In this episode, we go through the pathophysiology of hydrogen sulfide (H₂S)-dominant small intestinal bacterial overgrowth (SIBO), focusing on its role in intestinal motility disruption, microbial dysbiosis, and epithelial barrier dysfunction. We detail the interplay between the enteric nervous system (ENS), interstitial cells of Cajal (ICCs), and migrating motor complex (MMC) in regulating small intestinal transit and how excess H₂S modulates smooth muscle activity, inflammatory signaling, and gastrointestinal transit time. Lastly, we detail symptoms and related SIBO subtypes.Topics:1. Introduction to Hydrogen Sulfide Dominant SIBO     - Overview of hydrogen sulfide (H₂S) SIBO    - Sulfate as an electron acceptor     - H₂S production, sulfur-containing amino acids   - Physiological roles of H₂S vs. in excess   2. Hydrogen Sulfide and Gastrointestinal Motility     - Impact on transit time    - Association with IBS-like symptoms, diarrhea  3. Intestinal Motility and Regulation     - Role of the ENS, ICCs, smooth muscle cells, and neurotransmitters in motility   4. The Enteric Nervous System (ENS) and Gut Motility     - ENS as the "second brain" and its control over digestion     - Myenteric and submucosal plexuses: regulation of peristalsis, secretion, blood flow     - Gut-brain axis involvement via vagus nerve signaling     - ICCs as pacemaker cells generating slow-wave electrical activity    - MMC's role in clearing debris and bacteria 5. Dysregulated Motility and SIBO Development  6. Overlapping Symptoms Across SIBO Subtypes     - Shared symptoms   - Hydrogen and H₂S-dominant SIBO vs. IMO  7. Gastric Acid and Its Role    - HCl secretion by parietal cells and its role in digestion and microbial defense     - Hypochlorhydria and bacterial overgrowth   8. Addressing adverse food reactions goes way beyond simply cutting out the culprits, so I'm thrilled to have Dr. Tom Fabian join us again on New Frontiers to highlight the interventions that will move the needle for our patients. We explore the mechanisms through which specific strains can either exacerbate or alleviate both immune-mediated and non-immune-mediated food reactions, and discuss the fascinating connection between circadian rhythms of mast cells and intestinal permeability, feeding times and increased susceptibility to food reactivity. We look at why some patients may be more sensitive to symptoms and how histamine-producing microbes may lead to visceral hypersensitivity. But we also talk strategy, with one pearl after another, such as the potential of Lactobacillus reuteri in attenuating immune-mediated food reactions, the role of butyrate and indole in stabilizing mast cells, and using specific polyphenols to calm the immune system and promote a health microbiome. Hepatic Hydrogen Sulfide Levels are Reduced in Mouse Model of Progeria

A new research paper was published in Aging (listed by MEDLINE/PubMed as "Aging (Albany NY)" and "Aging-US" by Web of Science) Volume 15, Issue 12, entitled, “Hepatic hydrogen sulfide levels are reduced in mouse model of Hutchinson-Gilford progeria syndrome.” Hutchinson-Gilford progeria syndrome (HGPS) is a rare human disease characterized by accelerated biological aging. Current treatments are limited, and most patients die before 15 years of age. Hydrogen sulfide (H2S) is an important gaseous signaling molecule that is central to multiple cellular homeostasis mechanisms. Dysregulation of tissue H2S levels is thought to contribute to an aging phenotype in many tissues across animal models. Whether H2S is altered in HGPS is unknown. In a new study, researchers Stephen E. Wilkie, Diana E. Marcu, Roderick N. Carter, Nicholas M. Morton, Susana Gonzalo, and Colin Selman from the University of Glasgow, University of Edinburgh, Saint Louis University, and Karolinska Institute investigated hepatic H2S production capacity and transcript, protein and enzymatic activity of proteins that regulate hepatic H2S production and disposal in a mouse model of HGPS (G609G mice, mutated Lmna gene equivalent to a causative mutation in HGPS patients). “This study was designed and undertaken due to the lack of understanding in the mechanistic targets of known treatments against HGPS and considering the positive association between H2S and longevity in model organisms.” Here, the researchers employed the HGPS mouse model G609G to test the hypothesis that, in contrast to anti-aging increases in H2S production, the accelerated aging typical of progeroid mice is associated with reduced hepatic H2S production. G609G mice were maintained on either regular chow (RC) or high fat diet (HFD). HFD has been previously shown to significantly extend lifespan of G609G mice, and compared to wild type (WT) mice maintained on RC. RC-fed G609G mice had significantly reduced hepatic H2S