Dr. Monica Perez, the Scientific Chair at the Arms and Hands Lab at the Shirley Ryan AbilityLab and Professor of Physical Medicine and Rehabilitation at Northwestern University, is focused on developing drugs and strategies for treating those with spinal cord injuries. With no FDA-approved treatment, the current standard of care is surgery and physical rehabilitation, depending upon the location of the injury and cause. The AbilityLab is conducting a clinical trial with NVG-291, which targets the body's natural inhibitors for repair, regenerates neurons in the CNS, and aims to improve connections disrupted by spinal cord injury. Exercise is also a component of this treatment to improve outcomes. Monica explains, "The experiment we're running today at Shirley Ryan AbilityLab is the first of its kind study because we're using a medication that we have evidence in animal studies that actually can improve regeneration in the central nervous system and contribute at least to enhance recovery of function. This is NVG-291, a medication that targets the body's natural inhibitors for repair. And that, in theory, is supposed to improve connections disrupted by a spinal cord injury." "We are working with NervGen Pharma, a company that provides the medication. This is an injection. Our patients receive 84 injections during the trial. This is a randomized placebo-controlled trial, so they can receive either the real medication or a placebo medication, which is followed by exercise rehabilitation. So, it's pharmacology - the medication targets the body's natural inhibitors of repair, aiming to enhance the repair of connections that were affected by a spinal cord injury." #SRAbilityLab #SpinalCordInjury #NervGenPharma #CNS #CentralNervousSystem #Rehabilitation sralab.org Download the transcript here

Mike Kelly, CEO of NervGen Pharma, discusses the company's vision to build a startup biotech focused on repairing the central nervous system, helping patients regain function, not just slowing progression. Their first product, NVG-291, is currently in clinical trials for spinal cord injury. The drug aims to promote natural repair by targeting molecules called CSPGs that inhibit nerve growth and repair. NerveGen is also developing NVG-300 for other indications such as stroke, ALS, MS, and Alzheimer's. He draws parallels between his previous experience developing the Narcan Nasal Spray for opioid overdose and the unmet medical need in spinal cord injury. Mike explains, "We are a company focusing on a group of products, more like a platform, that will enable the nervous system to repair itself. So the vision is to build a startup biotech focused in CNS and to get our first product over the line in spinal cord injury." "There are no drugs available. So this is something that happens in an instant and someone's life has changed forever. The patient typically will then get hospitalized, have surgery to decompress the area that's injured, and then they're on the road to recovery. The only treatment is rehabilitation and exercise, and they push themselves to remake as many connections as possible. Still, that repair typically plateaus around six to nine months after their injury. And the level of function they regained during that period is where they'll be for the rest of their lives, unfortunately." "NVG-291 is a product that aims to promote natural repair. When you damage, say, you cut your finger or break your foot, your body repairs itself very easily. When you damage your central nervous system, your body protects itself very well and inhibits repair, actually. Our drug targets one of those mechanisms that inhibit repair, turns it off, and allows the body to repair itself naturally." #NervGenPharma #SCI #SpinalCordInjury #SpinalNews #SpinalInjury #Paralysis #NerveRepair #CNS NervGen.com Download the transcript here

Tonight I will be speaking to John. John writes “In 2017 we took our cadets out to an FTX (Field Training Exercise) near Tenino, WA. On the 1st night we heard what I now know to be tree knocks, which was curious but nothing unnerving, so we really paid no attention to it. But the 2nd night… around 0130 (1:30 am) we heard a LOUD, chilling, otherworldly yell, that got all the staff out of our tents at lightning speed and on immediate high alert. The length… of the howl / screaming growl; NOTHING I know of has a lung capacity that large. I'm a hunter & fisherman, so I spend a fair amount of time in nature (or at least, I used to) and I've never heard anything like that, ever, in my life. A sustained vocalization of approx 25-30 seconds in one breath. It was chilling. Almost paralyzing. What truly stood my hair on end, was some kind of infrasound reverb passing through my vital organs that accompanied the yell. Like a lion's huff. I Felt the sound waves ripple through my body, vibrating my guts. The voice was strange too. As if multiple sets of vocal cords were overlaid, activating all at once. Like 2 or 3 voices of different octaves and pitch, but altogether, wrapped up in a singular voice. I know that sounds insane, but it's difficult to describe. The 2nd vocalization had us back in our tents drawing our firearms… but for the first time, I had no confidence in my weapon against whatever made that sound. It was so intimidating. I felt thoroughly vulnerable, despite being armed. I mean Utterly helpless. We heard tree knocks again shortly after, and then another vocalization from the other side of the field in the opposite tree line and then tree knocks from that location as well. I don't know if there were two of them communicating across the field, or if it was the same one circling and repositioning around our perimeter. Whichever it was, it sounded aggressive. It certainly wasn't happy we were there. Like I said previously, we never saw it, because it was in the darkness beyond the glow of our bonfire in the center of camp. On the other hand, I hated being so exposed because we were at a tactical disadvantage. We were illuminated by the fire, so it could see us, but we couldn't see it. I wish I had brought NVG's. (I certainly will next time) A rock was thrown at our campfire where we, the staff were in befuddled conversation, about just what the hell to do. A few more rocks were thrown at us periodically but eventually it got quite. Obviously, we couldn't go back to sleep. We stayed up, and remained on watch until sunrise. We packed up and left the following morning.”
Today's conversation is a highly anticipated one. For years we've been hearing about the groundbreaking work of Jerry Silver to render the injury scar permeable to axon growth. That work led to development of the pharmacologic NVG-291 and the associated company NervGen Pharma. The research has finally advanced to a human SCI clinical trial, led by Monica Perez, PT, PhD, who's today's guest. Dr. Perez is the scientific chair of the Arm and Hands Lab at Shirley Ryan AbilityLab and a Professor of Physical Medicine and Rehabilitation at Northwestern University in Chicago. In this conversation we talk about what led Monica into SCI research, her role as Principal Investigator in this clinical trial, the background and study design of the trial and its current recruitment of participants.

