In this episode of The Mentors Radio, Host Tom Loarie talks with Ed Kopetsky, retired CIO for Stanford Health, to discuss the critical issue of addiction, a major career and family derailer today. Ed, who lost his son to an opioid overdose, shares 17 years of frontline experience, sharing invaluable insights on addressing addiction within families and communities. There is no one I know who is better to provide guidance on dealing with addiction within a family. He has been on the front lines of the addiction battle for 17 years. He lost his 31 year old son, Tim, to an overdose in 2017. Before he retired, he served as the Chief Information Officer at Stanford Health's Lucile Packard Children's Hospital and Stanford's Children's Health. He has served as the Board Chair for the College of Healthcare Information Management Executives (CHIME) and he is the founder and serves today as the Co-Chair of the CHIME Opiod Task Force. He was awarded the John Gall “CIO of the Year Award” awarded by CHIME and HIMSS. CHIME CEO Russell Branzell has called Ed “one of the most courageous people I know.” Kopetsky is now focused on mentoring families whose lives have been turned upside down with drug addiction. Dr. Craven is a licensed health psychologist and she's the Director of GI Health Psychology in the Division of Gastroenterology and Hepatology at Stanford Health. We spent quite a bit of time talking about gut directed hypnotherapy and other brain-gut behavioral health techniques. We also had a really interesting discussion about Dr. Craven's research and work on health disparities and diversity, equity, inclusion, and justice. We also talked about her interest in and practice of yoga and how she's helped her patients use it as well as diaphragmatic breathing and mindfulness to manage their IBD. As part of a CancerNetwork® Frontline Forum program, Alexander I. Spira, MD, PhD, FACP; Millie Das, MD; Joshua K. Sabari, MD; and Misako Nagasaka, MD, PhD, reviewed updates in the management of EGFR exon 20 non–small cell lung cancer (NSCLC), and how these findings may apply to clinical practice.  Spira, codirector of the Virginia Cancer Specialists Research Institute and director of the Thoracic and Phase I Program, and Das, a clinical associate professor of Medicine and Oncology at Stanford Health, led one part of the discussion regarding the evolution of molecular profiling and next-generation sequencing in the NSCLC space. Moreover, they spoke to clinical and non-clinical factors informing treatment decision-making in the second-line setting. Additionally, they reviewed data from the phase 1 CHRYSALIS trial (NCT02609776) assessing amivantamab as a treatment for patients with disease progression following chemotherapy. According to data from the CHRYSALIS trial, amivantamab elicited an objective response rate of 40% (95% CI, 29%-51%), and a median duration of response of 11.1 months (95% CI, 6.9-not reached). Additionally, the clinical benefit rate was 74% (95% CI, 63%-83%). Common any-grade adverse effects (AEs) in the study's safety population that were associated with EGFR inhibition included rash (86%), paronychia (45%), stomatitis (21%), pruritus (17%), and diarrhea (12%). Sabari, a thoracic medical oncologist at Perlmutter Cancer Center of NYU Langone Health, and Nagasaka, a thoracic oncologist at the University of California, Irvine, also discussed clinical updates in the EGFR exon 20 NSCLC population. They spoke about potential novel treatment options including mobocertinib (Exkivity), as well as potential next steps in research such as moving drugs to earlier lines of treatment and obtaining a better understanding of mechanisms of acquired resistance to treatment. According to findings from a phase 1/2 trial (NCT02716116), mobocertinib produced an ORR of 28% (95% CI, 20%-37%) by independent-review committee assessment and 35% (95% CI, 26%-45%) per investigator assessment in a cohort of platinum-pretreated patients with EGFR exon 20 insertion mutations. Tom and David shift from operations to health care finance and contemplate how things may change in the future.   