production capacity relative to WT mice, with a compensatory elevation in mRNA transcripts associated with several H2S production enzymes, including cystathionine-γ-lyase (CSE). H2S levels and CSE protein were partially rescued in HFD fed G609G mice. The data acquired here confirmed some aspects of the relevance of H2S in HGPS but raises more questions about the specific mechanisms at play. “Regardless, the work presented here addresses an area of research that remains critically understudied and provides new evidence that the accelerated ageing phenotype observed in HGPS may be partially explained by a reduction in hepatic H2S levels.” DOI - https://doi.org/10.18632/aging.204835 Corresponding authors - Colin Selman - colin.selman@glasgow.ac.uk, and Stephen E. In geologic history, extinctions have occurred after high levels of atmospheric CO2, when oceans are stratified and archae were well fed and proliferated, producing hydrogen sulfide, a poison often called "swamp gas." Here Franz Oeste, Brian von Herzen and Paul Beckwith discuss the possibility that current conditions on earth are leading to such an extinction. They are not much worried, believing that the proliferation of phytoplankton will help cool the planet, and it is the heat that causes ocean stratification, which is the condition necessary for these other dire conditions to arise. However, there is a LOT more to know about it, such as its ability to convert to potent antioxidant compounds, and its therapeutic roles in cardiovascular and metabolic disorders (diabetes and NAFLD), neurodegenerative disorders, and more. But how do we start to both broaden and nuance our approach to assessing hydrogen sulfide status? And what interventions can not only supply the substrates for beneficial hydrogen sulfide production, but also help achieve that all-important balance between too little and too much. We're taking a trip back to high school chemistry in this episode! The chemical compound H2S, better known as the gaseous chemical Hydrogen Sulfide--the one that puts off a "rotten eggs" smell, is found in many natural environment settings. However, did you know that Hydrogen Sulfide is produced in small amounts in our bodies? This over-looked compound has a variety of important physiological effects, especially as we age. Dr. Chris Hine has been studying these benefits and how H2S may actually help stave off Glioblastoma (GBM)! Both Dr.'s Hine and Lathia have been hard at work at the Lerner Research Institute at Cleveland Clinic finding out-of-the-box ways to better treat or help prevent brain cancers such as GBM. In this episode, we take a deeper look into their work. Season Presenting Sponsor: Gamma Tile (GT Medical Technologies)   Episode Sponsors: OncoSynergy, Inc. & Highmark BCBS of WNY

Unless you're new around here, you might have noticed it's been a while since our last episode. In fact, you may have thought you'd never hear from me again. But NO! I'm popping back into your podcast feed with a very important year of SIBO in review: an update on all the things that have happened in the world of IBS research since we hung up our mics after the end of the 4th and final season of the podcast. I am so thrilled to have a return guest, the king of SIBO himself, Dr. Mark Pimentel. If you're been down the SIBO rabbit hole for a while now, Dr. Pimentel likely needs no introduction. He's the director of The Pimentel Lab at Cedars-Sinai in Los Angeles, where he researches many pieces of the puzzle related to IBS, gastrointestinal motility and bacterial overgrowths. He's published dozens of studies on these subjects, and it's fair to say that much of the data-backed information we've covered throughout the 4 seasons of this podcast, has been discovered through Dr. Pimentel's work. He was last on the show almost 2 years ago in February 2020, and in that interview he promised that there would be many exciting developments for those with Post-Infectious IBS, Methane overgrowths, and Hydrogen Sulfide SIBO. So today we're going to hear a little status update. Resources, mentions and notes: The SIBO Made Simple Book – ON SHELVES NOW!! In this episode, I interview Dr. Mark Pimentel who is a leading researcher in the field of IBS and SIBO. Dr. Pimentel has done many great things in the field of gastroenterology, not the least of which is the development of a clinical breath test for hydrogen sulfide. Before the test was available, clinicians were forced to make an educated guess on whether their patient was suffering from hydrogen sulfide SIBO.  Now with the ability to test the hydrogen sulfide levels, the next big question to answer is this: What is the best treatment? Dr. Pimentel and his research group are working on this and hopefully we'll have the answer soon. A brief but information-packed episode.

