Though teaching is something I love doing, sometimes teaching PEM to an ED resident is challenging (for many reasons). Dr. Steven Nazario has been a physician and an educator for many years which is why I invited him on the show to talk about the challenges of teaching PEM to EM residents. - - Foto: Lea Aring / Deutsche Gesellschaft für ME/CFS

In this episode of The Hydrogen Podcast, we dive into three game-changing hydrogen stories:

Ep. 680: Cranford | Chapter 2 Book talk begins at 6:13. This week's chapter is a sobering reminder that small towns sometimes carry the heaviest stories. Also, we get another battle over Dickens vs. Johnson (yep, that again). --------------------------------------------------------------- • 02:40 - : Listen to “The Blog” (4 min vs 19 min on “The Gist—yes, he titled them in reverse). This is one of the Long-Covid papers we've been waiting for. PEM—post-exertional malaise is “feeling tired/sore a day after a workout” on crack. • PEM pain and brain fog can last up to two weeks after exertion…The Rank Study he mentions is the group saying “duh m'dude. If you've been in bed for a year yer gonna be tired after a workouta. You gotta get back on your exercise routine and qitcherbellyaching” —a theory that's led to permanent patient paralysis. The other study is big because it indicates that the damage is mitochondrial, not systematic (i.e., we're getting plenty of blood to our muscles. The muscles just can't do anything WITH that oxygen) • —and from the People who created the Visible app that has saved my life (MakeVisible.com) • 03:20 Snake Oil!!! • 04:03 RAFFLE of "Knitting Companion," the amazing ergonomic book. But the meaning is the same.) • 15:15 - Biblical DebORah (the pronunciation that should be used ahem) read about her in Judges 4:4. • 15:36 - “Strong minded” women - yeah, that was an insult. Kinda like “She was warned. She was given an explanation. Nevertheless, she persisted…” when Elizabeth Warren tried to read a letter from Coretta Scott King into the Congressional Record in February 2017. See Mary Wollstonecraft on masculine women on • 17:50 - - this is not the one I remembered (they haven't digitised their entire collection) but it DOES have a shocking poem in it—by a 14 year old girl. • 18:50 - Just a heads up: Gaskell sort of doesn't really quote things accurately—at this time code she's sort of quoting Hamlet, she also makes up words like Brunonian • 20:02 - Sort of quoting Alexander Pope “Imitations of Horace” 1733 • 20:17 - Sort of quoting/sub-referencing Spenser's “The Faerie Queene” IV:3;32, 1596 • 20:55 - Nasty cruel Railroads. Despite the recent upheaval in clean tech efforts, governments around the world are investing billions into green hydrogen. Analysts are calling it the missing piece in the clean energy puzzle, especially for industries that can't run just on batteries or solar power. But the future of green hydrogen may not be decided in Silicon Valley or Europe or even China. It might come from a factory just outside Bengaluru where a little-known American startup called Ohmium is building sleek, modular machines, the size of a fridge. These are designed to split water into hydrogen and oxygen using nothing but renewable electricity.Ohmium's unique technology called PEM (proton exchange membrane) electrolysers—are compact, scalable, and fast becoming the system of choice for green hydrogen production. But can India really lead the global green hydrogen race and will Ohmium be the company to take us there?Tune in.Daybreak is produced from the newsroom of The Ken, India's first subscriber-only business news platform. Dipl.-Wirt.-Ing.

