Dr. Michael Roizen, chief wellness officer emeritus of the Cleveland Clinic and author of nine best-selling books, has developed a concept, RealAge, that motivates people to take control of their lives in a way that will help them live longer and healthier. “The most important thing for people to understand is they're a genetic engineer,” he maintains. The choices people make in how they live can determine how long they live. His RealAge program, which suggests that people at the age of 90 will soon be able to live like 40-year-olds, advocates taking steps to remain physically active, reduce stress and continue social engagement. All can prolong life span and assure greater health. “When you do stress management or when you do physical activity, you change which of the genes are in or not in your cells,” Dr. Roizen says. Stressing a muscle, he explains, can send a protein to one's brain that can fertilize the hippocampus and act as Miracle Grow for the brain. This, in turn, can reduce the likelihood of dementia and other forms of cognitive dysfunction. Every person, he says, can reach their own “real age” by choosing their method of activity. ”If you don't like walking, you can do gardening, you can play with your kids, you can play ping pong,” Dr. Roizen says. “It's any activity. He is board certified in both internal medicine and anesthesiology. He was running a step-down ICU at the University of California San Francisco Medical Center studying the outcome of patients who had undergone cardiovascular surgery. “It wasn't their cardiac history or their lung function or their liver function or kidney function or their brain function that determined outcome,” he discovered. “What determined the outcome was their age.” He then embarked on a new mission as a physician - motivating patients to take charge of the way they aged. Dr. Roizen served as Cleveland Clinic's first Chief Wellness Officer from 2007 to 2019. He now serves fifty percent time as the Cleveland Clinic's Chief Wellness Officer Emeritus, and the other half as a Professor at the Learner College of Medicine of the Cleveland Clinic at Case Western Reserve University. He is a recipient of an Emmy, an Elle, and the Paul Rogers Best Medical Communicator Award from the National Library of Medicine. He initiated and developed the RealAge concept to motivate behavior change.  He believes that soon 90 will be the new 40, and how one can prepare for it is described in his most recent book, The Great Age Reboot, and Reboot Your Age app. Dr. Roizen is a Phi Beta Kappa graduate of Williams College and became a member of the American Osteopathic Association after graduating from UCSF School of Medicine.  He has authored over 195 peer reviewed scientific publications, four New York Times #1 bestsellers, and nine overall bestsellers.  He and Dr. Mehmet Oz co-authored a daily column syndicated to over 100 newspapers that translates current scientific reports into actionable steps for lay audiences. Dr. Jeffrey Gladden's foremost ambition is to help people live well beyond 100 years. That goal also entails assuring that individuals who live to be 120 or 130 years keep active and in tiptop shape — both physically and mentally. “Being in shape is being fast, agile, strong, quick, balanced, flexible with great cardiovascular endurance and good recovery,” he explains. “If you're going to have that later in life, you've got to focus on all of those things.” His medical program, Gladden Longevity, is engaged in working to realize those ambitions. “We currently have a research protocol that's in play for people to participate in, where that's literally what we're going for,” he says. “What we're really going for is making 100 the new 30.” While his program has therapies and devices that he says can help people achieve those objectives (though they can be costly), individuals can take direct action themselves to conquer the aging process. Rather than acquiescing to the traditional idea that aging means slowing down and losing impact, they can reject that belief and keep moving positively forward. Good sleep and eating habits, and remaining physically active, can contribute to that upward arc. “If you're asking questions,” he says, “that's growth. If you're stuck with your answers, that's decline, quite honestly. Having a growth mindset is critical.” Dr. Gladden's strategy involves four interlacing “circles” -  health, longevity, performance and life energy. He says people should approach these concepts exponentially rather than from a linear perspective. ***** Dr. Jeffrey Gladden is the founder, medical director and CEO of Gladden Longevity, a Texas-based concierge medical program established to optimize individuals' health and prolong their lives. Clients are given customized treatment plans configured to their own individual situations. The Salk Institute's Cellular Neurobiology Laboratory, which Pamela Maher, PhD heads, has been studying CBN. It is a chemical found in marijuana and findings show how it can protect nerve cells in the brain from oxidative damage, a major pathway to cell death. Their research also suggests that CBN works to protect the brain's mitochondria. Mitochondria is called the powerhouse of a cell.  Pair these two findings and the neuroprotective benefits CBN produces might be helpful in preventing diseases like Alzheimer's and Parkinson's, Maher says. However, human clinical trials are still years away. The lab's research thus far involves only mice. CBN is molecularly similar to THC, but is not psychoactive. Because it won't get one high, it is legal in every state. CBN is already produced commercially, sometimes in pill form, gummies or tinctures. Evidence suggests it is safe in animals and humans.  