Sme krajinou, kde sa umierania musíme báť, lebo na to nemáme, hovorí Simona Stískalová z Asociácie hospicovej a paliatívnej starostlivosti. Musíme prestať zatvárať oči pred realitou a naservírovať si pravdu, dodáva. Asociácia sa pridala k protestu voči tzv. ruskému zákonu. Ten totiž mieri i voči tým, ktorí sa o nás starajú na poslednej ceste do večnosti."Príde smrť a bude mať Tvoje oči," napísal svojho času taliansky básnik Cesare Pavese. Áno, smrť príde po každého z nás. No a keď to napokon príde, ide i o to, či bude v jej očiach pochopenie, súcit a aspoň kúsok dôstojnosti, s ktorou môžeme odísť. Na Slovensku je to však až príliš často nedosiahnuteľná ilúzia.O smrti najradšej mlčíme a umieranie je u nás neraz sprevádané nedôstojnosťou a zápasom o aspoň akú takú ľudskosť - neraz pritom vykúpenú poslednými úsporami či dokonca kostolnými zbierkami. Žijeme tak v krajine, kde sa ešte stále bojíme umierania - nie však pre jeho existenciálnu výzvu, ale pre hlboký nezáujem tohto štátu o ľudí i na tej ich naozaj poslednej ceste. Táto krajina tak zrejme pre umierajúcich nie je. V paráfráze na známy film bratov Coenovcov, sa tak natíska otázka: Pre koho je potom táto krajina, ak nie je ani pre tých, ktorí umierajú? "Verím, že keď si vstúpime do svedomia a zosobníme si problémy, ktoré sú tu naozaj problémami - a dobré a zlé mimovládky to nie sú, ale to s čím sa tu dennodenne stýkame - a smrť tam patrí, tak to sú tie problémy, s ktorými sa musíme ako spoločnosť zaoberať a len potom vieme spoločne žiť v takej krajine, kde sa nebudeme báť umierať - lebo si povieme, že na to nemáme," hovorí Stískalová z AHPS. O strastiach umierania na Slovensku vie svoje aj Asociácia hospicovej a paliatívnej starostlivosti. Tá koncom minulého roka takmer skrachovala a kvôli podfinancovaniu sa dokonca ako "charita" sama uchádzala o charitu nás všetkých."Hospice prežili vďaka darom, ale tá najväčšia čast bola, že sa samotní zamestnanci vzdávali svojich výplat v prospech hospicov," pripoína Simona Stískalová Ani dnes to hospice nemajú vôbec vyhraté no na Asociáciu sa - ako na mimovládku, už valí hnev vládnej moci, ktorá sa rozhodla Tretiemu sektoru vyhlásiť studenú vojnu. Tú vojnu pritom vedie práve s tými, ktorí tak často suplujú úlohy štátu v oblasti sociálnej, vzdelávacej či v oblasti zdravotníckej. Asociácia hospcovej a paliatívnej starostlivosti preto proti tzv. Amongst the weeds, the outline of a long-forgotten cabin and some greenhouse shaped objects could be seen in the distance. It was obvious the place was once loved, but those times had clearly passed. Vicky wiped the cobweb encrusted sign near the gates – sun faded and tired, it read – Severn Enterprise. “Oh, that's the old sheltered workshop that used to be here – it closed years ago, shame really, it gave people something purposeful and productive to do”. Vicky looked around the site that had obviously offered so much to so many and was crying out to fulfil that role once again. “Do you think we can make this something special again?” Vicky asked. “What have you got in mind?” exclaimed Robert Join Victoria Woodruff the Engagement, Activity and Physical Health Team (EAP) Team Lead/ Occupational Therapist and Robert Young, EAP Practitioner, from the Montpellier Unit – Wotton Lawn Hospital, Gloucestershire Health and Care NHS FT as they tell the story of an ever-expanding group of people who have collaboratively re-developed a therapeutic allotment resource to aid the recovery journeys of service users. On this episode of the podcast I'm in conversation with Laura Neil. Laura is Lead for Allied Health Professions and Interim Head of Clinical Governance and Quality Improvement in NHS 24. Laura is an occupational therapist. Although always interested in service development she first became involved more formally in quality improvement whilst working at UCLPartners, an Academic Health Science Network in London, initially as Frailty Fellow and then as programme manager, completing the Sheffield Microsystem Coaching Academy programme during this time. She then worked at North East London NHS Foundation trust as Director of Quality Improvement and Director of AHPs and Psychological Therapies. This role included establishing a new Quality Improvement Programme in the Trust; over the five years she was in post this grew to having delivered QI awareness sessions to over 5000 staff, training over 300 QI facilitators and 30 QI mentors. Whilst in this role she also completed the IHI Improvement Advisor programme, was Clinical Lead in the Incident Management Team from March 2020-Jan 2021 and AHP Director in the second iteration of the London Nightingale from Dec 2020-March 2021. In the first Unplugged episode on chief nursing information officer (CNIO) mentoring, Jordan Sollof chats to Amy Vercell, chief clinical information officer for nursing and AHPs at The Christie NHS Foundation Trust, and Peter White, CNIO at Alder Hey Children's Hospital. The pair discuss the importance of mentorship in the nursing profession, particularly in senior leadership roles such as CNIO and CCIO, and how nurses go about finding the right mentor for them. The discussion then moves onto how CCIOs and CNIOs can balance responsibilities of mentoring whilst managing their own professional development and the common misconceptions they face when it comes to mentoring. Finally, Amy and Peter give their thoughts on whether there are enough mentorship programmes and clear pathways of support for nurses, before sharing some success stories and examples of how mentoring has helped them or other mentees in their career.

