnancies and fetal alcohol syndrome.15:33 - Neural crest and facial development.19:18 - Fetal alcohol, nutrition, and growth.23:04 - Dr. Kathleen Sulick's contributions.25:26 - Alcohol's impact on family events.28:38 - Alcohol's cultural role and societal effects.30:57 - Alcohol's toxicity and informed consent.34:05 - Liquor profits and education funding.37:35 - Rethinking alcohol's role in society.42:56 - Global epidemic: Fetal alcohol impact.45:05 - Supporting neurodivergent youth transitions.47:50 - Taxing alcohol to aid impoverished children.51:01 - Alcohol's Hidden Health Impacts.54:12 - Rethinking alcohol's impact on children.DISCLAMER >>>>>>    The Ditch Lab Coat podcast serves solely for general informational purposes and does not serve as a substitute for professional medical services such as medicine or nursing. Steve and Susan give some background regarding the inspiration behind the conference as well as what will be covered during the symposium. The gathering is an opportunity to get the leading choline researchers together to update each other and build the momentum of choline research. The last time choline researchers gathered was in 1998, when requirements were set. (0:50)The next guest on our roster is Dr. Mark Manary, a professor of pediatrics at the Washington University School of Medicine. Mark's symposium talk discusses choline and food aid. Food aid products are specially designed to address needs from crisis situations. These specialized food aid products are standardized to meet great deficiency or inadequacy needs. On the most extreme side, there is a product called ready-to-eat therapeutic food for children who are starving to death. Other food aid products include those for children who are severely underweight. Dr. Manary's research consists of clinical trials in sub-Saharan Africa that include different nutrients in food aid to see if there are improvements in children's responses. One trial with the inclusion of DHA found a 6-15 IQ point difference by adding fish oil or DHA. Mark hypothesizes that a doubling of that effect will be observed when choline is added. (6:52)Next up is Dr. Kevin Klatt with the University of California - Berkeley. His symposium talk consisted of choline and DHA, focusing on two areas of his work. The first is dietary choline's impact on the production of phosphatidylcholine species enriched in the omega-three DHA, specifically in pregnancy. The second is interactions between lauric acid and choline, where a phosphatidylcholine species can actually bind to proteins that turn genes on and off. In one experiment, Kevin's group hypothesized that inadequate choline intake during pregnancy compromises the efficient handling of DHA by the liver. They showed in a randomized controlled trial that supplementation with choline dramatically improved the status indicators of DHA status. (17:33)Our fourth segment features Dr. Richard Canfield from Cornell University, whose symposium talk focused on choline and neurodevelopment. Rick is a developmental psychologist by training who works in infant and early child cognition. He has researched visual cognition and speed of information processing with babies in the first year of life for women who received a diet containing the recommended intake of choline and those who received double the recommended intake during pregnancy. They found that cognition improved for babies in the high choline group over their first year of age, which was maintained until seven years of age. The cohort is now 14 years old, and additional testing is being conducted to see if in utero exposure to choline still impacts the children 14 years later. (29:51)Dr. Robert Colin Carter from Columbia University is our next guest. His talk focused on choline and Fetal Alcohol Spectrum Disorder (FASD). His research has mainly been fetal alcohol spectrum disorders, with a particular interest in how both maternal and child nutrition might impact the teratogenic effects of alcohol. Prenatal alcohol exposure is the most common preventable cause of developmental delay worldwide, and a common view might be that women should just stop drinking. Dr. Carter argues that view is shortsighted because alcohol use is a really complicated problem for a lot of people. Asking someone who has an alcohol use disorder to stop drinking is probably not realistic for a lot of women. In animal models, supplementing a pregnant dam with choline seems to ameliorate at least some of the teratogenic effects of alcohol. Dr. Carter has completed a pilot study of 70 women from South Africa where beneficial effects of choline treatment during pregnancy were observed for growth, neurobehavior, and memory in their children. Another clinical study with 300 participants is now underway.  (51:38)We end our day one episode with a wrap-up from Dr. Dr. Susan Smith with the University of North Carolina Nutrition Research Institute and Dr. Joe McFadden with Cornell University. Susan emphasized the recurring message that choline is so important in prenatal health and in early postnatal periods. Pregnant and lactating women generally don't take enough choline, and choline is so important for healthy brain development in the fetus and the infant. Joe's takeaways from the livestock side of things include the impact of choline on colostrum production in animals and early-life supplementation in young livestock. Discover the groundbreaking FASCETS Neurobehavioral Model and its profound impact on understanding Fetal Alcohol Spectrum Disorder (FASD) within child welfare - essential listening for anyone passionate about improving the lives of children and families. In this episode, Patti Kasper and I delved into a profound discussion centered around the FASCETS Neurobehavioral Model, Fetal Alcohol Spectrum Disorder (FASD), and its implications in child welfare. The conversation was enlightening and touched on various critical aspects of these complex subjects. Patti began by highlighting her extensive background in child welfare and her journey through different academic disciplines, including social work and psychology. Her diverse experiences led her to develop a deep passion for understanding and assisting children with FASD, a condition caused by prenatal exposure to alcohol. One of the key takeaways from the conversation was the emphasis on viewing FASD as a developmental disorder that persists beyond childhood. Patti pointed out that this condition doesn't simply disappear when a child reaches a certain age, challenging the prevailing perception of FASD as a childhood disability. This understanding is crucial in ensuring that individuals with FASD receive adequate support throughout their lives. The heart of the discussion revolved around the FASCETS Neurobehavioral Model, a tool that helps parents and caregivers understand and support children with FASD. Patti explained how this model focuses on identifying brain tasks, assessing what is easy and difficult for the child and tailoring support accordingly. This approach allows caregivers to provide individualized care and accommodations, helping children with FASD thrive. Throughout the conversation, Patti passionately addressed the misconceptions and stigmas associated with FASD. She emphasized the need for a shift in societal perception, particularly regarding behavioral challenges. Patti urged parents and caregivers to consider that behaviors in children with FASD are not intentional choices but often the result of how their brains are wired. We also discussed the parallel between FASD and other neurobehavioral conditions. Patti highlighted the importance of recognizing the unique wiring of each child's brain and advocated for the application of the FASCETS model to support children with various neurobehavioral conditions, whether diagnosed or not. Our conversation also touched on the challenges faced by parents and caregivers, including the immense mental load of caregiving and the toll it takes on their own mental and physical health. Patti shared her own experiences as a caregiver for her mother, and the importance of self-care and support for caregivers. Patti highlighted the urgent need to dispel the assumption that behaviors in individuals with neurobehavioral conditions are intentional choices. She stressed the importance of considering the role of their brains in shaping their behavior. This conversation with Patti Kasper provided a comprehensive and insightful exploration of the FASCETS Neurobehavioral Model, FASD, and their broader implications in child welfare. We underscored the need for a shift in perception, increased understanding and tailored support to help individuals with FASD lead fulfilling lives. Patti's extensive knowledge and passionate advocacy for this cause made for a captivating and enlightening conversation. Welcome to Season 4, episode 22 of the FASD Family Life Podcast. This is the only show about FASD hosted by an FASD Specialist and parent with 30 years lived experience.  I am Robbie Seale, your host and mom to five incredible people; including three teens diagnosed with Fetal Alcohol Spectrum Disorder.  It is my passion to help families thrive.  To learn more about me and my work check out my website,  https://fasdfamilylife.ca/Fetal Alcohol Spectrum Disorder (FASD) refers to a constellation of disorders resulting from prenatal exposure to alcohol; the manifestations include birth defects, developmental disabilities, and neurological and behavioral problems. Fetal Alcohol Spectrum Disorder is a common disability impacting 1 in 20 people in the general population in the US, which is nearly 3 times more common than Autism. This week I am in the Netherlands where it is estimated that 2000 babies annually are born with FASD.I am speaking with Nienke Peters and Luke Schut to learn about an innovative photo book project that they have been involved in to raise awareness of Fetal Alcohol Spectrum Disorder in Holland.Luke Schut is the Project coordinator of the FAS-project. Her background is in pedagogics. She came in contact with the FAS-project through her Masters program.  