This podcast is about big ideas on how technology is making life better for people with vision loss. For hundreds of years, health professionals have dreamed of restoring vision for people who are blind or visually impaired. However, doing so, either through transplanting a functioning eye or using technological aids, is an incredibly complex challenge. In fact, many considered it impossible. But thanks to cutting-edge research and programs, the ability to restore vision is getting closer than ever. As a first for this podcast, this episode features an interview with Dr. Cal Roberts himself! Adapting audio from an interview on The Doctors Podcast, Dr. Cal describes his work as a program manager for a project on eye transplantation called Transplantation of Human Eye Allographs (THEA). Funded by a government initiative called ARPA-H, THEA is bringing some of the country's finest minds together to tackle the complexities of connecting a person's brain to an eye from a human donor. This episode also features an interview with Dr. Daniel Palanker of Stanford University. Dr. Palanker is working on technology that can artificially restore sight through prosthetic replacement of photoreceptors. Having proved successful in animals, Dr. Palanker and his team are working hard to translate it to humans. And if that can happen, then something once considered impossible could finally be accomplished! The Big Takeaways The Challenges of Eye Transplants: Although eyeball transplants have been done, they've only been cosmetic. So far, nobody has been able to successfully connect a donor eyeball to a recipient's brain. Dr. Roberts's work with THEA is bringing together multiple teams to tackle the challenges associated with a whole eyeball transplant, from connecting nerves and muscles to ensuring the organ isn't rejected, and much more. “Artificial” Vision Restoration: Dr. Palanker is working to replace the functions of photoreceptors through technological means. His photovoltaic array is placed underneath the retina and can convert light into an electrical current that activates the cells that send visual information to the brain. While it doesn't completely restore sight for people with Age-Related Macular Degeneration, this technology shows incredible promise. Decoding “Brain Language”: For both Dr. Roberts and Dr. Palanker, one of the biggest challenges with vision restoration is understanding how the eye and brain communicate. Dr. Roberts likens it to Morse Code — the eye speaks to the brain in “dots and dashes,” which the brain then converts into vision. Right now, the language is still foreign to us, but we're closer than ever to decoding it. The Evolution of the Brain-Machine Interface: Dr. Palanker imagines incredible possibilities in the interaction between the brain and technology. If we can find a way to truly translate the brain's signals into information, Dr. Palanker envisions the possibility of direct brain-to-brain communication without verbalization. In a way, this could make people telepathic, able to understand and digest vast amounts of information in an instant. Tweetables: So ideally in medicine, at least the ideal therapy is the restoration of full functionality. If we can grow back photoreceptors and make them reconnect to bipolar cells, undo all the rewiring that right now underwent during degeneration, and restore the full extent of vision, that would be the ideal outcome. — Dr. Daniel Palanker, Professor of Ophthalmology, Stanford University We can think about other aspects of brain-machine interface, which takes you maybe into the realm of capabilities that humans never had. All that work, all that research, and all that effort to come up with mechanical heart transplants are still state-of-the-art. Abstract: Photovoltaic subretinal prosthesis (PRIMA) enables restoration of sight via electrical stimulation of the interneurons in degenerated retina, with resolution limited by the 100 um pixel size. Since decreasing the pixel size below 75 um in the current bipolar geometry is impossible, we explore the possibility of using smaller pixels based on a novel 3-dimensional honeycomb-shaped design. We assessed long-term biocompatibility and stability of these arrays in rats by investigating the anatomical integration of the retina with flat and 3D implants and response to electrical stimulation over lifetime - up to 9 months post-implantation in aged rats. With both flat and 3D implants, VEP amplitude decreased after the day of implantation by more than 3-fold, and gradually recovered over about 3 months. With 25 um high honeycomb walls, the majority of bipolar cells migrate into the wells, while amacrine and ganglion cells remain above the cavities, which is essential for selective network-mediated stimulation of the second-order neurons. Retinal thickness and full-field stimulation threshold with 40 um-wide honeycomb pixels were comparable to those with planar devices - 0.05 mW/mm2 with 10ms pulses. However, fewer cells from the inner nuclear layer migrated into the 20 um-wide wells, and stimulation threshold increased over 5 months, before stabilizing at about 0.08 mW/mm2. Such threshold is significantly lower than 1.8 mW/mm2 with previous flat bipolar pixels, confirming the promise