Dr. Pamela Maher, a neuroscientist, is a research professor at the Salk Institute and the head of Salk's Cellular Neurobiology Laboratory. A senior staff scientist at Salk since 2004, she has concentrated her research on screening for compounds that can halt the progress of neurodegenerative brain disorders, including Alzheimer's disease. A good way to keep one's cognitive faculties strong is to go to bed at a decent hour, according to Dr. Sara Mednick, director of the Sleep and Cognition Lab at the University of California, Irvine. Her research shows the amount of deep sleep we get in mid-life is critical for preventing dementia as we age.  It's so important that Dr. Mednick recommends getting to bed at 9pm since deep sleep happens at the beginning of the night and you want to get as much of it as possible.  A phase of sleep she calls “downstate” is important to the rhythm of life because it allows people to restore energy for their waking hours. If toxic proteins accumulated in the brain during daytime are not flushed out at night, they can build into “plaques and tangles” that become part of dementia. Healthy long-term sleep patterns, Dr. Mednick says, can prevent “memory pathologies.”  Professor Sara Mednick, PhD is a cognitive neuroscientist at the University of California, Irvine and author of The Power of the Downstate. She is passionate about understanding how the brain works through her research into sleep and the autonomic nervous system. Dr. Mednick's seven-bedroom sleep lab works literally around-the-clock to discover methods for boosting cognition by napping, stimulating the brain with electricity, sound and light, and pharmacology. Her lab also investigates how the menstrual cycle and aging affect the brain. Dayan Goodenowe, PhD, founder, president and CEO of Prodrome Sciences, which promotes disease prevention through detection and treatment, believes supplements and behavior modification can play a role in combating dementia. His target is the little-known lipid, plasmalogen, which he cited in his book, Breaking Alzheimer's, as a cause of Alzheimer's. Like many molecules, plasmalogens that are typically healthy in youth and middle-age deteriorate during old age. That can lead to the onset of Alzheimer's, according to Dr. Goodenowe, because of the lack of plasmalogen in membranes in the neurological system. When membranes become deficient in plasmalogens, as happens during aging, they can create an impairment of synaptic function, and that, according to Dr. Goodenowe, reduces neurological function typically associated with dementia. He compares the neurological system to the wires in a house's lighting network. Lights are turned on by a switch, whereas neurons communicate with each other through biochemical molecules. And plasmalogens serve as biological fuses. They are a critical building block of the membranes that protect and compartmentalize the various functions of the body, including the neurological workings of the brain. Supplements provide a way to boost the number and efficiency of plasmalogens and warding off dementia in the elderly, Dr. Goodenowe says, by maintaining a proper cell level so that the lipids produce their own plasmalogens. Another strategy is to practice intermittent fasting, he explains, because it can help control fat metabolism. He also recommends a balanced diet and the consumption of animal fat to build plasmalogen and stave off dementia. Other bodily processes that can spur dementia, such as brain shrinkage, amyloid plagues, and neurofibrillary tangles, also can be measured and controlled, according to Dr. Goodenowe. In fact, brain shrinkage is impacted by plasmalogens, Dr. Goodenowe asserts. When the brain shrinks, it loses lipids, and one critical lipid it loses is plasmalogen. Dr. Goodenowe says that individuals with high plasmalogens live and function longer — by about 30 years — than those with low plasmalogens. In fact, he contends, plasmalogens are a better indicator of longevity than age. People can overcome the potential impact of their genes, Dr. Goodenowe maintains — even when someone has APOE4, the gene scientists say carries the greatest risk for dementia. One of the plasmalogens' many functions is to regulate cholesterol, and as humans age, and their plasmalogens become deficient, they are less able to export cholesterol, a condition that is more acute in those with the APOE4 gene. When an APOE4 carrier ages, it can lead to higher cholesterol in the membranes, which has an impact on the amyloid pathway. Amyloid plaque can form in the brain, which can lead to synapse dysfunction and dementia. People can measure their plasmalogens through blood tests. A finger-prick test can put you on a path to cut age-related disease in half. In this episode, Jane discusses the exciting new lab tests now available to help diagnose and address genetic causes of cognitive decline with biochemist and Vice President of business development Ryan Smith of TruDiagnostics in Lexington, KY. It's our biological age that matters, not our chronological one. So, blow out the birthday candles if you wish, but if you want to know how long you're able to live, you'll need to measure your biological age. We talk with Ryan Smith, Vice President of Business Development for TruDiagnostic, who shares how their lab can measure your biological age. It's a fairly pricey test, but in the resources tab below our community has been offered a $50 discount. This is an at-home, do-it-yourself blood test that you mail back.  You don't need a doctor's order. The results will also reveal your rate of aging based on your telomeres, methylation markers and indicators of epigenetic cell health.  I've always been a believer that “if you can measure it, you can change it.” As we explore avenues to slow the disease of aging, knowing one's biological age can be a marker of the effectiveness of our interventions. Knowing your biological age and rate of aging can be a valuable tool if you're worried about memory loss, brain fog, or if you have a history of Alzheimer's in your family. Once you establish this benchmark, you can shoot for a goal of a biological age seven years younger than your chronological age.  If you can achieve that, you cut your risk of age-related disease in half! Now that's a worthy goal!