Thanks for listening and watching! Don't just get on the job, get into the job! Was talking about a few incidents he heard about where people were eating a fruit that smelled so bad that people called the FD to investigate. The fruit is called Durian Fruit [Show pic of Durian fruit] The Durian Fruit is ugly. It's like a sea urchin. Its big, thorny, and super stinky! It depends on what side of the fence you sit, there are the people that think it smells a pleasant smell. Some say it smells like gym socks. [kind of like how europeans find our chocolate] It has been found that the overall odor of durian pulp could be mimicked by only two compounds: fruity smelling ethyl (2S)-2-methylbutanoate and roasted onion-like smelling 1-(ethylsulfanyl)ethane-1-thiol. Did you say Thiol?! Why yes I did… Then lets talk about some thiols and what they are It's a sulfur compound. Technically it's an organosulfur compound. Basically is a sulfur-hydrogen group thats attached to an organic molecule OR an alkyl group. Now this group can be called a few things, all meaning the same. You could say thiol group, sulfhydryl group or a sulfanyl group. All mean the same. All stinky. Oddly enough thiols are an analogue of alcohols. The word thiol means a mix of the works alcohol and sulfur in Greek.  You may be asking yourself, where would I find some of these little bombs? You can find it in Hydrogen Sulfide or mercaptans. Now taking a look purely at the periodic table, you would find the Sulfur element immediately south of the Oxygen element. Meaning I would assume it has many of the same properties as oxygen (and I think we're pretty comfortable with oxygen as an element) But the SH functional group is MUCH different than an OH function. The purely SH group is what they would call a sulfhydryl group in the IUPAC naming system.  On a side note, the word mercaptan is an ancient word. Its from the latin language. So the older ones were named by saying an alkyl group, then “mercaptan”. Old skool. You may also find them in the brewing process as a bug in the software. When you add hops to the wort there is one specific one  One of the unique properties of Thiols is that they don't form hydrogen bonds, so they have a low boiling point. What does that mean to you as a responder? [the low BP means high VP therefore its coming to find my nose] Right, The 2-buten-1- thiol above is one of the components of skunk spray. Mercaptan (aka Methanethiol) is purposely added to bottled gas and natural gas in order to reveal leaks.  More places are asparagus, cabbage, red grapes, apples and peaches. Are they acidic or basic? They tend to be more basic. Are they flammable? They can be. The mercaptans are, but not everyone is.

Question: Is hydrogen sulfide produced in the gut a bad thing, and what to do about it?I agree that most people that have this problem have some kind of deranged sulfur metabolism, but I don't see why that makes it a physiological adaptation rather than a pathological condition. The correlation is probably a direct result of the predominance of sulfur-metabolizing bacteria in the gut, which is a bad thing that is actually causing the derangement of sulfur metabolism. Well, you could go the antibiotic route. And I think metronidozole, and let me see if I can dig this up quickly because I have done a lot of research on this issue for one of my clients. Okay. So from the research I was looking at, metronidozole is very effective against sulfur-metabolizing bacteria. But isn't that reliably delivered to the stool and maybe could be used as an enema. But I think generally, it is used. And then I think there's one other one that's sometimes used. I forgot what the other one is. I think it's... Okay. I can't find it. Anyway, you can go the antibiotic route under the guidance of a prescribing specialist. This Q&A can also be found as part of a much longer episode, here. Understanding the Dangers of Hydrogen Sulfide Gas. Hydrogen sulfide (H2S) gas is produced as a result of the microbial breakdown of organic materials in the absence of oxygen. It can be found in tanks, vaults, voids, and other confined spaces at industrial facilities including power plants. Besides being flammable and corrosive, H2S is also colorless and toxic, even in relatively low concentrations, so it is extremely hazardous to workers. In fact, it is the second-most-common cause of workplace inhalation fatalities behind carbon monoxide. H2S is noticeable initially by its rotten egg smell, but the gas can deaden senses making it difficult for workers to detect without a gas monitor. Veriforce CEO Colby Lane and Chris Detillier, senior safety analyst with Veriforce, were guests on The POWER Podcast. Veriforce is a leading provider of software and services that enhance workforce and community safety. Among its offerings is a training course called H2S Clear, which provides students with life-saving information while meeting the compliance requirements of ANSI/ASSP standards. “It's extremely toxic. As little as 700 parts per million can cause someone to immediately collapse, and they can die from it,” Detillier said. “So, it is very important to have a good training program in place.” Lane explained that Veriforce's training model essentially credentials and accredits instructors. Then, those instructors provide the training to actual workers. Detillier said he's received a lot of positive feedback every time he's taught a “train-the-trainer” class for H2S Clear. “We've had guys that have been in the industry for years—some of them who have previously been through H2S training—and after class would tell me how much that they learned from the class and appreciate the content that we have in there.”

