Cilt ve yumuşak doku enfeksiyonları, acil servislerde sıkça karşılaşılan ve zamanında müdahale gerektiren tıbbi durumlardır. Bu enfeksiyonlar, hafif cilt iritasyonlarından yaşamı tehdit edebilecek sistemik enfeksiyonlara kadar geniş bir spektrumda seyredebilir. Özellikle immün sistemi zayıf olan bireylerde daha ciddi komplikasyonlara yol açabilir. Erken tanı ve uygun tedavi, enfeksiyonun yayılmasını ve komplikasyonların gelişmesini önlemek adına kritik öneme sahiptir. Bu yazımızda Kanada Acil Tıp Derneği'nin (CAEP) yakın zamanda yayınladığı klinik politika rehberi doğrultusunda cilt ve yumuşak doku enfeksiyonlarının tanısı, tedavisi ve acil servisteki yaklaşımlarını ele alacağız.​1​ Bu konu ile ilgili hazırlanan özeti içeren kısa broşür yazımızın sonunda yer almaktadır. Acil serviste (AS) selülit tanısı nasıl konulmalıdır?     (1)    Selülit tanısı koymak için klinik bulguları kullanın. Selüliti düşündüren tipik fizik muayene bulguları; hassasiyet, eritem, sıcaklık artışı, ödem ve endürasyondur. Bazen lenfanjit ve/veya ateş olabilir.     (2)    Selülit tanısı koymak için mevcut karar araçlarını veya spesifik incelemeleri (örn. beyaz kan hücresi sayımı, C-reaktif protein) kullanmayın. Bilateral semptomları olan hastalarda (örn. her iki bacak tutulumu) alternatif bir tanı düşünün. 2. Selülitli hastalarda kan kültürü tetkiği yapılmalıdır? Sistemik olarak iyi durumda olan selülitli hastalar için rutin olarak kan kültürü istemeyin. Aşağıdaki durumlarda kan kültürü istemeyi düşünün:     (1)    Sistemik olarak kötü hasta (örn. ateş, lenfanjit, inatçı taşikardi, takipne, hipotansiyon); veya     (2)    Bağışıklık sistemi baskılanmış (örn. antikanser tedavisi alan aktif malignite hastası, bilinen veya şüphelenilen nötropeni) 3. Acil servis klinisyenleri selülit için görüntüleme istemeli mi? Selülit için rutin olarak görüntüleme istemeyin. Deri apsesinin selülitten ayırt edilmesinde belirsizlik olduğu durumlarda yatak başı ultrasonu (Point of Care Ultrasonography-POCUS) uygulayın. Seçilmiş vakalarda görüntüleme (örn. röntgen, bilgisayarlı tomografi [BT], ultrason) istemeyi düşünün:     (1)    Osteomiyelit şüphesi     (2)    Yabancı cisimler     (3)    Nekrotizan fasiitten tanısının dışlanamaması (not: klinik şüphe varsa görüntüleme asla acil cerrahi konsültasyonu geciktirmemelidir). 4. Selülit tedavisi için önerilen oral antibiyotik ajanı, dozu, sıklığı ve süresi nedir? Oral antibiyotikler ilk basamak tedavidir (Tablo 1). 5. Acil servis klinisyeni selülit tedavisi için intravenöz (IV) antibiyotikleri ne zaman düşünmelidir? Aşağıdaki hastalarda IV antibiyotik ile tedavi edin:     (1)    Sistemik olarak kötü hasta (örn. ateş, lenfanjit, inatçı taşikardi, taşipne, hipotansiyon); veya     (2)    Başarısız oral antibiyotik tedavisi (en az 48-72 saat oral antibiyotiğe rağmen yeni/kalıcı ateş, kötüleşen ağrı ve/veya yayılan eritem); veya     (3)    Oral alamayan hasta (örn. kusma, malabsorpsiyon sendromu, vb.) 6. IV antibiyotik başlanırsa, selüliti tedavi etmek için önerilen antibiyotik ajan, doz, sıklık ve süre nedir? Tablo 1'de detaylı olarak verilmiştir. 7. Etkilenen bölgenin yükseltilmesi (elevasyonu) önerilmekte midir? Ekstremite selüliti olan hastalara etkilenen bölgeyi yükseltmelerini tavsiye edin, çünkü bu ödem ve inflamatuar maddelerin yerçekimi drenajını teşvik ederek iyileşmeyi hızlandıracaktır. 8. Selülit için antibiyotiklere ek olarak bir anti-inflamatuar ajan (örn. non-steroid anti-inflamatuar ilaç [NSAİİ], kortikosteroid) reçete edilmeli mi/önerilmeli mi? Selülitli hastalarda antibiyotik tedavisine ek olarak 5-7 gün süreyle (kontrendikasyon yoksa) oral bir NSAİİ önermeyi veya reçete etmeyi düşünün. 9. Acil servislerde sıkça karşılaşılan cilt ve yumuşak doku enfeksiyonları, bazen ciddi komplikasyonlar yaratabilir. Cilt apseleri de bu tür enfeksiyonlar arasında, acil serviste hızlı ve etkili bir müdahale gerektiren önemli bir durumdur. Tedavi edilmediğinde veya yanlış yaklaşımlar uygulandığında, bu apseler hastalar için ciddi sağlık sorunlarına yol açabilir. Yatak başı İ&D'yi takiben deri apsesi boşluklarını rutin olarak doldurmayın. 6.Deri apsesinin İ&D'sini takiben antibiyotikler ne zaman reçete edilmelidir? Pürülan lezyona yakın geniş selülit vakalarında veya ateş gibi sistemik semptomları olan hastalarda İ&D'ye ek olarak antibiyotik reçete edin. Bağışıklık sistemi baskılanmış hastalarda (örn. tedavi alan aktif malignite, bilinen veya şüphelenilen nötropeni) antibiyotikleri düşünün. 7. Deri apsesi olan bir hastaya oral antibiyotik reçete edilirse, hangi oral antibiyotik (1) ajan, (2) doz, (3) sıklık ve (4) süre ile önerilir? Lütfen tedavi tablosuna bakınız. This automation system is aimed at efficiently handling incoming absentee ballots and ensuring the county's readiness for future election cycles, particularly during pandemic situations. The funding for this purchase will primarily come from the Capital Project Fund, with $21,508 sourced from Gwinnett's General Tax Contingency project. Additionally, the commissioners approved a $708,000 contract with Fort Orange Press for ballot printing. This contract will cover the printing of emergency and provisional ballots for use in the 2024 elections and the printing and mailing of absentee-by-mail packets. The decision to award this contract to Fort Orange Press followed a competitive request for proposals process. These measures are part of the county's ongoing efforts to improve and secure its election infrastructure.  