What if the secret to better museums was … neuroscience? How can museums inspire human creativity? How much media should be in a gallery — or should there be any? How soon should you get feedback on your exhibition ideas? Can museums help us all “escape the algorithm”? What does knitting have to do with visitor satisfaction? In this episode, we'll learn some unexpected tricks of the trade from a renowned museum leader. Lynda Roscoe Hartigan (The Rose-Marie and Eijk van Otterloo Executive Director and CEO of the Peabody Essex Museum) discusses “Creating Effective Museum Experiences” with host Jonathan Alger (Managing Partner, C&G Partners | The Exhibition and Experience Design Studio). Huge momentum in the hydrogen industry! In this episode of The Hydrogen Podcast, I unpack three major developments redefining the global hydrogen landscape:
H2Tech hosts a conversation with Dennis Bauer, Area Sales Manager at Neumann and Esser, at the World Electrolysis Congress in Cologne, Germany. Dennis discusses the company's rich 200-yr history, their role in the H2 industry, and the integration of PEM electrolyzers into H2 value chains. In addition, Michael Sweet, Director of New Energy at Rockwell, discusses the company's focus on energy transition and decarbonization. He highlights the role of automation in driving down costs for H2 production, drawing parallels to the solar industry's past challenges. Michael emphasizes the importance of scale and data-driven optimization in reducing the cost of green H2.
Send us a textI am so pleased to be back recording the podcast, thank you for your continued support. I am pleased to have returning guest Patrick Ussher join me on this episode. Patrick Ussher has had ME/CFS for 7 years. He has written a book ‘Understanding ME/CFS & Strategies for Healing'. which is now available as an audible, ebook or paperback copy. Click on the link below to find out more. In the book he communicates some of the latest, most exciting ME/CFS research as well as strategies he has used to improve his quality of life. The book is laid out with a broad audience in mind, inclusive of those living with ME/CFS and Long covid, their families and/or carers and medical professionals. It contains links to the relevant research and chapter summaries which are really helpful. In this episode of the podcast we discuss his book and go into more detail around the section of the book covering (PEM) post exertional malaise, and the research findings of Professor Klaus Wirth and Professor Carmen Scheibenbogen. This is an interesting conversation with light bulb moments. We discuss how PEM is a nemesis for so many living with these conditions, and what the research findings suggest is happening at cellular level. We touch on pacing in the absence of medical treatment and how important, although extremely challenging pacing is to avoid PEM. Patrick, talks about mitodicure, a potential medication to treat patients with post exertional malaise. This is in its infancy, in terms of requiring funding and clinical trials, more information is available via the link in the show notes. Known for candid perspectives and sense of humor, pediatric emergency guru Dr. Richard Cantor shares how PEM reminds residents to be present with patients. The organic nature of PEM requires us to use all five senses, engage with patients and their families, and see past burdensome EMRs. Dr. Cantor recalls his transformation from day one as an intern to life as an attending, and he reminds new practitioners to always do what's best for patients and their families.
Send us a textIn this episode of the podcast, Robin McNelis joins me again for part 2 of a series where we aim to cover a variety of topics which will support pacing/micropacing and much more. In this episode we discuss the exertion types and what uses energy, we recap on what pacing is and why it is important when you live with a chronic condition such as ME and long covid. Robin discusses the principles of pacing and the varying forms of pacing, we discuss listening to your body and the challenges of this with PEM/PESE. Robin explains how monitoring your HR/HRV is important, what is available to monitor this data and how to understand what level to set your alerts at to remain within the anaerobic threshold. We discuss the lactate threshold and Robin explains the zones of this threshold and how important it is to remain within the lactate threshold, especially those with PEM. Robin advises how the lactate threshold can be tested at home, and clinical testing. We additionally discuss how those with Orthostatic intolerance, PoTs can find it challenging to maintain their heart rate within their threshold due to heart rate spikes. Robin gives some great advice on how to support this and lots of other amazing information and advice throughout this podcast, so grab a cuppa and have a listen. Send us a textThis is part 1 of a series with Robin McNelis, Clinical Specialist Cardiorespiratory physiotherapist. In this episode we discuss pacing and micro pacing, how it can be challenging to understand and to navigate. Robin explains the principles of pacing, identifying triggers, the lactate threshold and how to break down tasks and why this is