There is a third type of SIBO (besides hydrogen and methane-based SIBO) that currently does not have a test available: hydrogen sulfide SIBO. In fact, it can sometimes look like a flat line on a lactulose or glucose SIBO test. What can patients and clinicians do with the resources available? Guests Dr. Allison Siebecker, Gary Stapleton, and I discuss the testing methods available to assess and validate your likelihood of hydrogen sulfide SIBO, and their experiences in the clinic and in the lab, respectively. You’ll also learn some treatments that have shown promise. https://drruscio.com/?p=32910 My book Healthy Gut, Healthy You is available at https://drruscio.com/getgutbook/ Looking for more? Dr Greg Nigh co-founded the Immersion Health Centre in 2014 after completing the Naturopathic Doctor program, and the Master of Science and Oriental Medicine Program.  During his early years of work, he developed his interest and passion for researching and implementing alternative cancer therapies.  Dr Nigh has spoken several times at the annual convention of the American Association of Naturopathic Physicians and the National Conference of the Oncology Association of Naturopathic Physicians on cancer cell metabolism.  Dr Nigh is a prolific reader, researcher, and writer and has also contributed a chapter to the upcoming two-volume Foundations of Naturopathic Medicine textbook.   Recently Dr Nigh and Dr Jacobi both presented on the SIBO SOS Summit.  Dr Jacobi loved Dr Nigh's talk on hydrogen sulfide and has invited him on the show to discuss sulfur metabolism and his unique ideas about hydrogen sulfide and sulfur imbalances.   Topics discussed include: How did Dr Nigh come to specialise in sulfur? All about sulfur metabolism. Sulfur chelates so well that it can get bound up in our bodies with toxic things. The GAPS diet, how it has high sulfur content, and why some patients may not feel well on the GAPS diet. Sulfur issues as the ‘canary in the coalmine' of toxicity issues in general. How the demand for sulfur is higher due to increased exposure to pollutants in modern life, and the impact this has on sulfur binding throughout the body and not just in phase 2 liver detoxification. How does endogenous sulfur processing, and sulfur production and reduction by certain gut bacteria, connect? Sulfur exposure is all dietary, and is metabolised to sulfate in the body. Sulfate is required in the body in a constant supply, and sometimes the metabolism of sulfur to sulfate is hindered. Heparan sulfate - every surface is in the body is lined with this. Elements that sulfur is used to build in the body. The primary, and alternative, pathway/s of sulfate production. Cystathionine beta synthase (CBS) pathway, what it generates (hydrogen sulfide) PAPS - the activated form of sulfate, and the pathways the body uses to get it. When people are reactive to sulfur or sulfur containing foods or thiols, they are not having an efficient process that ends up in PAPS - what are they reacting to in the pathway? If people cannot get the end product PAPS, then people can overgrow the sulfur fixing bacteria so that they produce higher levels of hydrogen sulfide and sulfide, which can be directly oxidised into sulfate by the mitochondria in the body. Dr Nigh's view is that the overgrowth of hydrogen sulfide bacteria can be seen as an adaptive overgrowth. How to reduce desulfovibrio with diet. Reasons why sulfate production can be shut down, one being glyphosate - a direct hit to sulfate production in the body, chelation of minerals that are needed for sulfur production, interference of specific amino acids involved in the sulfur cycle, and more. The impact of glyphosate on the sulfur production in the body is profound. Glyphosate - a toxic herbicide, the active ingredient in roundup. Glyphosate chelates molybdenum. We need tiny amounts of it, and if we only need tiny amounts it does not take much to chelate and use it up. Dr Nigh's view on testing. Patient symptom red flags that point to sulfur issues, for example: Hydrogen sulfide is a vasodilator, so has links to orthostatic hypotension. Any gut issues - warrants sulfur issue suspicion. Alcohol