STORY 3: Gwinnett Police Searching for Suspect in Alleged Theft at Burger King in Lawrenceville   Gwinnett County police are searching for a man who reportedly followed two women to their car in a Burger King parking lot in Lawrenceville and stole their money. The incident occurred on October 6 at the Burger King on Duluth Highway. The unidentified suspect followed the women to their car, entered the back passenger seat, brandished a handgun, and demanded money. The victims eventually gave him $60 and $30, although they initially claimed to have no money. The suspect is described as a young Hispanic male wearing a black jacket, plaid pants, and an LA Dodgers cap with a decal on the bill. Their friendship began in Mr. Segool's class, where they discovered their personalities clicked. They spent their early years forming a close bond, spending time at each other's houses and creating dances, cheerleading routines, and playing teacher. Their middle school years led to a pause in their friendship as they had different schedules and classmates. However, fate has reunited them at McKendree Elementary School, where they are now teaching fifth grade. They are passionate about creating relationships with their students and enjoy the discussions and learning that happens in their classrooms.   STORY 7: The Gwinnett County Public Library is currently accepting applications for its New Start Entrepreneurship Incubator Program (NSEI), which provides business education to individuals who were formerly incarcerated. NSEI offers a six-month course aimed at helping ex-offenders, particularly minority populations and people of color in Georgia, to establish and sustain their own businesses. The program includes in-person classes, online coursework, access to mentors, and community partners. Participants receive training on various aspects of entrepreneurship, including finance, marketing, licensing, and business plan development. NSEI originated as a grant project in 2021, funded by Google in partnership with the American Library Association, and has already graduated 29 entrepreneurs across three cohorts. The program culminates in a pitch event called Launchpad, where entrepreneurs present their business ideas to community business leaders. Christie Thompson - Bio as Submitted Christie Thompson, School Counselor A native Arkansan, I relocated to Sherman, Texas, to begin my 20-year teaching career and was awarded New Secondary Teacher of the Year for the district. I then taught English in Plano ISD and earned my Master's degree in English from SMU. From CAEP 2019 I always love the activities and experiences at CAEP, but year after year I’m drawn to the Simulation Olympiad. Every time I think it’s just incredible. This year, I decided to talk with some of the people who make the SIMOlympiad such a great experience for audiences and participants alike. In my first interview, I heard about what the average SIM team looks like, how the competition runs, and what the scope of the material covers. We also got the details on what the characteristics of a winning team look like and how SIM hopefuls can start the team that will win it all at CAEP 2020. Here is my interview with Dr. Tamara McColl of the University of Manitoba and judge for the CAEP 2019 SIMOlympiad.   (If you like to follow along, the show questions are below) 1. Before the cheering throngs at Churchill Downs or Belmont or Saratoga watch young Thoroughbred racehorses charge from the starting gates, Shelley Bunning and her team at Timber Town Stables welcomed many of them into the world as newborn foals.   One of the youngest female Farm Managers at one of the greatest Thoroughbred racing broodmare operations in the US, Elise’s guest today is Shelley Bunning. Known for her utter devotion to the horses in her care and for her expertise, professionalism, and her readiness to mentor others, Shelley still retains the wonder of a former horse-crazy girl as she oversees multi-million dollar racing mares like Havre de Grace, Plum Pretty, Songbird, and Groupie Doll.   