Take our listener survey to receive our new CSR Whitepaper.Two U.S. states have already enacted laws requiring disposable hygiene packaging to list ingredients. Other states are considering the same. These policies, which mandate the disclosure of intentionally added ingredients, empower consumers in understanding what is in their favourite menstrual products. But is this level of transparency good news for manufacturers? In this episode of ‘Attached to Hygiene', host Jack Hughes explores these questions with Jane Wishneff, Executive Director for The Center for Baby and Adult Hygiene Products (BAHP). Together, they discuss emerging mandates and their current impact. Wishneff also shares what she expects to see in the future for absorbent hygiene products in the United States.Could 50 States Have 50 Conflicting Policies on AHPs?The absorbent hygiene product regulations in New York and California are rather different. The New York law, in effect now, is fairly brief. California's law, which begins January 2023, is quite detailed. They also vary in requirements and other important areas. Manufacturers are working to find solutions that will satisfy both sets of directives. Imagine the complexity the industry will face if all 50 U.S. states pass conflicting laws regarding transparency and sustainability. They may also disagree on where and how the information needs to be provided. Specific packaging for each state would be unworkable. It would also be a disservice to the consumers who are looking to the labels for information. So far, the FDA (America's Food and Drug Administration) is gathering data but has not enacted any regulations. This has led several industry players to become proactive in offering feasible, transparent solutions.The European Union (E.U.) has already established regulations and guidelines regarding disposable hygiene products. This brings pressure on America to do likewise. But it also allows lawmakers and manufacturers to learn what works, and what could—or should—be done differently in the U.S. Lastly, American laws can, in a way, borrow the E.U.'s expertise. For example, the California law states that companies cannot claim as a confidential ingredient anything that the E.U. has designated a chemical of concern. The COVID-19 pandemic has seen many health and social care services overwhelmed which has led to increased and sustained pressure and stress for health and social care staff. Nurses, midwives, AHPs, social care workers and social workers are on the frontline and have needed to respond to the increased need for reassurance and support for patients, women and their families while dealing with the additional challenge of wearing PPE and workload pressures. Some staff has been redeployed and all have had to quickly adapt to new ways of working including facilitating and managing visiting restrictions. Gov. Roy Cooper recently signed into law Senate Bill 584. It marks the latest step in an ongoing campaign to fight overcriminalization in North Carolina. Mike Schietzelt, John Locke Foundation criminal justice fellow, explains how the new legislation fits with the goal of cleaning up the state’s overly complicated criminal code. Overly burdensome occupational licensing rules restrict economic freedom in both North Carolina and South Carolina. During a recent meeting in Winston-Salem, the group Classical Liberals in the Carolinas learned how. Jon Sanders, John Locke Foundation director of regulatory studies, focused attention on North Carolina’s licensing restrictions. Jennifer McDonald, senior research analyst at the Institute for Justice, offered details about South Carolina’s rules. U.S. Sen. Thom Tillis, R-N.C., wants to do more to speed up the pace of government hurricane relief. Before Hurricane Dorian approached the N.C. coast, Tillis returned to Raleigh to discuss a bill that could help local governments bypass some layers of red tape in securing federal relief funding. Some state lawmakers want public schools to place more emphasis on phonics in early reading instruction. Rep. Larry Pittman, R-Cabarrus, tried to amend a recent education bill to mandate phonics instruction in the earliest elementary school grades. You’ll hear highlights from N.C. House debate of Pittman’s proposal. Without Cooper’s signature, the Small Business Health Care Act recently became law in North Carolina. It opens the door to Association Health Plans for small business owners and their employees. Jordan Roberts, John Locke Foundation health care policy analyst, assesses the significance of AHPs and their role in health care reform.

