Even more so if a woman is high risk for breast cancer or has a breast cancer diagnosis.  In this episode, I am joined by Dr. Corinne Menn, a board-certified Ob GYN and menopause expert who herself had a breast cancer diagnosis when she was in her 20s. After returning to the US in late April 2022, The War On Drugs took a short break berore heading back out on an intense 7-week tour that took the band to Mexico, across the US, and back to Europe.  After a massive show with The Strokes in Mexico City, the band returned to the US for a tour that concluded at Bonnaroo in June.  They made a quick turnaround, and headed back to Europe for a show opening for The Rolling Stones in London, as well as run of mostly festival dates across Europe.  Despite a Covid outbreak that took down 3 members of the tour party simultaneously, the band and crew rallied to support each other once more, and by the time they returned to the States in early July, they'd successfully crushed another 28 shows.Host: Dominic EastGuests: Adam Granduciel, Dave Hartley, Robbie Bennett, Charlie Hall, Anthony LaMarca, Jon Natchez, Eliza Hardy-Jones, Craig McQuiston

Only 1 of the 25 studies, the HABITS trial, demonstrated an increased risk of recurrence, which was limited to local or contralateral, and not distant, recurrence. None of the studies, including HABITS, reported increased breast cancer mortality associated with HRT. Even in the HABITS trial, the absolute increase in the number of women who had a recurrence (localized only) associated with HRT administration was 22. Think about why you are in the OR. If its an elective situation and can wait, consider bailing. If its emergent, you may have to do something more definitive. Exposure may be a challenge, you may have to alter your typical approach including where the assistant grabs and retracts. Extra hands are helpful. Bleeding can be a big deal. If possible, map out the abdominal wall ahead of time with cross-sectional imaging. Stay away from varices around the umbilicus or porta Ventral Hernia + Cirrhosis ·      Ideally, control ascites pre-operatively, if you can't consider leaving drains ·      Small (< 2cm) hernias close primarily ·      Larger (>2cm) hernias repair with mesh unless infected filed (controversial) ·      Minimally invasive repairs can be performed Benign Biliary Disease + Cirrhosis ·      Incidence of gallstones is 4-5 times higher in cirrhotic patients ·      Prophylactic laparoscopic cholecystectomy (LC) generally not done ·      LC generally considered acceptable in CP A or B but not C (exceptions: HD instability, gangrenous cholecystitis, hemorrhagic cholecystitis) ·      Cholecystostomy and ERCP are safe References:  Bleszynski, M. et. Essentially all women eventually enter menopause, and while most end up not taking hormones to treat their symptoms, they at least have the option to do so. Women with a breast cancer diagnosis, on the other hand, generally don't have the same choices at their disposal and are told, or assume, they are not candidates for estrogen therapy. Beth underwent gender transition from natal female to trans male and has since de-transitioned. In her early teens, Beth felt she was not like other women and began to question her gender. She saw people who were nonconforming, but although she adopted a non-binary identity in college, people still saw her as a woman. Beth became drawn to a masculine identity and associated transitioning gender with empowerment: she would be free from the perceived social constraints and physical vulnerabilities of womanhood. Beth's parents, the therapist she saw a few times, and the surgeon all affirmed her desire to transition. Beth underwent testosterone therapy and top surgery to remove her breasts, sacrifices in service of liberation. In time, however, Beth discovered that physical changes did not transform her inner world, and her post-surgical chest remains numb. Beth says, “de-transitioning brought me down to earth” and into adulthood. She feels some of her urges to transition were healthy, and some were self-annihilating—and wishes she had had more help in differentiating her inner world from her gender expression. Beth hopes to be a voice for the complexity of gender transitioning. Here's Beth's dream: “I met a woman in a lush forest. The forest was filled with strange relics—an abandoned child's playground, a little home, and abandoned campsites. I knew she was a detransitioner, but unlike me, she was proud, unabashed, and free. She told me I could be free like her once no one knew where to find me. Then she ran off into the jungle. Of the available studies, methods and definitions vary, suggesting a need to establish standardized criteria in the future. Five studies are mentioned below. One concludes, of 17,151 who identify as transgender and gender diverse people, 13.1% reported a history of detransition or going back to living as their sex assigned at birth. Another concludes that only 0.2-0.3% of patients expressed post-operative regret and requested reversal surgery. In another study, 6.9% of patients detransitioned.
