Episode 224: Community Health Workers Dr. Arreaza: Today we will discuss a topic that, frankly, every single person listening, whether you're a medical student, a resident, a nurse, a family doctor, or any primary care provider, needs to really understand. We're talking about community health workers (CHWs). We are joined by our stellar medical student; you may be familiar with her voice from previous episodes about insomnia. Moira, welcome, please introduce yourself.  Moira: I want to be upfront about why Community Health Workers matter to you specifically. If you've ever felt frustrated that your patient with uncontrolled diabetes keeps missing appointments because they can't get a ride, or that your heart failure patient was readmitted because nobody checked whether they could afford their medications, then you already understand the problem that CHWs are designed to solve. Dr. Arreaza: We're going to give you the definition of a CHW, the evidence behind their effectiveness, how they fit into your care team, the return on investment, and practical steps for integrating them into your practice. We have pulled information from a lot of peer-reviewed sources, and we want to share them with you. So, Moira, let's start with the basics. What exactly is a community health worker? Moira: Great question, and it's one that even literature struggles with, because there are so many titles for this role. Community Health Worker is an umbrella term that encompasses more than 20 different titles including outreach workers, promotores or promotoras de salud, community health representatives, lay health workers, peer educators, patient navigators, and many more. The American Public Health Association defines CHWs as frontline public health workers who are trusted members of or have an unusually close understanding of the communities they serve. Arreaza: And that trust is so important in health care. CHWs are not physicians. They are not nurses. They do not diagnose or prescribe. But they are like a bridge connecting the medical environment, social services, and the community to reduce gaps in healthcare delivery.  Moira: Exactly. Welcome to The Turf Zone podcast. This episode features the article “Minimizing Spring Dead Spot” written by M. Aaron Tucker, Assistant Professor, Auburn University and Wendell J. Hutchens, Assistant Professor, University of Arkansas. Introduction Spring Dead Spot (SDS) is one of the most persistent and damaging diseases affecting warm-season turfgrasses that undergo winter dormancy. First documented in 1954 on a bermudagrass lawn in Stillwater, Oklahoma, SDS continues to challenge turf managers across the southern and transition zone of the United States (Wadsworth & Young, 1960; Tredway, 2009). This article outlines the biology of the disease, key infection periods, and the latest cultural and chemical strategies for effective management. Pathogen Biology SDS is caused by soil-borne fungi in the genus Ophiosphaerella, specifically O. herpotricha, O. korrae, and O. narmari. These pathogens differ in geographic distribution and turfgrass host preference. In Alabama, O. korrae is likely the most prevalent species (Hutchens et al., 2025). Disease Cycle and Symptoms The fungi that cause Spring Dead Spot are ectotrophic root-infecting pathogens, colonizing the outer surfaces of roots, rhizomes, and stolons, leading to necrosis. This damage predisposes turfgrass to winter injury, with symptoms emerging in spring due to the combined effects of fall infection and harsh winter conditions. SDS is a monocyclic disease, with a single infection cycle per year. Infection occurs in late summer to early fall, when evening temperatures cool and soil temperatures drop consistently below 70°F. Symptoms appear in spring as circular or semi-circular patches of dead turf, ranging from six inches to several feet in diameter. Severe outbreaks can result in large, coalesced areas of turf loss, with recovery taking two to three months after green-up—making proactive management essential. Cultural Management Strategies Effective cultural management hinges on understanding the disease's biology. Thatch and organic matter serve as reservoirs for pathogen survival. Practices such as core aerification, verticutting, and fraze mowing during the summer remove thatch and dilute organic matter, thereby reducing inoculum and mitigating spring symptoms (Hutchens et al., 2025). Conversely, cultural practices such as verticutting and aerification on symptomatic turf in spring can actually hinder recovery. Mechanical stress on already damaged turf slows regrowth, so spring cultivation should be avoided. Proper fertilization, particularly nitrogen applications in late summer/fall and spring, supports plant health and recovery (Tredway, 2021; Hutchens et al., 2022). Ammonium sulfate has shown efficacy against O. herpotricha, while calcium nitrate is more effective against O. korrae—highlighting the importance of pathogen identification (Tredway, 2021). Chemical Management Strategies Chemical control with fungicides is most effective when timed correctly. Applications should occur during fall as soil temperatures drop consistently between 65-55°F. Current research is refining this window using the Battaglia-Hutchens cooling degree-day model to optimize timing (Battaglia et al., 2024). The modern demethylation-inhibiting (DMI) fungicide mefentrifluconazole offers strong SDS suppression, while older DMIs like propiconazole and tebuconazole provide moderate efficacy (Clarke et al., 2020). Select succinate dehydrogenase-inhibiting (SDHI) fungicides—like isofetamid, penthiopyrad, and pydiflumetofen—also provide excellent control but carry a higher risk of resistance. Rotating SDHIs with DMIs is recommended to preserve efficacy. Precision Application Technologies Advancements in GPS-guided sprayers allow for targeted