Our next AAOS 2025 Annual Meeting poster is titled Mid-term Follow-up of Patellofemoral Osteochondral Allograft Transplantation. This study was performed by Dr. Bill Bugbee and his team at the Scripps Clinic. Dr. Bugbee was a guest on our show back in November 2023.  That is episode 59 and 60, if you want to go check it out.  In that episode, we discussed osteochondral allograft transplantation for various cartilage defects of the knee, including medial and lateral tibiofemoral, which are the more common locations for OCA transplantation.  This study focuses specifically on outcomes of OCA transplantation for patellofemoral cartilage defects.This study identified 127 patients undergoing OCA transplantation in the patellofemoral compartment – 51 to the patella, 47 to the trochlea and 29 bipolar patella and trochlea.  The most common indication was a degenerative cartilage lesion at 47%, followed by a traumatic cartilage injury at 25% and osteochondritis dissecans at 15%.  All patients had a minimum follow-up of 2 years.  OCA failure was defined as any reoperation that involved removal of the allograft.  Patient reported outcomes were also assessed pre-op and post-op, including the IKDC score and KOOS score.So, what did this study find?  First, reoperations occurred in 39% of the knees.  Rate of reoperation was not statistically significant between patella, trochlea and bipolar grafts.  Second, OCA failures occurred in 16% of the knees at a median 4.4 years following the index surgery, and the most common revision procedure was arthroplasty.  Although it did not reach statistical significance, trochlear grafts had a lower failure rate of 9% compared to patellar grafts at 20% and bipolar grafts at 21%.Overall, graft survivorship at 5 and 10 years was 91% and 82%, respectively.  Patients with patellar, trochlear and bipolar grafts all had significant improvement in IKDC scores and KOOS scores from preop to the latest follow-up and no statistically significant differences were observed between the groups.  Overall, 77% of patients reported being satisfied with the results of the OCA transplantation with no statistically significant differences in satisfaction between the groups.Also check out:Episode 91: Dr. Tom DeBerardino 0n Advances in Patellofemoral Cartilage Restoration

Bring to a simmer, don't let it boil over. Whisk constantly for 3-5 minutes until the mixture thickens slightly. 3. Remove from heat, add sugar and salt. Stir until mixed and sugar melted. 4. On this episode, we're going to continue our discussion with Dr. William Bugbee and focus on OCA surgical technique and then discuss clinical outcomes including return to sports.Our conversation picks back up with a recent paper from the July issue of Cartilage this year titled “Young Age and Concomitant or Prior Bony Realignment Procedures are Associated with Decreased Risk of Failure of Osteochondral Allograft Transplantation in the Knee.” This retrospective nationwide database study represents the largest OCA cohort study to date and found that less than 2% of patients required salvage surgery. Young age, less than 29, and having a bony realignment procedure were associated with a significantly lower rate of salvage surgery – include revision cartilage procedures and arthroplasty.We finish up today with an article from the June 2017 issue of AJSM titled “Return to Sport and Recreational Activity after Osteochondral Allograft Transplantation in the Knee.” Dr. Bugbee and colleagues at Scripps Clinic in La Jolla California reported that at a mean follow up of 6 years, 75% of patients were able to return to sport or recreational activity. Patients who did not return were more likely to be female and have a large graft size. 25% of knees underwent further surgery and 9% were considered allograft failures. Of the patients without OCA failure, 91% were satisfied with the results of surgery.