Luke says "I am the most inspired by all these parents and caregivers, and the warm and caring FASD community. Let's keep sharing stories with each other to make FASD more visible."Nienke Peters is mother of 4 bio kids and grandmother of 4. Fostered 12 children with FASD and still fostering a boy with FASD and NAS. Met Allard de Witte in 2014 when he photographed one of our foster sons. Her family has been part of the Buddy Project of Witte Bos for over 4 years now.‘Het Witte Bos' is a non-profit organization, which initiates transmedia project on the cutting edge of journalism, art and society. The FAS-project started as a journalistic initiative, but eventually developed into a big multimedia awareness campaign, showing the potentially dangerous consequences of alcohol consumption during pregnancy. By storytelling, the project raises attention to a relatively unknown problem in a personal way. The FAS-project started in 2013 and launched with the book FAS-kinderen (Children with FAS). After this the fas-project continued under the name FAS2025. In the past years the project developed and gained a wider reach & more impact. FAS2025 tells the story of Isiah, Lorenzo, Marcella, Mila and Jasmijn, following their journey towards adulthood for ten years. Their stories are told by a collection of stories, portraits and mini-documentaries. To give children with FAS(D) a face, make FAS(D) less abstract and to tell what FAS(D) means to them and how it impacts their lives. The project also consists of expositions, books, a podcast, a college tour to inform future professionals about FAS, readings at professional organizations who (may) come in contact with FAS(D), and a buddy project in which children/youngsters with FAS(D) are matched to a buddy to have a nice time together.Link to purchase the book Children with FAS/Kinder mit FAShttps://fasproject.nl/en/product/children-with-fasd/The English website: https://fasproject.nl/enHave a comment or question about the show, email me at fasdfamilylife@gmail.comSupport the show

He began his legal career representing children in mental health facilities across Alabama. His work as an attorney and an advocate enforced children's constitutional rights to be free from staff abuse and to receive meaningful psychiatric care. Andrew returned home to Los Angeles as the CEO of The Alliance for Children's Rights. He defended children at MacLaren Hall, where he was once confined. He won the right for Los Angeles' 60,000 foster children to speak publicly about their treatment. He investigated the excessive removal of African-American babies from their parents, which led to one of the first government initiatives to stop it. Andrew is the Co-Founder of National Adoption Day. He went on to lead one of California's largest recruiters of foster and adoptive parents. Called to address the unjustified placement of foster children in mental institutions, Andrew has served as a Senior Advisor to the State of Illinois. His educational work resulted in the creation of New Village Girls Academy, California's first all-girls high school for parenting teens. Andrew serves on Arizona's Foster Care Review Board and he consults with child welfare systems and foundations. Andrew is also the author of the New York Times AND Los Angeles Times BEST SELLER book, "Hope's Boy: A Memoir". The Washington Post named "Hope's Boy" one of it's Best Books of the Year, with a review that called it "filled with vivid scenes and empathetic description...compulsively readable". In this THOUGHT-PROVOKING episode, Andrew shares: his personal journey and lived experience as a foster child in the Los Angeles Foster Care System, his journey in becoming an attorney and child rights advocate, insights and discussion points from "Hope's Boy", the punitive nature that was / still exists in child welfare systems, his thoughts on how to educate those in the child welfare system about FASD and other brain-based diagnoses, the beginnings of "National Adoption Day" in 2000 and how it's grown, his current work and initiatives and words of hope and encouragement. FASD Hope is a podcast series about Fetal Alcohol Spectrum Disorder (FASD), through the lens of parent advocates with over twenty years of lived experience. Episode 158 of FASD Hope explores a controversial topic: the School to Prison Pipeline (STPP).  The STPP or SPP is the significantly increased tendency of students from disadvantaged backgrounds, students with brain-based diagnoses, developmental disabilities and BIPOC students to become involved in the juvenile justice system. In today's episode, discussing the STPP and it's implications for students with brain-based diagnoses (including FASD, Autism, ADHD, etc.) is Dr. Deanna Westedt. Dr. Deanna Westedt received a doctorate in curriculum and instruction and spent 22 years in education. Currently, she homeschools and provides guidance for families, both homeschooled and traditionally schooled.  