Ben talks about ‘Plus Delta' debriefing with lead authors of 2 recent papers on the topic both published in Advances in Simulation. Adam Cheng starts the conversation talking about Embracing informed learner self-assessment during debriefing: the art of plus-delta, and giving us plenty of reasons to ensure this technique is on our repertoire as simulation educators. Ben goes on to talk with Raj Kainth about his team's experience of training faculty for clinical debriefing in the Nghtingale facility during the pandemic - Dynamic Plus-Delta: an agile debriefing approach centred around variable participant, faculty and contextual factors The conversation emphasises the importance of agility and adaptability in our learning conversations, and underscore the need for ongoing scholarly conversation on the topic. In this episode we look at the debrief in its entirety; why we debrief, when, how and where. We examine the evidence behind debriefing and the institutions that use debrief such as the military, pre-hospital practitioners, ward based staff, sports teams, athletes and others. We then look at the types of debrief such as the hot & cold debrief, Schwartz rounds, Critical Incident Stress Debriefing, the After Action Review amongst others. We then examine 6 tools of debriefing that can be used in practice. We briefly also touch on the disparity of evidence around mitigation of psychiatric pathology such as PTSD. The way we provide feedback makes a difference in how people receive and act on the feedback. If our goal is to provide feedback to change behavior for the right reasons, we need to message our feedback in the right way. Dr. Pilcher dissects real-life situations where feedback was given that wasn’t asked for, or that hurts the receiver, and offers solutions and tips to improve the interaction. Before I walk you through how to lead a Plus Delta exercise for your team, you might be wondering what it is or really what purpose it serves. In a nutshell, it's a team building exercise that celebrates their successes and the things you're doing well, which are the pluses, as well as highlighting the things that need to change, which are the deltas. Now, in math, the plus symbol means addition or a positive number, and the delta symbol, which is a triangle, is a symbol for change. So, this is how it works. Everyone in your leadership team gathers together and each of you take a turn being in the hot seat. When you're in the hot seat, everyone else takes turns telling you all the things that you're doing well, all of your pluses, and then they go around and tell you your deltas or things that you can change or improve on. You can see why this can be really powerful and really vulnerable. If you give every member on your leadership team an opportunity to speak directly to you and tell you the things that you're doing well and the things that you need to improve on or change, you get the opportunity to learn about blind spots you don't know you have. Likewise, when one of your team members gets to be in the hot seat and you and the other leaders on your team get to speak life into him, and tell him or her the changes they need to make, as well as the things that they're doing well, it calls them up to another level. It can build them up and challenge them in ways that they probably wouldn't realize they even needed. Now, one thing that's important to note is this is not a good versus bad exercise: “Here are all the good things you're doing, here all the bad things.” That's not what it's about. This entire exercise is a positive one because it highlights all the ways you're succeeding and what you're doing well while also highlighting the ways that you can improve. Now, it's up to you as a leader to set the tone, and we'll talk a little bit about that. But overall, it's an incredibly powerful exercise. So press play and I’ll walk you through how you can lead a Plus Delta exercise in five steps! For the full transcript, click here! It's a pretty common facilitation technique to do a "plus / delta" discussion at the end of a meeting, exercise, etc.In the plus column go the things that went well, were enjoyable, were helpful, and should repeated. The "deltas" (which is meant to seem more positive than saying "minuses," I guess) are the things that could be improved upon. A "delta" is a gap between where you are and where you want to be. After being in the doctor's office for just over an hour, nobody said "let's plus / delta your visit before you go." That never happens in a clinic setting. At best, you might get some sort of survey after the fact. But, the point of a good constructive "plus / delta" discussion is to give immediate feedback to those who might be able to make some improvements. --- This episode is sponsored by · Anchor: The easiest way to make a podcast. https://anchor.fm/app Support this podcast: https://anchor.fm/lean-blog-audio/support