As a young girl in England, Shelley knew that she wanted to work with horses and understood that it would take hard work, dedication, and an unrelenting drive to learn. Most of us in North America live in cultures that almost never talk about death and dying. And medical progress has led the way to a shift in the culture of dying, in which death has been medicalized. While most people wish to die at home, every decade has seen an increase in the proportion of deaths that occur in hospital. Death is often seen as a failure to keep people alive rather than a natural dignified end to life. This is at odds with what a lot of people actually want at the end of their lives: 70% of hospitalized Canadian elderly say they prefer comfort measures as apposed to life-prolonging treatment, yet as many as ⅔ of these patients are admitted to ICUs. Quality End of Life Care in Emergency Medicine is not widely taught. Most of us are not well prepared for death in our EDs – and we should be. There's no second chance when it comes to a bad death like there is if you screw up a central line placement, so you need the skills to do it right the first time. To recognize when comfort measures and compassion are what will be best for our patients, is just as important as knowing when to intervene and treat aggressively in a resuscitation. Emergency physicians should be able to recognize not only the symptoms and patterns that are common in the last hours to days of life, but also understand the various trajectories over months or years toward death, if they're going to provide the high quality end of life care that patients deserve. So, with the help of Dr. Howard Ovens, a veteran emergency physician with over 25 years of experience who speaks at national conferences on End of Life Care in Emergency Medicine, Dr. Paul Miller, an emergency physician who also runs a palliative care unit at McMaster University and Dr. Shona MacLachlan who led the palliative care stream at the CAEP conference in Edmonton this past June, we'll help you learn the skills you need to assess dying patients appropriately, communicate with their families effectively, manage end of life symptoms with confidence and much more... BACKGROUND Phleboedema, ankle oedema being a manifestation of Chronic Venous Disease (CVD) is a common condition with particularly high prevalence in women. The most common cause of CVD is a primary dilatative venous degeneration that may manifest itself in form of varicose veins oder deep vein dilatative phlebopathy. It is known that Common-Femoral-Vein (CFV) dilation correlates with the risk of skin changes corresponding to CAEP stage C3-C6. However, it remains to be demonstrated, if CFV dilation plays a role in the development of ankle oedema. OBJECTIVE The study objective was to test the hypothesis that there is an association between the diameter of the CFV and ankle oedema. METHODS A total of 40 subjects (including 34 females and 6 males) were enrolled in a randomised, placebo-controlled, double-blind study investigating the efficacy of diuretic therapy in ankle oedema. The investigational product contained as active substances bemetizid 5 mg and triamteten 10 mg. This analysis confines to women with primary CVD who received verum (n=16) or placebo (n=18), and only to their right legs. The diameter of CFV was measured by means of duplex scan during forced Valsalva manouver at the screening visit, whilst the impact of 7-day (or 14-day) diuretic therapy on ankle volume was determined using perometry (opto-electronic volumetry). An association between the two variables was examined. The data were analysed by means of descriptive and inferential statistics including ANOVA and logistic regression analysis. RESULTS In the group of women with a CFV dilation (diameter ≥ 16 mm) the risk of 3 % ankle oedema was 12-fold increased as compared to women with CFV diameters of 16 mm or less (OR = 12; p = 0.06). In contrast, other venous parameters including varicose veins (trunc, reticular, or spider-web varices), refluxes in deep or supperficial veins, degree of skin changes or results of photoplethysmography, were not predictive for the presence or extent of ankle oedema. The best discrimination between treatment effects of verum and placebo was observed if perometry included the distal 20 cm of the shank (p < 0.01). In contrast, if the examination was adjusted to the anatomical structures, i.e., involving the whole distance between the ankle and patella, discrimination was poor (insignifficant differences). The therapeutic effect on ankle volume was more pronounced after 7 days (43 mL) than after 14 days of treamtent (25 mL). Out of 9 women who initially experienced a volume reduction of at least 3 %, 6 noted a volume increase by at least 1 % during the second week of therapy. CONCLUSIONS The following conclusions refer to females and it is unknown if they might be extrapolated to males. 1. The diagnosis of phleboedema and/or its quantification should be based on perometry performed at the fixed distal 20-cm part of the shank. A supine position appears suitable for the assessment of ankle oedema, since it eliminates the influence of fluid shifting within the intravasal compartment. 2. Knowing the diameter of CFV one can assess the predisposition to phleboedema. A CFV diameter of 16 mm or more is consistent with an increased risk of phleboedema. 3. CFV dilation appears to be a marker of a generalised venous dilation (including capillaries). It is hypothesised, that venous dilation is a substrate of phleboedema. 4. The efficacy of the combination therapy with bemetizid 5 mg and triamteren 10 mg in women with primary degenerative CVD may markedly decline during second week of therapy; likely due to diuretic resistance.
