"The United States does not have a national cancer registry. We have a bunch of state registries. Some of those registries do collaborate and share information, but the issue is the registries that do exist typically do not report cancer by occupation. So, we cannot get our arms around the potential work-relatedness of the health outcome given the current way the state registries collect information. What we're trying to set up, is a way to make what is currently an invisible risk, visible," ONS member Melissa McDiarmid, MD, MPH, DABT, professor of medicine and epidemiology and public health director of the division of occupational and environmental medicine at the University of Maryland School of Medicine in Baltimore, told Jaime Weimer, MSN, RN, AGCNS-BS, AOCNS®, manager of oncology nursing practice at ONS, during a conversation about the University of Maryland School of Medicine Hazardous Drug Safety Center Exposure Registry. Highlights From This Episode "We thought that in order to answer some of the unclear questions about health risk, we would set up an exposure registry, in this case, for oncology personnel who handle the drugs. This would then create a cohort that we could ask questions to. For example, we could try to characterize whether there is a cancer excess in this group. Or characterize the reproductive abnormalities in excess that people are experiencing." TS 6:21 "It's sort of counterintuitive that the healthcare industry, whose mission itself is care of the sick, is a high-hazard industry. We typically think about the risk as being from infectious diseases, and certainly we've all lived in our practice lifetime through some examples of that. Even before COVID-19, some of us were doing preparation for Ebola and that sort of thing. So, we're kind of used to that. But the hazards that you kind of grew up with, we've routinized or normalized handling group one, human carcinogens, which a number of these drugs are—it's just something we do every day. Well, it is, but we have to do it with respect and with care every day. And I think sometimes in that routineness of it, we have sort of lost sight of the vigilance that we need to maintain." TS 11:19 "It's very easy in the life cycle of a drug in an organization to do something that doesn't just impact you, but unknowingly, you've contaminated a surface for somebody who comes behind you. Who maybe doesn't have plastic protective equipment on because something that got contaminated shouldn't have been contaminated in the first place. If we could all be thinking of it as more of a team sport, especially in terms of safe handling, that our disposition and drug handling affects not just us and our health, but those of our colleagues." TS 24:47 "For the job history pieces, we ask what year you started, what year you stopped, and we ask about estimations of handling. So we'll be able to come up with either a duration or some kind of metric for the intensity and duration of your handling history, which will then permit us to sort the population who completed the survey into sort of low, medium, high. And we'll see whether the health outcomes that are being reported are influenced by that drug handling history." TS 27:45 "The idea that we aren't exposed to the same therapeutic dose we give to our patients is absolutely true. However, the dosing schedule to them versus us is very different, and we are exposed frequently, if not daily, to very small concentrations. They don't reach a cytotoxic dose necessarily, but we do know from a lot of studies that either ourselves or our colleagues are taking up drug from contaminated work environments. And you've probably seen there is an awful lot of intermediate evidence looking at genotoxic insult in pharmacists and nurses who handle the drugs. So clearly we're showing uptake and we're showing that there are biologically plausible, concerning measures that are taking place in us. So, I think that we need to come back and circle around the idea that we need to have deep respect for the toxicity of these agents." TS 35:03

History advances through concealment. When the world darkens, we must know the inner light is gathering strength. In 1972, Rabbi Yaakov Asher Sinclair opened SARM Studios the first 24-track recording studio in Europe where Queen mixed "Bohemian Rhapsody". His music publishing company, Druidcrest Music published the music for The Rocky Horror Picture Show (1973) and as a record producer, he co-produced the quadruple-platinum debut album by American band "Foreigner" (1976). American Top ten singles from this album included, "Feels Like The First Time", "Cold as Ice" and "Long, Long Way from Home". Other production work included "The Enid – In the Region of the Summer Stars", "The Curves", and "Nutz" as well as singles based on The Hitchhiker's Guide To The Galaxy with Douglas Adams and Richard O'Brien. Other artists who used SARM included: ABC, Alison Moyet, Art of Noise, Brian May, The Buggles, The Clash, Dina Carroll, Dollar, Flintlock, Frankie Goes To Hollywood, Grace Jones, It Bites, Malcolm McLaren, Nik Kershaw, Propaganda, Rush, Rik Mayall, Stephen Duffy, and Yes. In 1987, he settled in Jerusalem to immerse himself in the study of Torah. His two Torah books The Color of Heaven, on the weekly Torah portion, and Seasons of the Moon met with great critical acclaim. Seasons of the Moon, a unique fine-art black-and-white photography book combining poetry and Torah essays, has now sold out and is much