fungicide applications. Mapping disease areas in spring and early summer enables precise fall fungicide treatments, reducing both cost and environmental impact (Booth et al., 2021; Henderson et al., 2025). Conclusion Understanding pathogen biology, the disease cycle, and optimal application timing is essential for managing SDS. Continued research is uncovering pathogen-specific responses to cultural and chemical strategies, offering promising avenues for more refined and effective control. Tre giorni di incontri con autori, studiosi e divulgatori che animeranno lo storico Castello Visconteo e Villa Jucker, sede della Fondazione Famiglia Legnanese, per esplorare e raccontare la Storia in tutte le sue più articolate declinazioni.Primo appuntamento dell'ampio calendario di eventi con cui la città si appresta quest'anno a celebrare gli 850 anni dalla Battaglia di Legnano, il Festival propone un ricco programma di incontri, tutti gratuiti, per adulti, ragazzi e bambini.Sarà Luigi Barnaba Frigoli ad aprire la manifestazione, venerdì 10 aprile alle 18 in sala Caironi a Villa Jucker, trasportando il pubblico nel 1299, con il racconto mese per mese della vita quotidiana nel Medioevo italiano (Un anno nel Medioevo). Alle 21 il viaggio continua con Alessandro Vanoli alla scoperta dell'Oriente e della sua storia (Oriente).Sabato 11 aprile gli incontri si spostano al Castello. Si parte alle 10.30 con Laura Fusaro e il racconto delle intense relazioni familiari nel Ritratto di una famiglia italiana (sala Crivelli). A seguire Sergio Costanzo ripercorrerà il cammino di due fratelli irlandesi sulle orme dell'Arcangelo Michele (La via dell'Arcangelo, sala Cenobio, ore 11), Chiara Montani porterà il pubblico alla scoperta della pittrice Sofonisba, prima donna ad affermarsi tra i protagonisti del Rinascimento (Il destino di Sofonisba, sala Cornaggia, ore 11.30) e Marco Tabilio racconterà la vita di uno degli scienziati più celebri e amati della storia (Archimede, sala Previati, ore 11.30).Il pomeriggio prosegue con Stefania Auci che chiude la saga dei Florio con il nuovo capitolo dedicato alle origini della famiglia (L'alba dei Leoni, Pala Castello, ore 14.30). Cristoforo Gorno racconterà invece l'avventura del dio ribelle che rifiutò l'Olimpo (Dionisio, sala Cenobio, ore 15), Franco Franceschi, Paolo Nanni e Gabriella Piccinni offriranno uno sguardo inedito sul Medioevo, presentato come un'epoca di creatività e innovazione (Medioevo che crea, sala Previati, ore 15.30), mentre Sarah Pellizzari Rabolini e Daniela Dawan presenteranno due romanzi accomunati da un filo narrativo che attraversa generazioni e periodi storici (L'alba del 2 giugno e La colpa di tacere, sala Cornaggia, ore 15.30). Lucia Cimini e Livio Galla racconteranno due simboli dell'arte e dell'imprenditoria italiane: Villa Necchi Campiglio e il Lanificio Rossi (Finché tutto splende e Il canto dei telai, Sala Capriate, ore 16). Serena Romano guiderà il pubblico in un viaggio storico-artistico dalle Alpi alla Sicilia alla scoperta dell'arte romanica (Il Romanico, sala Previati, ore 16.30). Fotografie e oggetti come strumenti per leggere la storia   saranno al centro degli incontri con Alessandra Mauro (Aprire lo sguardo. Le 15 fotografie che raccontano l'Italia, sala Cenobio, ore 17) e con Marina Montesano (Ars Magica, sala Cornaggia, ore 17.30). Gaddo della Gherardesca proporrà un tuffo, tra passato e presente, nel Castello di Castagneto Carducci, luogo della sua infanzia (Al tempo di una volta, sala Previati, ore 18), mentre Gaja Cenciarelli rifletterà sul tema della rivoluzione e sulla possibilità del cambiamento (Il rivoluzionario e la maestra, sala Crivelli, ore 18.30). L'ultimo appuntamento di sabato, alle 21 è con Heddi Goodrich e la storia di Turia, una donna dell'età imperiale divisa tra desideri e paure, alla quale non manca nulla ma che si sente prigioniera della vita (L'anno delle mille vite, sala Previati). Domenica 12 aprile la giornata inizia alle 10.30 con i miti, le consuetudini e le storie curiose sull'Antica Roma narrate con ironia da Mario Lentano, (I romani che non ti aspetti, sala Previati). Si prosegue con la storia di due donne che, a modo loro, hanno sfidato le convenzioni del tempo: Bertha Ringer, prima donna a compiere un viaggio in automobile, raccontata da Emilia Covini (sala Crivelli, ore 11), e l'iconica Elena nel ritratto inedito e attuale che ne fa Brunella Schisa (La più bella, sala Previati, ore 12).  Laura Minervini invece ricostruirà la leggenda medievale degli “assassini”, nata dall'incontro tra storia e mito (L'invenzione degli assassini, sala Cenobio, ore 11.30).Nel pomeriggio Livio Gambarini riporta in vita Margherita da Arco e fra Dolcino in una storia di fede e rivoluzione (La ribelle di Dio, sala Capriate, ore 14), mentre Renato Carlo Miradoli racconterà delitti, intrighi e nobiltà nella Milano post-napoleonica (L'omicidio del marchese Aliprandi Martinengo, sala Crivelli, ore 14.30) e Barbara Bellomo porterà il pubblico nella Catania di inizio Novecento attraverso il simbolo dell'arancia Tarocco e il mistero che la circonda (L'incartatrice di arance, sala Previati, ore 15). Giulio Leoni e Paolo Lanzotti racconteranno un'Italia del passato, tra morti misteriose e oscuri intrighi (L'anatomista delle ombre e Il canto del corvo, sala Capriate, ore 15.30), Dino Messina rievocherà una pagina cruciale della storia italiana, che ha visto intrecciarsi il destino di un giornale e quello della nazione (La Repubblica nasce in via Solferino, sala Cenobio, ore 16) e Sara Rattaro illustrerà la figura della partigiana Teresa Mattei, protagonista della costruzione democratica del Paese (Il vestito di mia madre, sala Cornaggia, ore 16.30), mentre Paolo Grillo ricostruirà il drammatico assedio e la distruzione di Milano del 1162 da parte di Federico I Barbarossa (L'assedio di Milano, sala Previati, ore 16.30).