important. Robin shares his own journey and how he identified his activity triggers and applied micro pacing. He explains how certain tasks can use the same energy requirement however the mechanics/movement of certain tasks can be more of a trigger spiking the heart rate and causing PEM/PESE. Robin discusses the difference between a strength issue and a fatigue issue. How it is not always understood that PEM is a physiological issue, not a muscle wasting issue. We talk about the lactate threshold and the protocol Robin has developed to test a person's lactate threshold. We will discuss this further on a future podcast episode. Have a listen to this episode of the podcast to find out so much more; if you would like to contact Robin his details can be found below. Dr. Melanie Hoppers never planned to become an expert on chronic fatigue syndrome (CFS). She learned about it briefly in medical school, but her professors largely brushed off the complaints of CFS patients. It wasn't until her own daughter got sick that Dr. Hoppers began to research CFS, becoming desperate to find anything that might help. That's when she discovered the work of Lucinda Bateman and the Bateman Horne Center, a clinical care and research institution specializing in CFS (also known as myalgic encephalomyelitis or ME) that focuses on educating not just patients, but healthcare providers about this complex illness. Dr. Hoppers learned so much about how to manage CFS and it's common comorbidities like mast cell activation syndrome (MCAS) that she soon decided to join forces with the Bateman Horne Center to fight back against these complex chronic diseases. In this episode of the Major Pain podcast, Dr. Hoppers discusses her journey from CFS caregiver to practitioner. She shares the personal history of her daughter's illness that brought CFS to her attention, and discusses how learning to combat this disease has changed the course of her professional life. We also discuss the disease itself, and some of the techniques the Bateman Horne Center is utilizing to manage it. She tells us that many people with CFS/ME also have to deal with MCAS, while sharing the basics of bringing mast cells under control. She also discusses the importance of pacing for those suffering from post-exertional malaise (PEM), her thoughts on low-dose naltrexone (LDN) and an overview of the important work being done at the Bateman Horne Center. Thank you for joining us for our 2nd Cabral HouseCall of the weekend! I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks… Ales: Dr. Cabral, what do you think of Palmitoylethanolamide (PEA) as a natural help with severe histamine intolerance? If everything else fails, would you recommend this compound to help with mast cells a and supress their excessive production of histamine? Is it even natural suplement like compound or a drug? Please help me understand. Thank you Sarah: I've had reoccurring UTIs and always have to take antibiotics to clear it despite knowing how destructive they can be. This has led to reoccurring yeast infections. I'm about to start the CBO protocol for the yeast, how do I tackle the UTIs? I've tried ALL the natural remedies (dmannose, uva ursi, oregano, probiotics etc) and still suffering and feel my only resort is antibiotics which I want to avoid, as I'm now dealing with yeast overgrowth too. Thank you!! Sarah: My boyfriend has a staph infection that is showing up as rash/pimples all over his body. Dermatologists are telling him the only option is antibiotics. He also has done lab testing and is showing an overgrowth of candida krusei. Trying to avoid antibiotic use as much as possible especially when he has an overgrowth of candida. He is about to start the CBO protocol. Is there anything that can be done for staph naturally/preventing it, other than washing with ACV, tea trea, coconut oil as we've tried that. Jim: Hello Dr. C! I can't thank you enough for all of the hard work you put into these podcasts. I binge listen to them when I travel. I've searched but can't find anything on Post Exertional Malaise. I've been dealing with a variety of health issues from Hashimoto's to, most recently, toxic mold poisoning. I'm currently doing your mold protocol and am having some really bad flare-ups that are very similar to ones I've been experiencing for a few years. To describe what happens, it's as if I am a battery powered robot and my battery go dead. I can literally feel my body shut off, like my battery has been drained. I've had "the virus" twice so I looked into Long Covid and ran across PEM. I checked all the boxes for the symptoms but how can I verify it is this and not the mold? Amanda: Hello! Just wondering what to do for a 3 year old with very inconsistent bowel movements/pellets. Tried high dose vitamin C/magnesium citrate, oxypowder, higher fiber diet, castor oil packs at night, energy work, osteopath, Epsom salt baths, prunes, pears etc. Diet is on point, only choline and ghee butter trigger bowel movements, but if I stop these then it gets irregular. Any advice on supplements/tinctures or foods that can support bile flow? Any advice on how to correct these bowel issues for a toddler, or where to start? Thank you very much for all your help !!!