sought as a collector's item fetching up to $250 for a mint copy. He is much in demand as an inspirational speaker both in Israel, Great Britain and the United States. He was Plenary Keynote Speaker at the Agudas Yisrael Convention, and Keynote Speaker at Project Inspire in 2018. Rabbi Sinclair lectures in Talmud and Jewish Philosophy at Ohr Somayach/Tannenbaum College of Judaic studies in Jerusalem and is a senior staff writer of the Torah internet publications Ohrnet and Torah Weekly. His articles have been published in The Jewish Observer, American Jewish Spirit, AJOP Newsletter, Zurich's Die Jüdische Zeitung, South African Jewish Report and many others. Rabbi Sinclair was born in London, and lives with his family in Jerusalem. He was educated at St. Anthony's Preparatory School in Hampstead, Clifton College, and Bristol University. Stoppard was never remotely religious, but his unwitting world view was so Jewish. He summed up in his immortal line which could have been a quote from our sages: "Every exit is an entrance somewhere else." In 1972, Rabbi Yaakov Asher Sinclair opened SARM Studios the first 24-track recording studio in Europe where Queen mixed "Bohemian Rhapsody". His music publishing company, Druidcrest Music published the music for The Rocky Horror Picture Show (1973) and as a record producer, he co-produced the quadruple-platinum debut album by American band "Foreigner" (1976). American Top ten singles from this album included, "Feels Like The First Time", "Cold as Ice" and "Long, Long Way from Home". Other production work included "The Enid – In the Region of the Summer Stars", "The Curves", and "Nutz" as well as singles based on The Hitchhiker's Guide To The Galaxy with Douglas Adams and Richard O'Brien. Other artists who used SARM included: ABC, Alison Moyet, Art of Noise, Brian May, The Buggles, The Clash, Dina Carroll, Dollar, Flintlock, Frankie Goes To Hollywood, Grace Jones, It Bites, Malcolm McLaren, Nik Kershaw, Propaganda, Rush, Rik Mayall, Stephen Duffy, and Yes. In 1987, he settled in Jerusalem to immerse himself in the study of Torah. His two Torah books The Color of Heaven, on the weekly Torah portion, and Seasons of the Moon met with great critical acclaim. Seasons of the Moon, a unique fine-art black-and-white photography book combining poetry and Torah essays, has now sold out and is much sought as a collector's item fetching up to $250 for a mint copy. He is much in demand as an inspirational speaker both in Israel, Great Britain and the United States. He was Plenary Keynote Speaker at the Agudas Yisrael Convention, and Keynote Speaker at Project Inspire in 2018. This week we dive into Solo Leveling Season 2. We start off a quick play-by-play, Jin-woo's Red Gate, Demon Castle grind, rising heat from the Association, Chae Hae-in catching on, and the Jeju Island shadow-army to round it off, then we argue adaptation choices. Rick's fine with trimming inner monologues and letting visuals carry the pace but Jack pushes back saying that tiny connective scenes like the post-Red Gate interviews, Jinho's dad's motives among other things were cut, muddying why guilds chase Jin-woo. We spar over what characters actually “know” in-anime, when Baek clocks Jin-woo's growth, and how a few 10 to 20 second inserts or stat card bumpers could've fixed a lot without bogging things down. About the anime: Jinwoo steps into the role of the Shadow Monarch while still posing as an E-rank hunter in public. The season follows his push through the Red Gate and the Demon Castle to secure the Elixir, his partnership with Yoo Jinho as they navigate guild requirements, and the growing scrutiny from the Hunters Association as his sudden leaps defy explanation. By 1985, Simple Minds were coming off a #1 album in the UK with Sparkle in the Rain and sales from all around the world. But it only went to #64 in the US and they needed something to spark greater interest across the pond. When Don't You Forget About Me from The Breakfast Club Soundtrack hit #1 in May 1985, a song they originally resisted, they had that spark and built on that success with memorable performance at Live Aid in Philadelphia. Their next album would need to be special to keep the momentum building. New York producer Jimmy Iovine (Bruce Springsteen, Stevie Nicks, Tom Petty) came to London to work with the boys as they welcomed in new bass player John Giblin. He encourage Jim to emote more - to really sing his heart out - in the studio and his soul is captured on Once Upon A Time. Jim's lyrics are optimistic about the future - the native Glaswegians know where they come from (Oh Jungleland) and can be proud of what they've accomplished (Come a Long Way). Alive and Kicking would be the first single to follow up #1 smash Don't You Forget About Me and soared up the charts, thanks in part to a music video shot in the Pocono Mountains - former Simple Minds drummer Mel Gaynor enlightens us with a funny story from that video shoot! They can get political without getting preachy (Ghostdancing) while bringing you to your feet with glorious salvation (Sanctify Yourself). Released October 21, 1985, Once Upon A Time would be the 2nd consecutive #1 in the UK and the 1st gold record for the band in the US. But what if they put Don't You Forget About Me on it? It was already a #1 and hadn't been on a Simple Minds album. The Wolf believes it would have resulted in millions of more sales and fans for the band in the US but that's a debate you'll have to hear!
