Radiation cystitis! Now this can be a really challenging condition for urologists, and especially for patients, to manage and to live with. And we frequently struggle to get good results. It is often years and years after radiation to the pelvis when patients return with problems like bleeding, pain, incontinence, fistula etc, which can have everything from minor, to ruinous effects on quality of life. And although these are radiation oncology patients, it is the urologist (and sometimes other surgeons) who have to manage these unfortunate side-effects of radiation.Today we travel to London to chat with two Urologists at Guy's Hospital who have set up a dedicated clinic to manage Radiation Cystitis patients from across the UK. Dr Ramesh Thurairaja and Dr Yasmin Abu Ghanem, explain why they set up this service, and share with us their strategy for assessing and managing radiation cystitis. This episode is full of useful advice for anyone out there managing radiation cystitis, and hopefully might inspire more people to set up similar clinics to look after these unfortunate patients. Cystitis is a condition in which the bladder becomes inflamed and irritated. While there are many physical causes of this condition, recent research has also shown that emotional factors and psychological issues can also contribute to cystitis symptoms. Psychosomatics of cystitis involves looking at how our minds and emotions interact with our bodies and how the condition can be both a result and cause of psychological distress. Hysterectomy: A surgical procedure in which the uterus is removed, either partially or completely. Etymology: From the Greek roots “hystera” (meaning “uterus”) and “tomy” (meaning “to cut”). 3. The initial evaluation of a pelvic pain or cystitis patient must address the patient's detailed symptoms and pain. On the first visit, Dr. Santiago-Lastra emphasizes the importance of discussing the frequency of symptoms, past treatments sought by the patient, and qualitative descriptions of the pain. She notes that some urologists will forget to consider genitourinary syndrome of menopause (GSM) as a cause for recurrent UTIs. Then, she outlines her 5-step pelvic examination procedure: vulvovaginal examination, urethral examination, classic pelvic examination, vaginal/cervix examination, and anal examination. If she finds something abnormal during the pelvic exam, she will use a hand mirror to show patients the anatomical location of their pain. Next, Dr. Santiago-Lastra discusses the kinds of medical treatment for patients presenting with recurrent UTIs and consistently positive urine cultures. She prefers to prescribe vaginal estrogen over long-term antibiotics, but acknowledges that some patients, such as premenopausal breast cancer patients, may refuse vaginal estrogen. In these cases, she recommends Refresh cream, methenamine, prophylactic post-coital/nightly antibiotics, and sometimes intravesical gentamicin instillation. Aside from medical treatments, Dr. Santiago-Lastra also recommends pelvic floor therapy and sometimes additional holistic treatment, as pelvic pain may originate from sexual trauma. Dr. Santiago-Lastra then discusses different options for treating pelvic pain and cystitis. For her, opioids play an extremely limited role for pelvic and bladder pain. She typically uses injections (nerve blocks), neuromodulation, pyridium, vaginal diazepam, vaginal lidocaine, gabapentin, and vaginal/systemic cannabis to treat pelvic and bladder pain. She does not usually prescribe NSAIDS because of their adverse effects from long-term use. For patients with confirmed localized bladder pain, she notes that IC cocktail (instillations) can provide some pain relief. In the rare case that all medical options have been exhausted, urinary diversion, an open surgery that removes the bladder completely, is a possible option. Finally, Dr. Santiago-Lastra and Dr. Silva discuss Botox and InterStim (sacral neuromodulation), two new treatments for patients who have both pelvic pain and incontinence/urgency symptoms. Although Botox and InterStim are equivalent treatments, there are certain indications for each treatment. For instance, InterStim is recommended for patients with voiding dysfunction and severe bowel symptoms because Botox only directs its efficacy to the bladder. To conclude, Dr. Santiago-Lastra emphasizes the importance of taking time to listen to pelvic pain and cystitis patients' concerns and desires, as they commonly become long-term patients.

Interstitial cystitis or painful bladder syndrome has been dubbed the evil twin of endometriosis, and often goes undiagnosed because the pain from the two in the pelvic can sometimes overlap and blur. I only realised that the bladder issues I was experiencing weren’t endo related when my endo symptoms resolved after I changed my diet. Over the years, my bladder pain has only worsened and has now evolved to that point where I experience pain pretty much 247. Histamines and oxalates can sometimes be behind bladder pain, and so this led me down a path of trying an elimination diet under the guidance of a nutrition at the start of this year. I started sharing my story on IG, but as the story unfolded, I decided it would be better to go quiet on the matter and share it once I was on the other side. So finally - I’m ready to talk! In today’s episode I discuss - The symptoms I was experiencing which motivated me to seek further help and try an elimination diet.