Guest speaker: David Entwistle President and CEO Stanford Health Care   Moderator: Tom Robertson Executive Director Vizient Research Institute   Show Notes: [01:16] Group practices with a collection of specialists that provide a one stop shop that considers the whole patient [02:09] Group specialty practices help you to remain competitive, manage care and costs [03:44] One of the biggest cost drivers is the variation of care   [04:24] The ability to reduce the avoidable variation will help reduce costs [04:37] Creating transparency of the variation data is monumental. Show physicians how they treated the same patient with the same diagnosis [05:15] Have to look at large data sets to show a true variation difference that's controllable to create a better-quality experience and lower costs [05:32] To make movement in variation, you can't just go after the costs. You have to show it's a better outcome and start with quality or it won't be successful. [06:47] To make the change, you have to ask how do you create the best outcome? Once you apply the data to the outcome, then the costs come down. [07:24] Scenario: if all-payers pay the same. How would you think differently – operationally and strategically? [08:42] All patients are treated equally. It wouldn't change anything operationally. [09:30] Strategically: Think how much we spend to be attractive to commercial insurance companies that are funding the private to public payer subsidy, there are nuances that collectively make a difference. [10:00] Margins produced in academics are different because they are funding research and teaching. DR. CYNTHIA JOVANOV - President Elect CANP and President Inland Empire Chapter (2018 - 2020) Dr. Jovanov is a practicing APRN with a vital skill set in the family and acute care setting. Her career began at Riverside Community College as an ADN in 2001. She is a first-generation college graduate from parents that migrated from Serbia and Central America. She obtained her Master's degree from UCLA in the Acute Care NP / CNS program, and shortly after, she received her FNP. She found that as an NP that business acumen was essential and went back for her MBA with a focus in finance. She completed her DNP from Duke University in 2019 with a focus on health literacy. She has worked in various institutions from small community settings to large academic institutions, including UC San Francisco and Stanford Health. Her specialty is Cardiac Surgery and Critical Care. She is a pioneer in the Inland Empire, becoming one of the first and few NP First assist in the operating room from Cardiac Surgery to assisting in delivering babies by C- section while billing independently. She wrote protocols and guidelines for local hospitals regarding the NP scope of practice. She mentors other NPs that are interested in her role. She teaches as an Adjunct Professor at California Baptist University. She advocates that APRNs are always aware of their contribution to the healthcare organization. She feels that every APRN should be informed and engaged in their practice; otherwise, other people make decisions for you. Dr. Jovanov feels it is imperative that you know your brand because it is your business. In this podcast, Dr. Jovanov shared her nursing career and how it evolved from being a Nurse Practitioner, Clinical Nurse Specialist, and First Assist. As a Nurse Practitioner Educator, Leader, and Advocate, she spoke on why Nurse Practitioners need to participate in policy-making. She has a strong passion for clinical, and she passes her love of clinical and critical thinking onto her graduate students. Resurgence In CaliforniaCalifornia is experiencing a new resurgence of COVID-19 in their hospitals.More people are on ventilators and in the ICU than ever before.A main contributor to this resurgence is that not enough people are sticking to the mask and social distance guidelines. Another obstacle for California is the number of counties within the state.Counties started to open up on a regional basis.So, you can be in one county and go to the gym and restaurants, but in the next county over, it is completely closed down.Lastly, people are starting to experience cabin fever and are beginning to travel freely and frequently to open counties.The Latest on COVID-19 TestingStanford has created a centralized testing system that allows their operations to run smoothly.They also opened a new hospital to combat COVID-19, and turned their ED into a testing site as well.Stanford is now able to test over 20,000 people per day and get results back between two hours and two days – which is significantly faster than the national average. Though Stanford has international patients, they have focused a lot of their marketing communications efforts on local and regional markets.Stanford has focused on being a credible source for their local are.Their goal is that the community can come to them for information that they know they can trust. Another standard that Stanford keeps is constantly testing employees as a part of taking all the necessary precautions to keep both patients and employees safe.That has been one of the most important measures taken in the pandemic.