Recipe: Sticky Squirrel ●    4 parts Hot Apple Cider ●    1 1/2 parts Skrewball Whiskey ●    Ground Cinnamon ●    Sugar ●    Honey ●     Whole Cinnamon Sticks Preparation: Pour sugar and ground cinnamon onto a small plate. Dip the rim of the mug in the honey then into a cinnamon/sugar mixture. Heat apple cider in a pot over medium heat. Once at desired temperature pour into the mug of your choice. On today's episode we're focusing on osteochondral allograft transplantation or “OCA” with Dr. William Bugbee. We have some great articles for you that contribute well to our conversation on OCA, including allograft preparation and storage, graft choice, surgical technique, clinical outcomes of OCA and return to sport. We'll start off our discussion today with an article authored by our guest, Dr. William Bugbee, from the May 2022 issue of AJSM titled “Fresh Osteochondral and Chondral Allograft Preservation and Storage Media: A Systematic Review of the Literature.” There was significant variability in experimental designs and incomplete reporting across the studies, so no real conclusions could be drawn regarding optimal storage conditions. While 60% of animal model studies suggest storage time may impact outcomes and 80% indicate inferior outcomes with frozen OCA compared to fresh OCA, the authors note that 75% of clinical studies reported no correlation between storage time and outcomes. So, we have a lot left to learn here.Then, from the March 2022 issue of AJSM, we review an article titled “Association of Sex Mismatch Between Donor and Recipient With Graft Survivorship at 5 Years After OCA Transplantation.” Dr. Gomoll and colleagues at Brigham and Women's reported that a significantly lower rate of graft survival was observed after different sex donor-recipient transplant compared to same sex donor-recipient OCA – 63% compared to 92%. Patients who received a donor-recipient sex-mismatch transplantation were 2.9X more likely to fail. Male donor to female recipient demonstrated the highest likelihood of failure compared to all other combinations.We are joined today by Dr. William Bugbee, a board-certified orthopedic surgeon specializing in joint reconstruction and cartilage restoration at Scripps Clinic in La Jolla, California. Dr. Bugbee received his medical degree from UC San Diego and remained at UC San Diego for his orthopedic surgery residency.  He then went on to complete a fellowship in joint reconstruction at the Anderson Orthopaedic Research Institute. Dr. Bugbee has published extensively on the topic of osteochondral allograft transplantation, so we're very excited that he is joining us today to share his expertise with all of you!

The Spiced Honey Bourbon Old Fashioned comes from Half Baked Harvest blog. If your 2 Girls could accomplish this you know you can pull it off. This episode also counts as Bugbee's Will. Ingredients: SPICED HONEY SYRUP ▢1/2 cup honey ▢2 sprigs fresh rosemary, plus more for serving ▢2 cinnamon sticks (or 1 teaspoon ground cinnamon) ▢1 tablespoon cardamom pods (or 1/2 teaspoon ground cardamom) ▢1 inch fresh ginger, sliced ▢zest of 1 orange ▢1 teaspoon vanilla extract OLD FASHIONED ▢2 ounces (1/4 cup) bourbon ▢1 ounce (2 tablespoons) lemon juice ▢1 ounce (2 tablespoons) orange or clementine juice ▢1-2 dashes orange bitters ▢1-2 ounces (2-4 tablespoons) spiced honey syrup Instructions 1. To make the spiced honey syrup. In a medium pot, bring 1/2 cup water, the honey, rosemary, cinnamon, cardamom, and ginger to a boil over high heat. Boil 2-3 minutes, then remove from the heat. Stir in the orange zest and vanilla. Let cool. This syrup makes enough for about 6 drinks. 2. In a cocktail shaker, combine 2-3 tablespoons honey syrup, the bourbon, lemon juice, orange juice, and orange bitters. Add ice and shake. Strain into a glass. Garnish with an orange slice and rosemary, if desired. To lead into the next season of Enduring Interest, we're re-releasing our first two seasons, covering totalitarianism and ideology and liberal education.  