Welcome to The Nonlinear Library, where we use Text-to-Speech software to convert the best writing from the Rationalist and EA communities into audio. This is: Ukraine Situation Report 2022/03/01, published by lsusr on March 2, 2022 on LessWrong. I love Russia, I love Russian and I love Russians. My favorite physics professor was a nuclear scientist from the Soviet Union. I studied Russian in college where a classmate gave me my first copy of Foreign Affairs Journal. Catherine the Great is #4 on my list of heroes. I don't know much about Ukraine or Ukrainians, but it is hard not to love them too after the swagger and humor they have exhibited since the Russian invasion. A week ago I wrote "The Russian Armed Forces is among the three most capable militaries in the world". Since then, I have been astonished by the incompetence of the Russian Armed Forces. Russian Equipment When I found out Ukraine issued 10,000 automatic rifles to civilians my first thoughts were "Those rifles could arm an insurgency but using them in conventional battle against Russian forces would be suicide". That's because there's more to fielding an effective soldier than giving a man or woman a gun. He or she needs to know what direction to point it. Here is an interesting comment thread between people who (I assume) are mostly American infantry. Having heard over the past 18-24 months about LSCO [Large Scale Combat Operations] and russia, I have to admit they have been exposed pretty badly. Also one of my friends noted during all of the combat footage we have not seen any night infantry combat, let alone any IR lasers. Now objectively I haven't seen every single piece of combat footage and presumably there is night combat. ―regularguyofthenorth I saw a report from Ukraine that said that Russian forces wait until dawn to attack because they don't have NVGs, nor the training on using them, not sure how accurate that is. ―IllustriusDot1866 Everything I've seen has said the same. No NODs, no optics, for anyone but leadership positions in infantry units. That's insane to me. I can't imagine life in a combat zone without NODs or optics. ―bang_the_drums That was something that stuck out to me too - even for the UA. I see maybe 1:20 with anything but an iron sight and a dream. ―Roastage I've seen an AS VAL [a Soviet assault rifle] and a VSS Vintorez [a silenced Soviet sniper rifle] as "combat trophies" so far, neither one of them had optics, which basically defeats the point of a suppressed gun shooting subsonic ammo that can penetrate armor. I also saw the VDV [Russian Airborne Forces] guys at the Hostomel airport, one of them had an AK-12 with no optic, no light, no laser, nothing. "Next gen" AK with ironsights. WTF? ―xyolikesdinosaurs Yep more or less the same for me. Expected for the Ukrainian Militia but surprising to see so few amongst the Russian infantry. There is a photo on combatfootage of some SF [Special Forces] guys who were fully kitted out with MCXs [gun] and all the jewellery. Unclear if they were FOG but there was more glass in that photo than I'd seen in all the footage to date. ―Roastage ―reddit thread "IR" stands for "infrared [nightvision]". "NVG" stands for "Night Vision Goggles". "NOD" stands for "Night Observation Device". These American troops are shocked to discover their Russian counterparts cannot see at night. My bicycle has better optics than Russia's paratroopers. American door-kickers in Iraq and Afghanistan operated at night because the best time to do battle is when you can see and the enemy can't. How can Russia's frontline troops be missing such basic equipment? Because Russia has a GDP the size of Florida's. Subtract oligarchic graft and what's left is Soviet hardware. When I found out the Ukrainians were defacing signs I thought "that's the right thing to do but I expect it won't do much because Russians can navigate via GPS instead. It appears unlikely that the majority of Russian military armored vehicles are using GPS, ...
Pv2 Caleb Smither was left to die in his barracks room on Ft Bragg only seven weeks and 4 days after reporting to base-his first duty station. After hitting his head on a vehicle in the motor pool. Pv2 Smither was turned away from Womack Army medical center 3 times. After being given a sedative and pain meds, he was put on quarters. Pv2 Caleb Smither died on the floor of his barracks room because no one checked on him. Womack failed this Soldier. His chain of command failed him. Who doesn't check on a brand spanking new troop who had to come to the formation with NVG's on because he was in so much pain? He lie in his room for 5 days before someone finally opened his door. Before that, Soldiers on his floor sprayed Febreeze around his door! But no one was held accountable. This is medical malpractice. This is negligent homicide.
Halloween episode, where Mr Frost ans Scotty Don't talk about what goes bump in the night.
This week, Matt Amos, Chet Sears and Troy Trussell cover the people stranded at the Kabul airport. Matt served in Afghanistan and provides us more insight than what we see in the news. Do the ideologies of the Taliban and the current Australian government have anything in common? Troy and Chet agree on the top sports movie of all-time. This is a Hardheaded Podcast first! Spoiler alert! Matt spends way too much time talking through the entire plot of a movie that he recommends we watch, Woodlawn (2015). "Amusing Ourselves to Death" Are we living frivolous lives and dying in tears and anxiousness? Are we in information overload? Do we spend too much time focused on everything going on and forget to actually do important things that will impact us and those around us? Chet poses these thoughts and leads us to Acts 17 where we find this isn't a new phenomenon brought on by 24 hour news! Topics discussed: What's On Your Mind: The Afghanistan debacle. Matt's experience with the Afghan people. Top 3: Hoosiers, Miracle, Remember the Titans, Karate Kid, Woodlawn, Friday Night Lights, Happy Gilmore, The Natural A Good Word: News. Ecc 1:9 "...there is nothing new under the sun." Acts 17:21 "Now all the Athenians and the foreigners who lived there would spend their time in nothing except telling or hearing something new."