Recently, a federal judge ruled that parts of the Trump administration’s 2018 final rule on association health plans were invalid. The court then directed the Department of Labor to reconsider how the remaining provisions of the final rule are affected. The court called the final rule an “end-run” around the Affordable Care Act and said that the DOL exceeded its authority under ERISA. And then just last week the DOL responded in disagreement with the court’s ruling and the DOL filed a notice of appeal. North Carolina’s ongoing congressional redistricting dispute returned to the U.S. Supreme Court recently. As justices decide whether Republicans engaged in too much partisanship when they drew election maps, a case challenging state legislative district maps is proceeding to trial in state court later this year. Rick Henderson, Carolina Journal editor-in-chief, analyzes the latest developments in both sets of legal challenges. School choice has boomed in North Carolina in recent years. Advocates hope to build on choice options in the coming year. Mike Long, president of Parents for Educational Freedom in North Carolina, shares his group’s top priorities. Some N.C. lawmakers hope to improve the state’s health care options through new legislation. It focuses on association health plans. AHPs would allow more small employers and self-employed workers to band together for insurance. They could seek insurance options that have been available only to large companies in recent years. During a recent Capitol Hill hearing on crumbling school buildings across the country, U.S. Rep. Virginia Foxx, R-5th District, highlighted another pressing issue. Foxx reminded her colleagues that education spending has increased substantially in recent years. But she says much of that increased spending has funded administrative bloat instead of teacher salaries and other high-priority items. North Carolina has one of the nation’s best laws limiting civil asset forfeiture abuse. But the state still could be open to problems if local law enforcement agencies circumvent that law when working with the federal government. Jon Guze, John Locke Foundation director of legal studies, discusses how other states have addressed the circumvention problem.

Rachael graduated from the University of Hertfordshire and after spending some time working for the British Army and London Hospitals settled at the Newcastle Upon Tyne NHS Foundation Trust in 2001. Rachael specialised in respiratory physiotherapy initially within cardiothoracic transplantation before moving into a rotational ICU Band 7 post. She developed a specialist interest in neuro-trauma and this role evolved to become the senior physiotherapy link within the North East long term ventilation team. More recently Rachael was Respiratory Lead at St Georges Hospital, London managing a diverse team and specialities to now working in a new Consultant Physiotherapy post at Royal Preston Hospital. Rachael’s area of expertise include complex ventilation and weaning and advanced airway clearance techniques for which she lectures and presents both in the UK and internationally and at pre and post graduate level. Rachael currently sits on BTS Council and the Critical Care Specialist Advisory Group representing AHPs, is an expert member of NHSE Patient Safety Group, AHP representative on the NIV NCEPOD study, Co-chair HMV-UK, Chair Respiratory Leaders in Physiotherapy UK, Board Member and Trustee St Catherines Hospice and most recently elected onto Physiotherapy Council.

