Ever since we started struggling to conceive, I have had the feeling something is not right. Maybe it's the yoga keeping me in touch with my body, maybe it's all the journalling and self-reflection. Maybe it's just gut instinct. But no matter that it was, we finally got the answers we have been looking for after a visit to a fertility clinic in Belgium. With any diagnosis, it can be a lot to process. So I chatted with my long-time friend and fertility midwife Tor Chivers about our recent tests, procedures and results. Together we unpack the blood tests, MRI and hysteroscopy procedures that brought us to the diagnosis that helps to explain the reason I have struggled to fall (and stay) pregnant. Also, my husband learns his sperm quality has been low, another important reminder that in fertility, it really is 50/50!The bumps in the road are not over for us. The treatment begins and that will bring its own set of challenges. But, we feel eternally grateful to have answers knowing so many people out there are still searching for theirs. If this is you, I'm sending so much love, and urge you to keep pushing your healthcare providers to look deeper.xxAnnaIf you would like to learn more about any of the discussed topics, I have found the following links particularly helpful:AMHhttps://www.littleangelivf.com/blog/fertility/amh-test-normal-range-and-amh-levels/ Dysmorphic (T-shaped) uterus and my up-coming metroplastyhttps://pubmed.ncbi.nlm.nih.gov/34319061/https://pubmed.ncbi.nlm.nih.gov/31146029/Decapeptyl injectionshttps://www.medicines.org.uk/emc/files/pil.963.pdfhttps://www.news-medical.net/drugs/Decapeptyl.aspxWANT TO FOLLOW ALONG?Bumps Podcast InstagramContact Anna:helloannachristie@gmail.comhttps://thebigpivot.substack.com/ Hosted on Acast. Hello, and welcome to Beauty and the Biz where we talk about the business and marketing side of plastic surgery and reputation management. I'm your host, Catherine Maley, author of Your Aesthetic Practice – What your patients are saying, as well as consultant to plastic surgeons, to get them more patients, more profits and stellar reputations. Now, today's episode is called "Reputation Management — with Jeffrey J. Segal, MD, JD". At the medical conferences I speak at, there's usually a talk about reputation management and how to keep %*@^ from hitting the fan and ruining your name.  So, I invited one of the country's leading authorities on medical malpractice and reputation management to update you on how to stay out of trouble and keep your reputation as stellar as possible.  Jeffrey Segal, MD, JD is a board-certified neurosurgeon, but he's also an attorney and partner at ByrdAdatto law firm. He's also a reputation management expert. Dr. Segal focuses on keeping doctors from being sued for frivolous reasons and to minimize the need for difficult reputation management. On this week's Beauty and the Biz Podcast on reputation management, Dr. Segal and I talked about: How to protect, preserve and manage your reputation online How to spot trouble early so your reputation management take far less effort How to handle patients with mental health issues who can cause havoc with your reputation management and a lot more… Every surgeon who has done enough surgery has or will experience patients who become problems, so this episode gives you strategies to help with your reputation management. *TW*Today I am joined with a fellow anorexia warrior, Brittney (@bites_by_b), to have a no-filter, raw conversation of our experiences with eating disorders. We raise awareness of  the psychologically damaging reality of anorexia nervosa, how isolating, terrifying and identity engulfing it can become – seeking comfort and control in abusing, malnourishing and depriving our bodies. How small habits and rituals can detrimentally develop and amplify into a sinister, life threatening situation, where your individual lifestyle, behaviours, characteristics are controlled and altered in aid of the harmful progression of the eating disorderWe also discuss our poor experience with doctors within the recovery process – where medical practitioners solely focus on reverting symptoms, rather than providing consideration towards psychological triggers and mental health issues.Often, and specifically within our situations, the severity of the illness is totally minimised – being refused treatment due to weight not being perceived as “low enough”, which only triggered the anorexia further, craving validation – only pushing us to pursue the disordered tendencies, putting us into a life threatening position. Thus raising the discussion on the essentiality of early intervention and the application of prevention techniques.Overall, we talk toxic environments, heavily dysmorphic mindsets and how this harmful illness influences and manipulates  you to totally dictate and dedicate your life to it – no matter the detrimental consequences upon your emotional and physical health, relationships or future.Due to the nature of the discussion, I would like to pre-warm any listeners of a fragile mindset and apply a trigger warning. The conversations we have regarding our previous circumstances are very raw, and can possibly be translated as triggering, so please skip this episode if you think it may impact you negatively.https://www.beateatingdisorders.org.uk/