We'll be back on September 8 with a new season covering free speech and censorship. In this episode we discuss a short essay by the philosopher Henry Bugbee, “Education and the Style of our Lives.” Bugbee taught for a number of years at the University of Montana. This short, beautiful and thought-provoking essay was occasioned by a report that a commission presented to the Montana legislature. In just over nine pages, Bugbee lays out the core of education as seen from the standpoint of both teacher and student. He seeks the revitalization of a dialogue that brings text and world together—experience is illuminated and meaning is discovered. The piece was published in Profiles, the magazine of the University of Montana in May of 1974. Our guest is Joseph M. Keegin. Joseph talks about Bugbee's insistence that both teacher and student must be capable of self-risk. We discuss Bugbee's reflections on the relationship between liberal learning and experience and how Bugbee's appeal to experience is quite different from the way people appeal to “lived experience” today. We end by thinking about Bugbee's appeal to the duty to bring the past to bear on the present. Joseph makes a plea for people to find a copy of Bugbee's only published book, The Inward Morning, which is a “philosophical exercise conducted through fifteen months of journal entries.” You can find Joseph's essay on Bugbee here and his blog is www.fxxfy.net. Richard has been learning traditional plant uses of southern California and the Kumeyaay language from Jane Dumas, a Kumeyaay Elder from Jamul Indian Village from 1980 to 2014. He was the ethnobotanist for the Traditional Indian Health Program through Riverside-San Bernardino Indian Health providing information on the interactions between traditional plant and pharmaceutical medicines. He teaches indigenous material cultures and traditional plant uses of southern California at many museums, botanical gardens, and reservations, and is an instructor for summer cultural programs for several Kumeyaay tribes. Indication: Bexacat is indicated to improve glycemic control in otherwise healthy cats with diabetes mellitus not previously treated with insulin. Cats treated with Bexacat may be at an increased risk of diabetic ketoacidosis or euglycemic diabetic ketoacidosis, both of which may result in death. Development of these conditions should be treated promptly, including insulin administration and discontinuation of Bexacat. Do not use Bexacat in cats with diabetes mellitus who have previously been treated with insulin, who are receiving insulin, or in cats with insulin-dependent diabetes mellitus. The use of Bexacat in cats with insulin-dependent diabetes mellitus, or the withdrawal of insulin and initiation of Bexacat, is associated with an increased risk of diabetic ketoacidosis or euglycemic diabetic ketoacidosis and death. Sudden onset of hyporexia/anorexia, lethargy, dehydration, diarrhea that is unresponsive to conventional therapy, or weight loss in cats receiving Bexacat should prompt immediate discontinuation of Bexacat and assessment for diabetic ketoacidosis, regardless of blood glucose level. Bexacat should not be initiated in cats with pancreatitis, anorexia, dehydration, or lethargy at the time of diagnosis of diabetes mellitus, as it may indicate the presence of other concurrent disease and increase the risk of diabetic ketoacidosis. Due to risk of severe adverse reactions, do not use Bexacat in cats with evidence of hepatic disease or reduced renal function. Consult a physician in case of accidental ingestion by humans. At the time of this recording, Dan and Greg were celebrating their 2nd year “kidneyversary.” When Greg's kidneys were poisoned because of an undiagnosed prostate issue, family and friends around the globe stepped up to help in many ways.   Listen as they retell the story of Greg's dramatic health decline. A previously energetic and youthful Greg shares the moment things went down, literally, as he was dancing the Cha-Cha at a wedding and could not get up after “going down low.” From that point many diagnostic tests and complications would lead to complete kidney failure. His son-in-law would ultimately be a willing match for a kidney transplant. This townie actually attended middle school and high school at the same time as me! Different paths never intersected our stories, however, until this interview. This Centennial grad chose to enlist in the military to gain discipline, direction and to see the world. He got much more than he bargained for after September 11th tested the defensive infrastructure of our nation. Thousands of men and women were sent overseas to fight in a war no one was anticipating or prepared for.   Listen as this United States Army Veteran talks about going from working at Bagelmen's one day to being deployed to the Middle East the next, saying goodbye to his dad, his friends and all of his possessions except for a backpack. From training in the Wisconsin winters to 130-degree desert, Brian and his fellow soldiers were initially tasked with setting up a military base before a lot of the proper equipment was even in place—like bathrooms and showers. As the opposition became more organized and unpredictable, his company would escort supply vehicles on day long treks up and down dusty, sandy, treacherous roads.