She has a special passion for twice-exceptional students and providing learning that goes outside of a classroom box. Her insights into twice-exceptionality have been featured in publications and venues such as Psychology Today, Future First Education, and CABE. Dr. Westedt's research was featured in AERA 2020. However, her best accomplishment is that of the growth her boys made during their time in homeschooling so far, where her ideas on how to accommodate their special combination of giftedness and learning disabilities paid off. She currently provides coaching and writing sessions, as well as specializes in early literacy that can be blended into any type of setting. Additionally, she provides parent sessions, diagnostics, and flexible differentiation training for homeschool/ educational organizations. In this highly informative and thought-provoking episode, Dr. Westedt shares the following: her professional background, her family's journey, her passion for creating academic equity for all students, how she became aware of the STPP, how students with brain-based diagnoses (including FASD) become even more susceptible to getting involved in the STPP, addressing how the traditional cycle of discipline reinforces the STPP (especially for students with developmental disabilities & brain-based diagnoses), how a systemic shift is required to break this cycle, how to better support those students involved in the STPP / JJ systems and her words of hope for listeners.  "Trust in the LORD with all your heart, And lean not on your own understanding; In all your ways acknowledge Him, And He shall direct your paths." - Proverbs: 5-6   EPISODE RESOURCES - Dr. Deanna Westedt - https://deannawestedt.com/ info@deannawestedt.com Facebook - https://www.facebook.com/drwestedt Instagram - https://www.instagram.com/dr.deannawestedt/ LinkedIn - https://www.linkedin.com/in/deanna-westedt-ed-d-64a2741b1/   FASD Hope Resources - https://www.fasdhope.com/ natalie@fasdhope.com Instagram - https://www.instagram.com/fasdhope/ Facebook - https://www.facebook.com/fasdhope1 Twitter - https://twitter.com/fasdhope LinkedIn- https://www.linkedin.com/in/natalie-vecchione-17212160/   HOPE FOR THE FASD JOURNEY ONLINE COMMUNITY - Co-Lead By Natalie Vecchione and Sandra Flach -  Hope for the FASD Journey is a faith-based, membership support community for parents and caregivers of individuals prenatally exposed to alcohol and other substances. My guest this week is Eileen Devine In this episode, Eileen and I discuss parenting through behavior and developmental challenges, with a spotlight on Fetal Alcohol Spectrum Disorder (FASD). Whether you have a child diagnosed with FASD or not, I encourage you to listen to this conversation as the content and tips can be applied to parenting kids with or without any behavioral or developmental challenges, at any stage of life.  We talk about why a brain-based parenting lens is so much more effective than a behavior-based approach, especially for kids with behavior challenges or developmental delays.  Eileen's personal and professional experience with FASD is incredibly insightful, where she shares what FASD does and does not look like, diagnoses, what research shows, and many realistic tips and tools to not just support a child in improving brain development and function, but also helping the parents in the process. Eileen Devine is a licensed clinical social worker living in Portland, Oregon and has over 20 years of clinical experience. She's also the mother of a teenager with Fetal Alcohol Spectrum Disorder, a serious brain based disability that has challenging behavioral symptoms. She believes that when we understand the way a child's brain works, we then understand the meaning behind challenging behaviors and begin to see them as symptoms of a child's differences. Through her work with parents, Eileen has created the Resilience Room membership community and the Brain First Parenting Program, both designed to support parents in their unique parenting journey. In addition to her one on one and group work with parents, she also facilitates dozens of workshops and trainings a year and is an instructor for the CASE Institute's Training for Adoption Competency Post-masters certification program.     Connect with Eileen Devine: Insta: @eileen.devine_brain.first Facebook: https://www.facebook.com/EileenDevineBrain Website: www.eileendevine.com     Episode Takeaways   Introduction to Eileen Devine & FASD … 00:01:45 What is Fetal Alcohol Spectrum Disorder and Diagnostic Process … 00:05:52 Outdated Assumptions of FASD Presentation … 00:11:20 FASD, Toxic Stress, & Other Prenatal Substance Exposures … 00:15:40 Common Behavioral Challenges Seen on the Spectrum … 00:18:15  Parenting Children Who are Physically, Behaviorally or Cognitively Impulsive … 00:26:42 Consequences Aren't Working: Better Behavioral Techniques … 00:30:15  Parenting Tips: A Brain-Based Reframe Rather Than a Behavior-Based Lens … 00:35:09 The Gap Between Expectations and What They are Capable of Doing … 00:40:42 Tips & Where to Start for FASD & Behavioral Challenges … 00:47:15  Resources & Episode Wrap Up … 00:50:10         Need help with improving your child's behavior naturally?