Session 21 Today, we’ve got some interesting case of a 14-year-old male with some malaise and abdominal pain. Once again, we're joined by Dr. Karen Shackelford of BoardVitals. Check out the resources they have to offer. Use the promo code BOARDROUNDS to save 15% off upon purchasing a QBank. Also, they have an Ask a Clinician feature where clicking a button gives you access to a physician who will help you through specific questions or content. This feature comes with their 3-month and 6-month plan. This podcast is part of the Meded Media network where we help premeds and medical students as they journey towards becoming great physicians. [02:15] Question of the Week A 14-year-old male is evaluated for malaise and abdominal pain. He reports passing dark urine this morning. His past medical history is significant for Streptococcal pharyngitis ten days ago, for which he received Amoxicillin. Today, his vital signs are within normal limits, except for an elevated blood pressure of 145/95 mmHg. Examination reveals spatial edema with pronounced periorbital swelling. He has 1+ pedal edema bilaterally. Abdominal exam reveals mild, diffuse tenderness without rebound or guarding. Laboratory studies are unremarkable except for a serum creatinine of 2 mg/dL. What is the common finding associated with the patient's condition? (A) Hypovolemia (B) Polyuria (C) Red blood cell cast (D) Hypokalemia [03:30] Thought Process Behind the Answers The correct answer here is C. Basically, the patient has glomerulonephritis. This is characterized by red blood cell casts that are almost pathognomonic for glomerulonephritis. Poststreptococcal glomerulonephritis is not common but also not unusual. The history of streptococcal pharyngitis should lead to that conclusion. Other symptoms of glomerulonephritis include white blood cell casts, hematuria, and proteinuria. But for this question, you have to hone in on the glomerulonephritis. Other findings in the urine sediment include granular casts. Dysmorphic red blood cells are strongly associated with glomerulonephritis and proteinuria. This is clinically manifested by a slow and progressive rise in serum creatinine and fluid hypertension, peripheral or periorbital edema, and sometimes, hypercoagulability. Rhabdomyolysis may come to mind but it wasn't really an option among the choices. This could happen on the boards. There might be systemic manifestations of some underlying disease process associated with glomerulonephritis. There's a group of immunologically triggered disorders that result primarily or characterized by glomerular inflammation. It can also manifest the proliferation of glomerular tissues that damage that basement membrane, mesangium, or the capillary endothelium. [07:15] Understanding the Other Answer Choices Glomerulonephritis is associated with hypervolemia. In this case, you have the proteinuria. But with the edema, you should immediately be able to figure out that it wasn't associated with hypovolemia. The patient had pedal edema and periorbital edema. It is also important to mention that there are three primary mechanisms of glomerular inflammation. And what distinguishes a nephrotic syndrome from glomerulonephritis is the inflammation as a mechanism of damaging the glomerular apparatus. This might come in the boards that the mechanisms are either immune complex deposition as in this case. Anti-glomerular basement disease is associated with Wegener's granulomatosis, eosinophilic granulomatosis with polyangiitis, or microscopic polyangiitis. The other mechanism is the antineutrophil cytoplasmic autoantibody (ANCA) or small vessel vasculitis. It causes damage to the glomerular filtration barrier. This results in the leakage of plasma proteins and inflammatory cells into the renal tubule. Patients may even have pulmonary edema in some cases. The hypertension results from retaining salt and water. The patient has a rising creatinine and so hyperkalemia is more likely to happen. So this is an easy one to rule out. Polyuria is associated with diabetes and causes diabetic ketoacidosis. This isn't that uncommon. Further laboratory testing is needed to determine the ideology. Typically on post-streptococcal glomerulonephritis, it is self-limited. A fair amount doesn't progress to renal failure. But testing is needed to determine the ideology on other cases whether it's long-lasting or the significant decline in glomerular filtration rate. There is usually a raise in serum complement levels, anti-neutrophilic cytoplasmic autoantibodies, antiglomerular basement membrane autoantibodies, antinuclear antibodies, anti-double stranded DNA. The treatment is just supported in these cases or treatment of the underlying disorder if there is one that you can determine from these other tests. Links: Meded Media BoardVitals

