In this episode of BackTable ENT, Dr. Julie Wei speaks with Dr. Gita Pensa, an emergency medicine physician, about her journey to becoming a physician coach. --- SHOW NOTES After enduring a 12 year malpractice lawsuit, Dr. Pensa explains why she chose to become a physician coach focused on helping physicians maintain their wellness through malpractice lawsuits. She started out by creating her own podcast to release 11 episodes about malpractice litigation basics. Once she realized her podcast was beneficial to many physicians, she started accepting speaking engagements and monetizing her skillset as a coach. To Dr. Pensa, physician coaching is not therapy for litigation. She doesn't ask about details of the case that have happened, but she does help her clients reflect on the thoughts and feelings that impact their actions and reactions to their cases. Additionally, the doctors discuss Dr. Pensa's client distribution in terms of sex and specialty. They also reflect on the relationships of malpractice attorneys with physician coaches. Although some attorneys may push back because of privacy concerns, some attorneys include Dr. Pensa on the team. The Land Before Podcast: Fossil Histories and Paleo Mysteries
SHOW NOTES (Bonus Pre-Season Episode) Evidence indicates the Stegosaurus did not swish its spiked tail back and forth like a Medieval weapon, but rather used it for locomotion and communication, and when need be for defense. Morrison Natural History Museum Director Matthew Mossbrucker provides the non-gory details in his interview with Michelle Howell. Dinosaur Ridge volunteer Jim Watson surmises how the Stego may have sounded — his interpretation is unlike any Hollywood version you've heard. Finally Dr. Nicole Peavy of the Colorado Department of Transportation (CDOT) talks about the work of Mitigation Paleontology, and what happens with fossils found on road construction sites. The Stegosaurus used its tail like a third leg! It helped move the animal, and also could be used to communicate with other Stegosaurus, and defend the animal from predators. Matthew Mossbrucker discusses the anatomy of the tail, including the lump of bone called the fourth trochanter, that served as a muscle attachment knob for the caudofemoralis, a muscle found in nearly all animals with tails. In 2009, Mossbrucker and Dr. Robert Bakker studied the tail and published “The Symposium on Stegosauria” In our conversation, Mossbrucker mentions Quarry 13 in Como Bluff, Wyoming which is referenced in this thorough wikipedia post if you would like more context. Peavy also referenced the Castle Rock Rainforest uncovered during a highway project. At the time the Dr. Kirk Johnson (Smithsonian's National Museum of Natural History Director) was with the Denver Museum of Nature and Science, which now holds 10,000 specimens from this site. He wrote an article about the finds that was published in 2003. Peavy also referenced Ian Miller, now National Geographic Society's Chief Science and Innovation Officer, who also worked at the Denver Museum of Nature & Science. Miller has led science research projects on major field expeditions across the western U.S. and Madagascar. If you've never heard how the term “Thagomizer” came about, you must first treat yourself to a popular newspaper comic strip from the 1980s called “The Far Side.” Author Gary Larson in a 1982 comic drew a group of cavemen attending a lecture in which the speaker points to the tail tip of a Stegosaurus. The caption explains the spikes are called Thagomizers "after the late Thag Simmons.” Denver Museum of Nature and Science paleontologist Kenneth Carpenter jokingly used the term at the Society of Vertebrate Paleontology Annual Meeting more than a decade later, and it caught on. Fossil Butte National Monument in Wyoming is the scene of world-renowned fossil finds including 27 types of fish. In this episode of Bowel Sounds, hosts Dr. Temara Hajjat and Dr. Peter Lu talk to Dr. Sam Kocoshis about caring for children with intestinal failure and intestinal rehabilitation. Dr. Kocoshis is a Professor of Pediatrics at Cincinnati Children's Hospital and Medical Center and the Medical Director of the Intestinal Care Center and the Small Bowel Transplantation Program. We talk to him about the management of children with short bowel syndrome, including recent advances in promotion of intestinal adaptation, prevention of central line-associated bloodstream infections (CLABSIs), detection and treatment of small intestinal bacterial overgrowth (SIBO), and prevention of intestinal failure-associated liver disease. We also discuss the indications for small bowel