The U.S. economy has posted impressive gains recently. Both President Trump and former President Barack Obama are claiming credit. Roy Cordato, the John Locke Foundation’s senior economist, puts the competing claims to the test. A Winston-Salem surgeon is taking North Carolina state government to court because of a law that blocks him from purchasing an MRI scanner. Dr. Gajendra Singh says the scanner would help him provide a valuable service to his patients at a reasonable cost. North Carolina’s certificate-of-need law blocks Singh from making the purchase. Singh and his attorney, Josh Windham of the Institute for Justice, explain why they’re challenging the CON law. One likely consequence of the 2018 elections is a renewed push for redistricting reform in North Carolina. John Locke Foundation Chairman John Hood offered that prediction during a recent post-election analysis. Hood says Republican legislative leaders looking ahead to 2020 elections might want to rethink their opposition to reform. Higher education faces significant challenges in North Carolina and across the United States. Jenna Robinson, president of the James G. Martin Center for Academic Renewal, highlighted key challenges during a recent speech in Raleigh. Robinson emphasized the lack of viewpoint diversity on college campuses, along with an overall decline in academic quality. As state and national politicians continue to debate the future of health care, the recent rise of Association Health Plans is offering a new option for many health care consumers. According to commenters, certain populations with specific needs, such as those with disabilities, could be disproportionately affected if their coverage does not include a robust level of benefits. Some of these commenters suggested that to mitigate these effects, the DOL should require AHPs to provide EHBs or some other minimum level of benefits, or require them to provide “minimum value” within the meaning of Code Sec. 36B(c)(2)(C)(ii) and Reg. §1.36B-6. Proponents of the rule, however, while acknowledging concerns that AHPs may provide inadequate benefits, did not believe that “legitimate” membership organizations would risk their goodwill and reputation by offering such health plans. Instead, they argued that economies of scale would enable AHPs to offer more comprehensive coverage to their members than they would be able to purchase on their own. One commenter noted that even though self-insured plans and large group market policies are not required to provide EHBs, most do, in fact, provide comprehensive coverage. The DOL decided not to make the provision of EHBs in an AHP a condition for a group or association to qualify as bona fide. Such a mandate would run contrary to the goal of leveling the playing field between small employers in AHPs, on the one hand, and large employers, on the other, who generally are not subject to the EHB requirements, according to the DOL. Moreover, such a mandate could reduce AHPs’ flexibility to tailor coverage to the particular needs of the members of the group or association offering the benefits, and thereby reduce access to AHPs by making them less attractive options. Thus, the DOL also decided that the final rule would not require the provision of “minimum value” coverage as a condition for a group or association to qualify as bona fide. The ability to design AHP benefit packages and set cost-sharing requirements without the burden of certain federal restrictions is critical to enabling AHPs to provide an additional, more affordable coverage option to small businesses and working owners who may otherwise have been unable or unwilling to obtain higher-priced coverage, the DOL said. The department also believes that concerns about adverse selection as result of AHPs not providing comprehensive coverage are “overstated.” The DOL sided with those commenters who asserted that AHPs are not likely to offer relatively low levels and scope of benefits, which could jeopardize their relationship with their members. Other protections that would apply. The DOL also said that other federal and state coverage requirements mayapply to AHPs, say for example, that AHPs must provide coverage for certain recommended preventive services without the imposition of cost-sharing. These services include: Evidence-based items or services that have in effect a rating of A or B in the current recommendations of the United States Preventive Services Task Force (Task Force) with respect to the individual involved; Immunizations for routine use in children, adolescents, and adults that have in effect a recommendation from the Advisory Committee on Immunization Practices of the Centers for Disease Control and Prevention (Advisory Committee) with respect to the individual involved. A recommendation of the Advisory Committee is considered to be “in effect” after it has been adopted by the Director of the Centers for Disease Control and Prevention. A recommendation is considered to be for routine use if it appears on the Immunization Schedules of the Centers for Disease Control and Prevention; With respect to infants, children, and adolescents, evidence-informed preventive care and screenings provided for in the comprehensive guidelines supported by the Health Resources and Services Administration (HRSA); and With respect to women, evidence-informed preventive care and screening provided for in comprehensive guidelines...