sensitivity Red, hot presentation Brain fog Histamine and sulfur overlaps. Histamine issue symptoms. Dr Nigh's protocols for enhancing sulfate production from sulfide. Taking the patient off sulfur foods for a week then gradual reintroduction. Detoxification protocols Infrared sauna Molybdenum supplementation Biotics Mo-Zyme Forte - food based tablet based on sprouted lentils 150mcg molybdenum Tablet chewed 2 times per day For those with bloating or gut issues Biotics Bio HPF - bismuth, demulcents, and others. Bismuth reduces hydrogen sulfide production in the gut. Butyrate The most underrated gut nutrient that there is. Biotics Butyric-Cal-Mag Apax EnteroVite Digestive enzyme Hydroxocobalamin injections or sublingually to lower hydrogen sulfide in the blood. Zinc acetate as a binder for hydrogen sulfide Glyphosate strongly chelates zinc. Detox support, coffee enemas, colonics, epsom salt baths (magnesium sulfate) - increasing sulfate transdermally, adrenal support if indicated Epsom salts (4 cups per bath) - a way to get sulfate in without the reactivity of digestion. Sensory deprivation tank option. Dr Yarnell's low sulfur diet Thiols and Dr Nigh's opinion on their reactivity for people. Red meat is the highest dietary source of sulfur and yet does not seem to have many people reacting to it, whereas garlic does. Why is this? Foods to watch - eggs, onion, kale, garlic, cabbage, brussel sprouts, cabbage, brassica family. Kale accumulates the heavy metal thallium (from waste-water used for crops). Do Allicin extracts still elicit issues in people with sulfur issues? Highly important to include any sulfur food items that are tolerated for the sulfur content (required by body). Dr Nigh's sulfur protocol and his success with SIBO patients. What is the connection between Dr Nigh's success with methane dominant SIBO clients and the sulfur protocol? How might glyphosate toxicity play into this? Methanobrevibacter smithii - not a pathogen, why? SIBO bacteria - why do we only grow certain bacteria? Dr Nigh's sulfur metabolism nuggets.   Resources Dr Greg Nigh Immersion Health Portland, Oregon drnigh@immersionhealthpdx.com SIBO SOS Summit Dr Yarnell Naturopathic Foundations of Gastrointestinal Diseases? In this episode Dr Nirala Jacobi is in conversation with Dr Christopher Shade. Dr Shade is the CEO and founder of Quicksilver Scientific, a lab that specialises in the environmental, biological, and analytical chemistry of mercury and all of its forms, and its interaction with sulphur compounds, particularly glutathione, and its enzyme systems.   Dr Jacobi met Dr Shade at the Bioceuticals Research Symposium in Sydney, 2017, and was riveted by his presentation on detoxification.   Topics discussed in this episode include: The liver, gallbladder, and GI connection, and how to align them so as to clear gastrointestinal issues. How failure to move bile is at the heart of detox reactions Bile as a detergent in the small intestine and how bile can be a core issue in SIBO. Bile acid destruction and fat malabsorption in SIBO. Emunctories and what this means. Drainage areas and how the extracellular space is cleared. Metal binder for the GI tract that is far better than chlorella - IMD microsilica Toxin conjugates and their interaction with bile salts. Phosphatidylcholine (PC), its requirement to fluidise the bile, and choline's relationship to cholestasis. Lipopolysaccharide (LPS) endotoxins from leaky gut (from SIBO) creating inflammation in the hepatocytes and the flow on effects from this. Phase 3 detoxification - The role of bile flow in lessening detoxification reactions through lowering toxic conjugate buildup in the blood, and instead moving them through the proper elimination channels. The importance of addressing emunctories in treating skin conditions. How LPS impairs phase 3 in the liver. Neurological consequences of leaky gut, SIBO, and of a backfiring liver. Neuroinflammation, LPS, and a leaky blood brain barrier. Movement of metals into the urinary flow and how it has nothing to do with glomerular filtration rate (GFR), even though this is what is commonly measured for kidney function. Organic