transplantation and a brief history of this procedure. Dr. Linguraru is also professor of Radiology and Pediatrics and secondary professor of Biomedical Engineering at George Washington University. We pre-recorded today's conversation with Dr. Jeffrey Feit, Valley Health's Population & Community Health Officer to learn more about the Delta variant and bust a few more vaccine myths. Dr. Feit explained how variants of the virus happen and why the Delta variant is more dangerous than the original version (COVID 1.0) of the virus. We talked about the effectiveness of the vaccines and the potential for boosters in the future. I gave Dr. Feit a few of the arguments I've heard for not getting the vaccine and he offered advice & perspective for each. He also outright busted a few of the common myths we've heard throughout this year. Hypertensive disorders of pregnancy (HDP) complicate about 5-10% of all pregnancies. HDP is a term that encompasses chronic hypertension, gestational hypertension, pre-eclampsia, and pre-eclampsia superimposed on chronic hypertension. Women with history of HDP are at increased risk for stroke, ischemic heart disease, and cardiovascular death later in life. It is important to include obstetric history as part of our assessment of cardiovascular risk. This obstetric history should include the number of pregnancies, history of gestational diabetes, gestational hypertension, history of pre-eclampsia as well as history of pre-term birth. Following delivery, these women need close monitoring for development of chronic hypertension as well as primary prevention of cardiovascular disease. 2. How is gestational hypertension distinguished from chronic hypertension? The key factor in distinguishing gestational hypertension from chronic hypertension is the timing of diagnosis. Before 20 weeks gestation, elevated blood pressure (defined as a reading > 140/90 on two occasions at least 4 hours apart) is considered chronic hypertension. After 20 weeks gestation, elevated blood pressures signify a pregnancy-induced process, namely gestational hypertension or pre-eclampsia if blood pressure elevati...
In today's podcast, we start to explore Lyme disease in children. Sarah welcomes her father, Dr. Ted Cormode, to the podcast. He understands Lyme disease both as a pediatrician and as a parent supporting and advocating for his child. Dr. Cormode talks about his experience as a parent when Sarah first became ill. In medical school, he did not learn about Lyme disease so he spent the next years digging deeper into medical research.Dr. Cormode walks us through some of the unique challenges of understanding Lyme disease in children. He describes an interesting case study of a child who became ill following a camping trip. The child had not received a diagnosis or treatment for Lyme disease, but her symptoms cleared up after being given antibiotics after the surgical removal of her tonsils. When the short course of antibiotics was completed, the child's symptoms returned. Another course of antibiotic treatment was ordered and Dr. Cormode is happy to report that many years later, the patient has remained in good health. For children and adults alike, knowing a patient over time can help a family physician determine when symptoms are out of the ordinary for that individual. Dr. Cormode also discusses three case studies recently published in the Canadian Medical Association Journal in which three patients with signs and symptoms of Lyme disease did not receive an early diagnosis and developed serious complications. These conversations aim to rethink narratives of history and culture, which have been traditionally boxed in by land-based territorial demarcations and regional studies frameworks. This series invites listeners to imagine the complex interconnectedness of life from East Africa to Southeast Asia and beyond. * In this introductory episode, Indian Ocean series host Lindsey Stephenson speaks with Fahad Bishara, who is an Assistant Professor of History at the University of Virginia. Dr. Bishara discusses with Lindsey the contours of the Indian Ocean world and the study of it. He draws out some of the similarities between the Indian Ocean and other oceanic spaces such as the Mediterranean and Atlantic worlds. He explains that many of the places central to the Indian Ocean have been treated as peripheral to area studies regions such as the Middle East and South Asia. Dr. Bishara suggests that rather than considering researching the Indian Ocean as working on a fixed space, it is more an approach to sources that helps us follow connections across seemingly disparate spaces.