WHO SAYS I'M NOT PERFECT???   "I first started obsessing over what i ate, before starting to hide food and eating only in private. It was a scary time in my life"   -Josh Wiggins   In the lead-up to recording this episode, I mentioned to several people that the topic was going to be on Male Anorexia. Unequivocally, the response was one of amazement and generally followed a reply to the effect of "that would be really rare wouldn't it??" And I'll tell you what I told them.... "NO, no it isn't in fact" Male anorexia and MDD (as it's more commonly known), are a bit like prostate cancer is to older men; a secret. Something that exists but isn't commonly known, nor discussed. Today's guest however has lived the nightmare and has promised himself that he will make a difference in the lives of others by sharing the fate he was dealt. The young, skinny kid I first met in the gym many moons ago was already under the heavy influence of body dysmorphia, and struggling in ways people would never know. There is nothing especially different about Josh Wiggins, or his story for that matter. What is different though is his tenacity to fight the good fight, and his brave nature in completely disclosing his own demons in an effort to help any person who currently finds themselves in the throes of a similar state.   On this episode Josh and I discuss   -Early detection and the warning signs of what could be a problem.   -The reality of living with an eating disorder & it’s effects on loved ones.   -Muscle Dysmorphic disorder / Bigorexia   -Anorexia   -Calorie observation & IIFYM   -The reality of drug use in young gym-goers   -The veil of delusion within the media & fitness industry & why ultimately the industries themselves are largely to blame.   Josh can be reached on his new IG account @_joshwiggins & is willing and able to support any of you who are currently struggling with similar life issues & requiring an open ear. These things don’t just go away or get better on their own guys, so make a choice to live your life abundantly and reach out to those who can help.     Enjoy today’s show   Shannon...

Ever heard of "orthorexia"? It's is an eating disorder characterized by an extreme or excessive preoccupation with avoiding foods perceived to be unhealthy.  In other words, it's being absolutely obsessed with eating all things paleo, gluten-free, vegan, raw, non-GMO, organic, and fair-trade, and counting every friggin' calorie of it. Or how about "body dysmorphic disorder?" This one is characterized by an obsessive preoccupation that some aspect of one's own appearance is severely flawed and warrants exceptional measures to hide or fix it. In other words, you must maintain a nice body, at all costs. Unfortunately, it's all-too-common to see eating-obsessed, exercise-obsessed people destroying their bodies and their minds with worrying and an obsessive-compulsive approach to nutrition and fitness. Heck, I , and while everything in the book is sound advice, you could also stress your brain and hurt your body trying to take an "all-or-nothing" approach and trying to do all 30 reboots all at once! My guest in today's podcast, Devin Burke, takes a completely opposite approach to health: a minimalist approach. He preaches that people are often obsessively looking for that "silver bullet" and unfortunately often miss tapping into the potential that is right in front of them - if they are only inspired, educated and disciplined enough to implement these necessary changes they could take them to the “next level", without an unhealthy obsession. Devin is a wellness educator, and passionate health coach. He is a Certified Holistic Health Coach and a Certified Personal Trainer by the American College of Sports Medicine. He is the founder of , a healthy lifestyle coaching business based in South Florida and author of “”. And he is also a graduate of the , the world's top online school for becoming a health, lifestyle and nutrition coach (save $1000 on an IIN certification if you contact them and mention this show). During our discussion, you'll discover: -How to completely reset your brain to eliminate an obsession with eating gluten-free, or getting X number of calories, or eating the perfect carb:fat:protein ratios... -How something called neurolinguistic programming can help you overcome an obsession with food... -Why you need to branch-out and eat a variety of different foods, even if that includes components such as dairy and gluten... -What to eat when you're traveling or at a party, and you want to "enjoy" food, but you still want to avoid making yourself sick or unhealthy... -Why what pop culture tells you is the perfect body completely flies in the face of your natural human ancestry and genetic hardwiring... -How to create exercise goals that go above and beyond just "getting a better body"... -How to quit obsessing over WiFi signals, mold, drinking water, personal care products and household cleaning chemicals... -Why true happiness is not tied to perfect health and how to truly achieve happiness... -And much more!