anion transport peptide's (OATP) role in bringing toxic conjugates from the blood into the proximal tubular epithelial cells in the kidneys, then the movement of the toxic conjugates into the urinary flow through the MRP2 transporter. MRP2 and how LPS and metals can block it. How the same things that shut down liver transport of toxic conjugates, can also shut down kidney transport. Does LPS affect GFR? Active transport of toxins happens in the proximal tubules of the kidney, not in the glomerulus. Kidney disfunction and how SIBO can be related to proximal tubule damage in the kidney due to the build up of LPS. Proximal tubule function test available - Mercury Tri-Test from Quicksilver Scientific. Genetic Single Nuclear Polymorphisms (SNPs) and how the transport protein SNPs are commercially uncharted as yet. How inflammation blocks detoxification. Chronic infections turning down detoxification. Ways to address phase 3 detox. Address under-functioning gallbladder with PC, for example. Address leaky gut so that toxins dumped into small intestine do not recirculate. Strong bitters before meals. Take multi-toxin binder (30 minutes after taking bitters) - this is important to do 30 minutes after bitters. Dr Nirala Jacobi uses Toxaprevent, but not in SIBO-C patients. Dr Shade uses: Charcoal Clay Chitosan Metal binder eg. IMD (much stronger than chlorella), Chlorella. Acacia gum For leaky gut Low FODMAPS Bifidogenic prebiotic Powdered Aloe Marshmallow root Slippery elm Is there a test to assess phase 3 detox function? NRF2 upregulators and their role in cellular detoxification, to be implemented AFTER liver and kidney detoxification is streamlined. The importance of NRF2 and the effects it has on the body. Dr Shade's universal binder - Ultra Binder How to access Dr Shade's products. Dr Shade's recipe for a self made toxin binder. The importance of Myrrh in the bitters The bitter detoxifier in ayurvedic medicine, qi and blood mover in chinese and ayurvedic medicine, and strong antimicrobial. Cautions with long term myrrh supplementation. Other herbals bitters to include: Dandelion Solidago (Golden Rod) Gentian Dr Jacobi's bitters blend Dandelion Oregon grape Gentian Baical Skullcap Bitters as opening up the bile, tonifying the kidneys, and restoring ileocecal valve function. What is the function of bitter receptors on the ovaries? If SIBO clients are Hydrogen Sulfide dominant note that the bacteria involved here are bile loving, so any cholagogues will exacerbate the SIBO issue. Does charcoal absorb all of your minerals? Using charcoal as the primary binder - amounts to take and rationale for diversifying binder portfolio. Is mercury stored in biofilm? Is it advised to disrupt biofilm, or is it dangerous to do so? EDTA as a biofilm breaker. Plaques in the vascular system are actually biofilms. What mercury testing is offered at Quicksilver Scientific? Is it worth testing for mercury toxicity? When is it good to test people for mercury toxicity? How healing the GI tract can heal the kidneys. What is the evil of amalgam fillings? Why people with amalgams often have to urinate a lot at night. The body has it's own inbuilt chelating system - glutathione, glutathione-s-transferase, and the transport proteins. What are the myths of chelation agents? Is lipoic acid a chelator, or is it upregulating the body's natural chelation system? The importance of lining up all the drainage, and lining up all the phases of detoxification. Hydrotherapy for the lymphatics. The issues in Australia with lead and aluminum - Dr Shade's recommendation on how to test these areas: Nutripath's Blood Metals Assay Excess copper in the presence of low zinc can become synergistically toxic with all toxic metals. Whole blood vs intracellular for testing? How do you measure intracellular stores? Residence time in the blood of different metals. Dr Shade's last pearls of wisdom - start with drainage, then move to mobilisation after levels come down first with good drainage.   Resources Quicksilver Scientific Dr Shade's Quicksilver Scientific Youtube channel Proximal tubule function test available - Mercury Tri-Test from Quicksilver Scientific.