of the 3D honeycomb-based approach to high resolution subretinal prosthesis. Abstract: In patients blinded by geographic atrophy, subretinal photovoltaic implant with 100 um pixels provided visual acuity closely matching the pixel pitch. However, such flat bipolar pixels cannot be scaled below 75 um, limiting the attainable visual acuity. This limitation can be overcome by shaping the electric field with 3-dimensional electrodes. In particular, elevating the return electrode on top of honeycomb-shaped vertical walls surrounding each pixel extends the electric field vertically and decouples its penetration into tissue from the pixel width. This approach relies on migration of the retinal cells into the honeycomb wells. Here, we demonstrate that the majority of the inner retinal neurons migrate into 25 um deep wells, leaving the third-order neurons, such as amacrine and ganglion cells, outside. This is important for selective stimulation of the second-order neurons to preserve the retinal signal processing in prosthetic vision. Comparable glial response to that with flat implants suggests that migration and separation of the retinal cells by the walls does not cause additional stress. Furthermore, retinal migration into the honeycombs does not negatively affect its electrical excitability. Copy rights belong to original authors. This Veterans' Day Special episode features United States Military Veteran, CEO of eZara, Inc and Veterans Mental Health Advocate Pasha Palanker. Pasha talks about: The uptick in suicides during the pandemic and Pasha checking himself into Walter Reed Medical Center for help. Emigrating from the Republic of Moldova at 16 to Los Angeles, CA, living in a rough area of the city and trying to buy a Big Mac with Food stamps. Coming face-to-face with a Suicide bomber during a deployment to Baghdad, Iraq and seeing horror on a daily basis. Standing over an IED Explosive and surviving not only the blast with all body parts intact, his unit thinking he was killed in action, but also coming to himself and walking away from it. Seeing US soil for the first time returning home after injury and being a walking miracle. Not thinking he would be impacted by Post Traumatic Stress Disorder (PTSD) and Traumatic Brain Injury (TBI), but underestimating the true extent of his injuries. Getting hurt again in 2015 on another deployment to Iraq and having a Panic attack after coming back home from the deployment. The internal conflict of being deployed in a mission but feeling emotionally overwhelmed. Having so much pain and feeling dangerously numb internally. Abstract: Objective: Patients with the photovoltaic subretinal implant PRIMA demonstrated letter acuity by ~0.1 logMAR worse than the sampling limit for 100m pixels (1.3 logMAR) and performed slower than healthy subjects, which exceeded the sampling limit at equivalently pixelated images by ~0.2 logMAR. To explore the underlying differences between the natural and prosthetic vision, we compare the fidelity of the retinal response to visual and subretinal electrical stimulation through single-cell modeling and ensemble decoding. Approach: Responses of the retinal ganglion cells (RGC) to optical or electrical (1mm diameter arrays, 75m pixels) white noise stimulation in healthy and degenerate rat retinas were recorded via MEA. Each RGC was fit with linear-non-linear (LN) and convolutional neural network (CNN) models. To characterize RGC noise level, we compared statistics of the spike-triggered average (STA) in RGCs responding to electrical or visual stimulation of healthy and degenerate retinas. At the population level, we constructed a linear decoder to determine the certainty with which the ensemble of RGCs can support the N-way discrimination tasks. Main results: Although LN and CNN models can match the natural visual responses pretty well (correlation ~0.6), they fit significantly worse to spike timings elicited by electrical stimulation of the healthy retina (correlation ~0.15). In the degenerate retina, response to electrical stimulation is equally bad. The signal-to-noise ratio of electrical STAs in degenerate retinas matched that of the natural responses when 78{+/-}6.5% of the spikes were replaced with random timing. However, the noise in RGC responses contributed minimally to errors in the ensemble decoding. The determining factor in accuracy of decoding was the number of responding cells. When Pasha Palanker was 15, he and his family immigrated to the U.S. from Moldova. Although his family lived in a rough neighborhood on a very low income, Pasha was happy and proud to be living in America, whose opportunities far outnumbered those of his birthplace. In an effort to repay the U.S. government for the assistance provided to him and his family after they arrived from Moldova, Pasha enlisted in the Army, shortly after the war in Iraq kicked off in 2003. Following multiple deployments to Iraq, Pasha had been blown up more than once, and suffered multiple Traumatic Brain Injuries. His experiences left him with the lasting effects of PTSD. In this episode he talks about his journey - how he survived multiple blasts and came face-to-face with a suicide bomber, and how he is now working through the issues caused by his injuries.
